Abstract

The Satisfaction with Appearance Scale (SWAP) was administered to 165 burn victims. SWAP showed a high level of internal consistency (Cronbach's alpha, r(a)=0.87); an 84-subject retest measured reliability (r(tt)=0.59). SWAP is both a reliable and valid measure of body image for a burn-injured population. (Author/MAK)

4. Early avoidance of traumatic stimuli predicts chronicity of intrusive thoughts following burn injury

Abstract

The role of avoidance behavior in perpetuating the experience of intrusive thoughts among burn survivors was investigated. The Impact of Events Scale (IES), which has subscales that measure the frequency of intrusive thoughts and avoidance behavior, was administered to burn survivors (n = 23) upon discharge and four months later. Both avoidance behavior and intrusive thoughts at discharge were significantly related to experiencing intrusive thoughts at four months (r = 0.59, P < 0.003, r = 0.45, P < 0.03, respectively). In a hierarchical regression analysis controlling for intrusive thoughts at discharge, avoidance behavior continued to significantly predict intrusive thoughts at four months [B = 0.43, t(20) = 2.9, P < 0.009]. Limitations of the study and implications for treatment are discussed.

12. Posttraumatic adaptation and distress among adult burn survivors

Abstract

Objective: The purpose of this study was to examine the prevalence, natural history, and psychosocial impact of posttraumatic symptoms in adult burn survivors. Method: Forty-three adult inpatients at a regional burn center were assessed at discharge with standardized instruments to determine the presence of psychiatric disorder, assess personality, and quantify depression. Thirty-one patients were evaluated 4 months after discharge. Results: Posttraumatic stress disorder was diagnosed in 7% of patients at discharge and in over 22% of patients at follow-up. Symptoms of avoidance and emotional numbing DSM-III-R criterion C symptoms) tended to emerge after discharge from the hospital. While posttraumatic symptoms were associated with symptoms of depression, they were not strongly associated with psychosocial adjustment to illness; psychosocial adjustment was more strongly related to aspects of personality, the injury itself, and its treatment. Conclusions: Since adult burn survivors often develop new symptoms of posttraumatic distress after leaving the hospital, longitudinal surveillance is required to detect new cases and provide appropriate treatment. Survivors at risk for poor psychosocial adjustment after discharge may be identifiable during hospitalization, and preventive treatment strategies should be developed and tested for this population.

13. Cultured epidermis for the coverage of massive burn wounds: a single center experience

Abstract

Patients who suffer massive wounds from burns require surgery to repair the full thickness of skin that was lost. There is controversy concerning the best method of achieving this. Normal skin is organized into an outer epidermal layer and a dermal layer below; duplication of this compartmentalization may be the best way to repair loss of full-thickness skin. The only method available to do this is expanded meshed autograft, which requires removal of skin from a donor site (another part of the patient's body), expansion of the donor tissue by making a series of slits in it and stretching it, and grafting to the area of the burn. The use of this technique for large burn areas is limited by the amount of donor skin available. The authors report their experience using cultured autologous epidermal tissue without a dermal underlayer. This procedure involves removal of a 5 millimeter (mm) by 40 mm piece of skin tissue; usually this is done within 48 hours after hospital admission. This tissue is sent to the laboratory to be cultured (grown). Preparation of cultured epidermis takes about three weeks. The patient is then brought to the operating room and the cultured epidermis is attached to vaseline gauze backing and applied to the carefully prepared burn wound. This procedure was carried out on seven patients who had an average burn size of 69.6 percent of total body surface. This group was compared, using chart review, with a historical control group of 18 patients who had an average burn size of 60 percent. The controls were treated with conventional meshed split-thickness graft. There was no significant difference between the two groups in duration of hospitalization, number of operations, or cost. In the cultured epidermis group there were no deaths and two major complications. In the historical control group there were six deaths and 11 major complications. In follow-up it was found that initially, cultured epidermis patients were more susceptible to blistering than controls, but after the second month this difference disappeared. Because of these encouraging results with cultured epidermis, a prospective randomized study should be performed. (Consumer Summary produced by Reliance Medical Information, Inc.)
