El papel de la medicina forense en la investigación de la muerte súbita es crucial, ya que desde el punto de vista jurídico, como en su forma de presentación, así como el desconocimiento de la existencia de una patología natural o, en su caso, la falta de testigos, hacen que se trate de una muerte “sospechosa”. De ahí que sea el patólogo forense el responsable de determinar su causa precisa. La autopsia puede representar en estos casos la primera y última oportunidad para hacer el diagnóstico correcto de la causa de la muerte, lo cual es de suma relevancia, especialmente para los familiares. A pesar de la relevancia de su estudio, aun se necesita realizar mas investigación, para su mejor comprensión; especialmente la relacionado a la Muerte Súbita no cardiovascular.

Ya que no fue la inmortalidad un don concedido a los hombres, claramente la muerte es un hecho ineludible. Sin embargo, a pesar de su carácter ineludible; la muerte súbita de un ser amado representa una pérdida irreparable, especialmente porque deja en los sobrevivientes profundos sentimientos de dolor, incertidumbre, temor y desesperanza frente a las interrogantes que se plantean, ante tal evento.

Introducción: La SUDEP, es la complicación más temida de la epilepsia y se reconoce como una de las principales causas de muerte prematura en adultos jóvenes con epilepsia; sin embargo, aún es poco reconocida, informada y escasamente investigada por los profesionales de la salud, lo que ha condicionado al escaso progreso en su detección y prevención. Metodología: Se realizó una revisión narrativa mediante la búsqueda de artículos, en Pub med, Google académico y Lilacs; utilizando las palabras clave en inglés y español: SUDEP, epilepsia, autopsia, autopsia molecular y sus combinaciones. Además, se analizaron cinco expedientes de la base de datos de la Dirección de Medicina Forense de Tegucigalpa, consignados como SUDEP. Resultados: De los cinco casos analizados, cuatro (80%) eran mujeres y uno hombre, cuatro (80%) no fueron presenciados y ocurrieron durante el sueño. El 80 % (4) de los casos eran menores de 40 años, los hallazgos de autopsia tanto macroscópicos, como microscópicos son similares a los reportado por otros autores. En el análisis de 27 artículos se encontró que el diagnostico de la SUDEP, aun representa un desafío. Conclusión: Pese a que hay avances significativos en la comprensión de los mecanismos y factores de riesgo que contribuyen a la SUDEP, aún es necesario realizar más investigación.

Introducción: la muerte súbita se trata de un evento fatal e imprevisible. Realizada la autopsia y estudios complementarios, en ausencia de otros hallazgos que expliquen la causa de muerte, se clasifica como muerte súbita inexplicada. Siendo recomendable en estos casos realizar análisis genéticos, especialmente con metodologías de secuenciación de siguiente generación, las que permiten explicar un porcentaje importante de estos casos. Objetivo: analizar las publicaciones más relevantes sobre secuenciación de siguiente generación, aplicada a la autopsia molecular, para determinar aquellas muertes súbitas inexplicables relacionadas a cardiomiopatías y canalopatías. Metodología: se realizó la búsqueda en PubMed del Instituto Nacional de Salud usando palabras clave en inglés y español: NGS, muerte súbita, autopsia molecular y sus combinaciones. Además, se realizaron búsquedas en OMIN y ClinVar relacionada a las diferentes afecciones cardiacas relacionadas a muerte súbita. Los criterios de inclusión: artículos completos en español e inglés de libre acceso, con antigüedad máxima de diez años, realizados en cualquier área geográfica y que trataran sobre la temática. Resultados: para secuenciación de siguiente generación, muerte súbita se encontraron más de 22000 y 65000 publicaciones, respectivamente. En cambio, al combinar las palabras clave se recuperaron 74 trabajos, según los criterios de inclusión y objetivo del trabajo se revisaron 67 artículos. La aplicación de las plataformas de secuenciación en la investigación de casos de muerte súbita tomo auge en el 2014 y en poco tiempo, demostraron su versatilidad para el análisis de una gran cantidad de genes simultáneamente, de forma rápida y a bajo costo. Conclusiones: las patologías asociadas a muerte súbita son múltiples, complejas y pueden generar fenotipos variables que dificultan el análisis genético de las mismas. Las plataformas de secuenciación de siguiente generación son sumamente útiles en los casos de muerte súbita inexplicada, además permiten la identificación de variantes genéticas en familiares para la implementación de medidas preventivas.

La hemorragia aguda por rotura de várices esofágicas es una de las complicaciones más graves de la cirrosis. Las varices esofágicas son venas anormalmente dilatadas que se encuentran en la parte final del esófago, por las que circula parte de la sangre que en condiciones normales debería pasar a través del hígado. Su dilatación es consecuencia de la dificultad que el hígado enfermo ofrece al paso de la sangre.

Justificación: La muerte súbita constituye un problema de salud pública a nivel global. Su presentación súbita e inesperada tiene implicaciones médico-legales por lo que su abordaje diagnóstico en el ámbito forense es de gran importancia. Objetivo: Analizar la muerte súbita y su abordaje diagnóstico en la Dirección de Medicina Forense de Honduras. Metodología: Estudio descriptivo, retrospectivo de autopsias por muerte súbita en población de 10 a 55 años, realizadas en la sede de Tegucigalpa durante un período de cinco años (2015-2019). Los cálculos estadísticos fueron realizados con el programa SPSS. Resultados: Se analizaron 165 casos de muerte súbita, 56% fueron de origen no cardiovascular y 44% de origen cardiovascular, 67% varones, con una media de edad de 39 años, DS+/-9,75. Las causas más frecuentes de muerte súbita no cardiovascular fueron digestivas (36%), respiratorias (34%), eventos cerebrovasculares hemorrágicos (14%) y muerte súbita en epilepsia (9%). Entre las muertes súbitas cardiovasculares las causas más frecuentes fueron la enfermedad coronaria aterosclerótica (67%), miocarditis (12%) y la muerte arrítmica con corazón estructuralmente normal (10%). Desde el punto de vista toxicológico destaca en ambas la asociación con el consumo de etanol (19%). Conclusiones: La muerte súbita de origen no cardiovascular fue la más frecuente, resultado condicionado por las limitantes del estudio. El abordaje actual de los casos de muerte súbita presenta importantes limitaciones por lo que se recomienda fortalecerlo implementando mejoras en la investigación medicolegal de la escena de la muerte, de los antecedentes familiares y el uso adecuado de los estudios complementarios toxicológicos e histopatológicos.

The definition of mitral annular disjunction (MAD) is still controversial. In the different areas of research, anatomical, clinical, and imaging studies, the separation of the junction points between the mitral valve annulus and the left ventricular wall (greater than 5 mm) has been proposed as a definition, which has been observed in patients with mitral valve prolapse, ventricular arrhythmias, and sudden death. Many of them presented myxomatous degeneration of the valve, a pathology that generates a weakening of the tissue and changes in its normal function. Changes in the geometry of the left ventricle are accompanied by changes in its function, modifying the valve apparatus and generating abnormal dynamics of the mitral annulus. Given the proximity to the anatomical structures of the conduction system, it is likely that these changes have an impact on the it, generating alterations such as arrhythmias, related to sudden death. Material and methods: 30 human hearts preserved with the Kaiserling technique were used. The dissections were performed with microsurgery and standard cardiovascular surgery forceps and scissors with the naked eye, with magnifying glasses and with an optical microscope. A cut was made at the level of the posterior leaflet of the mitral valve, between both papillary muscles, with part of the left atrium and ventricle, to assess whether there is disjunction or not. Results: We have managed to identify MAD in approximately 10% of cases. Conclusions: It is proposed to establish the measurement of MAD parameters in routine controls to perform cardiological follow-up of those patients who present them, their early detection may contribute to the prevention, monitoring and treatment of the aforementioned pathologies. Exercise improves quality of life and life expectancy, and its benefits outweigh its associated risks. However, athletes are at risk for sudden cardiac death, which occurs more frequently in them than in sedentary people, secondary to silent heart disease. Preparticipation evaluation varies by country and is a challenge for physicians in deciding whether to clear an individual for exercise or sports. Resistance training carries additional risks secondary to the cardiovascular adaptations it causes, such as cardiac remodeling, which may become pathological and increase the risk of arrhythmias, thus leading to sudden cardiac death. The main objective of this article is to propose a preparticipation evaluation in different age groups, based on current evidence in the literature for detecting and preventing cardiovascular diseases which can lead to sudden cardiac death in an athlete during sports practice. Introduction: This article presents data on implantable cardioverter-defibrillator implants in Spain in 2023., Methods: The registry is based on information provided by centers following device implantation, which is submitted to the Heart Rhythm Association of the Spanish Society of Cardiology via the national online registry platform (Cardiodispositivos). Additional information sources include: a) data transfer from the manufacturing and marketing industry; and b) local databases sent from the implanting centers. Population data from the National Institute of Statistics for the first quarter of 2024 was used to calculate implant rates., Results: In 2023, 180 hospitals participated in the registry. Data were reported for 8219 units, compared with 8523 reported by Eucomed (European Confederation of Medical Suppliers Associations). The total implant rate was 172 implants per million inhabitants (177 according to Eucomed), representing an increase compared with previous years. However, differences among autonomous communities persisted, and Spain continues to have the lowest implant rate among the European countries participating in Eucomed., Conclusions: The data from the 2023 registry reflects 96.4% of the implants performed in Spain. Despite the improvement observed in the implantation rate, Spain's position in Europe remains unchanged, with wide disparities among autonomous communities. La taquicardia ventricular polimórfica catecolaminérgica es una de las canalopatías más letales. Los síntomas de la enfermedad aparecen en la niñez o la adolescencia, los cuales están caracterizados por arritmias ventriculares desencadenadas por estrés o actividad física. Se presenta el caso de una adolescente que consultó por síncopes recurrentes precipitados por el ejercicio. En el abordaje diagnóstico se determinó como taquicardia ventricular polimórfica catecolaminérgica, con mutación en el gen del receptor de la rianodina cardíaco, heterocigoto c.14311G>A(p.v4771I exón 100), para el manejo fue necesario antiarrítmicos y el implante de un cardiodesfibrilador, con evolución satisfactoria. La sospecha clínica, la prueba de esfuerzo y las pruebas genéticas son fundamentales para un diagnóstico y manejo oportuno de esta patología.

Objetives: To assess if patients with Chagasic heart disease (CHD) received effective automatic implantable defibrillator (AID) shocks earlier than patients with ischemic heart disease (IHD). Methods: Retrospective cohort of patients with CHD and IHD who received an implantable cardioverter defibrillator (ICD) between 2009 and 2018, in a tertiary hospital. We evaluated the time between the implant of ICD and the first effective shock in patients with CHD and compared it with the IHD control population. Results: We included a total of 64 patients, 20 with CHD and 44 with IHD. CHD patients presented earlier an effective shock than patients with IHD during the first year (hazard ratio [HR]: 8.4; 95% confidence interval [95%CI]: 2.09-34.02; p = 0.0027), and at three years (HR: 4.61; 95%CI: 1.51-14.07; p = 0.0072). 100% of CHD patients who received the ICD as secondary prevention of sudden cardiac death presented an effective shock during the first 26 months of follow-up. Conclusions: Patients with CHD received effective ICD shocks earlier than the IHD patients. All patients with CHD and ICD as secondary prevention had an appropriate ICD shock at short term, representing the highest risk population, and supporting the indication of the device in a setting where randomized clinical trials are lacking. Introducción: Las enfermedades cardiovasculares han provocado el incremento de arritmias en la población, aumentando el uso de dispositivos como el cardiodesfibrilador, que, si bien previene las arritmias, puede afectar la calidad de vida. Objetivo: Describir la asociación que existe entre la calidad de vida relacionada con la salud, la ansiedad y la depresión en personas portadoras de un cardiodesfibrilador. Materiales y métodos: Estudio cuantitativo, descriptivo correlacional, de corte transversal, donde participaron 85 pacientes con implante de cardiodesfibrilador. Se aplicó una ficha sociodemográfica, los instrumentos Cuestionario de Salud (SF-36), el Inventario de Ansiedad y Depresión de Beck. Resultados: El 81,2% de los participantes fueron hombres con rango de edad 36-92 años. El síndrome QT largo (SQTL) congénito representa un grupo de enfermedades cardiacas de origen genético, caracterizado por la prolongación del intervalo QT y una onda T anormal en el electrocardiograma (ECG). Pueden tener una expresión dominante o recesiva, esta última asociada con sordera neurosensorial. En ambos casos su presentación clínica está asociada con síncopes recurrentes y muerte súbita como consecuencia de una taquicardia ventricular, específicamente a torsades de pointes. Actualmente se clasifican en función del defecto genético específico, pudiendo comprometer alrededor de 16 genes y casi 2000 mutaciones. Debe ser sospechada en individuos con la clínica relacionada, hallazgos electrocardiográficos y antecedentes familiares. El manejo está basado en la disminución o eliminación de los síntomas y, concomitantemente, la prevención de la muerte súbita (MS), en aquellos niños con sordera congénita el manejo requiere la aplicación de medidas propias del otorrinolaringólogo. Desde el punto de vista cardiovascular, el manejo implica la modificación de estilos de vida, principalmente la prohibición de deportes competitivos, entre ellos la natación, evitar la exposición a sonidos intensos o factores desencadenantes. La medicación usada abarca a los betabloqueadores y, más raramente, flecainida, ranozalina y verapamilo; el manejo invasivo consiste el implante de un cardiodesfibrilador o, incluso, la denervación simpática izquierda, cada una de ellas con sus propios riesgos y beneficios. En cualquiera de los casos debemos evitar las circunstancias que incrementen el intervalo QT, así como realizar el adecuado análisis de los beneficios y riesgos de cada posible medida invasiva.

Objetivo. Determinar las características epidemiológicas, clínicas, electrocardiográficas, imagenológicas y principales estrategias terapéuticas realizadas en los pacientes con cardiomiopatía arritmogénica tratados en un instituto cardiovascular de referencia nacional. Materiales y métodos. Estudio observacional, descriptivo y retrospectivo que busca identificar las características clínicas, exámenes complementarios y estrategias terapéuticas en pacientes con cardiomiopatía arritmogénica tratados en el Instituto Nacional Cardiovascular – INCOR EsSalud en Lima – Perú. Resultados. Se encontraron trece pacientes con cardiomiopatía arritmogénica. La mediana de edad en la que se realizó el diagnóstico fue 38,2 años y el 69,3% de los afectados era de sexo masculino. Las manifestaciones clínicas más frecuentes fueron las palpitaciones taquicárdicas (92,3%), el presíncope (84,6%) y la falla cardiaca (69,2%). El 23% de los pacientes sufrió un arresto cardiaco. Todos presentaron, al menos, un episodio de taquicardia ventricular, el 92,3% con morfología de bloqueo completo de rama izquierda y eje superior. Al 76,9% se le colocó un desfibrilador automático implantable (DAI), al 15,3% se le realizó ablación y al 15,3% un trasplante cardiaco. El 84,6% de los pacientes vive hasta la actualidad. Conclusiones. La cardiomiopatía arritmogénica afectó predominantemente a población joven y de sexo masculino. Todos los pacientes tuvieron una arritmia ventricular potencialmente fatal. La afección biventricular por ecocardiografía y por resonancia magnética cardiaca se evidenció en el 69,2 y 100% de los casos, respectivamente. Las estrategias terapéuticas empleadas fueron el tratamiento médico antiarrítmico, colocación de un DAI como prevención secundaria, la ablación y el trasplante cardíaco. Hasta la actualidad, el 84,6% de los pacientes sobrevive.

El riesgo de muerte súbita en la cardiomiopatía hipertrófica está relacionado a la presencia de arritmias ventriculares en la gran mayoría de los casos. Es y será un reto para la Cardiología moderna encontrar los mejores esquemas para valorar la probabilidad de complicaciones arrítmicas, siendo por ahora el enfoque multifactorial la mejor estrategia para evitar el implante innecesario de dispositivos como los cardiodesfibriladores. Si bien, el electrocardiograma (ECG) sigue siendo una excelente herramienta para el diagnóstico, incluso en fases sin expresión ecocardiográfica, no tiene un rol claro en la estratificación de riesgo. Sin embargo, la asociación de cambios en el ECG y otros hallazgos como la presencia de preexcitación y/o QT largo, así como variantes de presentación tipo cardiomiopatía hipertrófica apical, permitirían identificar a pacientes con alta probabilidad de muerte súbita. Desde hace unos años la resonancia cardíaca y la cuantificación de la fibrosis intramiocárdica (mecanismo básico de las arritmias ventriculares) tienen un rol cada vez más importante en la evaluación de estos pacientes. En especial, la población pediátrica debe tener una visión individualizada por el mal pronóstico a edades tempranas y por el rol incierto de diferentes herramientas para la valoración del riesgo y tratamiento

Disjunction of the mitral valve annulus is a structural abnormality consisting of an atrial displacement of the articulation point of the mitral valve, leading to an altered spatial relationship between the valve and the adjacent posterior ventricular wall. Studies have shown the relationship between mitral annular disjunction, ventricular arrhythmias, and myocardial fibrous degeneration, which increases the risk of sudden death, especially among young women. The case of a 30-year-old woman with no relevant pathological history with frequent palpitations is presented, in whom an increase in ventricular automatism with a progressive increase in the arrhythmic load of up to 20% with a weight of pharmacological management is documented. In a cardiac magnetic resonance, a 15 mm displacement towards the atrium of the mitral leaflets compatible with mitral annular disjunction and late enhancement foci of mesocardial, non-coronary distribution were observed. Given the findings and the persistence of symptoms, an electrophysiological study, 3D mapping, and ablation were performed. Two hyperechoic regions were seen on intracardiac ultrasound, one in the length of the anterior papillary muscle and the other in the mesocardium of the base of the posterior papillary muscle. Both foci related to the morphological sites of interest in which radiofrequency energy was applied. During the procedure presented an episode of ventricular fibrillation. A malignant ventricular extrasystole was considered and therefore a cardio defibrillator was implanted for the secondary prevention of sudden death. The literature was reviewed and the specific pathophysiological relationships between mitral annular disjunction, premature ventricular complexes, and risk of sudden death were analyzed. La Cardiomiopatía Hipertrófica es una patología en la que se caracteriza por presentar una hipertrofia septal, que a su vez, genera una obstrucción del flujo de salida del ventrículo izquierdo. Sin embargo, hay 2 formas de presentación: Una obstructiva, de la cual, es la de mal pronóstico; y la de tipo No Obstructiva, la cual, generalmente es de buen pronóstico. Puede esta patología ser hereditaria o deberse a una mutación. Afecta más a mujeres y a jóvenes. Se suele manifestar comunmente con disnea, fatiga, pressíncope, síncope, fibrilación auricular y progresar incluso a Insuficiencia Cardiaca. Hay diversas formas de diagnóstico, que incluyen como más importante: Electrocardiograma, Ecocardiograma, Eco-doppler, y Resonancia Magnética, que es el gold standard. Para el tratamiento, se suele usar betabloqueadores o bloqueadores de calcio; sin embargo, si es refractario, se suele recurrir a terapia de reducción septal o ablación septal con alcohol.

Introduction: Approved drugs for attention deficit hyperactivity disorder (ADHD) in Spain are methylphenidate, lisdexamphetamine, atomoxetine and guanfacine. Due to adverse cardiovascular effects, mainly increased blood pressure and heart rate, its use in patients with known or undiagnosed heart disease may be controversial. Objective: To obtain a consensus document from the Spanish Society of Paediatric Cardiology and Congenital Heart Diseases (SECPCC) and experts from other Agencies and Societies as a guide for the paediatric cardiologist and physicians who treat children and adolescents with ADHD. Method: An analysis was performed on the bibliography and Clinical Practice Guidelines, technical data sheets approved by the Spanish Agency of Medicines and Health Devices, and the Spanish Ministry of Health Guidelines. A Working Group was formed, with a Coordinator, as well as members of the Clinical Cardiology Working Group and Arrhythmia Group of the SECPCC. This Group produced a preliminary document that was reviewed by a group of external experts (List 1) and a group of internal experts of the SECPCC (List 2) with a consensus being reached on the final document. Results: The recommendations of the SECPCC and the group of experts are presented on cardiovascular evaluation prior to treatment in children and adolescents with unknown cardiovascular disease and with known cardiovascular disease.The recommendations of the SECPCC and the group of experts are also presented on the use of medications for ADHD in children and adolescents with cardiological symptoms with no evidence of heart disease, congenital heart disease, cardiomyopathy, Marfan syndrome and other aortic diseases, hypertension, and arrhythmias. Resumen: Introducción: Los fármacos aprobados para el trastorno por déficit de atención con hiperactividad (TDAH) en España son: metilfenidato, lisdexanfetamina, atomoxetina y guanfacina. Debido a los efectos adversos cardiovasculares que pueden producir, principalmente aumento de la tensión arterial y la frecuencia cardiaca, su uso en pacientes con cardiopatías conocidas o no diagnosticadas puede ser controvertido. Objetivo: Realización de un documento de consenso de la Sociedad Española de Cardiología Pediátrica y Cardiopatías Congénitas (SECPCC) y expertos de otras Agencias y Sociedades como instrumento para el cardiólogo infantil y los médicos que tratan niños y adolescentes con TDAH. Metodología: Análisis de la bibliografía y Guías de Práctica Clínica, fichas técnicas aprobadas por la Agencia Española del Medicamento y Productos Sanitarios y Guía del Ministerio de Sanidad español. Formación de un Grupo de trabajo con un Coordinador, miembros de los grupos de trabajo de Cardiología Clínica y Arritmias de la SECPCC. Este Grupo realizó un documento que fue revisado por un grupo de expertos externos (Anexo 1) y un grupo de expertos internos de la SECPCC (Anexo 2) llegando a un consenso para la obtención del documento final. Resultados: Se presentan las recomendaciones de la SECPCC y el grupo de expertos sobre la evaluación cardiovascular previa al tratamiento en niños y adolescentes sin enfermedad cardiovascular conocida y con enfermedad cardiovascular conocida. Se presentan las recomendaciones de la SECPCC y el grupo de expertos sobre el uso de medicamentos para el TDAH en niños y adolescentes con síntomas cardiológicos sin evidencia de cardiopatía, cardiopatías congénitas, miocardiopatías, síndrome de Marfan y otras aortopatías, hipertensión arterial y arritmias.

In-hospital sudden death and cardiopulmonary cerebral resuscitation problems could have a common structured response. However, most hospitals lack a solution- oriented strategy. This paper provides detailed update on the main alternatives to reduce lethality of in-hospital sudden death and presents a proposal from the "Hospital General de Cienfuegos". The institutional approach for greater survival and lesser disability when facing sudden death will be based on multidisciplinary initiatives primarily centered on quality and prevention in the different stages of cardiopulmonary cerebral resuscitation. A cardio-protected hospital comprises management, organizational, assistance, academic and logistical processes for every single link of the survival chain in a medical emergency event, mainly sudden cardiac arrest. The proposal of the "Hospital General de Cienfuegos", a cardioprotected facility, brings together all elements of an integrated cardiopulmonary cerebral resuscitation system aimed at quality, safety and satisfaction of patients and caregivers. Sudden cardiac arrest (SCA) and sudden death (SD) continues to be a global public health problem, although the true incidence is unknown, it is estimated that they are responsible for 30% of cardiac origin mortality and may represent 20% of total mortality in adults. Unfortunately, the majority of cases occur in the general population, at the out-of-hospital level, in homes and in people who were not known to have heart disease. Although the majority of SCD victims are considered to be of cardiac origin and more frequent ischemic, it is not possible to rule out other causes only with the clinical diagnosis. Autopsy, histological and toxicological studies are necessary in all victims of SCA and SD to determine the precise cause of death; when these studies are carried out, causes of non-cardiac origin have been found in up to 40% of victims. The type of arrhythmia responsible for an episode of SCA and SD has changed over the years, now asystole and pulseless electrical activity (PEA) are detected more frequently than ventricular fibrillation (VF) or pulseless ventricular tachycardia (pVT). Wolff-Parkinson-White syndrome is a rare and non-structural heart disease, which belongs to the ventricular preexcitation syndromes. It is associated with the d evelopment of sudden death, as it can induce the appearance of malignant a rrhythmias in the patient and its diagnosis can be established through electrocard iogram. If it takes place in people linked to sport, with the development of intense physical exercise, it can increase the risk of sudden death. The case of a highperformance athlete with a diagnosis of Wolff-Parkinson-White syndrome is d escribed, and the guidelines to be followed according to international criter ia for the interpretation of the electrocardiogram in sportsmen and women are presen ted. Hypertrophic cardiomyopathy is characterized by left ventricular hypertrophy without apparent cardiac justification. Sudden cardiac death may be the first manifestation of the disease. It occurs mainly in adulthood and can be seen in childhood and adolescence where genetic origin predominates. Primary HCM ("familial") is inherited in an autosomal dominant pattern in the 25 subtypes informed in Online Mendelian Inheritance in Man. The proteins encoded by the mutated genes are part of the sarcomere in the cardiac cells, being the thick filament the most frequently affected, with the worst prognosis. Anamnesis: en granja porcícola tecnificada‚ ubicada en el departamento de Antioquia‚ con producción en bandas de 28 días‚ se presentó 6 % de muertes súbitas de lechones de 3 a 6 días de vida‚ de diferentes camadas‚ paridad y módulos. La planta de tratamiento de agua no estaba en funcionamiento y los procesos de aseo y desinfección fueron afectados. Hallazgos clínicos: los lechones presentaron muerte súbita‚ sin ningún signo clínico aparente‚ oscurecimiento de la piel del abdomen luego de la muerte; a la necropsia algunos órganos abdominales presentaron coloración similar‚ sin pérdida de la integridad de los tejidos. Ayudas diagnosticas: análisis microbiológico de agua‚ análisis forense‚ muestra de sangre para hemograma‚ muestras de órganos afectados para histopatología‚ aislamiento y caracterización por los medios de cultivo agar‚ identificación por PCR convencional gen 16S específico para Clostridium spp. y PCR multiplex para identificación de genes de toxinas de Clostridium perfringens. Enfoque del tratamiento: se restringió el acceso de agua a los lechones‚ en forma metafiláctica se usó una Cefalosporina de 3a generación a dosis recomendada y se instauro una colonización intestinal con bacterias probióticas de Lactobacillus spp. entre otras‚ para restablecer el microbioma‚ inmunidad intestinal y promover mecanismo de exclusión competitiva contra bacterias patógenas. Resultados: en el análisis del agua se confirmó Clotridium spp‚ y otras bacterias patógenas considerando un agua no apta para consumo‚ en las muestras de tejidos se aisló y caracterizó Clostridium perfringens toxinotipo A‚ el aislamiento y PCR para Salmonella spp. y E coli fueron negativos; en histopatología se evidenció de forma general cuadro inflamatorio sistémico. Conclusiones: por el tipo de presentación del caso clínico‚ incluyendo la agresividad de las lesiones en órganos poco reportados como blanco de enterotoxemias causadas por Clostridium perfringens toxinotipo A‚ se evidencia que la presión de infección puede generar cuadros que alteran la patogenia de la enfermedad. Se ratifica la importancia de la calidad del agua de consumo y para realizar los procedimientos de aseo y desinfección. [ABSTRACT FROM AUTHOR]

La displasia arritmogénica del ventrículo derecho (DAVD) es una cardiomiopatía caracterizada por el reemplazo de miocitos por tejido fibroadiposo con herencia autosómica dominante. Ocupa el segundo lugar como muerte súbita en adultos jóvenes y es causante de un gran porcentaje en atletas. Su clínica es variable, debido a que puede presentarse en reposo o luego de actividad física con síntomas inespecíficos como palpitaciones, síncope y dolor torácico. Se presentan tres casos autópsicos de diferentes instituciones cuya manifestación clínica fue muerte súbita.

Introducción: El paro cardiorrespiratorio constituye una emergencia médica, con grave peligro de muerte en quien lo sufre. Objetivo: Caracterizar los factores clínicos asociados al paro cardiorrespiratorio en pacientes atendidos en el servicio de urgencias. Método: Estudio observacional transversal. Serie de 108 pacientes. Variables: edad, sexo, presencia de comorbilidad y grado de comorbilidad. Resultados: Los factores que mayores porcentajes mostraron con el riesgo de fallecer fueron: la edad superior a los 50 años (52,9 %), el sexo masculino (57,5 %), los antecedentes de hipertensión arterial (72,4 %), cardiopatía isquémica (37,9 %) y diabetes mellitus (28,2 %), y la presencia de una comorbilidad de por lo menos dos enfermedades (44,8 %). Conclusiones: El conocimiento de las variables clínicas y factores pronósticos identificados resultan de inestimable valor, ya que optimizan el proceso de toma de decisiones en el manejo del paro cardiorrespiratorio y por tanto en la salvación de vidas humanas.

In the literature there have been reported multiple cases of persons who have at the same time the diagnosis of Takayasu's arteritis and ankylosing spodylitis, these inflammatory diseases with inconclusive autoimmune component. This is the reason why in this article we present the case of a man that had been under treatment for ankylosing spondylitis confirmed by HLA-B27, who also had vitiligo. One week before his death, patient suffered precordial pain with treatment at hospital under vasodilator drug. During the intake of food in a restaurant patient had sudden respiratory distress and tegumentary pallor, arriving at hospital care with the presumptive diagnosis of food poisoning that caused death, for which a necropsy was performed and ruled out this diagnostic possibility. Introduction: Sudden death (SD) is a health problem worldwide affecting all strata of the population. The main cause of SD is ischemic heart disease (IHD). The aims of the study were as follows: (i) to analyze the incidence of deaths from IHD in two successive periods (1998-2006 and 2007-2015) to visualize the magnitude of the problem and (ii) to review the official reports of SD in the same lapse of time. Materials and Methods: During that period, instantaneous death (ISD) and death that occurred in the first 24 h after the onset of symptoms were analyzed according to the official databases of National Institute of Statistics and Geography (INEGI) and National Health Information System (SINAIS). Results: There was an under- registration of SD cases in Mexico. Only 1394 cases of ISD were recorded officially in 17 years period of study, whereas it is estimated that 33,000 cases occur annually, exclusively due to sudden cardiac death. Conclusion: There is a serious gap in the official epidemiological information; placing the real problem in perspective would help to establish the adequate public policies for both, prevention and investigation of the causes of SD in Mexico. In this research is presented an overview of heart failure related to arrhythmias and sudden death, which can coexist, worsen, or be cause or consequence of one another. Here are discussed the premonitory electrical signs that allow to stratify risk in patients with previous events, with a possible approach to reality, and in those who have not presented them (most of them, the unprotected ones) and where a prognosis is very difficult, or impossible, to be established. These signs are numerous, elusive, with low specificity and sensitivity, none is absolute or negligible, in order to interpret them, a comprehensive vision is required. Premature ventricular contractions are discussed as predictors and triggers of arrhythmias, sudden death and cardiomyopathy, as well as the usefulness of ablative procedures versus medications. The arrhythmogenic right ventricular dysplasia or cardiomyopathy is a genetic heart disease whose diagnosis is often a challenge for the clinician. It is one of the most common causes of sudden cardiac death in adolescence and in young adults. We present the case of a patient with a history of malignant ventricular arrhythmias and recovered sudden cardiac death due to arrhythmogenic right ventricular dysplasia, with QRS fragmentation in the right precordial leads, as a marker of the presence of a suitable substrate for the emergence of spontaneous ventricular fibrillation.
