Abstract

Background: Contraceptive use has complex effects on sexual behaviour and mood, including those related to reduced concerns about unintended pregnancy, direct hormonal effects and effects on endogenous sex hormones. We set out to obtain robust evidence on the relative effects of three contraceptive methods on sex behaviours, which is important for guiding contraceptive choice and future contraceptive developments. Methods: This is a secondary analysis of data from the Evidence for Contraceptive Options and HIV Outcomes (ECHO) randomized trial in which 7,829 HIV-uninfected women from 12 sites in Eswatini, Kenya, South Africa and Zambia seeking contraception were randomly assigned to intramuscular depot-medroxyprogesterone acetate (DMPA-IM), the copper intrauterine device (Cu-IUD) or the levonorgestrel (LNG) implant. Data collected for 12 to 18 months using 3-monthly behavioural questionnaires that relied on recall from the preceding 3 months, were used to estimate relative risk of post-baseline sex behaviours, as well as sexual desire and menstrual bleeding between randomized groups using modified Poisson regression. Results: We observed small but generally consistent effects wherein DMPA-IM users reported lower prevalence of specified high risk sexual behaviours than implant users than Cu-IUD users (the '>' and '<' symbols indicate statistically significant differences): multiple sex partners 3.6% < 4.8% < 6.2% respectively; new sex partner 3.0% < 4.0% <5.3%; coital acts 16.45, 16.65, 17.12 (DMPA-IM < Cu-IUD); unprotected sex 65% < 68%, 70%; unprotected sex past 7 days 33% <36%, 37%; sex during vaginal bleeding 7.1%, 7.1% < 8.9%; no sex acts 4.1%, 3.8%, 3.4% (DMPA-IM > Cu-IUD); partner has sex with others 10% < 11%, 11%. The one exception was having any sex partner 96.5%, 96.9% < 97.4% (DMPA-IM < Cu-IUD). Decrease in sexual desire was reported by 1.6% > 1.1% >0.5%; amenorrhoea by 49% > 41% >12% and regular menstrual pattern by 26% <35% < 87% respectively. Conclusions: These findings suggest that women assigned to DMPA-IM may have a modest decrease in libido and sexual activity relative to the implant, and the implant relative to the Cu-IUD. We found more menstrual disturbance with DMPA-IM than with the implant (and as expected, both more than the Cu-IUD). These findings are important for informing the contraceptive choices of women and policymakers and highlight the need for robust comparison of the effects of other contraceptive methods as well. 15. Endoscopic electrolytic ablation : feasibility and safety in the treatment of pancreatic pathologies

Abstract

Pancreatic cancer has a dismal prognosis. It is a condition that tends to present late meaning that the majority of patients are only suitable for palliative therapy. This research aims to assess the feasibility and safety of using electrolysis, a locally ablative technique, as a potential tool in the treatment of pancreatic cancer and other pancreatic mass lesions either independently or as an adjunct to other modalities. Experimental work investigated the use of pancreatic electrolytic ablation, both open and endoscopic, in a porcine model. Phase 1a: short term follow-up open pancreatic electrolysis. Twelve female white domestic pigs underwent laporotomy, duodenotomy and open pancreatic duct cannulation. Six received pancreatic electrolysis, six were used as controls. All animals were followed up for 72 hours and then killed. Phase 1b: medium and long term follow up open pancreatic electrolysis. Eight female white domestic pigs underwent open pancreatic electrolytic ablation. Four animals were killed at two weeks, the remaining at eight weeks. Phase 2: medium and long term follow up endoscopic pancreatic electrolysis. Fifteen female white domestic pigs all had endoscopic pancreatic duct cannulation. Ten pigs underwent endoscopic pancreatic electrolytic ablation and the other five pigs were used as controls. Five treatment animals and the five control animals were killed at two weeks, the remaining five animals at eight weeks. All animals tolerated the procedure and had returned to normal diet by day three. There were no significant (p>0.05) differences between the treatment and control groups with respect to biochemical parameters or cytokine concentrations by the end of the study period. Histological assessment of pancreata demonstrated significantly (p<0.05) increased inflammatory changes in all the treatment groups when compared with controls.

16. Academic Coping Styles, Self-Concept, and Stress.

Abstract

College students employ a number of cognitive strategies to help them cope with stress and anxiety. Some students expect to do well and have done well in the past (optimists), while others have done well in the past but expect to perform poorly on future tasks (defense-pessimists). Such coping strategies have been presumed to cushion self-esteem from threatening situations. This study questioned the assumption that coping strategies primarily cushion global self-esteem by examining which specific components or facets of self-concept differentiate optimists from defensive-pessimists. The relative strengths of 13 components of self-concept were compared between groups of college students (N=209) living on campus at a northwestern university, utilizing distinct coping strategies. The results indicated that strategies may have differential costs in the form of levels of stress. This study provides a new view of the dynamics and implications of coping styles used in academic situations. Rather than assume that coping styles are cushioning strategies for "self-esteem" per se, it is just as likely that they have more specificity and cushion weak or poorly developed facets of the self. Regarding stress, indeed defensive-pessimists and those without consistent coping strategies showed themselves to be more stressed that optimists. 22. Identification and validation of a multi-assay algorithm for cross-sectional HIV incidence estimation in populations with subtype C infection

Abstract

Introduction: Cross-sectional methods can be used to estimate HIV incidence for surveillance and prevention studies. We evaluated assays and multi-assay algorithms (MAAs) for incidence estimation in subtype C settings. Methods: We analysed samples from individuals with subtype C infection with known duration of infection (2442 samples from 278 adults; 0.1 to 9.9 years after seroconversion). MAAs included 1-4 of the following assays: Limiting Antigen Avidity assay (LAg-Avidity), BioRad-Avidity assay, CD4 cell count and viral load (VL). We evaluated 23,400 MAAs with different assays and assay cutoffs. We identified the MAA with the largest mean window period, where the upper 95% confidence interval (CI) of the shadow was 1000 copies/mL), and two MAAs previously optimized for subtype B settings. We compared these cross-sectional incidence estimates to observed incidence in an independent longitudinal cohort. Results: The optimal MAA was LAg-Avidity 400 copies/mL. This MAA had a mean window period of 248 days (95% CI: 218, 284), a shadow of 306 days (95% CI: 255, 359), and provided the most accurate and precise incidence estimate for the independent cohort. The widely used LAg algorithm had a shorter mean window period (142 days, 95% CI: 118, 167), a longer shadow (410 days, 95% CI; 318, 491), and a less accurate and precise incidence estimate for the independent cohort. Conclusions: An optimal MAA was identified for cross-sectional HIV incidence in subtype C settings. The performance of this MAA is superior to a testing algorithm currently used for global HIV surveillance. Abstract

The offshore Karoo Supergroup (+2 km thick) of the Seychelles has been divided into five members based mainly on sandstone/mudstone development as revealed by wireline logs and lithological samples. The four lower members are characterized by thick sandstones with minor mudstones and the lowest member distinguished by the presence of occasional bands of lignite; the topmost member has thick mudstones as well as thick sandstones. Members 2 and 3, and 4 and 5 form two upward-fining megacycles in which members 2 and 4 have coarser-grained pebbly, sandstones while the sandstones of 3 and 5 are medium and fine-grained. The sediments are fluvial in origin, possibly braid plain except for the uppermost member which may have formed under a meandering system. Paleogeographic reconstruction invokes a source area in north-east Madagascar shedding debris eastwards towards the Seychelles which at that time (pre-mid-Jurassic) lay between Madagascar to the west, and India to the east. Burial curves are presented from considerations of sediment thickness and texture, structure, including the positions of unconformities and diagenesis as an aid in assessing oil potential. With possible source rocks in overlying marine sediments, hydrocarbon generation may have occurred in late Mesozoic and early Tertiary. Migration could possibly have taken place into fault-juxtaposed Karoo sandstones. For comparative purposes petrographic features of samples from the Elk Point Group Alberta, Canada were studied. This Devonian sequence comprises marginal and marine sediments including carbonates and evaporites, but the sandstones studied come from marginal deltaic deposits. Sandstones studied are remarkably similar in both original composition and diagenetic history. Primary mineralogy was determined by similar source areas - predominantly granitic with minor metamorphic and volcanic components. Despite being deposited in different sedimentary environments both groups show extensive diagenesis involving dissolution of feldspars and lithoclasts, precipitation of quartz, and a second phase of dissolution of feldspar, lithoclasts and quartz. Kaolinite is the dominating clay in the sandstones of Karoo Supergroup up to depths of 10,000 ft, while illite prevails over other clays in the sequence between 10,000-12,500 ft. Illite and chlorite are present in the Elk Point Group: kaolinite is absent. The last stage of cementation in Elk Point sandstones was anhydrite precipitation during Cretaceous times. Precipitation of authigenic cements ended with the formation of carbonates in Karoo sandstones. Cementation, especially by quartz appears to have considerably reduced porosity in many places in both the successions. In both the study areas early diagenesis was controlled by meteoric waters while burial and marine influence are responsible for later diagenetic episodes. The presence of illite and chlorite against kaolinite in the Elk Point Group is due to greater depths of burial than Karoo sandstones. Presence of evaporitic environments lead to the formation of anhydrite in the sandstones of Elk Point Group, while feldspar overgrowths are formed as a result of retention of pore-fluids due to the formation of a closed system controlled by marine flooding surfaces.

Abstract

: Introduction: Global guidelines emphasize the ethical obligation of investigators to help participants in HIV‐endpoint trials reduce HIV risk by offering an optimal HIV prevention package. Oral pre‐exposure prophylaxis (PrEP) has increasingly become part of state‐of‐the‐art HIV prevention. Here we describe the process of integrating oral PrEP delivery into the HIV prevention package of the Evidence for Contraceptive Options and HIV Outcomes (ECHO) Trial. Methods: ECHO was an open‐label randomized clinical trial that compared HIV incidence among women randomized to one of three effective contraceptives. In total, 7830 women aged 16 to 35 years from 12 sites in four African countries (Eswatini, Kenya, South Africa and Zambia) were enrolled and followed for 12 to 18 months, from 2015 to 2018. Part‐way through the course of the trial, oral PrEP was provided to study participants either off‐site via referral or on site via trained trial staff. PrEP uptake was compared between different contraceptive users using Chi‐squared tests or t‐tests. HIV seroincidence rates were compared between participants who never versus ever initiated PrEP using exact Poisson regression. Results: PrEP access in ECHO began through public availability in Kenya in May 2017 and was available at all sites by June 2018. When PrEP became available, 3626 (46.3%) eligible women were still in follow‐up in the study, and of these, 622 (17.2%) initiated PrEP. Women initiating PrEP were slightly older; more likely to be unmarried, not living with their partner, having multiple partners; and less likely to be earning their own income and receiving financial support from partners (all p < 0.05). PrEP initiation did not differ across study randomized groups (p = 0.7). Two‐thirds of PrEP users were continuing PrEP at study exit. Conclusions: There is a need for improved HIV prevention services in clinical trials with HIV endpoints, especially trials among African women. PrEP as a component of a comprehensive HIV prevention package provided to women in a large clinical trial is practical and feasible. 38. 'What Would You Do, What If It's You?' Strategies to Deal with a Bully

Abstract

Bullying can be defined as a repeated negative interpersonal relationship characterized by an imbalance of power targeted at a person that is perceived to be weaker or more vulnerable, without apparent provocation. When bullying is not addressed, the effects are substantial. Bullying affects the targets (anxious, afraid, and failure to concentrate at school), witnesses (guilt, helplessness, and fear), and those who participate in bullying (leads to other antisocial/delinquent behaviors). Types of bullying include physical (hitting, tripping, and shoving), verbal (namecalling, taunts, teasing, and roasting), psychological (excluding, rumors, gossip, bossing, and threatening), ostracism based on sexual orientation, sexual harassment (taunts, touching, coercion, and slurs), and cyberbullying (through Internet, e-mail, and cell phone). In cases of school violence, the attackers felt bullied, persecuted, or injured by others prior to the attack. One aspect of creating a safe school environment is increasing student efficacy and reducing all forms of bullying. This article presents a lesson that was created to empower students to address bullying situations. In groups, the students will work to decide what they could do if they see bullying to others and then decide what they would do if they were being bullied. This article also presents activities and strategies to deal with a bully. (Contains 3 tables.)

39. Music Listening as Music Making

Abstract

One of the most fundamental, ongoing debates in music education involves scholars who argue for a "performance-based" curriculum and those who promote a curriculum that is fundamentally "aesthetic-listening based." Rather than arguing for the primacy of either performance or listening as a basis for a music education curriculum, the paper attempts to raise the profile of the musical listening process "tout a fait"--that is, not aesthetic listening in particular, but all forms of engaged listening. The author explores David Elliott's rationale for favoring performance over listening as well as his reasons for failing to include listening in his concept of "musicing." In connection with Elliott's notion of musical listening as a form of "thinking musically," the paper examines Matthew Lipman's work on higher-order thinking, mental acts and states, and his view of thinking as performance. The connections between listening and thinking, thinking and performing, and, ultimately, listening and performing are explored. The paper concludes with an argument that engaged music listening is "itself" a form of creative activity, that it "is" in fact a complex and bona fide mode of "making" music, and that such listening "may well" have some important implications for music education. Married veterinarians were surveyed about work satisfaction, work-related stress, marital-family stress, and spousal support for their career. Female veterinarians reported greater effect of martial/family stress on career and less perceived support than did their male counterparts.
