18. Understanding the effect of a participatory intervention with women's groups to improve maternal and neonatal health in rural Nepal

Abstract

Background: Rigorous evaluation of community based interventions is necessary to build the evidence base for maternal and child survival in developing countries. Randomised controlled trials may be the most reliable method of determining effectiveness of interventions, but they are unable to evaluate the impact of context and implementation, and explain how an outcome occurred. This thesis explores the outcome of a cluster randomised controlled trial using women’s groups in Nepal. Intervention areas experienced a 30% reduction in neonatal mortality, and increases in good care behaviours. I describe how communities experienced the intervention and explore how the implementation process and community context affected the community response. Methods: I used qualitative research methods, purposefully sampling stakeholders from two intervention areas, and one control area. Photoelicitation was used, and research assistants conducted semi-structured interviews, group interviews, focus group discussions, and observations. Data were tape recorded, transcribed and translated into English. Data were analysed using framework and NVIVO qualitative analysis software. Data were also fed back to respondents to increase validity. Results: Themes emerging from the data revealed a lack of trust in local health facilities, plurality in care seeking during illness, and a positive attitude towards working with non governmental organisations. Women’s groups created a learning forum for group members, and this knowledge was disseminated in communities. Strategies to address problems were of particular significance in enabling dissemination of information and enabling community participation. The intervention increased social networks and built community capacity. Discussion: The effect of context on the intervention is considered in order to assess the generalisability of the intervention. Potential mechanisms of effect are also discussed which help to explain the trial outcome. This thesis supports calls for the integration of concurrent process evaluations within randomised controlled trials, and contributes to the evidence for community-based interventions for maternal and newborn survival.

Abstract

Globally, researchers have been struggling with the ethical and practical questions of implementing community-based population-level research interventions during the Covid-19 pandemic. The safety of researchers and participants while implementing the interventions, and the need to avoid overburdening a health system already under pressure, are priorities, but it is also important to understand community perspectives on research. This article analyses community opinions about restarting a population-level community-based intervention for the prevention and control of type 2 diabetes in rural Bangladesh in a population of 120,000. We used qualitative phone interviews, discussions with key stakeholders and consultation with a trial steering committee. Community members were able and interested to attend intervention group meetings, but Covid-19 preventative measures were not being implemented routinely. People with diabetes did not perceive themselves to be at particular risk from Covid-19. Community members were meeting in groups and felt safe among friends and neighbours, but they did not feel safe among ‘outsiders’. As a result of our community engagement, we restarted the intervention but controlled the size of the groups and screened group members and group facilitators for Covid-19 symptoms. Abstract

Anaemia in pregnancy is a persistent health problem in Nepal and could be reduced through nutrition counselling and strengthened iron folic acid supplementation programmes. We analysed 24-hour diet recall data from 846 pregnant women in rural plains Nepal, using linear programming to identify the potential for optimised food-based strategies to increase iron adequacy. We then conducted qualitative research to analyse how anaemia was defined and recognised, how families used food-based strategies to address anaemia, and the acceptability of optimised food-based strategies. We did 16 interviews of recently pregnant mothers, three focus group discussions with fathers, three focus group discussions with mothers-in-law and four interviews with key informants. Dietary analyses showed optimised diets did not achieve 100 % of recommended iron intakes, but iron intakes could be doubled by increasing intakes of green leaves, egg and meat. Families sought to address anaemia through food-based strategies but were often unable to because of the perceived expense of providing an 'energy-giving' diet. Some foods were avoided because of religious or cultural taboos, or because they were low status and could evoke social consequences if eaten. There is a need for counselling to offer affordable ways for families to optimise iron adequacy. The participation of communities in tailoring advice to ensure cultural relevance and alignment with local norms is necessary to enable its effectiveness. [ABSTRACT FROM AUTHOR]

Abstract

Introduction People with disabilities have been disproportionately affected by the COVID-19 pandemic and there is a need to understand the diversity of experience globally to plan and implement responses. The experience of children and young people with disabilities is often overlooked as research focuses on younger children or adults, and they are not often engaged in the design of research methods to capture their experience. As part of a research project to develop participatory methods with children and young people with disabilities in rural Nepal to research their experience of COVID-19 (PARDAN), we sought to map the literature about experiences of children and young people with disabilities in emergency and COVID-19 contexts, and describe the methods used. This review will inform the development of our methods and will help identify gaps in the literature. Methods We will use Arksey and O’Malley’s (1) and Levac et al’s (2) framework to search for grey and published literature. We will search Scopus, Web of Science, ASSIA, Jstor, Source, Google Scholar, Academia.edu, Researchgate, and Google. We will also search websites of organisations working in emergencies such as United Nations High Commissioner for Refugees (UNHCR), World Health Organisation (WHO), United Nations Children's Fund (UNICEF), Humanity and Inclusion (HI), CBM International, International Disability Alliance (IDA), Women’s Refugee Commission (WRC), and National Federation of the Disabled Nepal (NFDN). We will approach our advisory committee and personal contacts in disabled persons organisations in the global south to help us locate additional relevant grey literature. Predetermined inclusion and exclusion criteria will be applied by five reviewers to determine study eligibility. Studies published in English and Nepali and between May 2011 and May 2021 will be reviewed. Methods and findings from literature will be extracted and a descriptive summary will be presented to an advisory committee of disabled persons organisations in Nepal. Discussion Through this review we will identify research gaps and develop implications of our findings which will be presented in a webinar and in publications. The review will be an important resource for those seeking to conduct research about children and young people with disabilities and will inform a global inclusive response to the pandemic which includes the perspectives of children and young people.
