1. Alcanzar la presión arterial y el colesterol LDL objetivo no previene la progresión de la carga de placa aterosclerótica en una población de alto riesgo

Abstract

INTRODUCION: La enfermedad aterosclerótica es una enorme carga para la salud en todo el mundo, y su prevención está basada en gran medida en el control de los factores de riesgo tradicionales, a pesar de la eficacia limitada en la prevención de eventos cardiovascular (ECV). La mejoría en la estratificación del riesgo se puede lograr a través de la detección de progresión del área total de la placa (TPA) medida por ecografía carotidea, la cual ha demostrado duplicación del riesgo basal en estos pacientes en un intervalo de 1 año. Nuestra hipótesis es que el control de la presión arterial y el colesterol LDL sérico en valores objetivo (según guías clínicas actuales) son insuficientes para reducir la progresión de la aterosclerosis en personas sin eventos previos, con alto riesgo cardiovascular. METODOS Y RESULTADOS: Estudio observacional prospectivo de 742 participantes con alto riesgo cardiovascular en un programa de prevención primaria cardiovascular. Se determinaron dos mediciones de ultrasonido de TPA para cada participante durante al menos un año. Incluimos en el análisis 57 participantes que mantuvieron una presión arterial por debajo de 130/80 mmHg y un colesterol sérico de lipoproteínas de baja densidad (LDL-C) por debajo de 100 mg/dl durante todo un año. Los participantes con progresión de la placa de TPA definida como aumento sobre el basal mayor de 5 mm2 se compararon con aquellos con cambios de TPA de 5 mm2 o menos (grupo sin progresión) mediante análisis de regresión logística multivariable. Después de una media de estadía en programa de casi 8 años, con Presión Arterial de 120.4 + 9/68.6 + 8 mmHg y LDL colesterol de 81 + 25 mg/dl, identificamos progresión de TPA en 22 de 57 (39%) participantes. No detectándose diferencias para ninguna covariable al comparar progresión versus no progresión. CONCLUSIÓN: La progresión de TPA ocurre hasta en el 39% de los individuos a pesar de mantener la presión arterial por debajo de 130/80 y el LDL-C sérico por debajo de 100 mg/dl. La evaluación de TPA puede ayudar a resolver las limitaciones de las pautas establecidas para la prevención de eventos de ECV en personas de alto riesgo.

Clinical findings and complementary studies suggested a bilateral atherogenic embolic event. Initially, she presented a superior branch retinal artery occlusion in her right eye followed by a central retinal artery occlusion with cilioretinal artery sparing in her left eye. Some conservative maneuvers performed did not improve visual acuity in the left eye. Supra-aortic Doppler ultrasonography revealed mild right internal carotid artery stenosis and moderate left internal carotid artery stenosis with a small, smooth, and homogeneous plaque. The transthoracic echocardiography showed a severe calcification of the mitral valve with a mild-moderate rim of stenosis. Central retinal artery occlusion and branch retinal artery occlusion are characterized by painless monocular loss of vision. Clinical approach and management attempt to treat the acute event, find the source of the vascular occlusion, and prevent further vascular events from occurring. Giant cell arteritis is a potentially treatable cause of central retinal artery occlusion and should be excluded in every single patient over 50 years old.Keywords: loss of vision, branch retinal artery occlusion, central retinal artery occlusion, Hollenhorst plaque

Abstract

This paper reports on the development of a novel CMOS device employing lead-sensing bacteria to assay lead in drinking water. The objective of the PlomBOX project is to develop a low-cost sensor ({\pounds}10) which can expedite access to on-demand assay methods and thus help mitigate lead intake through contaminated drinking water. The project follows three development paths: a) Certain bacteria can fluoresce or change colour when in the presence of lead. A genetically modified strain of Escherichia coli sensitive to lead concentrations up to 10 ppb is being developed. This constitutes the biosensor that fluoresces in proportion to the presence of lead. b) Bacteria response is imaged using a microprocessor (ESP32) with a camera module. This constitutes the optical metrology component of the PlomBOX. c) Data acquisition and control of the PlomBOX is achieved through a Bluetooth connection with the PlomApp, a custom-developed mobile phone application. Data are sent from the PlomApp to a database where a bespoke automated analysis software provides a result of the lead concentration in a sample of water. This paper reports on the instrumentation challenges of developing the electronics for the PlomBOX and on the first prototype.

Abstract

The aim of this work is to develop a model-based methodology for monitoring lateral track irregularities based on the use of inertial sensors mounted on an in-service train. To this end, a gyroscope is used to measure the wheelset yaw angular velocity and two accelerometers are used to measure lateral acceleration of the wheelset and the bogie frame. Using a highly simplified linear bogie model that is able to capture the most relevant dynamic behaviour allows for the set-up of a very efficient Kalman-based monitoring strategy. The behaviour of the designed filter is assessed through the use of a detailed multibody model of an in-service vehicle running on a straight track with realistic irregularities. The model output is used to generate virtual measurements that are subsequently used to run the filter and validate the proposed estimator. In addition, the equivalent parameters of the simplified model are identified based on these simulations. In order to prove the robustness of the proposed technique, a systematic parametric analysis has been performed. The results obtained with the proposed method are promising, showing high accuracy and robustness for monitoring lateral alignment on straight tracks, with a very low computational cost., Comment: 17 pages and 8 figures

Abstract

We highlight the power of the Gaia DR2 in studying many fine structures of the Hertzsprung-Russell diagram (HRD). Gaia allows us to present many different HRDs, depending in particular on stellar population selections. We do not aim here for completeness in terms of types of stars or stellar evolutionary aspects. Instead, we have chosen several illustrative examples. We describe some of the selections that can be made in Gaia DR2 to highlight the main structures of the Gaia HRDs. We select both field and cluster (open and globular) stars, compare the observations with previous classifications and with stellar evolutionary tracks, and we present variations of the Gaia HRD with age, metallicity, and kinematics. Late stages of stellar evolution such as hot subdwarfs, post-AGB stars, planetary nebulae, and white dwarfs are also analysed, as well as low-mass brown dwarf objects. The Gaia HRDs are unprecedented in both precision and coverage of the various Milky Way stellar populations and stellar evolutionary phases. Many fine structures of the HRDs are presented. The clear split of the white dwarf sequence into hydrogen and helium white dwarfs is presented for the first time in an HRD. The relation between kinematics and the HRD is nicely illustrated. Two different populations in a classical kinematic selection of the halo are unambiguously identified in the HRD. Membership and mean parameters for a selected list of open clusters are provided. They allow drawing very detailed cluster sequences, highlighting fine structures, and providing extremely precise empirical isochrones that will lead to more insight in stellar physics. Gaia DR2 demonstrates the potential of combining precise astrometry and photometry for large samples for studies in stellar evolution and stellar population and opens an entire new area for HRD-based studies., Comment: Published in the A&A Gaia Data Release 2 special issue. 45. Towards a holistic assessment of circular economy strategies: The 9R circularity index

Abstract

Our planet faces mounting environmental burdens due to linear production and consumption. Circular economy strategies offer a promising alternative, but evaluating their effectiveness requires robust measurement tools. Existing approaches lack a comprehensive framework incorporating the hierarchical strategies proposed by the 9R framework and the butterfly diagram. Based on the current circular economy indicators, this study gathers a set of indicators designed to assess the efficacy of circular economy strategies, considering the hierarchical levels outlined in the 9R framework. Moreover, it leverages the Analytic Hierarchy Process (AHP) fundamental scale as an integration tool to consolidate the suggested indicators into a unified metric termed the “9R circularity index”. This approach relies on a bottom-up approach to measure circular economy at various levels. The “9R circularity index” enhances the ability to compare results from a material flow analysis perspective. In addition, this paper presents a step-by-step approach and a supporting software tool that aims to facilitate the measurement, selection and comparison between circular and linear models. Ultimately, this proposed approach offers a workable, data-driven tool to support the transition towards a circular economy.

Abstract

The aim of this study is to show the optimal arrangement of a measurement system for estimating the thermal conductivity of thin films from temperature profiles. For this purpose, two different experimental setup systems, with square and circular cross sections, were designed to estimate the thermal conductivity of thin films and, in particular, of two ion exchange membranes. Both systems were placed horizontally and vertically in order to evaluate the best orientation to more accuratelydetermine thermal conductivity. A three-dimensional numerical simulation was performed using Comsol Multiphysics to predict the heat flow and temperature gradient and to evaluate the effect of the geometry and the orientation on the contact resistances. Each system was first calibrated without the membrane inside in order to estimate all the necessary thermal properties of the different materials of the model. Next, the membrane was placed inside the model, so that the model now includes the thermal conductivity of the membrane as the only unknown parameter. The numerical results were compared with the various measured temperature profiles to estimate the thermal conductivity. The thermal conductivity values of the well-known Nafion 117 membrane and other thicker membrane were determined. A very good agreement with reliable literature values was obtained. The approach presented here, combining experimental and simulated temperature profiles, may provide the basis for a practical alternative to better estimate the thermal conductivity of thin films.

Abstract

In this work, we study the performance of numerical methods based on the computation of topological energies to process data from synthetic and real experiments where only noisy limited-view data corresponding to a few frequencies are available. We show numerical experiments in two problems of practical interest. The first one corresponds to experimental measurements of the electromagnetic scattering produced by different objects extracted from the Fresnel database. The second one is related to synthetic experiments in a simplified model where steel welding joints are acoustically tested to identify possible flaws (air bubbles and inclusions) produced during the welding process.
