Purpose: The addition of midostaurin to standard chemotherapy (cytarabine and daunorubicin) has shown significant improvements in the survival of patients with acute myeloid leukemia with the FLT3 mutation (FLT3-AML). The objective of this study was to determine whether this intervention would be cost-effective in Spain. Methods: A partitioned survival model with five health states was developed (diagnosis and induction, complete remission, no complete remission, transplantation and death). A lifetime time horizon and the Spanish National Health System perspective were adopted. During the first three years, permanence in the different health states was determined according to the results of the RATIFY study. In successive years, the death rates of the Spanish population adjusted by a factor to reflect long-term disease-related mortality were used. Utilities were obtained from the literature. Pharmacological costs (first and second line) and the costs of other health resources (hospitalizations, visits and tests) were included. The robustness of the model was evaluated by deterministic and probabilistic sensitivity analyses. Results: The addition of midostaurin resulted in 1.46 life years gained (LYG) and 1.23 quality-adjusted life years (QALY) gained and implied an additional cost of € 47,955, resulting in an incremental cost-effectiveness ratio (ICER) of € 32,854/LYG and an incremental cost-utility ratio of € 38,985/QALY. In the univariate sensitivity analysis, the threshold of € 50,000/QALY was not exceeded in any case; taking into consideration potential discounts of 20-40% in the PVL of midostaurin the ICER would be below € 30,000/QALY, a commonly accepted threshold in Spain. In the probabilistic analysis, when the threshold was € 50,000/QALY, midostaurin was cost-effective in 82.3% of simulations. Conclusion: According to our modeling, midostaurin, in combination with standard chemotherapy, could be an efficient alternative for the treatment of FLT3-AML in Spain. © 2019 Arenaza et al.

espanolObjetivo: conocer las opiniones y vivencias de los usuarios y usuarias de un hospital universitario sobre el trato recibido por el personal durante la hospitalizacion, a traves de informantes clave y los propios sujetos de estudio. Metodo: estudio realizado mediante metodologia cualitativa, utilizando la Teoria Fundamentada segun la perspectiva de Glaser y Strauss. El estudio se llevo a cabo en el Complejo Hospitalario Universitario de Vigo (Pontevedra). Para la recogida de informacion se llevo a cabo una entrevista en profundidad, con una duracion aproximada de 60-90 min. El contenido fue registrado mediante una grabadora y transcrito literalmente. El muestreo fue intencional y se conto con la participacion de sujetos que respondian a tres perfiles diferentes: experto de la Unidad de Atencion al Usuario, expertos de asociaciones de pacientes y personas que habian estado hospitalizadas. Resultados: en total se realizaron 13 entrevistas en profundidad. Bajo la categoria nuclear denominada “trato” se identificaron cuatro grandes dimensiones: relacion personal (cinco categorias), informacion (seis categorias), ideal del compromiso profesional (13 categorias) y humanidad. Dentro del ideal de compromiso de todos los profesionales, se espera una conducta guiada por tres lineas maestras: no moralizar ni juzgar, no sentir que molestan cuando requieren cualquier intervencion de los profesionales y que sean atendidos cuando lo solicitan. Conclusiones: se puede afirmar que el termino “trato” se presenta como un constructo dinamico y complejo. Lo que supone a los profesionales sensibilidades y presupuestos morales y de conducta que, en no pocas ocasiones, se queda en un plano intangible. La “privacidad o intimidad” aparece permeada en su vertiente etica, moral y estetica por su ausencia. Le confieren gran importancia a la falta de identificacion por parte de los profesionales. EnglishObjective: to understand the opinions and experiences of male and female users of a University Hospital about the way they were treated by staff during hospitalization, through key reporters and the study subjects themselves. Method: a study conducted through qualitative methodology, using the Grounded Theory according to the perspective by Glaser and Strauss. The study was conducted at the Complejo Hospitalario Universitario de Vigo (Pontevedra). In-depth interviews with approximately 60-90 minute duration were used for collecting information. Their contents were audio recorded and literally transcribed. There was intentional sampling, and the subjects involved responded to three different profiles: expert from the Customer Care Unit, experts from patient associations, and persons who had been hospitalized. Results: overall, 13 in-depth interviews were conducted. Under the nuclear category called “treatment”, four major dimensions were identified: personal relationship (five categories), information (six categories), professional commitment ideal (13 categories) and humanity. Within the commitment ideal by all professionals, the performance expected was guided by three master lines: neither moralizing nor judgment, not feeling that they were bothering when they required any intervention by professionals, and being taken care of when they requested it. Conclusions: it can be stated that the term “treatment” is presented as a dynamic and complex construct; this assumes sensitivity and moral and behavioural suppositions in professionals that will often stay in an intangible plane. “Privacy or intimacy” appears permeated in its ethical, moral and aesthetic aspect, due to the lack of it. Major importance is assigned to lack of identification by professionals.

Los campos de actuación de nuestra profesión, incluso para el más profano de los clientes, siempre han pasado clásicamente por la dirección de obra (en riguroso cumplimiento de la LOE), la jefatura de obra (en contratas, subcontratas y en este sentido también en promotoras), coordinaciones de seguridad (sobre todo desde el año 95), valoraciones y peritaciones, laboratorios y empresas de control, empresas auxiliares de la construcción y algunas más que seguramente estaré dejando en el tintero. Sin embargo, y sobre todo a raíz de este periodo irrepetible de crecimiento en el sector de la construcción de los últimos doce años, se instauró con fuerza una nueva forma global de ver la profesión: la dirección integrada de proyectos o Project Management.

Objetivos: Conocer los comportamientos relacionados con la alimentación y hábitos tóxicos de las mujeres de edad comprendida entre 30-60 años del área sanitaria de Vigo. Averiguar si las mujeres menopáusicas tienen comportamientos más saludables que las no menopáusicas.Identificar si alguna de las variables sociodemográficas influye en los comportamientos sobre la alimentación y hábitos tóxicos. Determinar si las mujeres con conocimientos altos-muy altos tienen comportamientos adecuados Métodos: Estudio comparativo, observacional, transversal, realizado a 425 mujeres de 30-60 años. Se utilizó un cuadernillo con dos cuestionarios autocumplimentados, uno de conocimientos sobre la menopausia y otro sobre los comportamientos relacionados con la alimentación en la etapa del climaterio, incluidas las variables sociodemográficas. El análisis de los datos se realizó mediante SPSS 14.0. Resultados: Las mujeres de nuestra muestra tienen conocimientos altos-muy altos en un 74,3% y unos comportamientos que no se correlacionan con el nivel de conocimientos. Conclusiones: Las mujeres conocen qué es lo que deben realizar para mantener una vida saludable, pero a la hora de ponerlo en práctica no lo hacen mayoritariamente. Consideramos necesaria la implantación en los Centros de Salud, de un programa coordinado por el equipo de salud, dirigido a aplicar los conocimientos que han demostrado poseer las mujeres a los comportamientos diarios, con el objetivo de alcanzar una mayor calidad de vida. Este estudio forma parte de una investigación más amplia realizada sobre los “Conocimientos y comportamientos relacionados con el climaterio de las mujeres de 30-60 años del Área Sanitaria de Vigo” becado por el Fondo de Investigaciones Sanitarias (2007). Objectives: Knowing the behaviors related to food and toxic habits of women aged 30-60 years in the health area of Vigo. Determine whether menopausal women have healthier behaviors than non menopausal ones. Identify whether any of the socio-demographic variables has an influence on feeding habits or toxic behaviors. Determine whether women with high-very high knowledge have proper habits. Methods: Comparative, observational cross study made of 425 women between 30 and 60 years of age. We used a booklet with personally answered questionnaires, one about menopause and the other about feeding behaviors during the climacteric stage, which includes socio-demographic variables. Data analysis was carried out using SPSS 14.0. Results: A result of 74.3% of the women in the sample has a high/very high level of knowledge but behaviors that do not correlate with this. Conclusions: Women know what they must do to maintain a healthy life, but the majority does not put this into practice. We think it is necessary to introduce in health centers a program about the climacteric stage, coordinated by nurses and midwives and focused on applying to daily life the knowledge that women already have, with the aim of helping them to achieve a better quality of life. This study is part of a wider research on “Knowledge and behaviors related to climacteric stage in women between 30 and 60 years of age in the health area of Vigo sponsored by the “Fondo de Investigaciones Sanitarias” (Spanish Health Research Fund) in 2007.

Estudio observacional, transversal comparativo, realizado a los alumnos de los tres cursos de Enfermería matriculados en el curso 2004-05 en la Escuela do Meixoeiro (Vigo), con el objetivo de valorar qué situaciones de las prácticas clínicas, agrupadas por factores, resultan más estresantes para los/las estudiantes. La medición se ha realizado con los cuestionarios Kezkak y Stai. La participación en el estudio ha sido del 85,30%, con un rango de edad entre 19-41 años. Las puntuaciones medias obtenidas en nuestra Escuela reflejan una puntuación alta en casi todos los ítems considerados como factores desencadenantes de estrés, en comparación con los alumnos de otros estudios. Al realizar la correlación entre ansiedad rasgo, ansiedad estado y las situaciones estresantes, hemos encontrado significación entre algunos de los factores y la ansiedad estado, pero no en la ansiedad rasgo, lo que parece indicar que las situaciones de estrés que viven con mayor intensidad están más relacionadas con la inseguridad que con un rasgo de su carácter. Como medida para mejorar el nivel de ansiedad de nuestros alumnos en la realización de sus prácticas clínicas proponemos intervenciones que faciliten la enseñanza de técnicas de relajación y afrontamiento. SUMMARY Observacional, cross-sectional comparative study given to the 3 courses students of infirmary registered in course 2004-05 in the School of Meixoeiro (Vigo), with the objective to valuate situations of the clinical practices, grouped by factors, are but more stressant for the students. The Kezkak and Stai questionnaires have been used to make. Participation in the study has been of the 85.30%. with a rank of ages between 19-41 years. The obtained average scores in our school reflect a high score in almost all the items considered like leading factors of stress, in comparison with the students of other studies. When making the correlation between anxiety characteristic, anxiety state and the situations , we have found significant between some of the factors and anxiety state, but not in anxiety characteristic, which seem to indicate that the stress situations that arelived with greater intensity are more related to the insecurity than with a characteristic of its character. As a measure to improve the level of anxiety of our students in the accomplishment of its clinical practices we propose interventions that facilitate the education of relaxation techniques and facing facts.

Se estudia la prevalencia del síndrome de Burnout o quemad@ mediante el Maslach Burnout Inventario (MBI) en las enfermeras del Complexo Hospitalario Universitario de Vigo que está integrado por cinco hospitales y se relaciona con distintas variables sociodemogáficas. Es un estudio comparativo entre los distintos hospitales, observacional y de carácter transversal, considerando un nivel de confianza del 0,95% (a=0,05), una precisión del 5% (i=0,05) y para una p=q=0,05 .Las fechas de entrega y recogida de las encuestas han sido del 18 de mayo al 31 de Agosto de 2005, obteniéndose un nivel general de Burnout del 13%. ABSTRACT We studied the prevalence of Burnout with the Maslach Burnout Inventory in a sample of nurses working at the Complexo Hospitalario Universitario of Vigo - CHUVI- (Spain). The CHUVI is composed of five hospitals. We carried out a comparative, cross-sectional and observational study in these hospitals. We used a confidence interval of 95%, (a=0.05), a statistical precision of 5% (i=0.05) and a p value of 0.05. The questionnaires were delivered and collected between May and August 2005. The study showed a burnout prevalence of 13%.

En la actualidad no cabe duda de la importancia de la nutrición como indicador de salud, por tanto, conocer el estado nutricional de la población anciana permite valorar de forma indirecta su estado de salud, pero también detectar y tratar de forma precoz situaciones de riesgo de malnutrición. Se sabe que las necesidades nutricionales del individuo varían con la edad, aunque aun no ha sido determinado cuál sería el aporte nutricional óptimo para la población anciana. El objetivo de este trabajo es conocer los cambios que se producen en la nutrición durante el proceso de envejecimiento. La metodología utilizada ha sido la revisión bibliográfica. Se analizan los cambios en la composición corporal, en los diferentes órganos y los factores de riesgo de desnutrición, planteándose finalmente distintos tipos de intervención nutricional, concluyendo que el envejecimiento normal no es, por sí mismo, una causa de desnutrición en la población anciana sana.

The nursing personnel at the C.H. Xeral-Cíes participated in a study of various types of self-protection methods over two time spans. The hypotheses investigated were the following: 1. Is there a divergence in the percentage of personnel vaccinated against hepatitis B?, 2. Is there an increase in the level of knowledge about universal precautions?, and 3. Is there any improvement in self-protection methods in the latter study? The statistical analysis used has a variable range of0.05. The results indicated significant differences in the percentage of personnel vaccinated against hepatitis B, in knowledge about universal precautions, and in the number of accidental punctures reported to the Preventative Medicine Service.
