2. Improving the efficiency of scale-up and deployment of community health workers in Mali: A geospatial analysis.

Optimising the scale and deployment of community health workers (CHWs) is important for maximizing geographical accessibility of integrated primary health care (PHC) services. Yet little is known about approaches for doing so. We used geospatial analysis to model optimised scale-up and deployment of CHWs in Mali, to inform strategic and operational planning by the Ministry of Health and Social Development. Accessibility catchments were modelled based on travel time, accounting for barriers to movement. We compared geographic coverage of the estimated population, under-five deaths, and plasmodium falciparum (Pf) malaria cases across different hypothetical optimised CHW networks and identified surpluses and deficits of CHWs compared to the existing CHW network. A network of 15 843 CHW, if optimally deployed, would ensure that 77.3% of the population beyond 5 km of the CSCom (community health centre) and CSRef (referral health facility) network would be within a 30-minute walk of a CHW. The same network would cover an estimated 59.5% of U5 deaths and 58.5% of Pf malaria cases. As an intermediary step, an optimised network of 4 500 CHW, primarily filling deficits of CHW in the regions of Kayes, Koulikoro, Sikasso, and Ségou would ensure geographic coverage for 31.3% of the estimated population. There were no important differences in geographic coverage percentage when prioritizing CHW scale-up and deployment based on the estimated population, U5 deaths, or Pf malaria cases. Our geospatial analysis provides useful information to policymakers and planners in Mali for optimising the scale-up and deployment of CHW and, in turn, for maximizing the value-for-money of resources of investment in CHWs in the context of the country's health sector reform. Countries with similar interests in optimising the scale and deployment of their CHW workforce may look to Mali as an exemplar model from which to learn.

Abstract: This study examines the challenges faced by first-year English students at Université des Lettres et des Sciences Humaines de Bamako (ULSHB) in using the English prepositions "in," "on," and "at." Given the global significance of English, proficiency in its use is critical, yet French-speaking Malian students encounter difficulties due to differences between English and French prepositional systems. The study involved 60 students who completed a 15-minute test consisting of sentences requiring the insertion of the appropriate prepositions through a questionnaire. The results reveal a mix of positive and negative transfer from French, affecting students' accuracy. Notably, the highest accuracy was observed in sentences where English prepositional use aligns with French patterns, such as in expressions of time. In contrast, lower accuracy rates were found in contexts where prepositional use diverges significantly between the two languages. The quantitative method was adopted in this study. The findings show that we need specific teaching methods to help students better understand and use English prepositions. The study recommends focusing on how English and French prepositions differ and giving students practice in real-life situations to improve their skills. - Abstract
The identification of haplotypes influencing traits of agronomic interest, with well-defined effects across environments, is of key importance to develop varieties adapted to their context of use. It requires advanced crossing schemes, multi-environment characterization and relevant statistical tools. Here we present a sorghum multi-reference back-cross nested association mapping (BCNAM) population composed of 3901 lines produced by crossing 24 diverse parents to three elite parents from West and Central Africa (WCA-BCNAM). The population was characterized in environments contrasting for photoperiod, rainfall, temperature, and soil fertility. To analyse this multi-parental and multi-environment design, we developed a new methodology for QTL detection and parental effect estimation. In addition, envirotyping data were mobilized to determine the influence of specific environmental covariables on the genetic effects, which allowed spatial projections of the QTL effects. We mobilized this strategy to analyse the genetic architecture of flowering time and plant height, which represent key adaptation mechanisms in environments like West Africa. Our results allowed a better characterisation of well-known genomic regions influencing flowering time concerning their response to photoperiod with Ma6 and Ma1 being photoperiod sensitive and candidate gene Elf3 being insensitive. We also accessed a better understanding of plant height genetic determinism with the combined effects of phenology dependent (Ma6) and independent (qHT7.1 and Dw3) genomic regions. Therefore, we argue that the WCA-BCNAM constitutes a key genetic resource to feed breeding programs in relevant elite parental lines and develop climate-smart varieties.

Background The African Surgical OutcomeS-2 (ASOS-2) trial tested an enhanced postoperative surveillance intervention to reduce postoperative mortality in Africa. We undertook a concurrent evaluation to understand the process of intervention delivery. Methods Mixed-methods process evaluation, including field notes, interviews, and post-trial questionnaire responses. Qualitative analysis used the framework method with subsequent creation of comparative case studies, grouping hospitals by intervention fidelity. A post-trial questionnaire was developed using initial qualitative analyses. Categorical variables were summarised as count (%) and continuous variables as median (inter-quartile range [IQR]). Odds ratios (OR) were used to rank influences by impact on fidelity. Results The dataset included eight in-depth case studies, and 96 questionnaire responses (response rate 67%) plus intervention fidelity data for each trial site. Overall, 57% (n=55/96) of hospitals achieved intervention delivery using an inclusive definition of fidelity. Delivery of the ASOS-2 interventions and data collection presented a significant burden to the investigators, outstripping limited resources. The influences most associated with fidelity were: surgical staff enthusiasm for the trial (OR=3.0; 95% confidence interval [CI], 1.3–7.0); nursing management support of the trial (OR=2.6; 95% CI, 1.1–6.5); performance of a dummy run (OR=2.6; 95% CI, 1.1–6.1); nursing colleagues seeing the value of the intervention(s) (OR=2.1; 95% CI, 0.9–5.7); and site investigators' belief in the effectiveness of the intervention (OR=3.2; 95% CI, 1.2–9.4). Conclusions ASOS-2 has proved that coordinated interventional research across Africa is possible, but delivering the ASOS-2 interventions was a major challenge for many investigators. Future improvement science efforts must include better planning for intervention delivery, additional support to investigators, and promotion of strong inter-professional teamwork. Summary Background Risk of mortality following surgery in patients across Africa is twice as high as the global average. Most of these deaths occur on hospital wards after the surgery itself. We aimed to assess whether enhanced postoperative surveillance of adult surgical patients at high risk of postoperative morbidity or mortality in Africa could reduce 30-day in-hospital mortality. Methods We did a two-arm, open-label, cluster-randomised trial of hospitals (clusters) across Africa. Hospitals were eligible if they provided surgery with an overnight postoperative admission. Hospitals were randomly assigned through minimisation in recruitment blocks (1:1) to provide patients with either a package of enhanced postoperative surveillance interventions (admitting the patient to higher care ward, increasing the frequency of postoperative nursing observations, assigning the patient to a bed in view of the nursing station, allowing family members to stay in the ward, and placing a postoperative surveillance guide at the bedside) for those at high risk (ie, with African Surgical Outcomes Study Surgical Risk Calculator scores ≥10) and usual care for those at low risk (intervention group), or for all patients to receive usual postoperative care (control group). Health-care providers and participants were not masked, but data assessors were. The primary outcome was 30-day in-hospital mortality of patients at low and high risk, measured at the participant level. All analyses were done as allocated (by cluster) in all patients with available data. This trial is registered with ClinicalTrials.gov , NCT03853824 . Findings Between May 3, 2019, and July 27, 2020, 594 eligible hospitals indicated a desire to participate across 33 African countries; 332 (56%) were able to recruit participants and were included in analyses. We allocated 160 hospitals (13 275 patients) to provide enhanced postoperative surveillance and 172 hospitals (15 617 patients) to provide standard care. The mean age of participants was 37·1 years (SD 15·5) and 20 039 (69·4%) of 28 892 patients were women. 30-day in-hospital mortality occurred in 169 (1·3%) of 12 970 patients with mortality data in the intervention group and in 193 (1·3%) of 15 242 patients with mortality data in the control group (relative risk 0·96, 95% CI 0·69–1·33; p=0·79). 45 (0·2%) of 22 031 patients at low risk and 309 (5·6%) of 5500 patients at high risk died. No harms associated with either intervention were reported. Interpretation This intervention package did not decrease 30-day in-hospital mortality among surgical patients in Africa at high risk of postoperative morbidity or mortality. Further research is needed to develop interventions that prevent death from surgical complications in resource-limited hospitals across Africa. Le maraîchage est une pratique bien connue en zone rizicole au Mali. Face à une pluviométrie déficitaire entrainant de mauvaises récoltes des céréales, le maraîchage est un espoir pour combler ce déficit et une source de revenus des paysans. Ils fertilisent les parcelles avec des engrais chimiques et organiques, font recours aux pesticides pour combattre les ravageurs. Le but de cette étude était de déterminer le rôle socioéconomique du maraîchage à Baguinéda. Ainsi, une enquête sociodémographique a été conduite auprès des maraîchers à Baguinéda-Camp, Sonougouba et Sébéla. Sous l’égide des points focaux, les questionnaires ont été soumis individuellement à 84 producteurs dont 63 hommes et 21 femmes. Les résultats ont donné 41,7% de maraîchers d’âge inférieure à 36 ans. Parmi les maraîchers, 76 étaient mariés, 5 célibataires 3 veuves. Les intrants employés dans les jardins ont été jugés de mauvaise qualité par 52,0%. Tous utilisaient des pesticides et 35,7% observaient plus de 15 jours entre le dernier traitement et la récolte des légumes. Les revenus générés par la vente des légumes permettent à 88,1% des maraîchers de faire face aux dépenses familiales. En raison de son importance dans la vie socioéconomique, le maraîchage est pratiqué dans la plaine innondable en saison sèche.

12. Improving the efficiency of scale-up and deployment of community health workers in Mali: a geospatial analysis

Optimising the scale and deployment of community health workers (CHWs) is important for maximizing geographical accessibility of integrated primary health care (PHC) services. Yet little is known about approaches for doing so.We used geospatial analysis to model optimised scale-up and deployment of CHWs in Mali, to inform strategic and operational planning by the Ministry of Health and Social Development. Accessibility catchments were modelled based on travel time, accounting for barriers to movement. We compared geographic coverage of the estimated population, under-five deaths, and plasmodium falciparum (Pf) malaria cases across different hypothetical optimised CHW networks and identified surpluses and deficits of CHWs compared to the existing CHW network.A network of 15843 CHW, if optimally deployed, would ensure that 77.3% of the population beyond 5 km of the CSCom (community health centre) and CSRef (referral health facility) network would be within a 30-minute walk of a CHW. The same network would cover an estimated 59.5% of U5 deaths and 58.5% of Pf malaria cases. As an intermediary step, an optimised network of 4500 CHW, primarily filling deficits of CHW in the regions of Kayes, Koulikoro, Sikasso, and Ségou would ensure geographic coverage for 31.3% of the estimated population. There were no important differences in geographic coverage percentage when prioritizing CHW scale-up and deployment based on the estimated population, U5 deaths, or Pf malaria cases.Our geospatial analysis provides useful information to policymakers and planners in Mali for optimising the scale-up and deployment of CHW and, in turn, for maximizing the value-for-money of resources of investment in CHWs in the context of the country’s health sector reform. Countries with similar interests in optimising the scale and deployment of their CHW workforce may look to Mali as an exemplar model from which to learn.

La presente etude est une contribution a la connaissance des plantes ornementales utilisees en medecine traditionnelle par les tradipraticiens de sante (TPS) du District de Bamako. Ainsi, une enquête ethnobotanique a ete menee auprès de 60 TPS sur 27 marches de la ville de Bamako et un criblage phytochimique des quatre plantes les plus impliquees dans le traitement des differentes affections a ete realise. Les resultats ont montre que sur un total de 128 espèces de plantes medicinales recensees, 51 sont ornementales parmi lesquelles les Verbenacees et les Arecacees sont les plus representees avec les pourcentages respectifs de 9,80% et 7,84%. Plusieurs affections sont traitees avecces plantes dont les plus frequentes sont le paludisme, lhypertension arterielle, le diabète et les dermatoses.Les resultats ont aussi montre que les feuilles et les rameaux feuilles sont les plus sollicites (39,51%) et que la majorite des remèdes est preparee sous forme de decoction (32,50%). Le screening deDelonix regiaRaf., deKigelia africanaBenth, deLantana camaraL., et deTectona grandisL. a montre la richesse de ces plantes en alcaloïdes, tanins, flavonoïdes, saponosides, coumarines et terpenoïdes. La presence de ces metabolites justifierait lutilisation de ces plantes ornementales dans le traitement des differentes affections citees.

In Sub-Saharan Africa, Genotype-Environment interaction plays a key role in formulating strategies for crop improvement. Multi-location trials have created enabling structure to determine varieties yield performance and stability. Crop modeling led to prediction of long-term and spatial effects of climate variability. Three improved varieties were compared to three landraces. Optimum cultivation areas minimizing the risk of crop failure were delineated by comparing predicted flowering dates and end of rainy seasons. Agronomic values were determined in trials from three climatically different zones in 27 farms. Yield stability was determined using linear regression depending on each environmental mean and the AMMI model. Photoperiod sensitive varieties have wider optimal cultivation areas whereas early-maturing varieties (photoperiod insensitive) are subjected to strong constraints on sowing date. In low productivity conditions, landraces and improved varieties are not distinct. As the environmental cropping conditions increase, improved lines become significantly superior to landraces. Photoperiod insensitive landrace is subservient to climate conditions of its area of origin and its productivity drops sharply when moved to a wetter area. Varieties studied combined productivity and stability traits. These findings are important steps toward breeding climate resilient varieties for meeting the challenges of climate smart agriculture and sustainable intensification. 20. Plants grown on sewage sludge in South China and its relevance to sludge stabilization and metal removal

The production of sewage sludge in China has been increasing sharply in order to treat 40% of the municipal sewage in 2005 as planned by central government. The main sludge disposal method is landfill owing to heavy metal contamination, but it presents an attractive potential for agricultural land application. Experiments were carried out to study the simultaneous metal removal and sludge stabilization by plants. The sludge samples were collected from Datansha Wastewater Treatment Plant of Guangzhou, it contained excessive Cu and Zn compared with the Chinese National Standard for Agricultural Use of Sewage Sludge. Plants growing on sludge beds were investigated to follow their growth and metal uptake. 30 sludge plants were identified during 1 year's observation. A Zn high-accumulating and high growth rate plant(Alocasia macrorrhiza) was selected and grown on sludge beds in plots. The water, organic matter, heavy metals and nutrients contents, the E. coli number and the cress seed germination index were monitored for the sludge samples collected monthly. The plant growth parameters and its heavy metals contents were also determined. The sewage sludge treated by plants could be stabilized at about 5 months, the E. coli number was significantly decreased and the cress seed germination index attained 100%. Crop on sludge could ameliorate the sludge drying. The experiments are continuing to find out the appropriate plant combination for simultaneous sludge stabilization and metal removal for an acceptable period. Comparisons between the proposed processes and other methods for treating produced sludge such as composting, chemical andbacterial leaching were discussed.

21. Challenges and opportunities in implementing competency-based approach: insights from EFL teachers in Mali

Abstract: The present study investigates the implementation of the Competency-Based Approach (CBA) in Malian high schools. The main objective is to examine how teachers implement CBA in their classroom. The study also aims to demonstrate if teachers receive training about the method of teaching in use. In order to reach the research objectives, a questionnaire was designed. Thus, a quantitative research method is used for this purpose. In all, 137 teachers completed the questionnaire. The findings revealed that the majority 72% of teachers attended training sessions in CBA with 55% of them attending the sessions more than two times; 16% attending more than three times. The results also revealed that the majority asserted that they apply CBA in their classroom with 55% asserted they always apply it; 34% sometimes apply it. However, many of them replied that they encountered difficulties. For example, 94% encountered difficulties when applying CBA due to lack of materials, large classes, lack of textbooks, lack of sufficient training for some teachers, and time constraints.
