2. New Medical Device and Therapeutic Approvals in Otolaryngology: State of the Art Review of 2022.

Abstract

OBJECTIVE: To review new drugs and devices relevant to otolaryngology approved by the Food and Drug Administration (FDA) in 2022. DATA SOURCES: Publicly available FDA data on drugs and devices approved in 2022. REVIEW METHODS: A preliminary screen was conducted to identify drugs and devices relevant to otolaryngology. A secondary screen by members of the American Academy of Otolaryngology-Head and Neck Surgerys (AAO-HNS) Medical Devices and Drugs Committee differentiated between minor updates and new approvals. The final list of drugs and devices was sent to members of each subspecialty for review and analysis. CONCLUSION: A total of 1251 devices and 37 drugs were identified on preliminary screening. Of these, 329 devices and 5 drugs were sent to subspecialists for further review, from which 37 devices and 2 novel drugs were selected for further analysis. The newly approved devices spanned all subspecialties within otolaryngology. Many of the newly approved devices aimed to enhance patient experience, including over-the-counter hearing aids, sleep monitoring devices, and refined CPAP devices. Other advances aimed to improve surgical access, convenience, or comfort in the operating room and clinic. IMPLICATIONS FOR PRACTICE: Many new devices and drugs are approved each year to improve patient care and care delivery. By staying up to date with these advances, otolaryngologists can leverage new innovations to improve the safety and quality of care. Given the recent approval of these devices, further studies are needed to assess long-term impact within the field of otolaryngology.

ObjectiveTo evaluate new medical devices and drugs pertinent to otolaryngology-head and neck surgery that were approved by the Food and Drug Administration (FDA) in 2021.Data sourcesPublicly available FDA device and drug approvals from ENT (ear, nose, and throat), anesthesia, neurosurgery, plastic surgery, and general surgery FDA committees.Review methodsFDA device and therapeutic approvals were identified and reviewed by members of the American Academy of Otolaryngology-Head and Neck Surgery's Medical Devices and Drugs Committee. Two independent reviewers assessed the relevance of devices and drugs to otolaryngologists. Medical devices and drugs were then allocated to their respective subspecialty fields for critical review based on available scientific literature.ConclusionsThe Medical Devices and Drugs Committee reviewed 1153 devices and 52 novel drugs that received FDA approval in 2021 (67 ENT, 106 anesthesia, 618 general surgery and plastic surgery, 362 neurosurgery). Twenty-three devices and 1 therapeutic agent relevant to otolaryngology were included in the state of the art review. Advances spanned all subspecialties, including over-the-counter hearing aid options in otology, expanding treatment options for rhinitis in rhinology, innovative laser-safe endotracheal tubes in laryngology, novel facial rejuvenation and implant technology in facial plastic surgery, and advances in noninvasive and surgical treatment options for obstructive sleep apnea.Implications for practiceFDA approvals for new technology and pharmaceuticals present new opportunities across subspecialties in otolaryngology. Clinicians' nuanced understanding of the safety, advantages, and limitations of these innovations ensures ongoing progress in patient care.

Two independent reviewers assessed the drug's or device's relevance to otolaryngology, classified to subspecialty field, with a critical review of available scientific literature.ConclusionsThe Medical Devices and Drugs Committee reviewed 53 new therapeutics and 1094 devices (89 ENT, 140 anesthesia, 511 plastic and general surgery, and 354 neurology) approved in 2020. Ten drugs and 17 devices were considered relevant to the otolaryngology community. Rhinology saw significant improvements around image guidance systems; indications for cochlear implantation expanded; several new monoclonal therapeutics were added to head and neck oncology's armamentarium; and several new approvals appeared for facial plastics surgery, pediatric otolaryngology, and comprehensive otolaryngology.Implications for practiceNew technologies and pharmaceuticals offer the promise of improving how we care for otolaryngology patients. However, judicious introduction of innovations into practice requires a nuanced understanding of safety, advantages, and limitations. Working knowledge of new drugs and medical devices approved for the market helps clinicians tailor patient care accordingly.

Abstract

Genetic polymorphisms within the IFNL3/IFNL4 genomic region, which encodes type III interferons, have been strongly associated with clearance of hepatitis C virus. We hypothesized that type III interferons might be important for the immune response to other pathogens as well. In a cohort of 914 Malian children, we genotyped functional variants IFNL4-rs368234815, IFNL4-rs117648444, and IFNL3-rs4803217 and analyzed episodes of malaria, gastrointestinal, and respiratory infections recorded at 30,626 clinic visits from birth up to 5 years of age. Compared to children with the rs368234815-TT/TT genotype (IFN-λ4-Null), rs368234815-dG allele was most strongly associated with an earlier time-to-first episode of gastrointestinal infections (p = 0.003). The risk of experiencing an infection episode during the follow-up was also significantly increased with rs368234815-dG allele, with OR = 1.53, 95%CI (1.13-2.07), p = 0.005 for gastrointestinal infections and OR = 1.30, 95%CI (1.02-1.65), p = 0.033 for malaria. All the associations for the moderately linked rs4803217 (r2 = 0.78 in this set) were weaker and lost significance after adjusting for rs368234815. We also analyzed all outcomes in relation to IFN-λ4-P70S groups. Our results implicate IFN-λ4 and not IFN-λ3 as the primary functional cause of genetic associations with increased overall risk and younger age at first clinical episodes but not with recurrence or intensity of several common pediatric infections.

47. Looking beyond COVID-19: Arts Education Policy Implications and Opportunities

Abstract

In this concluding article for the special issue on COVID-19 and K-12 arts education, the authors look forward to some positive possible changes for the delivery of arts education as students return to school past COVID-19 and will address some of the barriers to these positive changes, including policy opportunities and implications. The barriers discussed are not new to arts education -- resource and funding challenges, a focus on remediation pulling students from arts classes, and a continued inequitable access to arts education for students in poverty and students of color. The article will discuss how the pandemic has exacerbated some of these long-standing challenges, as well as ways to address those challenges from a policy perspective as well as through advocacy work at the federal, state, and local levels. The article suggests ways to rethink how we approach areas of educational policy, particularly the accountability structures of public education, in order to address inequitable access now and in the long term.

48. Cognitive Control as an Underpinning of Relational Abstraction and Reasoning from Diagrams

Abstract

Diagrams are powerful opportunities for grappling with and learning abstract relationships, for example learning the relations between elements in an ecosystem rather than simply memorizing the objects within the system. Further, what is crucial from any diagrammatic learning opportunity is the ability to use this relational knowledge in a new context or with new materials, beyond simply understanding the initial presentation. This is cognitively effortful, however, and individual differences in how reasoners benefit from such relational learning opportunities are not well understood. We describe a computational simulation that examines how cognitive control of attention enables relational learning from visual stimuli such as diagrams. Specifically, we propose that cognitive control is critical for both abstracting relational representations from that visual stimuli, and to the ability to use these representations in subsequent problem solving. 49. Individual Differences in Relational Learning and Analogical Reasoning: A Computational Model of Longitudinal Change

Abstract

Children's cognitive control and knowledge at school entry predict growth rates in analogical reasoning skill over time; however, the mechanisms by which these factors interact and impact learning are unclear. We propose that inhibitory control is critical for developing both the relational representations necessary to reason and the ability to use these representations in complex problem solving. We evaluate this hypothesis using computational simulations in a symbolic connectionist model of analogical thinking, DORA/LISA (Discovery Of Relations by Analogy; Doumas, Hummel, & Sandhofer, 2008). Longitudinal data from children who solved geometric analogy problems repeatedly over six months show three distinct learning trajectories though all gained somewhat: analogical reasoners throughout, non analogical reasoners throughout, and transitional - those who start nonanalogical and grew to be analogical. Varying the base level of lateral inhibition in DORA affected the ability to learn relational representations, which, in conjunction with lateral inhibition levels used in LISA during reasoning, simulated accuracy rates and error types seen in the three different learning trajectories. These simulations suggest inhibitory control may not only impact reasoning ability, but may also shape the ability to acquire relational knowledge given reasoning opportunities.
