The exercise heart rate (HR) profile and its relation to cardiac function and arrhythmias were investigated in patients with hypertrophic cardiomyopathy (HC). Chronotropic response (CR) and heart rate recovery (HRR) were computed during and after treadmill exercise testing in 273 patients with HC and 95 age-matched healthy controls. Patients with HC had higher prevalence of chronotropic incompetence and lower HRR1e5min compared with controls. Exercise capacity, diastolic function (assessed by E/e') and left atrial volume index were associated with HRR1min and CR in HC. Septal myectomy was associated with reduction in chronotropic incompetence but did not affect HRR1min. In conclusion, impaired CR and HRR1min are associated with advanced disease and do not appear to be independent clinical markers indicating high-risk status in HC. Improving CR by titrating doses of negative chronotropic agents, myectomy, and atrial pacing may be useful to increase exercise capacity in patients with HC. [ABSTRACT FROM AUTHOR]

Objective: To identify risk factors to pre-surgical complications in patients with cystic echinococcosis (CE) in endemic areas of Peru., Materials and Methods: Study of cases (n=43) and controls (n=127) not paired was designed, with a case-control ratio of 1:3 based on a secondary source. The case was defined as the patient with CE who presented with pre-surgical complications related or not with the disease and the control without pre-surgical complication. The response variable was preoperative complication and the exposition ones were age, gender, degree of instruction, occupation, number, size (diameter in centimeters) and location of the cyst. To identify the risk factors, a bivariate logistic regression analysis was performed to obtain the crude odds ratios (cOR) and then the variables with significant association were adjusted by a multivariate logistic regression analysis using the Stata® version 14 software, obtaining theadjusted odds ratios (aOR)., Results: Fifty eight point eight percent (24/43) of the cases were women and the median age was 35 years and 59.1% (75/127) of the controls correspond to the female gender and the median age was 27 years. The multivariate logistic regression analysis found that being over 56 years old has an aOR 2.91 (95% confidence interval [CI]: 1.007-8.433) and having ≥3 cysts has an aOR 4.26 (95% CI: 1.62-11.22) for present pre-surgical complications in patients with CE., Conclusion: It was identified that being over 56 years old and having ≥3 cysts are risk factors for presenting pre-surgical complications in CE.

Objetives.: Our aim was to evaluate the psychometric properties of the FACES-III among Peruvian high school students., Materials and Methods.: This is a psychometric cross-sectional study. A probabilistic sampling was applied, defined by three stages: stratum one (school), stratum two (grade) and cluster (section). The participants were 910 adolescent students of both sexes, between 11 and 18 years of age. The instrument was also the object of study: the Olson's FACES-III. The analysis included a review of the structure / construct validity of the measure by factor analysis and assessment of internal consistency (reliability)., Results.: The real-cohesion scale had moderately high reliability (Ω=.85) while the real-flexibility scale had moderate reliability (Ω=.74). The reliability found for the ideal-cohesion was moderately high (Ω=.89) like for the scale of ideal-flexibility (Ω=.86). Construct validity was confirmed by the goodness of fit of a two factor model (cohesion and flexibility) with 10 items each [Adjusted goodness of fit index (AGFI) = 0.96; Expected Cross Validation Index (ECVI) = 0.87; Normed fit index (NFI) = 0.93; Goodness of fit index (GFI) = 0.97; Root mean square error of approximation (RMSEA) = 0.06]., Conclusions.: FACES-III has sufficient reliability and validity to be used in Peruvian adolescents for the purpose of group or individual assessment.

Objectives.: To assess the implementation of an intervention based on the sending of text messages and e-mails to internal medicine residents in order to achieve an increase in the prescription of thromboprophylaxis according to Clinical Practice Guidelines (CPG)., Materials and Methods.: A quasi-experimental study was conducted in the inpatient units of internal medicine at two general hospitals in Lima, Peru: one selected to implement the intervention and the other as control. The unit of analysis was the therapeutic order sheet. It was considered that the intervention should increase the order of thromboprophylaxis according to CPG with respect to the initial assessment and also in relation to the control hospital., Results.: Atotal of 394 therapeutic order sheets, 227 (57.4%) from the intervention hospital and 167 (42.4%) from the control, were evaluated. In the intervention hospital, 64.5% initially had an order of thromboprophylaxis according to CPG, which decreased to 54.4% after the intervention; overuse increased from 13.6 to 26.8%., Conclusions.: Intervention based on sending text messages and e-mails had no impact, showing a tendency to overuse of thromboprophylaxis post-intervention.
