1. Impact of sex on outcomes after surgery for non-muscle-invasive and muscle-invasive bladder urothelial carcinoma: a systematic review and meta-analysis

Abstract

Purpose To assess the prognostic value of sex for non-muscle-invasive/muscle-invasive bladder urothelial carcinoma (NMIBC/MIBC) treated with radical surgery. Methods The PubMed, Web of Science, and Scopus databases were searched in November 2021 according to the Preferred Reporting Items for Systematic Review and Meta-analysis statement. Studies were deemed eligible if they involved the comparison of the overall, cancer-specific, progression, and recurrence-free survival of patients with NMIBC/MIBC. Formal sex-stratified meta-analyses of these outcomes were performed. Results Thirty-one studies, which included 32,525 patients with NMIBC, and 63 studies, which included 85,132 patients with MIBC, were eligible for review and meta-analysis. Female sex was associated with worse cancer-specific survival (pooled hazard ratio [HR], 1.21; 95% confidence interval [CI], 1.11–1.31) and overall survival (pooled HR, 1.02; 95% CI, 1.00–1.05) in patients with MIBC. In contrast, however, sex was not associated with cancer-specific survival (pooled HR, 1.01; 95% CI, 0.70–1.46), progression-free survival (pooled HR, 1.04; 95% CI, 0.88–1.24), and recurrence-free survival (pooled HR, 1.06; 95% CI, 0.98–1.16) in patients with NMIBC. Conclusions Sex is associated with an increased risk of worse survival outcomes in patients with MIBC but not in those with NMIBC. Given the genetic and social differences between sexes, sex may represent a key factor in the clinical decision-making process.

3. Sustainability Evaluation of a Concrete Gravity Dam: Life Cycle Assessment, Carbon Footprint Analysis, and Life Cycle Costing

Abstract

Exploring the life cycle of infrastructures, like dams, is important for decision-makers since it allows for evaluating its overall sustainability, and identifying ways to balance the benefits and costs of dam development. In this study, the life cycle assessment (LCA) of the existing concrete gravity Pine Flat dam in the stages of construction to destruction, disposal, and recycling is investigated through ReCiPe 2016 methodology and the effect of two approaches of seismic retrofitting and non-retrofitting in the life cycle of the dam is studied. For a comprehensive understanding of the sustainability of the dam's life cycle, carbon footprint analysis (CFA) and life cycle costing (LCC) are also conducted to identify the main sources of greenhouse gas (GHG) emissions and evaluate the economic performance. These demands begin by assessing the dam's life cycle across three distinct stages, namely, initial construction, seismic retrofitting, and decommissioning, assuming recycling 20% of demolished concrete for the last stage. The outcomes of the evaluation are then presented for four different life cycle scenarios. The findings have underscored the importance of reducing air pollution and emphasize that human health is the most significant environmental concern as compared to the ecosystem and resource indicators. The concrete recycling considered during the decommissioning stage led to a 32% reduction in pollution caused by the dam disposal process. Additionally, the effect of retrofitting dams in decreasing environmental impact indicators such as carbon footprint and human health has been considered when compared to dam disposal. The economic and environmental costs of retrofitting Pine Flat dam were obtained about half of the equivalent expenses for its disposal and recycling stage.

4. Fully automated operational modal identification of regular and irregular buildings with ensemble learning

Abstract

Modal identification, a type of system identification that employs modal analysis, can be automated to reduce user interference in extracting modal parameters. Modal analysis is a valuable tool for obtaining a structure's modal parameters, which define its dynamic characteristics. Operational modal identification is a specific type of modal analysis that uses only system outputs to extract modal parameters. The present study aims to introduce a fully automated modal identification algorithm to identify modal parameters for an unknown system based on its output information. The algorithm uses a stochastic subspace algorithm to extract modal parameters, requiring only the appropriate degree of the state-space model estimated using the Singular Value Criterion. Several features are used to create a feature space that separates physical and spurious modes. The Improved Distance Evaluation algorithm selects appropriate features, making the identification process more accurate and flexible. The identification algorithm employs clustering algorithms with different approaches and the Dempster-Shafer data fusion algorithm to integrate clustering algorithm results and avoid divergence among them. Due to limited data diversity for identification in cases where only one dataset is available, statistical resampling methods are used as an alternative. The proposed algorithm can identify modal parameters using only one dataset of the building structure. Signal processing, data collection, clustering, feature extraction, model reduction, and data fusion are all aspects of this algorithm. To assess its performance, four regular and irregular buildings are considered as case studies. The presented modal identification algorithm is highly accurate and robust with the repeatable result as compared to similar algorithms, with extracted modal parameters demonstrating high stability.

5. A novel development of HPC without cement: Mechanical properties and sustainability evaluation

Abstract

In the current study, a new type of high-performance concrete (HPC) called HPC-CAS (calcium oxide-activated slag) is presented. The HPC-CAS is made from calcium oxide-activated slag and is reinforced with steel, basalt, and recycled polyethylene terephthalate (PET) fibers to make it less brittle. To assess the mechanical properties of the samples, five mix designs were produced, including HPC, HPC-CAS without reinforcement, and HPC-CAS reinforced with fibers. Compressive, splitting tensile, and flexural strengths were then determined. In addition, the study evaluated the environmental impact of different mix designs using two methods: IMPACT 2002+ and CML baseline 2000. The findings indicate that in HPC-CAS, calcium oxide serves as the activator for slag, resulting in a compressive strength of 105 MPa, which is only 4.54% lower than that of HPC. Notably, incorporating steel and recycled PET fibers addresses the issue of brittle fracture in HPC and HPC-CAS samples, leading to compressive strengths of 121 MPa and 78 MPa, respectively. When compared to samples without fibers, the inclusion of steel, basalt, and recycled PET fibers in HPC-CAS resulted in improved splitting tensile strengths of 159%, 89%, and 55%, respectively. The samples containing steel, basalt, and recycled PET fibers also revealed flexural strengths with 3.68 and 1.20 times, and 31% increase, respectively, as compared to those without fibers. Overall, the incorporation of fibers significantly enhanced the brittle behavior of the HPC-CAS geopolymer matrix. However, it is important to note that the addition of 3% fibers to HPC-CAS resulted in higher environmental impacts in all categories as compared to HPC-CAS without fibers. Furthermore, the embodied carbon footprint associated with the production of HPC-CAS was found to be significantly lower, with a reduction of approximately 61%. 6. A new approach of ensemble learning in fully automated identification of structural modal parameters of concrete gravity dams: A case study of the Koyna dam

Abstract

This study proposes a new automatic modal identification algorithm that identifies the system's modal parameters based on output-only data. The identification process is based on a stochastic subspace algorithm. Stochastic subspace algorithms only need the model degree to identify the modal parameters of the system. A model degree is an appropriate degree of the state-space model and it is estimated by using the Singular Value Criterion. Several clustering algorithms with different approaches have been used. The most powerful data fusion algorithm, which is called Dempster-Schaffer Theory is applied for integrating the results of the algorithms and preventing divergence of results. Furthermore, it focused on measuring the uncertainty of the identified modal parameters. In the proposed modal identification algorithm, only one data set is used for identification. In this situation, the variety of data is small and limited, and statistical resampling methods seem to be a good option. Establishing the above cycle enables the identification algorithm to identify modal parameters and quantify their uncertainty based only on a data set. This algorithm uses many fields, such as data collection, signal processing techniques, model reduction, feature extraction, clustering and data fusion. The Koyna gravity dam is selected as a case study to evaluate the performance of the developed algorithm. In the first step, a comparative study is carried out to verify the developed finite element model of the Koyna dam. Afterwards, the dam is excited by the Koyna earthquake, and dynamic responses of the dam are collected from the minimum number of points required for system identification. Abstract

Item does not contain fulltext, CONTEXT: During the past decade, several urinary biomarker tests (UBTs) for bladder cancer have been developed and made commercially available. However, none of these is recommended by international guidelines so far. OBJECTIVE: To assess the diagnostic estimates of novel commercially available UBTs for diagnosis and surveillance of non-muscle-invasive bladder cancer (NMIBC) using diagnostic test accuracy (DTA) and network meta-analysis (NMA). EVIDENCE ACQUISITION: PubMed, Web of Science, and Scopus were searched up to April 2021 to identify studies addressing the diagnostic values of UBTs: Xpert bladder cancer, Adxbladder, Bladder EpiCheck, Uromonitor and Cxbladder Monitor, and Triage and Detect. The primary endpoint was to assess the pooled diagnostic values for disease recurrence in NMIBC patients using a DTA meta-analysis and to compare them with cytology using an NMA. The secondary endpoints were the diagnostic values for high-grade (HG) recurrence as well as for the initial detection of bladder cancer. EVIDENCE SYNTHESIS: Twenty-one studies, comprising 7330 patients, were included in the quantitative synthesis. In most of the studies, there was an unclear risk of bias. For NMIBC surveillance, novel UBTs demonstrated promising pooled diagnostic values with sensitivities up to 93%, specificities up to 84%, positive predictive values up to 67%, and negative predictive value up to 99%. Pooled estimates for the diagnosis of HG recurrence were similar to those for the diagnosis of any-grade recurrence. The analysis of the number of cystoscopies potentially avoided during the follow-up of 1000 patients showed that UBTs might be efficient in reducing the number of avoidable interventions with up to 740 cystoscopies. The NMA revealed that diagnostic values (except specificity) of the novel UBTs were significantly higher than those of cytology for the detection of NMIBC recurrence. Abstract

A simple approach was developed for performance-based analysis of reinforced concrete columns subjected to shear, flexure and axial loads. This method is based on a simplification of a consistent but relatively more complex approach known as the axial-shear-flexure interaction (ASFI) method, which is able to predict the full load-deformation relationships of reinforced concrete columns subjected to axial, flexure and shear force. The uniaxial shear-flexure model (USFM), presented herein, can also predict comparable full load-deformation responses. In it, however, the computation-intensive iteration process for shear modeling used within the ASFI approach is eliminated and the formulation is simplified. This paper describes the formulation, implementation and verification of the USFM approach.
