1. Dysphagia Therapy in Adults with a Tracheostomy: A Scoping Review Protocol

Abstract

Background: Swallowing impairment (dysphagia) and tracheostomy coexist. Research in this area has often provided an overview of dysphagia management as a whole, but there is limited information pertaining to specific dysphagia therapy in the tracheostomy population. The aim of this scoping review is to provide a detailed exploration of the literature with regard to dysphagia therapeutic interventions in adults with a tracheostomy. The scoping review will describe current evidence and thus facilitate future discussions to guide clinical practice. Methods: A scoping review using the Joanna Briggs Institute and Preferred Reporting Items for Systematic Reviews guideline will be used. Ten electronic databases from inception to December 2023 and grey literature will be searched. From identified texts forward and backward citation chasing will be completed. Data extraction will compose of population demographics, aetiology and dysphagia therapy (type, design, dose and intensity). A number of citations and papers included in the scoping review will be presented visually. Discussion: The scoping review aims to expand upon the existing literature in this field. A detailed description of the evidence is required to facilitate clinical discussions and develop therapeutic protocols in a tracheostomised population. 19. The Effect of a Zoo-Based Experiential Academic Science Program on High School Students' Math and Science Achievement and Perceptions of School Climate

Abstract

The purpose of this study was to determine the effect of an 11th-grade and 12th-grade zoo-based academic high school experiential science program compared to a same school-district school-based academic high school experiential science program on students' pretest and posttest science, math, and reading achievement, and student perceptions of program relevance, rigor, and relationships. Science coursework delivery site served as the study's independent variable for the two naturally formed groups representing students (n = 18) who completed a zoo-based experiential academic high school science program and students (n = 18) who completed a school-based experiential academic high school science program. Students in the first group, a zoo-based experiential academic high school science program, completed real world, hands-on projects at the zoo while students in the second group, those students who completed a school-based experiential academic high school science program, completed real world, simulated projects in the classroom. These groups comprised the two research arms of the study. Both groups of students were selected from the same school district. The study's two dependent variables were achievement and school climate. Achievement was analyzed using norm-referenced 11th-grade pretest PLAN and 12th-grade posttest ACT test composite scores. Null hypotheses were rejected in the direction of improved test scores for both science program groups-students who completed the zoo-based experiential academic high school science program (p <0.001) and students who completed the school-based experiential academic high school science program (p <0.001). The posttest-posttest ACT test composite score comparison was not statistically different (p = 0.93) indicating program equipoise for students enrolled in both science programs. No overall weighted grade point average score improvement was observed for students in either science group, however, null hypotheses were rejected in the direction of improved science grade point average scores for 11th-grade (p < 0.01) and 12th-grade (p = 0.01) students who completed the zoo-based experiential academic high school science program. Null hypotheses were not rejected for between group posttest science grade point average scores and school district criterion reference math and reading test scores. Finally, students who completed the zoo-based experiential academic high school science program had statistically improved pretest-posttest perceptions of program relationship scores (p <0.05) and compared to students who completed the school-based experiential academic high school science program had statistically greater posttest perceptions of program relevance (p <0.001), perceptions of program rigor (p <0.001), and perceptions of program relationships (p <0.001). 35. Aspiration, risk and risk feeding: A critique of the Royal College of Physicians guidance on care of people with eating and drinking difficulties

Abstract

The Royal College of Physicians has recently published guidance on supporting people with eating and drinking difficulties. Although much of the advice in the guidance is sensible and helpful, in this paper we argue that the recommendations regarding ‘risk feeding’ decisions are flawed. In particular, there is a failure to clearly identify the nature, frequency and severity of different risks. There is an undue emphasis on aspiration as a risk and as a potential cause of pneumonia, and the limited evidence base for many interventions to manage risk is not adequately acknowledged. There is an emphasis on multidisciplinary team decision making at the expense of individual professional responsibility. We conclude that this guidance regarding risk feeding supports an unduly defensive approach to oral intake and should not be adopted as a standard of medical practice. more...

38. Underreporting of Death Certification in a University Teaching Hospital – A Hospital Based Study in Ireland.

Abstract

Summary Background Death certificates are frequently used as the source for epidemiological data on the prevalence of diseases. We postulated that comorbidities may be under-documented, particularly in older patients with multiple coexisting conditions. Methods Death certificates completed during a six-month period notification forms completed between January and June 2016 To investigate the accuracy of death certificate completion in Galway University Hospital, with specific emphasis on accurate documentation of common comorbidities. A retrospective review of and comparison with the deceased patients' casenotes. All death certificates were divided into those relating to patients aged over and under 75 years. Death certificates were examined for accuracy and documentation of comorbidities and these, (and the number of omissions) were compared with the actual diagnoses documented in the patients' casenotes. Results The cause of death was accurately documented in all Death Certificates. Overall, comorbidities were more common and omissions were more frequent in the older group compared with the younger cohort, with at least one comorbidity omitted in 71% of death certificates versus 56% (p = 0.0481). For individual diagnoses, under-documentation rates were similar in both age-groups. Conclusions While the actual cause of death was accurately completed in the death certificates reviewed in this audit, the majority of certificates in both age groups omitted one or more important comorbidity. The readability of 40 healthcare policies published by the NHS or the Health Service Executive were compared and contrasted against 40 newspaper articles regarding healthcare from the Irish Timesor the Irish Mirror.MethodsReadability was measured using the Flesch reading ease score and the standardised measure of gobbledygook grade.FindingsFlesch and the standardised measure of gobbledygook were significantly worse in health policies than in newspaper articles: none of the healthcare policies met a minimum standard of a Flesch score of 50 or more (P<0.0001); 60% of health policies and no newspaper article had a standardised measure of gobbledygook grade of 13 or more, suggesting a need for a university level education for comprehension (P<0.0001).ConclusionsThe readability of healthcare policies in Britain and Ireland is poor. 41. Melena as an unusual presentation of gastrointestinal stromal tumour, a case report.

Abstract

Introduction Gastrointestinal Stromal Tumors (GISTs) are a rare slow growing malignancy, accounting for less than 1% of all gastrointestinal (GI) tract tumors. These tumors are usually discovered incidentally by endoscopy, surgery or radiology. However on occasions they may present with significant symptoms including GI blood loss. This case report discusses an atypical presentation of a GIST in a 57-year-old female. Case presentation A 57-year-old woman presented to the emergency department following one episode of melena. This occurred on a background of two previous presentations with melena over a 10-year period. She had a preceding surgery for a Meckel’s Diverticulum. She was admitted for monitoring and investigation. An emergency upper endoscopy showed no upper gastrointestinal pathology to account for the bleeding. Her condition deteriorated with development of hypovolemic shock, requiring blood transfusion. An urgent CT angiogram identified a large mass in the distal ileum. The patient underwent an emergency laparotomy, where a 9.1 cm tumor located on the distal one-third of the ileum was resected. Histopathology confirmed the mass was a GIST. The patient had a successful post-operative period and subsequent treatment with Imatinib. Discussion The majority of GISTs are found incidentally. This case report describes an unusual presentation of a GIST in which the tumor bled into the intestinal lumen causing significant melena and life threatening hemorrhage. 42. The de-alzheimerisation of dementia? An analysis of internet searches.

Abstract

Purpose Alzheimer's disease is often seen by the public as synonymous with dementia but this may have changed in recent years. Methods We used Google Trends to examine the relative volume of internet searches from 2004 to July 2017 for the terms 'dementia' and 'Alzheimer' in English-speaking countries. For each country, a linear regression model was fitted for each search term and the slopes of the lines were compared. Results The slopes (standard error) for 'dementia' and 'Alzheimer', respectively, were: Australia--0.26 (0.02) and - 001 (0.002); Canada--0.23 (0.01) and 0.004 (0.01); Ireland: 0.33 (0.02) and - 0.04 (0.01); United Kingdom--0.36 (0.01) and 0.01 (0.001); and United States--0.23 (0.01) and - 0.01 (0.004). Differences between slopes were all significant at p < 0.001. 44. How well does Anorexia Nervosa fit with personal values? An exploratory study

Abstract

Despite an increasing clinical interest in the use of personal values as a motivational tool in psychological therapies for Anorexia Nervosa (AN), research is limited. This study explored personal values among individuals with AN, with a particular focus on the ‘fit’ between participants’ values and their AN. A qualitative research design was employed in this study. In-depth, semi-structured interviews were carried out among eight female outpatients and inpatients with a diagnosis of AN or Eating Disorder Not Otherwise Specified – AN type (EDNOS-AN type). Data was analysed using Interpretative Phenomenological Analysis (IPA; Smith, Jarman & Osborne, 1999). Three super-ordinate themes emerged from analysis: ‘Balancing Values’(difficulty finding balance in relating to and acting on values), ‘Congruence and Clashes between AN and Values’(experiences of AN representing a mixed-fit with values) and ‘From Ambivalence to Motivation’(ambivalence toward both AN and recovery – in the context of its mixed-fit with values – and experiences of values as a motivational tool in recovery). Study findings support a role for psychological therapies in working with personal values as a means of promoting recovery in AN, through supporting individuals to explore AN’s workability in the context of their values. Further research investigating the optimal stage of treatment to work with values as a motivational tool is warranted. more...

Abstract

AbstractObjective:The aim of this study was to investigate the attitudes to medication in relation to insight, purpose in life, symptoms and sociodemographic factors among a cohort of stable patients with a diagnosis of schizophrenia and schizoaffective disorder.Method:We included 70 patients with a diagnosis of schizophrenia and schizoaffective disorder attending a Dublin suburban mental health service. All participants were 18 years or older and were excluded if they had a learning disability, acquired brain injury resulting in unconsciousness, and psychosis secondary to a general medical condition or illicit substance misuse. All participants were given self report questionnaires which included Drug Attitude Inventory (DAI-30), Birchwood Insight Scale, and Purpose in Life test. Symptoms were assessed using the Scale for Assessment of Positive and Negative symptoms. All data was analysed using the Statistical Package for the Social Sciences.Results:We found that 86% (n = 60) of the participants had positive attitudes to medication, and 82% (n = 58) had good insight into their illness. Only 27% (n = 19) were found to have a definite purpose in life. There was a significant negative relationship between attitudes to medication and delusions (r = -0.25, n = 70, p < 0.05) and a significant positive relationship between insight and attitudes to medication (r = 0.0.28, n = 70, p < 0.05).Conclusion:Many factors are involved in the multifaceted issue of attitudes to medication.
