Novel triple formulations of ICS, LABAs and LAMAs are becoming the mainstay of treatment, however there is still a lack of clinical evidence for personalized therapy.Patients and Methods: RATIONALE was a non-interventional, prospective, 52 week study, assessing the effectiveness of beclometasone/formoterol/glycopyrronium-bromide (BDP/FF/G), in symptomatic COPD patients, with moderate airflow obstruction. The study included 4 visits, where data on demographic parameters, exacerbations, symptoms, quality of life (based on the EQ-5D-3L questionnaire) and lung function were collected. Data on adherence to treatment, based on prescriptions filled was collected from the database of the National Health Insurance Fund, with the patients’ consent. The primary objective was the change of adherence to treatment during the study, compared to baseline.Results: Altogether 613 patients had been enrolled. Their average age was 64.56 years and 50.5% were female. The average CAT score was 20.86, and most patients had suffered minimum one exacerbation (82.2%). Average FEV1 was 59.6%. Most patients had some limitation in one or more dimensions of EQ-5D-3L, with an average visual analogue scale score (VAS) of 60.31. After 12 months of treatment, adherence improved significantly – proportion of patients in the highest adherence group increased from 29.8% to 69.7% (p< 0.001). The average CAT score improved by 7.02 points (95% CI 5.82– 8.21, p< 0.001). There was a significant improvement in all dimensions of EQ-5D-3L, with an average increase of 17.91 (95% CI 16.51– 19.31, p< 0.001) points in the VAS score. Exacerbation frequency also decreased significantly.Conclusion: Although limitations of observational studies are present, we observed that early introduction of fixed triple combination results in a marked improvement in adherence to treatment, symptom scores, exacerbation frequency and quality of life. We show interior H\"older continuity for a class of quasi-linear degenerate reaction-diffusion equations. The diffusion coefficient in the equation has a porous medium type degeneracy and its primitive has a singularity. The reaction term is locally bounded except in zero. The class of equations we analyse is motivated by a model that describes the growth of biofilms. Our method is based on the original proof of interior H\"older continuity for the porous medium equation. We do not restrict ourselves to solutions that are limits in the weak topology of a sequence of approximate continuous solutions of regularized problems, which is a common assumption.

We present a Bayesian phase-space reconstruction of the cosmic large-scale matter density and velocity fields from the Sloan Digital Sky Survey-III Baryon Oscillations Spectroscopic Survey Data Release 12 CMASS galaxy clustering catalogue. We rely on a given ∧ cold dark matter cosmology, a mesh resolution in the range of 6-10 h-1 Mpc, and a lognormal-Poisson model with a redshift-dependent non-linear bias. The bias parameters are derived from the data and a general renormalized perturbation theory approach. We use combined Gibbs and Hamiltonian sampling, implemented in the ARGO code, to iteratively reconstruct the darkmatter density field and the coherent peculiar velocities of individual galaxies, correcting hereby for coherent redshift space distortions. Our tests relying on accurate N-body-based mock galaxy catalogues show unbiased real space power spectra of the non-linear density field up to k ~ 0.2 h Mpc-1, and vanishing quadrupoles down to r ~ 20 h-1 Mpc. We also demonstrate that the non-linear cosmic web can be obtained from the tidal field tensor based on the Gaussian component of the reconstructed density field. We find that the reconstructed velocities have a statistical correlation coefficient compared to the true velocities of each individual lightcone mock galaxy of r ~ 0.68 including about 10 per cent of satellite galaxies with virial motions (about r = 0.75 without satellites). The power spectra of the velocity divergence agree well with theoretical predictions up to k ~ 0.2 h Mpc-1. This work will be especially useful to improve, for example, baryon acoustic oscillation reconstructions, kinematic Sunyaev- Zeldovich, integrated Sachs-Wolfe measurements or environmental studies.

We present a survey of some recent results concerning joint numerical ranges of n-tuples of Hilbert space operators, accompanied with several new observations and remarks. Thereafter, numerical ranges techniques will be applied to various problems of operator theory. In particular, we discuss problems concerning orbits of operators, diagonals of operators and their tuples, and pinching problems. Lastly, motivated by known results on the numerical radius of a single operator, we examine whether, given bounded linear operators T1, . . ., Tn on a Hilbert space H, there exists a unit vector x ∈ H such that |〈Tjx, x〉| is “large” for all j = 1, . . . , n.

Our objective was to conduct a study that provides data on the level of symptom control and the frequency of specific risk factors for poor asthma outcomes on a large patient cohort.Methods: A cross-sectional, non-interventional real-life study was conducted among asthmatic patients treated by respiratory specialists in Hungary. Asthma control and risk factor assessment were done according to Global Initiative for Asthma guideline (Box 2–2). In the data analysis, phase descriptive statistics, graphical outputs, and Fisher’s exact tests were used.Results: Of 12743 patients enrolled by 187 specialists, asthma was well controlled in 36.0%, partially controlled in 29.29%, and uncontrolled in 34.71% of the cases. The most common comorbidities were rhinitis/sinusitis (66.84%), cardiovascular diseases (43.81%), and gastroesophageal reflux disease (20.11%). The following risk factors had the strongest relationship with uncontrolled disease: incorrect inhaler technique causing side effects (odds ratio, OR 4.86, 3.51–6.8), previous severe exacerbation (OR 4.79, 4.02–5.72), high short-acting beta agonist (SABA) use (OR 4.46, 4.03–4.93), incorrect inhaler technique associated with an exacerbation (OR 3.91, 3.06–5.03), and persistently low forced expiratory volume in 1 s (FEV1, OR 3.14, 2.8–3.52). The most frequent risk factors were smoking (OR 1.47, 1.36–1.59) and obesity (OR 1.34, 1.24–1.45). Furthermore, high loss of control was associated with an initial low FEV1 (OR 2.21, 2.01–2.44), frequent oral corticosteroid (OCS) use (OR 1.83, 1.64–2.05), poor adherence to treatment (OR 2.51, 2.21–2.86), and allergen exposure (OR 1.63, 1.47–1.81).Conclusions: This study indicated that the presence of risk factors for poor asthma outcomes listed by the Global Initiative for Asthma document significantly influenced actual control level in a real-world large patient cohort, with high SABA use, previous severe exacerbation, incorrect inhaler technique, persistently low FEV1, and poor adherence to treatment having the highest impact.Keywords: asthma, risk factors, poor outcomes, exacerbation, reliever use, comorbidity

We calculated required sample sizes to power for OS using published data from recent mBC trials.Patients and methods: Randomized superiority trials of first-line chemotherapy/targeted therapy for HER2-negative mBC including >150 patients, meeting the primary efficacy objective, and published in 2000–2018 were identified. The sample sizes required to power for PFS and OS were calculated retrospectively for each trial using observed results and study/recruitment follow-up durations (α=0.05, two-sided log-rank test, 80% power), and summarized as a factor (x) relative to actual sample size.Results: Nine of 13 identified trials reported all information required for retrospective sample size calculation. Six had sample sizes larger than required to demonstrate a significant PFS benefit but all would have required larger sample sizes to demonstrate significant OS benefit with the observed results. In ten trials, the required sample size was ≥5-fold larger to power for OS than PFS.Conclusion: Designing trials to test potential new treatments for HER2-negative mBC is challenging, requiring a balance of regulatory acceptability, feasibility, and realistic medical assumptions to calculate sample sizes. Powering for OS is particularly difficult in heterogeneous populations with long postprogression survival, potential crossover, heterogeneous poststudy therapy, and evolving treatment standards. Validated surrogate endpoints are critical. Ongoing trials of cancer immunotherapy (new mode of action) in triple-negative mBC (more homogeneous, shorter OS and postprogression survival, fewer treatment options) may show a new pattern. We present results from a set of high (512^3 effective resolution), and ultra-high (1024^3) SPH adiabatic cosmological simulations of cluster formation aimed at studying the internal structure of the intracluster medium (ICM). We derive a self-consistent analytical model of the structure of the intracluster medium (ICM). We discuss the radial structure and scaling relations expected from purely gravitational collapse, and show that the choice of a particular halo model can have important consequences on the interpretation of observational data. The validity of the approximations of hydrostatic equilibrium and a polytropic equation of state are checked against results of our simulations. The properties of the ICM are fully specified when a 'universal' profile is assumed for either the dark or the baryonic component. We also show the first results from an unprecedented large-scale simulation of 500 Mpc/h and 2 times 512^3 gas and dark matter particles. We present some of the results of an ongoing collaboration to sudy the dynamical properties of galaxy clusters by means of high resolution adiabatic SPH cosmological simulations. Results from our numerical clusters have been tested against analytical models often used in X-ray observations: $\beta$ model (isothermal and polytropic) and those based on universal dark matter profiles. We find a universal temperature profile, in agreement with AMR gasdynamical simulations of galaxy clusters. Temperature decreases by a factor 2-3 from the center to virial radius. Therefore, isothermal models (e.g. $\beta$ model) give a very poor fit to simulated data. Moreover, gas entropy profiles deviate from a power law near the center, which is also in very good agreement with independent AMR simulations. - Abstract
Acyrthosiphon pisum Harris (Hemiptera: Aphididae) is a globally distributed and economically important aphid pest of leguminous crops including peas, broad beans, and alfalfa. Infestations weaken plants directly by feeding, and indirectly, by the transmission of yield-affecting plant viruses. Although pea aphid control strategies largely rely on the application of insecticides, cases of insecticide resistance under applied conditions have not yet been described. Here we investigated the molecular basis of high levels of pyrethroid resistance (up to >180-fold) in two strains of A. pisum, PYR-R1 and VR, collected in the field in France and Italy, respectively. Foliar bioassays revealed significant synergistic effects between deltamethrin and piperonyl butoxide (a cytochrome P450 (P450) inhibitor), and, to a lesser extent, S,S,S-tributyl phosphorotrithioate (an esterase inhibitor). P450 and esterase activities in both resistant strains were significantly higher compared to an insecticide susceptible reference strain SUS-1. Target-site resistance mutations in the voltage-gated sodium channel known to confer pyrethroid resistance in other aphid pest species were not detected in either resistant strain. However, transcriptome profiling identified the P450 gene CYP6CY12 as highly over-expressed in the resistant PYR-R1 strain compared to the SUS-1 strain, and real-time quantitative PCR showed that this gene is also significantly overexpressed (>30-fold) in the VR strain. Functional expression of recombinant CYP6CY12 revealed its catalytic capacity to hydroxylate deltamethrin, thus confirming its causal role in pyrethroid resistance. Our study uncovered a P450-mediated mechanism of pyrethroid resistance in A. pisum for the first time and will inform future resistance management tactics to control this pest.
