Purpose: Oral cancer risks have been shown to be modified by improving public awareness and reducing barriers to preventive care. The purpose for this study was to assess oral cancer knowledge and awareness and provide oral cancer screenings and education to a population of rural farmers in Wisconsin. Methods: Attendees 18 years of age and older at a rural farming exposition in Wisconsin were invited to complete a 12item oral cancer awareness paper survey and to receive a visual and tactile head and neck examination/ oral cancer screening. Completing both the survey and the screening were optional. Participants also received educational materials on oral cancer. Individuals with abnormal lesions were provided with dental referrals. Results: A total of 236 attendees consented to participate either the survey or oral cancer screening (n=236). Most (72%) reported seeing a dentist in the past six months regardless of insurance status. In spite of having had recent dental encounters, only 28% of women and 46% of men were able to identify at least one risk factor associated with oral cancer. Among participants consenting to the oral cancer screening (n=194), 17% (n=33) presented with oral lesions requiring additional assessment and were recommended for follow-up care. Conclusions: Knowledge and awareness of oral cancer risk factors, signs and symptoms was low among the participants in this rural population despite high rates of dental care access. Oral cancer screenings and education provided in varied settings could improve oral cancer knowledge and awareness and early detection of malignant oral lesions in rural communities. Abstract

Characterization of the dynamic change in the immunological landscape during malignant transformation from precancerous lesions to cancerous lesions in squamous cell carcinoma (SCC) is critical for the application of immunotherapy. Here, we performed single-cell RNA-Seq (scRNA-Seq) of 131,702 cells from 13 cancerous tissues of oral squamous cell carcinoma (OSCC), 3 samples of precancerous oral leukoplakia, and 8 adjacent normal samples. We found that tumor-infiltrating [CD4.sup.+] and [CD8.sup.+] T cells were functionally inhibited by immunosuppressive ligands expressed on various types of myeloid cells or neutrophils in the process of oral carcinogenesis. Notably, we identified a subset of myofibroblasts that exclusively expressed tryptophan 2,3-dioxygenase (TDO2). These [TDO2.sup.+] myofibroblasts were located distally from tumor nests, and both [CD4.sup.+] and [CD8.sup.+] T cells were enriched around them. Functional experiments revealed that [TDO2.sup.+] myofibroblasts were more likely to possess the ability for chemotaxis toward T cells but induced the transformation of [CD4.sup.+] T cells into Tregs and caused [CD8.sup.+] T cell dysfunction. We further showed that use of the TDO2 inhibitor LM10 attenuated the inhibitory states of T cells, restored the T cell antitumor response, and prevented the progression of OSCC malignant transformation in murine models. * A data set has been developed for the reporting of excisional biopsies and resection specimens for malignant odontogenic tumors by members of an expert panel working on behalf of the International Collaboration on Cancer Reporting, an international organization established to unify and standardize reporting of cancers. Odontogenic tumors are rare, which limits evidence-based support for designing a scientifically sound data set for reporting them. Thus, the selection of reportable elements within the data set and considering them as either core or noncore is principally based on evidence from malignancies affecting other organ systems, limited case series, expert opinions, and/or anecdotal reports.
