Orientador: Lucy Seki Tese (doutorado) - Universidade Estadual de Campinas, Instituto de Estudos da Linguagem Resumo: A tese apresenta uma descrição da fonologia e gramática da língua Aguaruna (família lingüística Jívaro), falada por 45 mil pessoas que habitam a região norte de Amazônia Peruana. O trabalho vem estruturado em seis capítulos, a bibliografia e um apêndice contendo um vocabulário básico e mapa das aldeias aguarunas. O capítulo 1, introdutório, é dedicado a informações gerais sobre a nação e a língua Aguaruna, incluindo discussão sobre as possíveis origens dos etnônimos Jívaro e Aguaruna, e sobre a classificação do Jívaro no contexto das classificações lingüísticas de autores como Brinton (1891), Greenberg (1960, 1987), Mason (1950), Payne (1981) e Kaufman (1991). Esse capítulo inclui ainda: 1) descrição da situação sócio-econômica e da localização mais específica dos falantes, 2) apresentação da metodologia de trabalho de campo e os objetivos da tese. O capítulo 2 trata da fonologia da língua: os fonemas (vogais e consoantes), estrutura da sílaba, características do acento e principais processos fonológicos como: alongamento, desvozeamento, queda e nasalização de vogais, queda e desnasalização de consoantes. Discute-se também a relação entre a consoante nasal velar /f)/ e a fricativa glotal nasalizada [h]. No capítulo 3 são apresentadas as características tipológicas da língua Aguaruna, principalmente a ordem de constituintes, e são definidos alguns conceitos gramaticais como Frase, Oração, Sintagma, Palavra, Tema e Raiz, todos eles em relação à língua. O capítulo 4 trata dos processos de flexão e derivação e as características morfológicas e sintáticas relacionadas com as categorias Nome, Adjetivo, Advérbio, Pronome e Partículas. Os verbos, seus processos de flexão e derivação e suas categorias, são tratados no capítulo 5. Aims: While the associations of health-related quality of life scores in heart failure (HF) [e.g. the Kansas City Cardiomyopathy Questionnaire (KCCQ)] with clinical outcomes are well established, their interpretation in the context of what magnitudes of change are clinically important to patients is less clear. The main objective of this study was to correlate the changes in the KCCQ and Patient Global Assessment (PGA) in patients with HF with reduced ejection fraction (HFrEF) to determine minimal clinically important difference (MCID). Methods and results: We analysed data from 459 patients of the FAIR-HF trial. Both KCCQ and PGA were assessed at 4 and 24 weeks after enrolment. An anchor-based approach was used to calculate MCID at week 4 and 24. PGA was chosen as the clinical anchor against which changes in the KCCQ scores were calibrated. For each category of change in PGA, the corresponding differences were calculated by the mean scores of various domains of KCCQ along with 95% confidence intervals (CIs). There was fair correlation between PGA and changes in overall summary scores (OSS) (r = 0.31; P < 0.001), clinical summary scores (CSS) (r = 0.36; P < 0.001) and physical limitation scores (PLS) (r = 0.31; P < 0.001) from baseline to week 4. KCCQ OSS, CSS and PLS MCID for ‘little improvement’ at week 4 were 3.6 (1.0–6.2), 4.5 (1.8–7.2) and 4.7 (1.4–8.0) points, respectively. OSS, CSS and PLS MCID for ‘little improvement’ at week 24 were 4.3 (0.2–8.4), 4.5 (0.5–8.5) and 4.0 (−0.9–9.0) points, respectively. Conclusion: The MCID threshold for KCCQ score was generally consistent and numerically lower than the threshold of 5-point change considered for clinical outcome prognosis and were stable between 4 and 24 weeks. This suggests that even changes smaller than the traditional 5-point improvements in KCCQ may be clinically meaningful. Also, these results can aid in the clinical interpretation of patient-reported outcomes, and better endpoint selection in future studies. © 2020 European Society of Cardiology

Tomosynthesis (DBT) has proven to be more sensitive than digital mammography, but it requires longer reading time. We retrospectively compared accuracy and reading times of a simplified protocol with 1-cm-thick slabs versus a standard protocol of slabs + 1-mm-spaced planes, both integrated with synthetic 2D. We randomly selected 894 DBTs (including 12 cancers) from the experimental arm of the RETomo trial. DBTs were read by two radiologists to estimate specificity. A second set of 24 cancers (8 also present in the first set) mixed within 276 negative DBTs was read by two radiologists. In total, 28 cancers with 64 readings were used to estimate sensitivity. Radiologists read with both protocols separated by a 3-month washout. Only women that were positive at the screening reading were assessed. Variance was estimated taking into account repeated measures. Sensitivity was 82.8% (53/64, 95% confidence interval (95% CI) 67.2–92.2) and 90.6% (95% CI 80.2–95.8) with simplified and standard protocols, respectively. In the random screening setting, specificity was 97.9% (1727/1764, 95% CI 97.1–98.5) and 96.3% (95% CI 95.3–97.1), respectively. Inter-reader agreement was 0.68 and 0.54 with simplified and standard protocols, respectively. Median reading times with simplified protocol were 20% to 30% shorter than with standard protocol. A simplified protocol reduced reading time and false positives but may have a negative impact on sensitivity. • The adoption of digital breast tomosynthesis (DBT) in screening, more sensitive than mammography, could be limited by its potential effect on the radiologists’ workload, i.e., increased reading time and fatigue. • A DBT simplified protocol with slab only, compared to a standard protocol (slab plus planes) both integrated with synthetic 2D, reduced time and false positives but had a negative impact on sensitivity.

Aims: Iron deficiency worsens symptoms, quality of life, and exercise capacity in chronic heart failure (CHF) and might do so by promoting fluid retention. We assessed whether iron repletion improved congestion in CHF and appraised the prognostic utility of calculated plasma volume status (PVS), a novel index of congestion, in the FAIR-HF data set. Methods and results: In FAIR-HF, 459 iron deficient CHF patients were randomized to intravenous ferric carboxymaltose (FCM) or saline and assessed at 4, 12, and 24 weeks. Using weight and haematocrit, we calculated PVS in 436 patients. At baseline, PVS and weight were −5.5 ± 7.7% and 76.9 ± 14.3 kg, with peripheral oedema evident in 35% of subjects. Higher PVS values correlated to other congestion surrogates such as lower serum albumin. At 4 weeks, FCM was associated with greater reductions in weight (0.02) and PVS (P −4% at baseline predicted worse outcomes even after adjustment for treatment assignment (hazard ratio 1.88, 95% confidence interval 1.01–3.51, 0.046). Conclusions: Intravenous iron therapy with FCM is associated with early reductions in PVS and weight, implying that decongestion might be one mechanism via which iron repletion aids CHF patients. O estudo aplicou a Técnica Multivariada de Análise de Correspondência Múltipla para avaliar a existência de associação entre o retorno econômico da atividade leiteira e os indicadores de produtividade, composição de rebanho, custo de produção, receita bruta e assistência técnica. Os dados são provenientes de um questionário estruturado aplicado em 374 propriedades leiteiras de Minas Gerais, Brasil. Concluiu-se que o retorno econômico superior na produção de leite, expresso pela margem bruta por área de terra, está associada a maior produção de leite por vaca, maior número de vacas por área de terra e maior percentual de vacas no rebanho, que por sua vez está associado ao maior tempo de assistência técnica recebida pelo produtor rural. Ainda que o custo de produção operacional por animal tenha sido superior quando houve aumento de produtividade, a receita bruta por animal suplantou o custo, resultando em margem bruta superior. A assistência técnica do produtor rural é um mecanismo importante para capacitar o produtor rural e levar conhecimento sobre novas tecnologias, o que resulta em ganhos de produtividade no campo. A técnica de Análise de Correspondência Múltipla mostrou-se adequada para aplicação e tomada de decisão em análises do setor agropecuário. Made available in DSpace on 2019-08-15T00:44:03Z (GMT). This study presents floristic inventories and the description of the vascular vegetation of fourteen small granitic islands around surrounding Lavezzu Island (Bouches-de-Bonifacio Natural Reserve) in Southern Corsica. These islets differ in the following characteristics : total area, area of flat surfaces, maximum altitude, shape, distance to Lavezzu Island, degree of exposure to storms, and number of breeding gull pairs. The number of vascular plant species ranges from 2 (South Becchi Island) to 50 (Achiarinu Island). Their plant communities can be classifiaed into eight phytosociological alliances (Allion triquetri, Asplenion marini, Crithmo maritimi-Limonion articulati, Dactylido hispanicae-Daucion commutati, Halimionion portulacoidis, Juniperion turbinatae, Molinio arundinaceae-Holoschoenion vulgaris, Saginion maritimae). The multivariate analyzes indicate the importance of the "flat surface area" of each island as the main explanatory parameter of the floristic richness and the number of phytosociological alliances., Cette étude présente les inventaires floristiques et la description de la végétation vasculaire de quatorze îlots granitiques entourant l’île Lavezzu (Réserve naturelle des Bouches-de-Bonifacio) en Corse-du-Sud. Ces îlots diffèrent par les caractères suivants : superficie totale, superficie des surfaces planes, altitude maximale, forme, distance à l’île Lavezzu, degré d’exposition aux tempêtes et quantité de couples de goélands nicheurs. Le nombre d’espèces de plantes vasculaires varie de 2 (îlot Becchi sud) à 50 (îlot de l’Achiarinu). Leurs groupements végétaux sont classables dans huit alliances phytosociologiques (Allion triquetri, Asplenion marini, Crithmo maritimi-Limonion articulati, Dactylido hispanicae-Daucion commutati, Halimionion portulacoidis, Juniperion turbinatae, Molinio arundinaceae-Holoschoenion vulgaris, Saginion maritimae).
