Knowledge-based systems, such as expert systems, provide a means of automating both foundry and environmental expertise. This approach may be further leveraged by implementing the expert system on the internet. This type of knowledge-based system can provide geographically wide distribution of specialized expertise in a consistent manner. The purpose of this research was to develop a prototype web-based expert system. The system considers the recycling potential of such foundry residuals as fly ash, steel slag, and non-ferrous slags for their potential use in road construction applications. This assessment considers Environmental Protection Agency regulations and standards for hazardous waste. In operation, users answer a series of screen prompts regarding the details of their specific industrial process. The software then leads the user through the process of systematically identifying constituent materi-als, identifying potential health or safety issues, and prescribing recommendations for their re-cycling. Background: The prevalence of dialysis-dependent chronic kidney disease (CKD5D) patients in Australia is increasing., Aims: To describe the long-term outcome and resource utilisation of CKD5D patients requiring critical care admission., Methods: Retrospective matched cohort study conducted in the intensive care unit (ICU) of Alice Springs Hospital. CKD5D patients admitted between 1 January 2011 and 31 December 2013 were matched by sex, age, Indigenous status and APACHE III score in a 1:1 ratio with a patient without CKD5D. Primary outcome was mortality censored at 31 December 2018, to allow a minimum 5-year follow up. Secondary outcomes explored resource use including ICU and hospital length of stay (LoS)., Results: During the study period, 178 (9%) admissions were coded as having CKD5D, 148 (83%) of which were successfully matched. CKD5D patients were older (52 vs 49 years, P < 0.01), with more chronic cardiovascular disease (34 vs 23%, P < 0.01). Five-year mortality was similar (49.5 vs 41.5%, P = 0.28), with a significantly longer time to death in the CKD5D cohort (1179 vs 341 days, P < 0.01). CKD5D patients had a shorter median ICU LoS (1.9 vs 3.0 days, P < 0.01) and lower rates of mechanical ventilation (12.2 vs 35.4%, P < 0.01)., Conclusion: CKD5D patients frequently require intensive care during acute illness. While they have an equivalent 5-year survival rate, time to death is longer suggesting mortality is related to chronic disease progression rather than their acute illness. Background: Because of the potential gravity of finding a person incompetent, assessment of mental capacity is challenging for clinicians. We aimed to test validity of a new structured professional judgement tool designed to assess functional mental capacity in three domains - finances, welfare and healthcare., Methods: Fifty-five male forensic psychiatric patients with Schizophrenia were interviewed using the Dundrum Capacity Ladders - a new semi-structured interview, and scores were assigned on a stratified scoring system, measuring ability to understand, reason, appreciate the personal importance of the decision at hand and communicate a decision. Data were also gathered pertaining to level of therapeutic security at the time of interview, diagnosis, neurocognitive function and a validated measure of real world function., Results: The results show that internal consistency and inter-rater reliability were high for all items. There were correlations between higher scores of functional mental capacity, neurocognitive function and measures of real world function in this population. Correlations were in the range 0.358 to 0.693, effect sizes that were moderate to high., Conclusions: The DUNDRUM Capacity Ladders appear to be a valid measure of functional mental capacity in this population. Further prospective studies of functional mental capacity as a measure of recovery are now required.

Few-layer black phosphorus (BP) is a new two-dimensional material which is of great interest for applications, mainly in electronics. However, its lack of environmental stability severely limits its synthesis and processing. Here we demonstrate that high-quality, few-layer BP nanosheets, with controllable size and observable photoluminescence, can be produced in large quantities by liquid phase exfoliation under ambient conditions in solvents such as N-cyclohexyl-2-pyrrolidone (CHP). Nanosheets are surprisingly stable in CHP, probably due to the solvation shell protecting the nanosheets from reacting with water or oxygen. Experiments, supported by simulations, show reactions to occur only at the nanosheet edge, with the rate and extent of the reaction dependent on the water/oxygen content. We demonstrate that liquid-exfoliated BP nanosheets are potentially useful in a range of applications from ultrafast saturable absorbers to gas sensors to fillers for composite reinforcement.

Subarachnoid haemorrhage is a condition that usually presents with headache and altered mental state. We report the case of a 50-year-old man with subarachnoid haemorrhage who presented with Terson's syndrome. Few data on stress symptoms related to the World Trade Center disaster in law enforcement personnel have been reported. Most New York City Police Department (NYPD) officers had significant exposure to the events of Sept. 11, 2001. Approximately 5,000 officers responded within the first 2 days, and more than 25,000 officers worked at ground zero, the morgues, or the Staten Island landfill. Because the police are the first line of defense against terrorist attacks, it is imperative that they maintain optimal health and functioning. Concern for the long-term effects from traumatic exposure is warranted. In partnership with Project Liberty, peer officers and clinicians from the Police Organization Providing Peer Assistance performed outreach, support work, and screening for stress symptoms related to the disaster in the NYPD from December 2002 until December 2003. Psychological issues in law enforcement personnel, a description of the outreach program, and data from these screenings are presented.

Objectives: The purpose of this article is to outline our methods for the prevention and management of cerebrospinal fluid (CSF) leak after transtemporal skull base surgery., Methods: A total of 589 patients underwent a variety of transtemporal surgical approaches for the extirpation of skull base tumors at our institution from July 1988 to October 1999. The medical records were retrospectively reviewed to identify the tumor histology, size, and location as well as the surgical approach, defect reconstruction technique, and the incidence of postoperative CSF leak., Results: The risk of CSF fistulae was greatest in utilizing the restrosigmoid approach (8%) and least in those who underwent a translabyrinthine approach (4%). Tumor size had no bearing on the incidence of the CSF leak and the overall incidence of meningitis was 1.0%., Conclusion: The proper surgical technique will minimize the risk of CSF leak after transtemporal skull base surgery. Immediate management of CSF fistulae helped prevent meningitis in the majority of these patients.

Itching purpura is a benign disorder characterized by the sudden onset of markedly pruritic, purpuric lesions with a clinical appearance similar to the progressive pigmented purpuric dermatoses. The pruritus is severe and unremitting and may lead to depression and loss of sleep. Hematologic and serologic evaluation is normal in most cases. The disease tends to be chronic with exacerbations and remissions lasting over a period of months to years. No current therapy has been uniformly successful in treating itching purpura.
