Mucormycosis is a fatal invasive illness most frequently seen in immunocompromised hosts with uncontrolled diabetes, hematological malignancies, organ transplantation, or long-term steroid treatment. It has a poorer outcome than other fungal diseases due to its rapid spread and resistance to antifungal agents. We report a rare case of disseminated mucormycosis including the bone marrow, peritoneum, lung, and lymph nodes in an apparently immunocompetent 58-year-old gentleman who presented with two months of ascites and weight loss. After a thorough analysis, we found aseptate fungal hyphae in the bone marrow and ascitic fluid. In addition, a cottony white, woolly growth indicative of mucor species was seen in the ascitic fluid culture. CT scans of the chest and abdomen indicate characteristics consistent with mucor invasion. We began the patient on tablet posaconazole, but he died on the fifth day. The atypical presentation in an apparently immunocompetent patient and broad dissemination with rare bone marrow involvement emphasizes the disease's invasiveness.

Melioidosis has lately expanded its endemic areas to encompass the Indian subcontinent, despite being well-known in Southeast Asia and Northern Australia.Though cases have been reported from Bangladesh, it might be a significantly underdiagnosed cause of infection & death in this agricultural country. We report a case of melioidosis with multiple abscesses & fulminant sepsis in a 55-year-old farmer diagnosed based on pus samples, which revealed gram-negative bacillus Burkholderiapseudomallei. Industrial zones are considered one of the most important sectors of economic development in any country. All countries seek to develop these zones and transform them into the fourth and fifth industrial revolution zones. This paper relies on an extrapolation approach and comparative analysis of the industrial and economic zones that include the currently enforced policies dealing with these zones all over the world. Accordingly, this study consists of three main stages. The first stage concerns reviewing the concepts and definitions of industrial and economic zones, defining policies for dealing with them, and then concluding the bases and criteria of those policies. The second stage focuses on defining the criteria of the fourth industrial revolution industrial zones, then determining the criteria for differentiating between the current economic zones according to their suitability to absorb the fourth-generation industries, and finally determining the criteria for the localization of these industries spatially, relying on Fuzzy GIS methods to identify the best sites for localizing high-quality industries. The third stage is about conducting a field application to the Suez Canal area in Egypt to determine the best high-tech industrial site within the selected area in Egypt. The site identified as the most suitable for the settlement of high-tech industries is the area located in the northwest and southeast of the special economic zone (SEZ), northwest of the Suez Canal, with an area of 22 km², a weight of 0.357724, and a score of 20.

7. The prevalence and associated factors of shisha use among smokers attending three primary healthcare centers in Qatar, 2020

Smoking is one of the most serious health issues. One form of tobacco use, shisha, is prevalent in the Eastern Mediterranean Region. There is a misconception among smokers that waterpipe use poses no harm to human health. In Qatar, the Global Adult Tobacco Survey in 2013 has shown that 3.4% of adults in the country were current waterpipe users. Nevertheless, there remains a scarcity of data on the knowledge and practice of shisha smokers in Qatar.This was a cross-sectional study across three primary healthcare (PHC) centers in Qatar between September and December 2020. A convenient sampling technique was employed to enroll participants. Upon consent, an interview was conducted to collect data from adult smokers (aged ≥18 years). Descriptive and analytic statistics were used when appropriate.A total of 281 individuals participated in this study. Nearly half (48.8%) were current shisha users and almost three-quarters (72.9%) had inadequate knowledge in this regard. In addition, only one-fifth (19.5%) of participants attended smoking cessation clinics at the primary healthcare centers and a quarter (25%) of the respondents managed to quit smoking before relapsing. A significant association was found between the shisha knowledge of participants and their nationality (Qatari), employment status (unemployed), smoking status (current smoker), and duration of smoking.The study results convey a high prevalence of shisha use (47.8%), inadequate shisha knowledge (72.9%), and lack of awareness about available smoking cessation services (46.6%) among the participants. Further research is recommended to generate evidence on the determinants of shisha use and knowledge among the attendees of primary healthcare centers in Qatar.

Business's strength arises from the strength of its supply chain. Therefore, a proper supply chain management is vital for business continuity. One of the most challenging parts of SCM is the contract negotiation, and one main aspect of the negotiation is to know the risk associated with each range of quantity agreed on. Currently Managers assess the quantity to be supplied based on a binary way of either full or 0 supply, This paper aims to assess the corresponding quantities risks of the suppliers on a multilayer basis. The proposed approach uses fuzzy logic as an artificial intelligence tool that would develop the verbal terms of managers into numbers to be dealt with. A company that produces fresh frozen vegetables and fruits in Egypt who faces the problem of getting the required quantities from the suppliers with a fulfilment rate of 33% was chosen to apply the proposed model. The model allowed the managers to have full view of risk in their supply chain effectively and decide their needed capacity as well as the negotiation terms with both suppliers and customers. Future work should be the use of more data in the fuzzy database and implement the proposed methodology in an another industry.

This study evaluated the degree of parasitic contamination of vegetables which are commercialized and consumed fresh in Benha, Egypt. It included 530 vegetables: lettuce, watercress, parsley, green onion, and leek. Vegetables were collected randomly from markets within Benha. Samples were washed in saline, and the resulting washing solution was filtered and centrifuged to concentrate the parasitic stages. Sediments and supernatants were examined by iodine and modified Ziehl-Neelsen stained smears. Intestinal parasites were detected in 157/530 (29.6%) samples. Giardia lamblia cysts were the most prevalent parasite (8.8%) followed by Entamoeba spp. cysts (6.8%), Enterobius vermicularis eggs (4.9%), various helminth larvae (3.6%), Hymenolepis nana eggs (2.8%), Hymenolepis diminuta eggs (2.1%), and Ascaris lumbricoides eggs (0.6%). The highest contaminated vegetable was lettuce (45.5%) followed by watercress (41.3%), parsley (34.3%), green onion (16.5%), and leek (10.7%). Problem solving is a critical skill for computer science. However, the complexity and overhead of most programming tasks make problem solving difficult to explicitly teach. Previous studies describe explicit instruction of problem-solving strategies, but provide few details about how the strategies have been developed. We describe the development of a checklist tool that guides students in solving general programming tasks. The tool was developed by observing problem-solving strategies used by advanced computer science students in practice. We believe that the tool will provide a basis for explicit problem-solving instruction for novice programmers.

Diastolic dysfunction is the early sign of myocardial ischemia that usually occurs earlier than ECG changes.To determine the existence of a correlation between ST segment shift and diastolic dysfunction among patients with AMI.Fourty six patients with significant ST segment elevation or depression and having symptoms of acute myocardial infarction for12 h were enrolled in this study. Patients were examined for serial ECG, cardiac enzymes, and echocardiography.There was no significant correlation between ST segment elevation or depression and majority of the diastolic indices. Few diastolic parameters; such as, E/A ratio of the mitral valve and deceleration time of the tricuspid valve showed a direct correlation with the ECG changes. Whereas, the Tei Index of the LV function showed a borderline correlation to the ST segment elevation at discharge.Overall, there was no correlation between either ST segment elevation or depression and the LV or RV diastolic function in patients with acute coronary syndrome (ACS). Improvement of the ST segment total score was associated with improvement of the diastolic function grades at discharge. Moreover, this association has shown an evidence of dose response relationship, the more improvement in total score at discharge the more improvement in diastolic function grade level.

13. Comparative study between PCR and microscopic examination in diagnosing Entamoeba histolytica

Differentiation between E. histolytica and non pathogenic species as Entamoeba dispar, which are morphologically identical species, is essential for rapid treatment decision, precaution of the invasive disease and public health. This study evaluated the real time PCR for detection of intestinal amoebiasis in comparison with microscopic examination. Stool samples were obtained from symptomatic 40 patients from outpatient clinic of Benha University Hospitals and Benha Educational Hospital. Twenty suffered from dysentery and 20 cases were not suffering dysentery. In addition 10 stool samples of other parasitic infection as Giardia lamblia and Cryptospordium parvum were examined by direct smear, iodine stained smear, formol ether concentration technique and real time PCR for detection of E. histolytica DNA. Formol ether concentration technique showed that 20 (40%) samples were positive for E. histolytica but real time PCR showed 26 (52%) positivity. All samples positive by microscopy were also positive by PCR and additional 6 PCR positive cases. There was no cross reaction with other parasites as G. lamblia and C. parvum.

In the most of the drilling operations a considerable amount of money is spent for drilling related problems; including stuck pipe, lost circulation, and excessive mud cost. In order to decrease the percentage of non-productive time (NPT) caused by these kind of problems, the aim is to control annular frictional pressure losses especially in the fields where pore pressure and fracture pressure gradient is too close which is called narrow drilling window. By solving these problems, drilling cost will fall, therefore enabling the industry to be able to drill wells that were previously uneconomical. Managed pressure drilling (MPD) is a new technology that enables a driller to more precisely control annular pressures in the wellbore to prevent these drilling related problems. As the industry remains relatively unaware of the full spectrum of benefits, this paper involves the techniques used in Managed Pressure Drilling with an emphasis upon revealing several of its lesser known and therefore less appreciated applications.
