Resumo: Introdução: A avaliação do estudante deve induzir aprendizagem e ser baseada em competência, ou seja, avaliar (habilidades cognitivas, psicomotoras e afetivas). Para avaliar conhecimento e a habilidade para sua utilização no contexto profissional, o Teste de Progresso (TP) tem sido usado em larga escala, com finalidade somativa e principalmente formativa. Objetivo: Este estudo teve como objetivo verificar a adequação e qualidade de itens que compõem os TP realizados pelos estudantes. Método: Trata-se de estudo exploratório descritivo e retrospectivo que analisou todos os itens de seis provas do TP aplicado a estudantes de Medicina do primeiro ao sexto ano da Faculdade de Medicina de Ribeirão Preto/USP, no período de 2013 a 2018. Os sete indicadores de boas práticas foram: 1. abordar tema relevante na formação médica; 2. ter enunciado maior que as alternativas; 3. avaliar aplicação do conhecimento; 4. definir pergunta clara para o item no enunciado; 5. avaliar apenas um domínio do conhecimento em cada item; 6. ter resposta correta e distratores homogêneos e plausíveis; 7. ausência de erros no item que acrescentam dificuldade desnecessária ou dão pistas da resposta correta. Dois avaliadores independentes analisaram as questões e, quando necessário, revisavam em conjunto os itens discordantes. Resultado: A análise das provas permitiu identificar boa qualidade técnica na maioria dos itens das seis provas, além de indicar que a não adesão foi mais frequente nos indicadores 4 e 5, que podem comprometer tanto a validade quanto a interpretação dos resultados da prova em termos de lacunas do conhecimento por parte dos estudantes. Conclusão: A qualidade das questões das provas analisadas é muito boa, mas foi possível identificar oportunidades de melhoria no processo de elaboração de itens, que servem de base para o desenvolvimento docente dos elaboradores da instituição. Abstract: Introduction: Assessment drives learning and should follow a competence-based approach. The Progress Test (PT) has been used on a large scale for summative and mainly formative purposes to assess knowledge and the ability to use it in the professional context. Objective: To check the adequacy and quality of the items and that make up the progress tests sat by students. Method: Descriptive and retrospective exploratory study that analyzed all the items of six PT exams applied to medical students from the first to the sixth year of the Faculty of Medicine of Ribeirão Preto/USP, from 2013 to 2018. The seven indicators of good practices were: 1. Addresses a relevant topic in medical training; 2. Statement longer than key answer and distractors; 3. Application of knowledge evaluated; 4.Clear lead-in defined for the item in the statement; 5. Only one domain of knowledge assessed in each item; 6.Plausible and homogeneous key answer and distractors; 7.Absence of flaws that add unnecessary difficulty or give clues to the correct answer. Two independent evaluators analyzed the items and, if necessary, they jointly reviewed any disagreement. Result: The analysis showed a good technical quality of most items in the six PT exams. In addition, they indicated that non-adherence was a bit more frequent for indicators 4 and 5, which can compromise both the validity and the interpretation of the test results in terms of knowledge gaps on the part of students. Conclusion: In general, the quality of the items was very good but there are some opportunities for improvement in the process of item writing based on faculty development within the institution.

Background: Seasoning is one of the recommended strategies to reduce salt in foods. However, only a few studies have studied salt preference changes using seasoning. Objectives: The aim of this study was to compare preference for salty bread, and if seasoning can change preference in hypertensive and normotensive, young and older outpatients. Methods: Outpatients (n = 118) were classified in four groups: older hypertensive subjects (OH) (n = 32), young hypertensive (YH) (n = 25); older normotensive individuals (ON) (n = 28), and young normotensive (YN) (n = 33). First, volunteers random tasted bread samples with three different salt concentrations. After two weeks, they tasted the same types of breads, with seasoning added in all. Blood pressure (BP), 24-hour urinary sodium and potassium excretion (UNaV, UKV) were measured twice. Analysis: Fisher exact test, McNamer’s test and ANCOVA. Statistical significance: p < 0.05. Results: Systolic BP, UNaV, and UKV were greater in HO and HY and they had a higher preference for saltier samples than normotensive groups (HO: 71.9%, HY: 56% vs. NO: 25%, NY; 6%, p

Aging is accompanied by changes in body composition, including a decrease the free fat mass (FFM) and a progressive increase in the fat mass (FM). The objective this study was to report the body composition in elderly active women, using the anthropo­metric and deuterium oxide methods, and to analyze the concordance between them, the last one being considered the gold standard. Twenty two non-dependent elderly women aging 65-75 years old were evaluated. Body weight was assessed using a digital scale (Filizola) and height was measured with a vertical bar stadiometer. The level of physical activity was evaluated using the international physical activity questionnaire (IPAQ-long version). Body composition was assessed by anthropometry, using the equations of Jackson et al. and Durnin and Womersley, and by the deuterium oxide (2H2O). Statistical analysis was done using the Lin concordance correlation coefficient and the Bland and Altman graphs. Mean age was 69.3 ± 3.6 years, weight 67.2 ± 10.6 kg, height 1.55 ± 0.04 m and body mass index 27.9 ± 5.0 kg/m2. The concordance correlation coefficient obtained by the equations of Jackson et al. and Durnin and Womersley, compared with the deuterium were: %FM 0.72 and 0.71; FM 0.90 and 0.91; and FFM 0.46 and 0.57. The equations used in this study showed good concordance between the anthropometry and deuterium, but the equation of Durnin and Womersley showed better results in the assessment of body composition in elderly active women. Com o envelhecimento, ocorrem alterações na composição corporal, observando­-se uma redução da massa magra (MM) e um aumento progressivo da massa gorda (MG). O objetivo deste estudo foi descrever a composição corporal de mulheres idosas ativas, pelos métodos de antropometria e óxido de deutério e verificar a concordância do método antropométrico com o método óxido de deutério, considerado como referência nesse estudo. Participaram do estudo 22 idosas independentes, com faixa etária entre 65 a 75 anos. O peso corporal foi avaliado usando balança digital e a altura usando um estadiômetro em barra vertical. Para identificar o nível de atividade física foi usado o questionário internacional de atividade física (IPAQ, versão longa). A composição corporal foi avaliada pela antropometria pelas equações de Jackson et al. e Durnin e Womersley e pelo método de óxido de deutério (2H2O). Para análise estatística, usaram-se o coeficiente de concordância de Lin e o gráfico de Bland e Altman. A média de idade foi 69,3±3,6 anos, o peso 67,2±10,6Kg, a altura 1,55±0,04m e o índice de massa corporal 27,9±5,0 kg/m2. Os coeficientes de concordância obtidos pelas equações de Jackson et al. e Durnin e Womersley comparados ao deutério foram: %GC 0,72 e 0,71; MG 0,90 e 0,91; e MM 0,46 e 0,57. As equações utilizadas neste estudo apresentaram boa concordância com o deutério, sendo que, a equação de Durnin e Womer­sley apresentou melhores resultados para avaliar a composição corporal de idosas ativas.

Embora a relevância de se estudar a composição corporal de idosos já esteja bem definida, são escassos os estudos que se propuseram a analisar a composição corporal de idosas fisicamente ativas. Esse estudo se propôs avaliar a composição corporal (percentual de gordura corporal %GC, massa gorda MG e massa magra MM) de idosas ativas, pelos métodos absorciometria de duplo-fóton (DXA) e antropométrico e verificar a concordância entre os mesmos, considerando o DXA como referência. Participaram do estudo 22 idosas com idade entre 65 a 80 anos. A composição corporal foi avaliada pelos métodos de antropometria (utilizando as equações de Jackson et al. e Durnin e Womersley) e DXA. As idosas foram consideradas ativas segundo o questionário internacional de atividade física (IPAQ). Para análise estatística usou-se o coeficiente de concordância de St. Laurent e o gráfico de Bland e Altman. A média de idade foi 69,3 (3,6) anos, o peso 67,2 (10,6) Kg, a altura 1,55 (0,04) m e o IMC de 27,9(5,0) kg/m2. Os coeficientes de concordância das equações de Jackson et al. e Durnin e Womersley, comparados ao DXA foram: (%GC 0,71 e 0,77; MG 0,89 e 0,92; e MM 0,56 e 0,72), respectivamente. Portanto, as equações antropométricas utilizadas, apresentaram de moderada a quase perfeita concordância com o DXA, sendo que, a equação de Durnin e Womersley foi a que apresentou melhores resultados para avaliar % GC, MG e MM em idosas fisicamente ativas.

OBJECTIVES: To characterize the geriatric population under dialysis in a specialized center. To identify factors possibly involved with death risk in this population. METHODS: Sixty two volunteers, (83% of the older patients of that center, six were excluded) were submited to the application of the Mini-Mental State Examination (MMSE), the Clock-Drawing test (CDT), the Verbal Fluency test and the Geriatric Depression Scale (GDS). Biochemical tests were also performed. After 12 months, the patients' files were reassessed as to mortality and associated factors data. RESULTS: Volunteers alive after 12 months were younger (68.18 ± 5.7 years) than those that died (74.06 ± 7.36 years), p = 0.001. Male gender was predominant (60.7%). In alive and deceased groups, the results for blood tests were, respectively: parathormone 369.87 ± 318.90 and 368.50 ± 501.47 (p > 0.05); creatinine 6.89 ± 2.05 and 5.23 ± 1.15 (p = 0.002); albumine 3.66 ± 0.35 and 3.74 ± 0.644 (p > 0.05); total cholesterol 165.55 ± 45.16 and 169.35 ± 38.68 (p > 0.05); triglycerides 144.57 ± 80.95 and 126.94 ± 77.71 (p > 0.05); HDL 38.27 ± 14.26 and 44.22 ± 10.78 (p > 0.05); calcium 9.40 ± 0.77 and 9.14 ± 0.68 (p > 0.05); KT/V 1.64 ± 0.32 and 1.63 ± 0.27 (p > 0.05). MMSE differed between groups (p = 0.03) and creatinine showed to be a potential death-related factor. GDS had similar scores between the two groups (p = 0.06). CONCLUSIONS: MMSE scores were directly associated with mortality in older patients submitted to hemodialysis. The correlation between lower serum creatinine level and mortality is possibly caused by a worse nutritional status of those patients.

Benign Paroxysmal Positional Vertigo is the most common peripheral vestibular disorder, especially in the elderly and presents as the predominant etiology in this population of the degeneration of the utricular macula.Aim: To compare the effectiveness of the approaches after Epley maneuver. Study Design: longitudinal cohort.Materials and Methods: The study included 53 volunteers with Benign Paroxysmal Positional Vertigo of the posterior semicircular canal, divided into Group 1, who underwent Epley maneuver associated with the use of neck collar and post-maneuver instructions, Group 2 underwent the Epley maneuver without the use cervical collar and/or post-maneuver restrictions, and Group 3 underwent the Epley maneuver associated with the use of a mini vibrator, without the use of neck collar and/or post-maneuver restrictions.Results: In the three groups, the number of Epley maneuvers ranged from one to three. We employed the Brazilian Dizziness Handicap Inventory – pre-and post-treatment and observed a statistically significant difference on most scores pre-and post-treatment for both groups.Conclusion: Regardless of the post Epley maneuver treatment selected for the treatment of Benign Paroxysmal Positional Vertigo, it was effective when comparing the Brazilian Dizziness Handicap Inventory pre-and post-treatment.

Orientadores: Sandra Cecilia Botelho Costa, Maria Elena Guariento Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciencias Medicas Resumo: Há crescente interesse no entendimento da evolução e da eventual prevenção da Fragilidade, bem como do processo fisiopatológico das doenças desencadeante desta síndrome de importante repercussão funcional. Pesquisas têm demonstrado que a infecção crônica latente por CMVH na população geronte apresenta alta prevalência em idosos frágeis sendo um importante desencadeante de limitações, mas poucos artigos são encontrados ao buscar associação entre esta síndrome e infecção ativa por CMVH. Objetivos: Detectar e comparar a incidência de infecção ativa por CMVH em idosos que apresentem critérios de Fragilidade, utilizando Real-Time -PCR para CMVH, e verificar a correlação entre estes critérios de Fragilidade e a positividade para CMVH. Metodologia: trata-se de um estudo transversal com 55 idosos, acompanhados no Ambulatório de Geriatria e em outros ambulatórios do Hospital de Clínicas da Unicamp, sendo utilizados 04 dos 05 critérios de Fragilidade proposto por Fried e colaboradores (2001); avaliou-se a presença de infecção ativa por CMVH através do Real-Time PCR e posteriormente fez-se à análise dos prontuários desses pacientes. Resultados: o número de pacientes por cada critério de Fragilidade e as médias de medicamentos em uso foram concordantes com a literatura. Relacionando critérios para Fragilidade com o número de medicamentos em uso e número de enfermidades por pacientes, observou-se associação positiva entre esses fatores. Quanto ao número de medicamentos em uso, houve prevalência do sexo feminino; as quatro principais categorias de enfermidades classificadas pelo CID-10, apresentadas pelos pacientes incluídos no estudo foram: doenças do aparelho circulatório (76,47%), doenças endócrinas (47,05%), doenças do sistema osteomuscular e do tecido conjuntivo (45,09%) e doenças metabólicas (33,33%). A reação de amplificação em tempo real das 55 amostras para detecção do CMVH foi negativa em todas amostras, caracterizando a ausência de atividade do CMVH nesta população estudada. Conclusão: mesmo havendo importante associação entre citomegalovirose latente e Síndrome da Fragilidade, neste estudo não foi possível verificar a presença de infecção ativa do CMVH e sua relação com os critérios de Fragilidade estudados; achamos necessária a realização de novos estudos para elucidação da provável associação entre infecção ativa por CMVH e critérios de Fragilidade Abstract: There is growing interest in understanding the evolution and possible prevention of Frailty, as well as the pathophysiological process of diseases that trigger this syndrome with important functional consequences. Research has shown that chronic latent HCMV in the elderly population is highly prevalent in the frail elderly and is an important trigger of limitations, but few articles are found to seek an association between this syndrome and active infection by HCMV. Objectives: To determine and compare the incidence of HCMV active infection in the elderly presenting Frailty's criteria, using Real-Time PCR for HCMV, and verify the correlation between these criteria of Frailty and positivity HCMV. Methodology: This is a cross-sectional study with 55 individuals, followed at the Department of Geriatrics and other outpatient clinics of the Hospital de Clinicas da Unicamp, being used 04 of the 05 Frailty's criteria proposed by Fried and colleagues (2001), we evaluated the presence of active infection through Real-Time PCR and subsequently became the analysis of records of these patients. Results: The number of patients for each criterion of Frailty and the averages of medications were consistent with the literature. Frailty's criteria relating to the number of medications and number of illness for patients, we observed a positive association between these factors. The number of medications in use were females prevalence, and the four main categories of diseases classified by ICD-10, presented by the patients included in the study were: cardiovascular diseases (76.47%), endocrine disorders (47, 05%), diseases of the musculoskeletal system and connective tissue (45.09%) and metabolic diseases (33.33%). The amplification reaction in real time from 55 samples for detection of HCMV was negative in all samples, characterizing the absence of HCMV activity in this population.
