Abstract

As is well known, it is impossible to model reality with its true level of detail. Additionally, it is impossible to make an infinite number of observations, which are always contaminated by noise. These circumstances imply that, in an inverse problem, the misfit of the best estimated model will always be less than that of the true one. Therefore, it is not possible to reconstruct the model that actually generated the collected observations. The best way to express the solution of an inverse problem is as a collection of models that explain the observations at a certain misfit level according to a defined cost function. One of the main advantages of global search methods over local ones is that, in addition to not depending on an initial model, they provide a set of generated models with which statistics can be made. In this paper we present a technique for analyzing the results of any global search method, particularized to the particle swarm optimization algorithm applied to the solution of a two-dimensional gravity inverse problem in sedimentary basins. Starting with the set of generated models, we build the equivalence region of a predefined tolerance which contains the best estimated model, i.e., which involves the estimated global minimum of the cost function. Retaining ligaments (RL) bind soft tissue layers of the face to the underlying facial skeleton. It remains controversial whether RL suffers laxity with aging or if changes in 42 bone and other structures where ligaments are inserted lead to altered mechanical function of the latter. Enhancement of the supportive effect of the ligaments could help restore the soft tissues to their original anatomical positions and achieve a lifting effect. While injectable hyaluronic acid implants have been used to improve ligament support, calcium hydroxylapatite has one of the highest viscoelasticities when compared with other dermal fillers and can induce collagen synthesis, which could provide long-term laxity correction. In this preliminary report of a case series, our goal was to describe the use of calcium hydroxylapatite in different dilutions and combinations for different aging profiles to treat the midface as an alternative for hyaluronic acid injections and to describe the ultrasonographic behavior in the long run.Keywords: rejuvenation, retaining ligaments, calcium hydroxylapatite, hyaluronic acid, soft tissue repositioning, aging pattern

Abstract

El cáncer de mama (CM) es la primera causa de muerte por cáncer en mujeres tanto a nivel mundial como en Chile. Basados en características clínicas, histológicas y moleculares, múltiples estudios han identificado cuatro subtipos básicos de CM, los cuales están asociados a estrategias de tratamiento específicas y diferenciadas. El CM HER2-positivo representa un 15-25% de todas las neoplasias mamarias y se caracteriza por su agresividad, recurrencia temprana y mayor tendencia a presentar compromiso del sistema nervioso central. En las últimas décadas, nuevas terapias dirigidas se han posicionado como el estándar de tratamiento y han cambiado la historia natural de esta enfermedad, transformándola en una enfermedad potencialmente curable incluso en etapas avanzadas. Esta revisión busca entregar un resumen de las bases biológicas de esta enfermedad. Por otro lado, dada la aparición de un creciente número de nuevas estrategias de manejo sistémico, nos proponemos revisar sus mecanismos de acción analizando reportes de datos clínicos publicados y la experiencia de nuestro grupo.

28. Impacto del dupilumab en la calidad de vida de pacientes con dermatitis atópica grave tratados en un servicio de dermatología

Abstract

Objetivo: Determinar el impacto del dupilumab en la calidad de vida de pacientes con dermatitis atópica grave a tratamiento en nuestro Servicio de dermatología.Metodología: Estudio epidemiológico longitudinal descriptivo. Tras la prescripción con dupilumab, los pacientes eran atendidos en la consulta de enfermería dermatológica, seresolvían dudas, se le explicaba el tratamiento, técnicas de inyección, cuidados y recomendaciones del tratamiento. Se recogieron y analizaron varias escalas de valoración degravedad de dermatitis atópica (EASI, IGA), Extensión (BSA), calidad de vida (DLQI) y prurito (NSR), así como los efectos adversos asociados.Resultados: La gravedad disminuyó progresivamente a lo largo de las semanas de tratamiento con dupilumab, pasando de una media de 50% en la semana 0, a 7% en la 24. Del mismo modo el EASI mostró una disminución similar (de 28,75 a 1,48); así como el IGA (de 3,5 a 1) lo que indica mejoría objetiva de la dermatitis atópica. El DLQI también disminuyó pasando de 19 puntos en la semana 0 a 1,5 en la 24. También disminuyó la escala NRS prurito de 8,33 puntos a 1,83. Conclusiones: En nuestro estudio, el tratamiento con dupilumab resultó efectivo para el abordaje terapéutico de la dermatitis atópica grave, obteniendo los pacientes una disminución del nivel de picor y un aumento de la calidad de vida percibida. [Impact of dupilumab on the quality of life of patients with severe atopic dermatitis treated in a dermatology service] Objective: To determine the impact of dupilumab on the quality of life of patients with severe atopic dermatitis under treatment in our dermatology department.Methodology: Descriptive longitudinal epidemiological study. After prescription of dupilumab, patients were seen in thedermatological nursing office, doubts were solved, treatment, injection techniques, care and treatment recommendations were explained. Several atopic dermatitis severity assessment scales (EASI, IGA), extension (BSA), quality of life (DLQI) and pruritus (NSR) were collected and analyzed, as well as the associated adverse effects.Results: Severity decreased progressively over the weeks of treatment with dupilumab, from a mean of 50% at week 0 to 7% at week 24. Similarly, the EASI showed a similar decrease (from 28.75 to 1.48), as did the IGA (from 3.5 to 1), indicating objective improvement of atopic dermatitis. The DLQI also decreased from 19 points at week 0 to 1.5 at week 24. The NRS pruritus scale also decreased from 8.33 points to 1.83.Conclusions: In our study, treatment with dupilumab was effective for the therapeutic approach to severe atopic dermatitis, with patients obtaining a decrease in the level of itching and an increase in perceived quality of life.
