Abstract

Background/Objectives: Neuroendocrine neoplasms (NEN) are heterogeneous entities. Despite considerable advancement in the field, almost 50% of patients have metastatic disease, when liver transplantation (LT) is one of the possible treatments offering a cure in well-selected patients. Methods: The present study aims to systematically review all the literature from 2000 onwards on using LT for patients with NEN-LM, with particular attention to the risk factors of death and recurrence. Results: LT offers 5-year OS ranging from 52 to 74% and 5-year TFS rates ranging from 39 to 62%, with even better results published from 2009 onwards. The main risk factors for patient deaths are related to unfavorable primary tumor pathology, higher liver involvement, and simultaneous LT and primary resection. Similarly, recurrence is higher related to poor tumor grade and differentiation, and in the case of an older recipient age. Conclusions: Applying uniform criteria and a more in-depth understanding of the relevant prognostic factors contribute to a better selection of candidates for curative LT due to NEN metastases. LT for unresectable or liver-restricted NENs has a relevant place in the treatment algorithm and has achieved excellent results in recent decades, but more international efforts are needed to further improve outcomes. Abstract

Lockdown measures to control disease transmission were implemented at the start of the COVID-19 era, worsening the already existing sedentary lifestyle. Reduced physical activity (PA) and unhealthy eating habits have a negative impact on mental health in chronically ill patients, including diabetes patients. Mental illness, on the other hand, encourages a sedentary lifestyle, exacerbating all components of metabolic syndrome. While well-controlled diabetic patients with an HbA1c of less than 7% had a less severe clinical presentation and COVID-19 mortality rates, the favorable effect of PA on immunomodulation and immunoregulation should not be neglected. Given recent data indicating that a sedentary lifestyle is the third independent risk factor for COVID-19 complications and death (after advanced age and organ transplant), including regular PA has never been more vital. Since PA has a major impact on both glycemic control and mental health, implementing structured home-based activity programs could improve glycemic control and psychological well-being, hence positively impacting COVID-19 outcomes.

22. The Impact of Total Physical Activity on Microvascular Complications in Type 1 Diabetes Mellitus Patients

Abstract

The incidence of diabetes is increasing worldwide, emphasizing an emerging need for blood glucose control optimization to prevent the development of chronic complications and improve the quality of life. This retrospective cohort study aimed to investigate the effects of total physical activity on microvascular diabetic complication development in patients with type 1 diabetes mellitus (T1DM). The study included 71 T1DM patients, average age 41 years and HbA1c 7.78%. Most patients (82.1%) reported having hypoglycemia, while the minority of patients developed microvascular complications, mostly nonproliferative retinopathy (17.7%). All subjects included in the study were moderately or vigorously physically active. No association was observed between total physical activity and regulation of glycemia, hypoglycemic incidents, or development of microvascular complications. Until sufficient data from prospective studies become available, our data support the findings of no negative effect of higher intensity physical activity on the development of microvascular complications in T1DM patients., Učestalost šećerne bolesti globalno raste, što naglašava potrebu za optimizacijom kontrole glukoze u krvi kako bi se spriječio razvoj kroničnih komplikacija i poboljšala kvaliteta života. Cilj ove retrospektivne kohortne studije bio je istražiti učinke tjelesne aktivnosti na razvoj mikrovaskularnih dijabetičkih komplikacija u bolesnika s dijabetes melitusom tip 1 (T1DM). U studiju je uključen 71 bolesnik s T1DM prosječne starosti 41 godina i HbA1c od 7,78%. Većina bolesnika (82,1%) imala je hipoglikemiju, dok je kod manjeg broja bolesnika došlo do mikrovaskularnih komplikacija, uglavnom neproliferativne retinopatije (17,7%). Svi ispitanici uključeni u istraživanje bili su umjereno ili intezivno tjelesno aktivni. Nije primijećena povezanost između ukupne tjelesne aktivnosti i regulacije glikemije, hipoglikemijskih incidenata ili razvoja mikrovaskularnih komplikacija. Sve dok ne postanu dostupni dovoljni podaci iz prospektivnih studija, naši podaci potkrepljuju dokaze da nema negativnog učinka intenzivnije tjelesne aktivnosti na razvoj mikrovaskularnih komplikacija u bolesnika s T1DM.

Abstract

There is increasingly growing evidence and awareness that prehabilitation in waitlisted solid organ transplant candidates may benefit clinical transplant outcomes and improve the patient’s overall health and quality of life. Lifestyle changes, consisting of physical training, dietary management, and psychosocial interventions, aim to optimize the patient’s physical and mental health before undergoing surgery, so as to enhance their ability to overcome procedure-associated stress, reduce complications, and accelerate post-operative recovery. Clinical data are promising but few, and evidence-based recommendations are scarce. To address the need for clinical guidelines, The European Society of Organ Transplantation (ESOT) convened a dedicated Working Group “Prehabilitation in Solid Organ Transplant Candidates,” comprising experts in physical exercise, nutrition and psychosocial interventions, to review the literature on prehabilitation in this population, and develop recommendations. These were discussed and voted upon during the Consensus Conference in Prague, 13–15 November 2022. A high degree of consensus existed amongst all stakeholders including transplant recipients and their representatives. Ten recommendations were formulated that are a balanced representation of current published evidence and real-world practice. The findings and recommendations of the Working Group on Prehabilitation for solid organ transplant candidates are presented in this article.

Uzroci anemije jednaki su onima u općoj populaciji poput nedostatka željeza ili specifični kao posljedica odbacivanja, disfunkcije presatka, nedostatka eritropoetina, virusne infekcije, imunosupresivne ili antiviralne terapije. Većina je anemija nakon transplantacije reverzibilna, a osnovu liječenja čine nadoknada željeza i/ili eritropoetina, koncentrata eritrocita uz korekciju imunosupresivne i potporne terapije. Inhibitori hipoksija-inducibilnih faktora predstavljaju nadolazeće mogućnosti liječenja. Pravovremeno prepoznavanje i liječenje ove komplikacije značajno utječe na ishode pacijenata nakon transplantacije solidnih organa. U ovome preglednom radu donosimo dosadašnja saznanja o anemijama nakon transplantacije solidnih organa poput bubrega, jetre, srca i pluća s naglaskom na etiologiju i liječenje., Anemia after solid organ transplantation is a common complication. The causes of anemia can be the same as those in the general population such as iron deficiency, or specific due to rejection or graft dysfunction, erythropoietin deficiency, viral infection, immunosuppressive or antiviral therapy. Most anemia after transplantation is reversible, and the basis of treatment is iron and/or erythropoietin supplementation, erythrocyte concentrate transfusion, with correction of immunosuppressive and supportive therapy. Inhibitors of hypoxia-inducible factors represent emerging treatment options. Timely recognition and treatment of this complication significantly affects patient outcomes after solid organ transplantation. In this review, we present the current knowledge about anemia after transplantation of solid organs such as kidneys, liver, heart and lungs, with an emphasis on etiology and treatment.

Abstract

Infekcija citomegalovirusom (CMV) još uvijek predstavlja jednu od najčešćih komplikacija u primatelja solidnih organa. Povezana je s povećanim rizikom različitih komplikacija, uključujući gubitak presatka i smrt primatelja. Multidisciplinarni panel stručnjaka koji se bave transplantacijom solidnih organa okupio se radi donošenja preporuka za dijagnostiku, prevenciju i liječenje CMV infekcije. Preporuke su prvenstveno posvećene suvremenom pristupu dijagnozi CMV infekcije, mogućnostima liječenja novim antivirusnim lijekovima i načinima liječenja rezistentne/refraktorne CMV bolesti., Cytomegalovirus (CMV) infection remains one of the most common complications in solid organ transplant recipients. It is associated with an increased risk of different complications, including graft loss and mortality. A multidisciplinary panel of solid organ transplant experts gathered to revise and adopt consensus recommendations on CMV diagnostics, prevention, and treatment. Recommendations focus on the current approach to diagnosing CMV infection, different methods of prevention, possibilities of treatment with novel antiviral therapies, and approaches to treating resistant/refractory disease.

Abstract

Medical professional environments are becoming increasingly multicultural, international, and diverse in terms of its specialists. Many transplant professionals face challenges related to gender, sexual orientation or racial background in their work environment or experience inequities involving access to leadership positions, professional promotion, and compensation. These circumstances not infrequently become a major source of work-related stress and burnout for these disadvantaged, under-represented transplant professionals. In this review, we aim to 1) discuss the current perceptions regarding disparities among liver transplant providers 2) outline the burden and impact of disparities and inequities in the liver transplant workforce 3) propose potential solutions and role of professional societies to mitigate inequities and maximize inclusion within the transplant community.

37. Parvovirus B19 status in liver, kidney and pancreas transplant candidates: A single center experience

Abstract

Background: Parvovirus B19 (B19V) is associated with a wide range of clinical manifestations. The major presentation is erythema infectiosum. However, a persistent infection may cause pure red cell aplasia and chronic anemia in immunocompromized patients. The B19V seroprevalence varies with age and geographical location. Aim: To determine the B19V serological status and DNAemia in kidney, liver, and pancreas transplant candidates. Methods: Patients who underwent kidney, liver, or simultaneous kidney and pancreas/liver transplantation between January 2021 and May 2022 were included in the study. The serum samples were collected before transplantation. For detection of B19V DNA, a LightMix Kit B19V EC (TIB MOLBIOL, Berlin, Germany) was used. B19V IgM and IgG antibodies were detected using a commercial ELISA test (Euroimmun, Lübeck, Germany). Results: One hundred and thirty-one transplant candidates were included in the study, 71.0% male, with an average age of 53.27 years ± 12.71 years. There were 68.7% liver, 27.5% kidney, 3.0% simul taneous pancreas/kidney transplant (SPKT), and 0.8% simultaneous liver/kidney transplant recipients. No patients had detectable B19V DNA. B19V IgG seroprevalence was 77.1%. No acute or recent infections were detected (IgM antibodies). There was no difference in the mean age of seronegative and seropositive patients (51.8 years ± 12.9 years vs 53.7 years ± 12.7 years, t = -0.603; P = 0.548). Although seropositivity was lower in patients aged less than 30 years (66.6%) compared to the patients aged 30-59 years and > 60 years (80.4% and 78.1%, respectively), this difference was not significant. In addition, there was no difference in seropositivity between male and female transplant candidates, 76.3% and 78.9% (χ 2 = 0.104; P = 0.748). The seroprevalence did not differ among organ recipients, with 77.8%, 80.6%, and 50.0% for liver, kidney, and SPKT, respectively, (χ 2 = 5.297; P = 0.151). No significant difference was found in the seroprevalence in kidney transplant patients according to dialysis modality. Seroprevalence was 71.1% in hemodialysis patients, and 100% in peritoneal dialysis patients (χ 2 = 0.799; P = 0.372). Conclusion: The B19V seroprevalence is expectedly high among kidney, liver, and pancreas transplant candidates, but there are still 22.9% of seronegative individuals who remain at risk for primary disease and severe manifestations. Further research should elucidate the necessity of B19V screening in peri-transplant management.

Abstract

Background: Management and follow‐up strategies for primary sclerosing cholangitis (PSC) vary. The aim of the present study was to assess patient‐reported quality of care to identify the most important areas for improvement. Methods: Data were collected via an online survey hosted on the EU Survey platform in 11 languages between October 2021 and January 2022. Questions were asked about the disease, symptoms, treatment, investigations and quality of care. Results: In total, 798 nontransplanted people with PSC from 33 countries responded. Eighty‐six per cent of respondents reported having had at least one symptom. Twenty‐four per cent had never undergone an elastography, and 8% had not had a colonoscopy. Nearly half (49%) had never undergone a bone density scan. Ursodeoxycholic acid (UDCA) was used in 90–93% in France, Netherlands and Germany, and 49–50% in the United Kingdom and Sweden. Itch was common (60%), and 50% of those had received any medication. Antihistamines were taken by 27%, cholestyramine by 21%, rifampicin by 13% and bezafibrate by 6.5%. Forty‐one per cent had been offered participation in a clinical trial or research. The majority (91%) reported that they were confident with their care although half of the individuals reported the need for more information on disease prognosis and diet. Conclusion: Symptom burden in PSC is high, and the most important areas of improvement are disease monitoring with more widespread use of elastography, bone density scan and appropriate treatment for itch. Abstract

Vitamin D has been a focus of attention in liver cancer due to its direct and indirect antineoplastic effects. This review critically evaluates data from recently published basic and clinical studies investigating the role of vitamin D in liver cancer. Basic studies indicate that vitamin D plays an important role in liver cancer development by suppressing the activity of hepatic stellate cells and Kupffer cells. Furthermore, vitamin D has a direct anti-proliferative, anti- angiogenic, proapoptotic, and prodifferentiative effect on liver cancer cells. Recent investigation suggested several interesting mechanisms of these actions, such as inactivation of Notch signaling, p27 accumulation, and tyrosine-protein kinase Met/extracellular signal-regulated kinases inhibition. On the other hand, data from clinical observational studies, although promising, are still inconclusive. Abstract

Aim: To evaluate the effect of diabetes mellitus type 2 (T2DM) on the outcomes after treatment of hepatocellular carcinoma (HCC). ----- Methods: PubMed and Cochrane Central Register of Controlled Trials Databases were systematically searched. Three HCC clinical outcomes were explored: death, progressive disease after locoregional therapies, and recurrence. Sub-analysis was performed according to the use of potentially curative (resection, transplantation, termo-ablation) or non-curative therapies. Odds ratios (OR) and 95% confidence intervals (CI) were calculated to compare the pooled data between T2DM and non-T2DM groups. ----- Results: A total of 27 studies were analyzed. T2MD was associated with an increased risk of death (OR 3.60; 95%CI 2.18-5.95; P<0.001), irrespective of the treatment approach: curative (OR 1.30 95%CI 1.09-1.54; P=0.003) or non-curative (OR 1.05; 95%CI 1.00-1.10; P=0.045), increased HCC recurrence (OR 1.30; 95%CI 1.03-1.63; P=0.03), and increased disease progressiveness (OR 1.24; 95%CI 1.09-1.41; P=0.001). Conclusions Current data provide strong evidence that T2DM unfavorably affects HCC progression and recurrence, and patients’ survival after treatment, irrespective of the approach used.
