Following first-line treatment, second-line options are significantly limited. Classically, first-line treatment consisted of either gemcitabine or 5-fluorouracil based systemic chemotherapy. Upon progression of disease or recurrence, subsequent second-line treatment is still gemcitabine or 5-fluorouracil based chemotherapy, depending on what was used in the first line and the timing of progression or recurrence. A better understanding of the molecular underpinnings of pancreatic adenocarcinoma (PDAC) has led to new treatment strategies including specifically targeting the desmoplastic stroma, cytokine signaling and actionable mutations. Furthermore, efforts are also directed to enhance the immunogenicity profile of PDAC’s well-established immunologically “cold” tumor microenvironment. More recently, the outstanding response rates of chimeric antigen receptor T (CAR-T) cells in hematologic malignancies, have led to clinical trials to evaluate the treatment modality in PDAC. In this review, we summarize recently presented clinical trials for metastatic pancreatic adenocarcinoma with novel treatment approaches in the second line and beyond.Keywords: pancreatic adenocarcinoma, immunotherapy, CAR-T, KRAS

يهدف هذا البحث إلى تسليط الضوء على دور الغرافين في مقاومة التآكل الحاصل في البرونز المصنف وفق DIN1705, والمستخدم في تصنيع شفرات المضخات على نطاق واسع,حيث تم ترسيب طبقة من أكسيد الغرافين المحضر بطريقة هومر(Hummer's method) على سطح المعدن باستخدام تقنية الترسيب الكهركيميائي, و تم التآكد من طبقة التغطية عن طريق جهاز انعراج الأشعة السينيةXRD)),و المجهر الالكتروني (SEM), تم اجراء اختبار التآكل بالغمر على المعدن ضمن وسط يحتوي على محلول هيبوكلوريد الصوديوم,وتم حساب معدل التآكل الناتج عن النقصان في الوزن ,ومقارنة النتائج بعينة غير مغطاة,حيث كانت النتائج تشير إلى زيادة مقاومة التآكل للعينة المغطاة بأكسيد الغرافين على حساب العينة الأخرى,مما يوضح أهمية أوكسيد الغرافين كمقاوم للتآكل.

A 26-year-old male presented to the hospital with refractory diabetic ketoacidosis and multiple electrolyte abnormalities, 9 days after the first dose of brentuximab vedotin and nivolumab for recurrent classical Hodgkin lymphoma. During his hospitalization, he developed multi-organ failure. His workup showed elevated cytokine levels concerning severe cytokine release syndrome (CRS) and hemophagocytic lymphohistiocytosis (HLH)-like syndrome. Despite treatment with CRS- and HLH-directed therapies, his clinical status deteriorated due to ongoing multifactorial shock and worsening multi-organ dysfunction, and comfort care measures were eventually pursued. To our knowledge, there have been no other cases reported of HLH-like syndrome after the combination of BV and nivolumab in patients with cHL. This case of a fatal adverse event following one dose of BV and nivolumab underscores the vital need for close monitoring of patients receiving this treatment regimen.Keywords: immunotherapy, immune-related adverse event, antibody drug conjugate, CRS

جمعت أزهار نبات العرن المثقوب (Hypericum Perforatum.L) الذي ينتمي إلى (جنس العرن, الفصيلة العرنية), من منطقتين مختلفتين في طرطوس: منطقة القدموس (900m) ومنطقة العنازة (450m), تم استخلاص الزيت العطري منها بطريقة التقطير البخاري باستخدام جهاز كليفينجر (Clevenger), حيث بلغت النسبة الوزنية للزيت العطري المستخلص 0.39% , 0.26% لكل من أزهار القدموس و العنازة على التوالي. تم تحديد التركيب الكيميائي للزيت العطري المستخلص من أزهار نبات العرن باستخدام تقنية الكروماتوغرافيا الغازية المقترنة مع طيف الكتلة GC\MS. أظهرت نتائج التحليل أن الزيت العطري المستخلص من أزهار منطقة القدموس يحتوي على 26 مركباً والتي شكلت نسبة 99.98% من إجمالي الزيت العطري, وأن المكون الرئيسي هو: exo -6-carbonyl methoxy tricyclo [2.2.1.0(2,7)] heptane, بنسبة 21.89%. أظهرت نتائج التحليل أن الزيت العطري المستخلص من أزهار منطقة العنازة يحتوي على 55 مركباً والتي شكلت نسبة 99.96% من إجمالي الزيت العطري, وأن المكون الرئيسي هو: Trispiro(2.1.2.1.2.1)dodecane-4,8,12-trione, بنسبة 11.38%. الكلمات المفتاحية: نبات العرن المثقوب, الفصيلة العرنية, حشيشة القلب, عشبة القديسين, عشبة جون, الزيت العطري, GC/MS.

Objective: To present a case of adrenocorticotropic hormone (ACTH) hypersecretion caused by a metastatic acinic cell carcinoma (AcCC) of the parotid. Only 6 cases have been reported prior to October 2019. We believe that this condition is under-reported and hope that improved recognition will improve its reporting. Methods: Diagnosis in this case was done using surgical pathology of the primary tumor, involving lymph nodes, and a metastatic lesion. Following an initial misdiagnosis, a final diagnosis of AcCC was made using immunohistochemical staining. ACTH hypersecretion was diagnosed by testing for random ACTH, cortisol, and 24-hour urine aldosterone and cortisol levels. Results: A 57-year-old man presented with hypokalemia, lower-extremity edema, and left-side rib pain 7 months following excision of a 4-cm left-parotid tumor. Immunostaining positive for DOG-1, CK7, pan-cytokeratin (including CAM5.2), and SOX10 led to the diagnosis of AcCC. ACTH hypersecretion was diagnosed based on a random ACTH level of 307 pg/mL (normal morning value, 7.2-63 pg/mL), a cortisol level of 33 μg/dL (normal morning value, 4.3-19.8 μg/dL; normal PM value, 3.1-15.0 μg/dL), a 24-hour urine aldosterone level of

New derivative N-(3-methyl, 5-isothiazolyl) maleimide have been Synthesized via Diels- Alder Reaction between maleimide with each of furan and 2,3-di methyl butadiene. The structure of synthesized compound have been established on the basic of their spectral (IR, 1HNMR, elemental analysis) data. Maleimide was treated with furan to give two isomers were (endo),(exo). The proportion of each isomer has been different dependence reaction temperature. The thermodynamic properties of reaction have been determined using the theoretical Methods CBS-4m, B3PW91, MPW1PBE and MPW1PW91. The all theoretical methods results have been accorded with the experimental method results .

The objective of this research was to study the effect of plant extracts on wheat seeds during storage periods (3, 6, 12 and 18 months). Plant extracts used were: Azadirachta indica (Neem), Ocimum basilicum L. (Basil) and Datura stramonium (Datura) at concentrations of 8%, 10% and 12% concentrations. The results showed that increasing storage time after harvest to 3, 6, 12 and 18 months significantly decreased the mean germination percentage, seedling parameter [root length, shoot length and seedling dry weight], relative density (R.D), seedling vigour index (S.V.I), field emergence (F.G), field survival (F.S), relative field emergence (R.F.E) and relative field survival (R.F.S).On the other hand, the decline in germination percentage was associated with the increase in electrical conductivity (E.C) value, acidity and moisture content. The maximum germination percentage, seedling parameters [root length, shoot length and seedling dry weight] and R.D were recorded in the treatment where neem extract was applied at 10% concentrations. However, the treatment with neem extract (8%) concentration gave the lowest viability by increasing EC value and acidity percentage. [ABSTRACT FROM AUTHOR]
