Introduction: Deprescribing is a complex and pivotal process in the healthcare system that requires the involvement of different healthcare professionals as well as patients and family members. Given the multifaceted healthcare professionals involved in deprescribing, fostering interdisciplinary healthcare teams during the didactic and experiential educational engagement is imperative. Thus, this study aimed to to characterize healthcare professional students’ opinions on the importance of an interdisciplinary approach to deprescribing during the didactic and experiential components of the curriculum. Methods:This qualitative study was conducted with healthcare professional students enrolled in a professional degree program at one Mid-South health science located in the US. Focus groups were conducted via an online platform over three months in 2022, and recruitment continued until thematic saturation was achieved.Using Thematic Analysis, the corpus of the transcribed data was imported into the Dedoose®, a qualitative software that facilitated the analysis. Results: A total of 36 healthcare professional students participated in four focus groups. Three themes emerged from the data: 1) setting the stage for deprescribing, 2) developing interprofessional deprescribing simulations, 3) potential positive patient outcomes. In the first theme, the healthcare professional students describe current challenges in the deprescribing process that call attention to the interprofessional nature of deprescribing. The second theme demonstrates the necessity of developing interprofessional simulations to educate healthcare professional students in the practice of deprescribing. Finally, in the third theme, the healthcare professional students illustrate the potential of interprofessional education on deprescribing to improve patient outcomes. Conclusion:The data highlights that there was strong agreement between extant literature and health professions participants on barriers to deprescribing, including interprofessional communication issues, and on the recommendation to enact interprofessional healthcare simulation education on deprescribing. In addition, the potential benefits of increased interprofessional education will provide immeasurable benefits and create an ample understanding of the roles of health professionals. These findings suggest that deprescribing focused interprofessional healthcare simulation educational activities should be conducted and assessed.

Background & Objective: Medical students form an integral part of future taskforce for the healthcare sector. We aimed to assess the impact of demographic charcateristics and COVID-19 pandemic affecting choice of speciality among medical graduates in Pakistan. Materials & Methods: A cross-sectional study was carried out on fresh medical graduates using two self-designed online questionnaire surveys before and after two months of house job, inquiring choice of specialty, reasons for choosing it, exposure to pandemic and its effect on choice of specialty using Chi-Square test. Results: Fifty nine percent of medical graduates had chosen field of speciality, significantly impacted by higher socioeconomic class (p=0.006), fathers with higher education (p=0.046) and advice about specialty selection (p=0.044). Internal medicine (20%), surgery (19%), obstetrics and gynaecology (O&G) (17%) and dermatology (13%) were most commonly selected. Age (p=0.027), female gender (p=0.009) and mothers with higher education (p=0.022) significantly affected choosing O&G, while advice to choose this specialty impacted choosing O&G (p=0.027) and dermatology (p=0.022). Better patient outcomes, challenging specialty, reputation/prestige and inspiration from a doctor were common reasons to choose a specialty. 9.1% changed their initial choice of specialty based on exposure to COVID-19 pandemic of whom majority switched to COVID unrelated fields, but it was not statistically significant. Conclusion: This study identifies that demographic, patient outcome and personal factors were primary determinants of specialty choice. COVID-19 pandemic did not significantly affect career choices among medical graduates.

Abstract Background Dental anxiety is very much common among the patients and could be due to different factors like the behavior of the dentist, past experiences, Needle phobia, or word of mouth from other patients. According to recent studies, a strong association between sound and anxiety has been found, so this observational study has been conducted to find out the link between the activation of anxiety with the sound of a handpiece between experienced patients, who have already gone through the dental treatments and non-experienced patients. Methods Total of 297 participants were part of this study. These participants were divided into 2 groups according to the experienced and non-experienced dental patients. The researcher first filled out the CORAH Dental Anxiety Scale (DAS) form to mark the anxiety level of the patients, and then noted the readings of the heart rate in 3 intervals which were before during, and after the treatment with the pulse oximeter. Later the data was analysed using the SPSS independent t-test. Results Results show that patients in group 1 who have gone through the dental treatment before were less anxious and had a lesser effect on their heart rate than the patient who were having the treatment for the first time who were in group 2. Another interesting factor was noticed that in both the groups female were found to be more anxious than male participants. Participants with younger age were found to be more anxious than older age patient in both groups Conclusions The sound of the handpiece can provoke anxiety in the patient, affecting the heart and increasing the heart rate. Participants who were experienced were found to be less anxious than the participants who were inexperienced.

Plants have been used to cure epileptic seizures, since ancient times. Approximately 70 percent people living in developing countries still rely on complementary and alternative medicines (CAM) regardless of the improvement in conventional medicine. People, particularly living in rural and tribal areas, have faith in Hakeem’s. According to them, alternative treatments do not have toxic effects and are more easily accessible, cost effective and have been used traditionally. Standard drugs therapies have several adverse effects; people become resistant and require a combination of treatments which may double the risk of adverse effects. Herbal agents could be a better alternative to conventional medicine; hence to prove the effectiveness and safety of medicinal plants, evidences-based researches are required from different parts of world. In order to prove the efficacy of herbal agents, different animal models of seizures are used to evaluate efficacy of plants and among all of them Maximum electroshock (MES) induced seizure model (a model of tonic-colonic seizure), is used as gate keeper to assess the anti-seizure efficacy of newly tested plants. We report on the use of 2D Ruddlesden–Popper (RP) perovskites as optoelectronic materials in building-integrated applications, addressing the challenge of balancing transparency, photoluminescence, and stability. With the addition of polyvinylpyrrolidone (PVP), the 2D RP films exhibit superior transparency compared to their 3D counterparts with an average visible transmittance (AVT) greater than 50% and photoluminescence stability under continuous illumination and 85 °C heat for up to 100 h as bare, unencapsulated films. Structural investigations show a stress relaxation in the 3D perovskite films after degradation from thermal aging that is not observed in the 2D RP films, which retain their phase after thermal and light aging. We also demonstrate ultrasmooth, wide-bandgap 2D Dion–Jacobson (DJ) films with PVP incorporation up to 2.95 eV, an AVT above 70%, and roughnesses of ~2 nm. These findings contribute to the development of next-generation solar materials, paving the way for their integration into built structures.

ABSTRACT Acinetobacter baumannii is a Gram-negative pathogen, accountable for lethal nosocomial infections, owing to high pathogenicity and multi-drug resistance (MDR). Hence, the World Health Organization has prioritized this bacterium against which new pharmacophores are required urgently. Therefore, this study evaluates the antimicrobial potential of lithium complex based on salicylic acid, and 1,10-phenanthroline {[Li(phen)2sal]} against MDR A. baumannii (MDRAB) through growth inhibition, growth curve assay, biofilm inhibition, and eradication alongside microscopy. The mechanism of action was proposed by flow cytometry and proteomic analysis. Moreover, in vivo efficacy was assessed using an A. baumannii-induced pneumonia model in mice. Results showed that lithium complex significantly inhibited the bacterial growth at 16–32 µg/mL. Growth kinetics revealed that it retarded the bacterial growth after 2–3 h of treatment. Lithium complex significantly inhibited and reduced >70% of biofilms at the MIC level which is a key problem of indwelling and prosthetic devices. Light and atomic force microscopy further indicated disarticulated biofilm and ruptured cellular membrane post-lithium complex treatment. Proteomic analysis revealed that the compound exerted oxidative stress on A. baumannii which may contribute to cell rupturing. Data are available via ProteomeXchange with identifier PXD044293. Flow cytometry confirmed its role in cytoplasmic membrane disintegration. The lithium complex ameliorated the pneumonia symptoms at non-toxic doses of 20 and 40 mg/kg in the pre-clinical A. baumannii-induced pneumonia model as evident by the significant reduction in bacterial burden and parenchymal inflammation. Taken together, these findings suggest that the lithium complex can be a potential drug candidate against MDRAB. IMPORTANCE Multi-drug resistance (MDR) by virtue of evolving resistance and virulence mechanisms among A. baumannii is a global concern which is responsible for lethal hospital-acquired infections. Therefore, it is crucial to develop new therapeutics against it. Metal complexes are compact structures with diverse mechanisms that the pathogens cannot evade easily which make them a strong drug candidate. In this study, we assessed the in vitro and in vivo efficacy of lithium complex {[Li(phen)2 sal]} against biofilm-forming MDR A. baumannii. The lithium complex displayed strong antimicrobial activity and reduced the pre-formed mature biofilm which is key barrier for antimicrobial action. Moreover, it employs oxidative stress as one of its mode of actions and causes cellular rupturing. Lithium complex was non-toxic and was significantly effective to overcome pneumonia in mice model. These results highlight the untapped potential of metal complexes that can be explored and utilized for combating notorious A. baumannii infections.

ABSTRACT: Background and objective: Candida albicans belongs to the genus Candida. It is a commensal yeast in healthy individuals and can cause wide variety of infections in immune-compromised patients. The aim of the present study was to observe the frequency of Candida species, resistant profile of C. albicans against fluconazole and over-expression of CDR1 gene in both fluconazole resistant and sensitive C. albicans. Methodology: A total number of 206 Candida species were collected on Sabouraud Dextrose agar from Jinnah hospital Lahore. The isolates were re-confirmed by using various microbiological techniques like wet preparation, Gram stain and Germ tube test. Identification upto the species level was done by APIC 20AUX. Antifungal susceptibility profile of C. albicans against fluconazole was determined by broth microdilutiom method. Real Time PCR was performed on both the sensitive and resistant C. albicans to observe the role of CDR1 gene against fluconazole resistance. Results: • tropicals was the most common species with the frequency of 66 (32.0%) followed by C. albicans 41(19.9), C. glabrata 24 (11.7%), C. lusitaniae 14 (6.8%) and other less common species. The majority of infected patients were females (59.2%) than males (41%). Antifungal susceptibility testing showed that 80% C. albicans were sensitive to fluconazole. Results of gene expression showed that CDR1 gene play a major role towards fluconazole resistance in C.albicans. Conclusion: Fluconazole was found to be effective in treating infections caused by C. albicans. With the rise of global warming and the growing energy crisis, scientists have pivoted from typical resources to look for new materials and technologies. Perovskite materials hold the potential for making high-efficiency, low-cost solar cells through solution processing of Earth-abundant materials; however, scalability, stability, and durability remain key challenges. In order to transition from small-scale processing in inert environments to higher throughput processing in ambient conditions, the fundamentals of perovskite crystallization must be understood. Classical nucleation theory, the LaMer relation, and nonclassical crystallization considerations are discussed to provide a mechanism by which a gellan gum (GG) additive—a nontoxic polymeric saccharide—has enabled researchers to produce quality halide perovskite thin-film blade coated in ambient conditions without a quench step. Furthermore, we report on the improved stability and durability properties inherent to these films, which feature improved morphologies and optoelectronic properties compared to films spin-coated in a glovebox with antisolvent. We tune the amount of GG in the perovskite precursor and study the interplay between GG concentration and processability, morphological control, and increased stability under humidity, heat, and mechanical testing. The simplicity of this approach and insensitivity to environmental conditions enable a wide process window for the production of low-defect, mechanically robust, and operationally stable perovskites with fracture energies among the highest obtained for perovskites.

We combined field-based data with mathematical modeling to estimate the effectiveness of smartphone-enabled COVID-19 exposure notification in Pennsylvania, USA. We estimated that digital notifications potentially averted 7–69 cases/1,000 notifications during November 8, 2020–January 2, 2021. Greater use and increased compliance could increase the effectiveness of digital notifications.

The aims of this study is to define a cubic fuzzy set based logarithmic decision-making strategy for dealing with uncertainty. Firstly, we illustrate some logarithmic operations for cubic numbers (CNs). The cubic set implements a more pragmatic technique to communicate the uncertainties in the data to cope with decision-making difficulties as the observation of the set. In fuzzy decision making situations, cubic aggregation operators are extremely important. Many aggregation operations based on the algebraic t-norm and t-conorm have been developed to cope with aggregate uncertainty expressed in the form of cubic sets. Logarithmic operational guidelines are factors that help to aggregate unclear and inaccurate data. We define a series of logarithmic averaging and geometric aggregation operators. Finally, applying cubic fuzzy information, a creative algorithm technique for analyzing multi-attribute group decision making (MAGDM) problems was proposed. We compare the suggested aggregation operators to existing methods to prove their superiority and validity, and we find that our proposed method is more effective and reliable as a result of the comparison and sensitivity analysis.

Introduction: Obesity causes various diseases due to systemic oxidative stress. Young healthy obese are unaware of oxidative burden in their body. Total oxidant status (TOS) is a marker of oxidative stress in obesity. Aims & Objectives: This study was planned to determine and compare levels of TOS in young healthy non obese and obese, males and females and individuals with active versus sedentary lifestyle. Place and duration of study: This study was conducted over one month duration at six women and co-ed colleges of Lahore. Material & Methods: This cross-sectional comparative study was conducted on samples collected within one month from 160 healthy males and females, aged 20 to 40 years at APWA College, Lahore College Womens University, Government College University, Gulberg College, PAC Lahore, Ameeruddin Medical College Lahore (n=30 each college). The participants were divided into two groups based on the respondents BMI. TOS was measured colorimetrically. Data was analyzed using IBM-SPSS version 25. Mann Whitney U test compared TOS levels between non obese and obese subjects. Results: Fifty percent of the subjects had a normal BMI while 50% were obese according to the Asian Pacific criteria for BMI.Fifty-two percent of the population were males while forty-eight percent of the population was females. Group I (non obese) consisted of 55% males and 45% females with a significant difference in the levels of TOS. The population in Group II (obese) comprised of 50% males and females each also showing significant difference. Thirty-six percent (n=58) of the total population had active lifestyle while sixty-four percent (n=102) of the population had a sedentary lifestyle. While in Group 1(non obese) (64%) had an active lifestyle whereas the rest were sedentary. In comparison only 9% of group II subjects (obese) had an active lifestyle while the remainder were sedentary. A significant difference in level of TOS were seen when compared between subjects with active versus sedentary lifestyle in respective groups. A significant difference (p=0.03) was seen when TOS was compared between subjects with Active lifestyle 4.15(3.17- 5.42) µmol/lversus subjects with sedentary lifestyle 4.9(3.70-7.22) µmol/l. Conclusion: Increased levels of TOS were seen in young healthy obese as compared to non-obese. Both non obese and obese females had higher levels of TOS in comparison to males due to greater levels of body fat percentage. Subjects with a sedentary lifestyle have raised levels of TOS in comparison to subjects with active lifestyle.

Objective: To assess the efficacy and safety of IPL in patients with inflammatory Acne Vulgaris. Study Design: Quasi-experimental study. Place and Duration of Study: Department of Dermatology, Pakistan Air Force Hospital, Fazaia Medical College, Islamabad Pakistan, from Aug 2019 to Jan 2020. Methodology: Ninety patients with inflammatory facial acne were enrolled and received four Intense Pulsed Light (IPL) sessions at two weeks intervals with fluence 25J/cm2 and 420nm cut-off filter. The duration of treatment for each patient was two months. Clinical improvement was assessed using the Global Acne Grading System score (GAGs score). Results: Mean age of the patients was 18.5±3.3years, of which nine were males (10%), and 81 were females (90%). IPL was effective in 42(46.7%) patients with inflammatory acne vulgaris. Abstract In the current study, APOB (rs1052031) genotype-guided proteomic analysis was performed in a cohort of Pakistani population. A total of 700 study subjects, including Coronary Artery Disease (CAD) patients (n = 480) and healthy individuals (n = 220) as a control group were included in the study. Genotyping was carried out by using tetra primer-amplification refractory mutation system-based polymerase chain reaction (T-ARMS-PCR) whereas mass spectrometry (Orbitrap MS) was used for label free quantification of serum samples. Genotypic frequency of GG genotype was found to be 90.1%, while 6.4% was for GA genotype and 3.5% was for AA genotypes in CAD patients. In the control group, 87.2% healthy subjects were found to have GG genotype, 11.8% had GA genotype, and 0.9% were with AA genotypes. Significant (p = 0.007) difference was observed between genotypic frequencies in the patients and the control group. The rare allele AA was found to be strongly associated with the CAD [OR: 4 (1.9–16.7)], as compared to the control group in recessive genetic model (p = 0.04). Using label free proteomics, altered expression of 60 significant proteins was observed. Enrichment analysis of these protein showed higher number of up-regulated pathways, including phosphatidylcholine-sterol O-acyltransferase activator activity, cholesterol transfer activity, and sterol transfer activity in AA genotype of rs562338 (G>A) as compared to the wild type GG genotype. This study provides a deeper insight into CAD pathobiology with reference to proteogenomics, and proving this approach as a good platform for identifying the novel proteins and signaling pathways in relation to cardiovascular diseases.

Objective: To determine the association between psoriasis and serum uric acid levels. Study Design: Case control study. Place and Duration of Study: Study was conducted at Department of Dermatology, Pakistan Air Force Hospital Islamabad, from Jan 2017 to Jul 2017. Material and Methods: This study involved 31 psoriatic subjects of both genders aged between 18-70 years. Subjects were matched with 31 healthy volunteers from PAF Hospital staff. Serum uric acid level was tested in all the study participants. Outcome variable was frequency of hyperuricemia among cases and controls. Results: The age of the subjects ranged from 18 years to 70 years with a mean of 39.47 ± 13.92 years. Majority (n=37, 59.7%) of the subjects were aged under 45 years. There were 35 (56.5%) male and 27 (43.5%) female subjects with a male to female ratio of 1.3:1. There were 20 (32.3%) obese subjects. Mean serum uric acid was significantly higher among cases (5.89 ± 1.28 vs. 4.76 ± 1.44 mg/dl; p=0.002) as compared to controls. Frequency of hyperuricemia was significantly higher among cases (25.8% vs. 6.5%; p=0.038; OR=5.043; 95% CI: 0.975 – 26.086) as compared to controls. Conclusion: Mean serum uric acid and frequency of hyperuricemia was found to be significantly higher among cases (25.8% vs. 6.5%; p=0.038) as compared to controls irrespective of age, gender and obesity status of patient.

The scoping review study was carried out from January 18, 2019, to February 18, 2019, by following the Arksey and O'Malley method of scoping review. An extensive search in the bibliographic databases, PubMed, ERIC, and Google scholar for the gray literature was done. The search was narrowed by a set of inclusion and exclusion criteria. The studies included in the literature search were sort by the PRISMA flow chart. The selected studies address the use of a positive reinforcement strategy to manage disruptive behavior in the classroom. The positive reinforcement strategies identified were praise (41%), feedback (33%), and other classroom management studies (25%). Skinner's operant learning principle has a classroom implication for increasing the likelihood of the desired behavior. The results of the review can be used to implement evidence-based practice and policy regarding improving the desired behavior in the classroom.

Wide bandgap (Eg) perovskite solar cells (PSCs) are emerging as the preferred choice for top cells in a tandem architecture with crystalline silicon solar cells. Among the wide bandgap perovskites, a mixed cation mixed halide composition containing CsyFA1-yPbI3−xBrx is a popular choice because the presence of bromine widens the bandgap and addition of cesium stabilizes the crystal structure. These perovskite layers are commonly fabricated using one-step spin coating technique; however, sequential spin coating followed by dip coating has been successful in offering better control over the crystallization process for low bandgap absorber layers. In this paper, the fabrication of a Cs0.2FA0.8PbI3−xBrx perovskite absorber layer using the sequential deposition route is reported. The concentration of bromine was varied in the range 0 ≤ x ≤ 1 and optical, structural, and morphological properties of the films were studied. As the concentration was increased, the perovskite showed better crystallinity and the presence of large grains with high surface roughness, indicating the formation of the CsPbBr3 phase. Optically, the perovskite films exhibited higher absorbance in the ultraviolet (UV) range between 300 and 500 nm, hence up to x = 0.3 they can be profitably employed as a wide bandgap photon absorber layer in solar cell applications.

Postpartum depression (PPD) is defined as the onset of major depressive disorder in mothers, occurring during pregnancy or within 4 weeks post-delivery. With 7% of pregnancy-related death in the United States owing to mental health conditions, including PPD, and a global prevalence of 12%, PPD is a growing public health concern. In 2019, the Food and Drug Administration (FDA) approved brexanolone, an exogenous analog of allopregnanolone, as the first ever drug to be specifically indicated for treating patients with PPD. This approval was preceded by an open-label study and three randomized placebo-controlled trials, each assessing the safety, tolerability, and efficacy of brexanolone, using mean Hamilton Rating Scale for Depression (HAM-D) score reduction as the primary outcome. In each randomized controlled trial, the drug was administered as an intravenous infusion given over 60 h. Enrolled participants were followed up on days 7 and 30 to evaluate the sustained effect. A statistically significant reduction in mean HAM-D score compared to placebo was observed in all three studies, supporting brexanolone’s use in treating moderate-to-severe PPD. Therefore, this article attempts to briefly review the pharmacology of brexanolone, evaluate the latest available clinical data and outcomes concerning its use, reevaluate its position as a ‘breakthrough’ in managing PPD, and review the cost-related barriers to its worldwide standardized use.

In Everyday Sectarianism, anthropologist and filmmaker Joanne Nucho examines the inextricable links between sectarian belonging, Lebanon’s confessional system of governance, and neighborhood infrastructures developed in the absence of the state (a refrain throughout the book is wayn al dawleh?). Departing from orientalist accounts that represent sectarianism as a static and primordial conflict of identities, Nucho argues that sectarianism in Lebanon is a modern, relational, and political process of continual (re)construction. In this sense, her account draws from existing literature on the Lebanese state that emphasizes sectarianism’s contingent character (see, for example, Ussama Makdisi 2000; Max Weiss 2010; Suad Joseph 2008). For these scholars, sectarianism is not a given mode of being in the world. Rather, it is a project inseparable from questions of gender, class, geography, and the state, and cannot be “collapsed onto religion or theology” (4).

Multidrug resistant A. baumannii has emerged as an important and problematic human pathogen as it is the causative agent of several types of infections especially in neonates and immunocompromised patients because they have least capacity to fight against infections. Carbapenems are used as last resort antibiotics for treating these infections but currently resistance against carbapenems due to MBL production is on the rise. The objective of this study was to determine the frequency of antibiotic resistance in A. baumannii and also to compare the efficacy of combined disk test and double disk synergy test for detection of metallo-beta-lactamases. A total of 112 A. baumannii were identified from various clinical samples and antibiotic susceptibility profile was determined by Kirby-Bauer Disk Diffusion method. Out of 112, 66 (58.9%) isolates were resistant to both imipenem and meropenem (OXOID). These resistant isolates were tested for carbapenemase production, and 55 (83.3%) were carbapenemase producers by Modified Hodge Test. These isolates were further tested for MBL production by combined disk test and double disk synergy test. Out of 66, 49 isolates were positive by both methods, CDT and DDST, and only one isolate was detected as negative (with kappa value = 0.038). All MBL producing strains showed remarkable resistance to cephalosporins, fluoroquinolones, aminoglycosides, and piperacillin/tazobactam (OXOID). The antibiotic resistance was very high in A. baumannii which were isolated from children in Pakistan specially attending a nephrology unit.
