Thermal management is crucial for effective heat regulation and minimal energy loss across fields like electronics cooling, chemical processing, and energy storage etc. Therefore, present work numerically analyzes the convection of heat and entropy production for a lid driven flow in a tilted square enclosure containing viscoelastic nanofluid exposed to a static magnetic field with a magnitude denoted as ‘B0’. A second-grade model is employed to characterize the viscoelastic behavior. Tc and Th are the constant temperatures at the upper and bottom walls, respectively whereas the vertical walls are considered insulated. Moreover, upper wall moves at a uniform velocity in positive x-axis direction. The stream function approach removes the pressure gradient term from the linear momentum equation, and the resulting partial differential equations are discretized via finite differences and then resulting algebraic equations are solved with SOR and SUR methods employing self-developed MATLAB codes. The computations were carried out with the values relevant parameters ranging from: 10 ≤ Re ≤ 50, 0 ≤ Ha ≤ 20, 0 % ≤ ϕnf ≤ 4 %, 10−1 ≤ Ec ≤ 10−3, 0 ≤ En ≤ 10−2 and inclination angle (ξ)= -300, 00, 300, 450. The contour profiles of streamlines, isotherms, local entropy generation (SGEN); local Bejan number (Be) are plotted to demonstrating their effects. It is noted that Nuavg and Ns both increase significantly as the ϕnf increases. However, rise in ‘Ha’, reduces Nuavg and Ns by 47.19 % and 43.42 % for Ri = 0.1, and by 4.17 % and 3.46 % for Ri = 5, respectively. The elasticity effect opposes flow and reduces Nuavg and Ns. At Ri = 0.1, increasing ‘En’ from 0 to 0.1 reduces Nuavg and Ns by 2.22 % and 2.56 %, while at Ri = 5, Nuavg and Ns decreases by 0.76 % and 0.28 %. Moreover, with Eckert number (Ec) average Nusselt number diminishes, accompanied by an increase in both Ns and Beavg. Heat convection and entropy production are enhanced with forward inclination of the square enclosure and is slightly diminished with backward inclination.

Type 2 Diabetes Mellitus is an endocrine metabolic disorder leading to impaired glucose utilization. This disease is managed by both commercial drugs and herbal products eliciting various interactions. The evaluation of one such interaction between saxagliptin (SAXA, substrate of CYP3A4), and Guggul extract (GE) from Commiphora wightii is done in this present study. The concomitant administration of the SAXA with GE had restored the levels of otherwise increased blood glucose and biochemical parameters and otherwise decreased total glycogen and oxidative stress markers more effectively as compared to the solely consumed SAXA or GE. These results were corroborated by histopathological evaluation where the combination treatment showed better mitigation of hepatic, renal and pancreatic tissue damage that occurred due to induced diabetes. Furthermore, CYP3A11 mRNA (murine homolog to human CYP3A4) expression levels were observed to be reduced to non-diabetic levels in combination therapy which was otherwise unachievable. The molecular docking studies predicted improved interaction between CYP3A4 and bioactive content of GE, i.e., guggulsterone E-Z, possessing binding energy of −9.96 kcal/mol along with SAXA, confirming the co-existence of both the ligands at different binding sites of the metabolizing enzyme. The synergistic interactions of GE and SAXA helped in better restoration of tissue damage and increased CYP3A11 mRNA expression levels caused by induced diabetes.

We present the case of a 36-year-old male patient with a posttraumatic composite defect of the lower two-thirds of the anterior aspect of the left leg with exposed necrotic tibia in an old, neglected type 3b fracture of the tibia of 9-month duration. The options for definitive soft-tissue cover include microvascular free tissue transfer and cross-leg flaps. In trauma cases, the surrounding tissue is usually damaged, and the recipient vessels are frequently implicated, ruling out the use of a microvascular free flap. Cross-leg flaps are unachievable due to large defect sizes and lack of appropriate donor tissue. In this case report, we highlight the use of cross-leg microvascular free anterolateral thigh (ALT) flap cover for the composite leg defect. By presenting our case, we aim to advocate the clinical application of cross-leg free flap surgery in vessel-depleted limbs as a safe and viable option.

Abstract Hydromagnetically associated heat convection can greatly enhance the performance of high-efficiency thermal appliances and renewable energy sources through an optimized design. This investigation examines the production of thermodynamic irreversibility and heat convection for a double lid-driven flow within a partially porous stratified hexagonal enclosure. The top and bottom-wall are moving in the opposite direction with an equal velocity U0. The top-wall and the bottom-wall are kept at temperature Tc and Th (Th $$>$$ > Tc) while the slanted walls are assumed to be thermally insulated. A constant magnetic field is employed in the horizontal x-direction. The hexagonal cavity was filled with a micropolar hybrid nanofluid Ag-MgO/water. The system of dimensionless equations was solved by the finite difference method (FDM) associated with successive over-relaxation (SOR), successive under-relaxation (SUR), and Gauss–Seidel iteration tactics and required results are computed with problem specific program in MATLAB code. The results indicate that the Ra and the thickness of the porous layer (Xp) significantly influences heat convection and thermal irreversibility processes. The Nuavg and STotal rises 6.299% and 3.373% as ‘ $${\upphi }_{{\text{hnf}}}$$ ϕ hnf ’ enhances from 0 to 4%, respectively. Furthermore, as the values of Ra, Ha, K0, and $${\upphi }_{{\text{hnf}}}$$ ϕ hnf increase, Beavg experiences a decline of 53.73%, 11.04%, 38.36%, and 0.09% respectively. Also, movement of wall has a significant impact on heat transfer rates and entropy production. The present study may be extended in numerous areas to mimic the problems like—(1) onset of thermo-mechanical process for solid–fluid interaction in a conduit. (2) Thermos-chemical process with extraction of ions in two-phase fluid for double layer plating on a continuously moving sheet, as region of porous stratum saturated with a class of fluid and region without porous medium occupied with other fluid.

Abstract This study presents a novel biosorbent developed by immobilizing dead Sp2b bacterial biomass into calcium alginate (CASp2b) to efficiently remove arsenic (AsIII) from contaminated water. The bacterium Sp2b was isolated from arsenic-contaminated industrial soil of Punjab, a state in India. The strain was designated Acinetobacter sp. strain Sp2b as per the 16S rDNA sequencing, GenBank accession number -OP010048.The CASp2b was used for the biosorption studies after an initial screening for the biosorption capacity of Sp2b biomass with immobilized biomass in both live and dead states. The optimum biosorption conditions were examined in batch experimentations with contact time, pH, biomass, temperature, and AsIII concentration variables. The maximum biosorption capacity (qmax = 20.1 ± 0.76 mg/g of CA Sp2b) was obtained at pH9, 35 ̊ C, 20 min contact time, and 120 rpm agitation speed. The isotherm, kinetic and thermodynamic modeling of the experimental data favored Freundlich isotherm (R2 = 0.941) and pseudo-2nd-order kinetics (R2 = 0.968) with endothermic nature (ΔH° = 27.42) and high randomness (ΔS° = 58.1).The scanning electron microscopy with energy dispersive X-ray (SEM–EDX) analysis indicated the As surface binding. The reusability study revealed the reasonable usage of beads up to 5 cycles. In conclusion, CASp2b is a promising, efficient, eco-friendly biosorbent for AsIII removal from contaminated water.

Jacob's disease is characterized by osteochondroma arising from the coronoid process of the mandible. Patients usually present with limitations of mouth opening and cheek prominence. Computed tomography of facial bones with three-dimensional reconstruction was considered the gold standard for diagnosis. Management was principally surgical which involved the removal of the coronoid process either by intraoral or extraoral approach. Postsurgical mouth-opening exercises are considered important for maintaining the obtained results.

Background: Facial dog bite injuries result in significant emotional, psychological, and physical trauma to the victims involved and should be considered a significant health issue. The purpose of this study is to share our experience in the management and to add to the existing medical literature regarding the epidemiological patterns of facial dog bite injuries.

Overall, the integration of blockchain and artificial intelligence has the potential to offer several benefits, and as these technologies continue to evolve, new and innovative applications will continue to emerge.

Antimicrobial resistance is a serious public health concern across the world. Gram-negative resistance has propagated over the globe via various methods, the most challenging of which include extended-spectrum β-lactamases, carbapenemases, and AmpC enzymes. Gram-negative bacterial infections are difficult to treat in critically extremely sick persons. Resistance to different antibiotic treatments nearly always lowers the probability of proper empirical coverage, sometimes resulting in severe outcomes. Multidrug resistance can be combated with varying degrees of success using a combination of older drugs with high toxicity levels and novel therapeutics. The current therapies for multidrug-resistant Gram-negative bacteria are discussed in this review, which includes innovative medications, older pharmaceuticals, creative combinations of the two, and therapeutic targets.

The contamination of environmental sully with antibiotics is regarded as a major problem today and predictable to attain more recognition in near future. However, human intervention resulting in antibiotic consumption is being enhancing all around the world. Our review of literature revealed the role of microbiome in sully and how antibiotic resistant genes raised. The structure of antibiotics basically influenced by natural components such as biotic and abiotic push which shifts based on different soils. Therefore, management of microbiome in soil and their expression studies were distinctively revealed. The assessment of antibiotic resistance genes with help of next generation sequencing provided a clear comprehension on genome and transcriptome of the bacterial genes. Thus, interaction of microbiome with soil can also be well understood. The current findings in our study will guide every researcher to follow logical protocol in analyzing microbiota composition is covered as well and also to understand its metagenomic and sequenced with next-generation sequencer which helps to comprehend the diverse micro-flora present in soil and its operation. Finally, later progresses in bioinformatics computer program, flow of work, and applications for analyzing metagenomic information are put in a nutshell.

Background: Burn injuries involving the dorsum of the hand are very common. Most burn injuries heal with dressings or skin grafting. Deep burns can lead to soft tissue defects and require durable cover to protect the underlying extensor tendons and the bone. Dorsal metacarpal artery-based flaps are reliable locoregional flaps that provide good soft tissue cover. Materials and Methods: This study included nine patients with postburn dorsal finger defects who underwent debridement and coverage using dorsal metacarpal artery perforator (DMAP)-based flaps from January 2019 to January 2022. Parameters in terms of flap dimensions, reliability, complications, and donor-site morbidity were assessed. Results: The flap size ranged from 5 cm × 2.5 cm to 7 cm × 2.5 cm. Two flaps had marginal necrosis, and one had venous congestion, which settled over time and healed with dressings. All the flaps settled well with a satisfactory outcome. The patients were followed up for 6–24 months. Conclusion: DMAP-based flaps are reliable in providing good soft tissue cover for reconstructing postburn dorsal finger defects with acceptable donor-site morbidity.

Diabetes is a significant public health issue. The global diabetes epidemic has had a tremendous impact on India, and the disease burden has increased dramatically. Diabetes is quickly increasing in prevalence, especially in Indian cities, according to data. Therefore, an ideal drug is sought that has better safety and tolerability and the most effective control of diabetes. Many effective medications come from plant sources. Natural products like onion and garlic can effectively control diabetes. In this review, we should pay attention to Aloe vera and its bioactive compounds, that with the development of traditional medicine, Aloe vera can be used to treat various diseases. Some reports have questioned the safety and efficacy of Aloe vera or its compounds, especially at different doses, and some studies have shown no side effects. In this review we also focus on benefits on human health so that Aloe vera is part of the daily diet in many countries and appears to be non-toxic, it is necessary to investigate whether aloe vera dietary supplement can be a beneficial preventive or nutritional mitigation strategy to reduce the effects of diabetes. This review focuses on Aloe vera and its biologically active compounds that play a role in the treatment or prevention of this morbid disease: diabetes, including its underlying mechanism of blood sugar lowering properties, and herbal products that have been marketed for the treatment of diabetes or the therapeutic effect of diabetes.

The medicinal plant C. roseus synthesizes biologically active alkaloids via the terpenoid indole alkaloid (TIAs) biosynthetic pathway. Most of these alkaloids have high therapeutic value, such as vinblastine and vincristine. Plant signaling components, plant hormones, precursors, growth hormones, prenylated proteins, and transcriptomic factors regulate the complex networks of TIA biosynthesis. For many years, researchers have been evaluating the scientific value of the TIA biosynthetic pathway and its potential in commercial applications for market opportunities. Metabolic engineering has revealed the major blocks in metabolic pathways regulated at the molecular level, unknown structures, metabolites, genes, enzyme expression, and regulatory genes. Conceptually, this information is necessary to create transgenic plants and microorganisms for the commercial production of high-value dimer alkaloids, such as vinca alkaloids, vinblastine, and vincristine In this review, we present current knowledge of the regulatory mechanisms of these components in the C. roseus TIA pathway, from genes to metabolites.

Background Burn is a leading cause of fatality in a developing country. C-reactive protein levels (CRP) and procalcitonin (PCT) can be prognostic indicators for the burn patients' mortality. Aim To assess serial levels of serum PCT and serum CRP as prognostic indicators in burns. Patient and Methods In patients admitted with burns, alternate-day serum PCT and CRP were measured from the time of admission until the time of discharge or until survival. The change in trends of CRP and PCT serum levels were studied, and it was then correlated with mortality among these burn patients. Results The first-day value of serum PCT > 1772 pg/mL and serum CRP > 71 mg/mL or any value of serum PCT > 2163 pg/mL and of serum CRP > 90 mg/L indicate a poor prognosis in burns. Conclusions The day-1 values of PCT and CRP were significantly higher in nonsurvivors than survivors in burns. The increasing trends of serum PCT and CRP levels are independent predictors of mortality in burns requiring prompt intervention. Rising PCT and CRP level denote poor prognosis in burns with an increased likelihood of death by 4.5 and 23.6 times, respectively.

Medicinal herbs that are in use for centuries to treat infections and other illnesses. Prunella vulgaris L. is traditionally used for its therapeutic attributes for the alleviation of various infectious diseases. The objective of this study on Prunella vulgaris was to reveal relevant pharmaceutical information to understand its beneficial medicinal uses for human beings. The methanolic and petroleum ether extracts after removal of the solvent under reduced pressure from the Prunella vulgaris plants were prepared and these extracts were analyzed in vitro for their activity against B. subtilis, E. coli, S. aureus and S. typhi (with ATCC numbers 6051, 25922, 23235 and 14028 respectively). Likewise, in vivo studies were conducted using the E. coli-induced peritonitis in laboratory rat models where the rats were given allopathic antibiotic ofloxacin and the results were compared with those rats who received plant extracts under controlled conditions. The results were analyzed for the efficacy of the plant extracts was compared with ofloxacin; the methanol extracts exhibited equally if not better results in clearing the pathogen from the system of animals. The petroleum ether extracts exhibited the least antimicrobial activity in comparison to those extracted in methanol isolates. In conclusion, the herbal extract demonstrated significant antibacterial activity both under in-vitro and in-vivo studies which is a major outcome of the study. During this study, all standards and norms were followed as per government animal authority.

A congenital nevi is a pigmented patch which requires complete surgical excision for cosmetic reasons. Here, we report a case of a patient with facial hairy pigmented lesion, occupying the right half of her face since birth, who underwent complete surgical excision and staged reconstruction utilizing, preexpanded forehead and neck skin. We used two rectangular tissue expanders with 150 and 300 cubic cm of volumes inserted in the forehead and the neck, respectively. The length of the expanders selected were equal to 1.2 to 1.5 times the length of their respective lesions, whereas the width of the base of the expanders were approximately similar to the width of their defects. It is concluded with this case report that tissue expansion provides a good cosmetic and anatomical correction to cover large defects, with adjacent skin having similar properties.

Ionizing radiation exerts various biological effects, including cell killing and carcinogenesis, mainly through generating damage on DNA. Among various types of DNA damage, DNA double-strand break (DSB) is considered the most deleterious and most intimately related to biolog?ical effects of radiation. DNA-dependent protein kinase (DNA-PK), consisting of DNA-PK catalytic subunit and Ku80-Ku70 heterodimer (Ku), is activated upon binding to the end of double-stranded DNA and acts as the molecular sensor for DSB. While DSB is repaired mainly through homologous recombination and nonhomologous end joining in eukaryotes, DNA-PK is shown to be essential in the latter pathway. Moreover, DNA-PK is reported to be capable of phosphorylating a number of proteins, suggesting versatile functions of DNA-PK in cellular response to DSB. Here, we review the advance in our understanding on DNA-PK in three decades and remaining problems.

Background: The estimated incidence of spina bifida is 1–2 cases per 1000 population. In earlier literature, the global prevalence of meningomyelocele (MMC) is reported as 0.8–1.0 per 1000 live births. This retrospective study analyses the outcome of various surgical procedures performed for the closure of MMC defects. Method: A total of 22 patients with MMC defects who underwent repair at our institute from July 2016 to August 2018 were included in the study. A retrospective review of all the cases operated was completed to analyse patient demography including defect size, defect location, surgical procedures, complications and the final outcome. Results: Out of 22 cases, wherein the neurosurgery department sought help from the plastic surgery department, 11 defects were closed using the Limberg flap technique, 4 defects were closed with either primary closure or the double flap rotation flaps, one defect was closed using the triple rotation flap and 2 defects were closed using the local transposition flap cover technique. Complications were noted in only three cases. One patient had a local wound infection, while in two other cases, wound dehiscence was observed. All 3 cases were managed conservatively. On average, it takes approximately 70 days in India to close such defects. Conclusion: MMC defects can be effectively managed with local flap options such as Limberg flap, local transposition flap or rotation flaps. Various reasons for the delay in closure were reported in patients late to our centre, when the first point of contact was with other departments. The hemodynamics provides a way to predict effect of atherosclerosis by means of mathematical models. The pulsatile flow of blood through an artery with two side-to-side axisymmetric stenoses has been considered. A static transverse magnetic field to the flow is taken into account. The velocity profile, Wall Shear Stress and Wall Shear Stress Gradient to the flow have been simulated under the influence of magnetic field for various values of length and thickness of the stenosis. The upstream flow velocity in the subsequent stenosis region is significantly lower down from the velocity in the preceding stenosis region. The flow velocity decreases with the increase of Hartmann number. In the stenosis region wall shear stress (WSS) increases from unstenosed region to maximum thickness of stenosis. The wall shear stress (WSS) increases with the increase of Hartmann number and Womersley number. The WSSG have local maximum value in the vicinity of the throat of the stenoses and oscillates in the stenosed portion of the artery. The magnitude of WSSG is directly proportional to the Hartmann number. WSSG increases in magnitude on the upstream and downstream section of both the stenoses with the increase of Womersley number. Generated data are analyzed and discussed through graphs.

The main aim of the relativistic heavy-ion experiment is to create extremely hot and dense matter and study the QCD phase structure. With this motivation, experimental program started in the early 1990s at the Brookhaven Alternating Gradient Synchrotron (AGS) and the CERN Super Proton Synchrotron (SPS) followed by Relativistic Heavy Ion Collider (RHIC) at Brookhaven and recently at Large Hadron Collider (LHC) at CERN. These experiments allowed us to study the QCD matter from center-of-mass energies (sNN) 4.75 GeV to 2.76 TeV. The ϕ meson, due to its unique properties, is considered as a good probe to study the QCD matter created in relativistic collisions. In this paper we present a review on the measurements of ϕ meson production in heavy-ion experiments. Mainly, we discuss the energy dependence of ϕ meson invariant yield and the production mechanism, strangeness enhancement, parton energy loss, and partonic collectivity in nucleus-nucleus collisions. Effect of later stage hadronic rescattering on elliptic flow (v2) of proton is also discussed relative to corresponding effect on ϕ meson v2.
