1. Tropical cyclogenesis in warm climates simulated by a cloud-system resolving model

Abstract

Here we investigate tropical cyclogenesis in warm climates, focusing on the effect of reduced equator-to-pole temperature gradient relevant to past equable climates and, potentially, to future climate change. Using a cloud-system resolving model that explicitly represents moist convection, we conduct idealized experiments on a zonally periodic equatorial β-plane stretching from nearly pole-to-pole and covering roughly one-fifth of Earth’s circumference. To improve the representation of tropical cyclogenesis and mean climate at a horizontal resolution that would otherwise be too coarse for a cloud-system resolving model (15 km), we use the hypohydrostatic rescaling of the equations of motion, also called reduced acceleration in the vertical. The simulations simultaneously represent the Hadley circulation and the intertropical convergence zone, baroclinic waves in mid-latitudes, and a realistic distribution of tropical cyclones (TCs), all without use of a convective parameterization. Using this model, we study the dependence of TCs on the meridional sea surface temperature gradient. When this gradient is significantly reduced, we find a substantial increase in the number of TCs, including a several-fold increase in the strongest storms of Saffir–Simpson categories 4 and 5. This increase occurs as the mid-latitudes become a new active region of TC formation and growth. When the climate warms we also see convergence between the physical properties and genesis locations of tropical and warm-core extra-tropical cyclones. While end-members of these types of storms remain very distinct, a large distribution of cyclones forming in the subtropics and mid-latitudes share properties of the two.

4. Convective self-aggregation and tropical cyclogenesis under the hypohydrostatic rescaling

Abstract

The behavior of rotating and nonrotating aggregated convection is examined at various horizontal resolutions using the hypohydrostatic, or reduced acceleration in the vertical (RAVE), rescaling. This modification of the equations of motion reduces the scale separation between convective- and larger-scale motions, enabling the simultaneous and explicit representation of both types of flow in a single model without convective parameterization. Without the RAVE rescaling, a dry bias develops when simulations of nonrotating radiative-convective equilibrium are integrated at coarse resolution in domains large enough to permit convective self-aggregation. The rescaling reduces this dry bias, and here it is suggested that the rescaling moistens the troposphere by weakening the amplitude and slowing the group velocity of gravity waves, thus reducing the subsidence drying around aggregated convection. Separate simulations of rotating radiative-convective equilibrium exhibit tropical cyclogenesis; as horizontal resolution is coarsened without the rescaling, the resulting storms intensify more slowly and achieve lower peak intensities. At a given horizontal resolution, using RAVE increases peak storm intensity and reduces the time needed for tropical cyclogenesis-effects here suggested to be caused at least in part by the environmental moistening produced by RAVE. Consequently, the RAVE rescaling has the potential to improve simulations of tropical cyclones and other aggregated convection in models with horizontal resolutions of order 10-100 km.

8. Student teachers’ beliefs about diversity: analysing the impact of a ‘diversity week’ during initial teacher education

Abstract

This article reports findings from a week of enrichment placements framed around ‘diversity’ within a secondary Initial Teacher Education (ITE) programme in England. The authors outline the demographics of the county – a largely rural, White county in the East Midlands of England – and describe the challenges this presents for ITE. A mixed-methods approach was used to study student teachers’ (n = 56) beliefs about diversity, generating data through: pre- and post-survey of beliefs and attitudes; student-created reflective videos; journaling; and one pre- and post-diversity week interview. The findings reveal shifts in student teachers’ perceptions about gender, race and sexuality, and these attitudinal shifts were more significant in those attending all week than those attending only the first day. This is particularly interesting because for some topics the only formal input was on the first day, and so the authors argue for the importance of time and space for creative reflection in beginning teachers’ professional development.

Abstract

As of the academic year 2017/2018, autism education is a required element in Initial Teacher Education (ITE). This is, of course, excellent news for all of us trying to increase understanding of the perplexing and fascinating conundrum that is autism. \ud \ud Unfortunately, ITE is very squeezed. There are many, many\ud elements to fit into the course, and in this context, elements of autism education may be very restricted.\ud \ud But what if that were not the case? What if we could take the time to educate our new teachers fully in the many ways that autism will affect our pupils? In this context, what would we want our new teachers to learn and think about?\ud \ud This book attempts to answer these questions. It aims to be a cornucopia of interesting angles on autism and how autism might be considered in schools and classrooms. Each chapter contains the\ud material for a session to be delivered to trainee teachers (or to currently working teachers through continuing professional development (CPD)), or to provide a unit of self-study. Each chapter challenges us to reconsider autism within different educational contexts. Many of the chapters contain tasks and points for discussion. Each of the subjects discussed in these chapters has wider educational implications, and each is valid ITE in its own right. \ud \ud It is hoped that, with these subjects available to teacher educators, they may begin to become core content in teacher education as we begin to incorporate ‘the autism angle’ more centrally\ud within the wider education debate.

24. HIV-1 remission following CCR5 Delta 32/Delta 32 haematopoietic stem-cell transplantation

Abstract

A cure for HIV-1 remains unattainable as only one case has been reported, a decade ago(1,2). The individual-who is known as the 'Berlin patient'-underwent two allogeneic haematopoietic stem-cell transplantation (HSCT) procedures using a donor with a homozygous mutation in the HIV coreceptor CCR5 (CCR5 Delta 32/Delta 32) to treat his acute myeloid leukaemia. Total body irradiation was given with each HSCT. Notably, it is unclear which treatment or patient parameters contributed to this case of long-term HIV remission. Here we show that HIV-1 remission may be possible with a less aggressive and toxic approach. An adult infected with HIV-1 underwent allogeneic HSCT for Hodgkin's lymphoma using cells from a CCR5 Delta 32/Delta 32 donor. He experienced mild gut graft-versus-host disease. Antiretroviral therapy was interrupted 16 months after transplantation. HIV-1 remission has been maintained over a further 18 months. Plasma HIV-1 RNA has been undetectable at less than one copy per millilitre along with undetectable HIV-1 DNA in peripheral CD4 T lymphocytes. Quantitative viral outgrowth assays from peripheral CD4 T lymphocytes show no reactivatable virus using a total of 24 million resting CD4 T cells. CCR5-tropic, but not CXCR4-tropic, viruses were identified in HIV-1 DNA from CD4 T cells of the patient before the transplant. CD4 T cells isolated from peripheral blood after transplantation did not express CCR5 and were susceptible only to CXCR4-tropic virus ex vivo. HIV-1 Gag-specific CD4 and CD8 T cell responses were lost after transplantation, whereas cytomegalovirus-specific responses were detectable. Similarly, HIV-1-specific antibodies and avidities fell to levels comparable to those in the Berlin patient following transplantation. Although at 18 months after the interruption of treatment it is premature to conclude that this patient has been cured, these data suggest that a single allogeneic HSCT with homozygous CCR5 Delta 32 donor cells may be sufficient to achieve HIV-1 remission with reduced intensity conditioning and no irradiation, and the findings provide further support for the development of HIV-1 remission strategies based on preventing CCR5 expression.

Background: Human memory B cells play a vital role in the long-term protection of the host from pathogenic re-challenge. In recent years the importance of a number of different memory B cell subsets that can be formed in response to vaccination or infection has started to become clear. To study memory B cell responses, cells can be cultured ex vivo, allowing for an increase in cell number and activation of these quiescent cells, providing sufficient quantities of each memory subset to enable full investigation of functionality. However, despite numerous papers being published demonstrating bulk memory B cell culture, we could find no literature on optimised conditions for the study of memory B cell subsets, such as IgM+ memory B cells. Methods: Following a literature review, we carried out a large screen of memory B cell expansion conditions to identify the combination that induced the highest levels of memory B cell expansion. We subsequently used a novel Design of Experiments approach to finely tune the optimal memory B cell expansion and differentiation conditions for human memory B cell subsets. Finally, we characterised the resultant memory B cell subpopulations by IgH sequencing and flow cytometry. Results: The application of specific optimised conditions induce multiple rounds of memory B cell proliferation equally across Ig isotypes, differentiation of memory B cells to antibody secreting cells, and importantly do not alter the Ig genotype of the stimulated cells. Conclusions: Overall, our data identify a memory B cell culture system that offers a robust platform for investigating the functionality of rare memory B cell subsets to infection and/or vaccination.

Abstract

Objective: Self-perceived health status may be helpful in identifying patients at high risk for adverse outcomes. The Euro Heart Survey on Coronary Revascularization (EHS-CR) provided an opportunity to explore whether impaired health status was a predictor of 1-year mortality in patients with coronary artery disease (CAD) undergoing angiographic procedures. Methods: Data from the EHS-CR that included 5619 patients from 31 member countries of the European Society of Cardiology were used. Inclusion criteria for the current study were completion of a self-report measure of health status, the EuroQol Questionnaire (EQ-5D) at discharge and information on 1-year follow-up, resulting in a study population of 3786 patients. Abstract

Aims: Revascularization in patients with coronary artery disease changed over the last two decades, favouring the number of patients treated by means of percutaneous coronary interventions (PCI) when compared with coronary artery bypass grafting (CABG). Many randomized controlled trials (RCTs) have been performed to compare these two competing revascularization techniques. Because of the strict enrolment criteria of RCTs in which highly selected patients are recruited, the applicability of the results may be limited in clinical practice. The current study evaluates to what extent patients in clinical practice were similar to those who participated in RCTs comparing PCI with CABG. Methods and results: Clinical characteristics and 1-year outcome of 4713 patients enrolled in the Euro Heart Survey on Coronary Revascularization were compared with 8647 patients who participated in 14 major RCTs, comparing PCI with CABG. In addition, we analysed which proportion of survey patients would have disqualified for trial participation (n = 3033, 64%), aiming at identifying differences between trial-eligible and trial-ineligible survey patients. In general, important differences were observed between trial participants and survey patients. Patients in clinical practice were older, more often had comorbid conditions, single-vessel disease, and left main stem stenosis when compared with trial participants. Almost identical differences were observed between trial-eligible and trial-ineligible survey patients. In clinical practice, PCI was the treatment of choice, even in patients who were trial-ineligible (46% PCI, 26% CABG, 28% medical). PCI remained the preferred treatment option in patients with multi-vessel disease (57% in trial-eligible and 40% in trial-ineligible patients, respectively, P < 0.001); yet, the risk profile of patients treated by PCI was better than that for patients treated either by CABG or by medical therapy. In the RCTs, there was no mortality difference between PCI and CABG. In clinical practice, however, we observed 1-year unadjusted survival benefit for PCI vs. CABG (2.9 vs. 5.4%, P < 0.001). Survival benefit was only observed in trial-ineligible patients (3.3 vs. 6.2%, P < 0.001). Conclusion: Many patients in clinical practice were not represented in RCTs. Moreover, only 36% of these patients were considered eligible for participating in a trial comparing PCI with CABG. We demonstrated that RCTs included younger patients with a better cardiovascular risk profile when compared with patients in everyday clinical practice. This study highlights the disparity between patients in clinical practice and patients in whom the studies that provide the evidence for treatment guidelines are performed. © The European Society of Cardiology 2006. © 2016 Japan Atherosclerosis Society.Familial hypercholesterolemia (FH) is the most common and serious form of inherited hyperlipidaemia. Dominantly inherited with high penetrance, untreated FH leads to premature death from coronary artery disease due to accelerated atherosclerosis from birth. Despite its importance, there is still a major shortfall in awareness, detection and treatment of FH worldwide. International models of care for FH have recently been published, but their effective implementation requires the garnering of more knowledge about the condition. The “Ten Countries Study” aims to investigate diagnostic, epidemiological and service aspects, as well as physician practices and patient experiences of FH in several countries in the Asia-Pacific Region and the Southern Hemisphere. Five observational studies are being undertaken that will systematically investigate the following aspects of FH: the phenotypic predictors of low-density lipoprotein receptor mutations, the point prevalence in available community populations, current knowledge and clinical practices among primary care physicians, availability and utilisation of services and facilities, and patient perceptions and personal experiences of the condition. The information gathered will inform better clinical practice and will enable the development of country-specific models of care for FH.

The ubiquitous SAR11 bacterial clade is the most abundant type of organism in the world's oceans, but the reasons for its success are not fully elucidated. We analysed 128 surface marine metagenomes, including 37 new Antarctic metagenomes. The large size of the dataset enabled internal transcribed spacer (ITS) regions to be obtained from the Southern polar region, enabling the first global characterisation of the distribution of SAR11, from waters spanning temperatures -2ºC to 30ºC. Abstract

Introduction: Developmental Dysplasia of the Hip (DDH) is the most common notifiable musculoskeletal birth defect in South Australia (SA). Despite routine screening by physical examination of the hips in the neonatal period and at 6 weeks of age, the risk of late diagnosis is increased in rural areas. It is assumed this is due to the examining doctors' reduced clinical expertise. Introducing Anterior Dynamic Ultrasound (ADUS) has reduced the late detection rates in Sweden to almost zero, and may benefit Australian infants in rural areas if routine screening was introduced. This study reports on a small implementation pilot in a SA regional hospital where volunteer postnatal mothers consented to their babies having ADUS examinations. Methods: The pilot was evaluated by collecting results of physical examination, ADUS, and surveying parental impressions of the screening test. Results: Hips of 86 infants underwent ADUS during the implementation pilot. Parents' perceptions were mainly very positive and indicated ADUS was an accessible and acceptable screening test. Of the hips scanned, three were found to have maximum movement of the femoral head of >3 mm and were deemed to demonstrate increased laxity. Four hips described as loose or mobile on clinical examination were found to be within normal limits of maximum mobility on ADUS. Conclusions: This study has demonstrated that a larger scale implementation project would be feasible in regional Australia, and would enable researchers to better understand how to reduce the late diagnosis rate of DDH in rural areas.

In particular, we explore whether the SST response is interhemispheric in nature, specifically as reflected in the Atlantic SST Dipole index, or whether the response is localized more in the North Atlantic Ocean. In the absence of direct observational data, this Dipole index has been proposed to approximate AMOC variations over the duration of the instrumental temperature record. We find that typically, on timescales between decadal and centennial, the SST Dipole index correlates with the AMOC with coefficients ranging from 0.2 to 0.7, typically with a 0-6 year lag, and thus explains less than half of the AMOC variance. In just two models this value slightly exceeds 50 %. Even for the models with the highest correspondence between the AMOC and the Dipole index, the correlation between the two variables is controlled mainly by SST variations in the North Atlantic, not the South Atlantic, both for the model control and historical simulations. Consequently, in nearly all models, the North Atlantic SST provides a better indicator of AMOC variations than the Atlantic Dipole. Thus, on decadal to centennial timescales AMOC variability affects mainly the North Atlantic Ocean, with the sensitivity of the North Atlantic SST between 40 and 60°N, given by the multi-model average, of about 0.3 °C per 1 Sv of AMOC change, explaining roughly one third of the SST variance.
