Abstract Glioblastoma (GB) is a devastating tumor of the central nervous system characterized by a poor prognosis. One of the best-established predictive biomarker in IDH-wildtype GB is O6-methylguanine-DNA methyltransferase (MGMT) methylation (mMGMT), which is associated with improved treatment response and survival. However, current efforts to monitor GB patients through mMGMT detection have proven unsuccessful. Small extracellular vesicles (sEVs) hold potential as a key element that could revolutionize clinical practice by offering new possibilities for liquid biopsy. This study aimed to determine the utility of sEV-based liquid biopsy as a predictive biomarker and disease monitoring tool in patients with IDH-wildtype GB. Our findings show consistent results with tissue-based analysis, achieving a remarkable sensitivity of 85.7% for detecting mMGMT in liquid biopsy, the highest reported to date. Moreover, we suggested that liquid biopsy assessment of sEV-DNA could be a powerful tool for monitoring disease progression in IDH-wildtype GB patients. This study highlights the critical significance of overcoming molecular underdetection, which can lead to missed treatment opportunities and misdiagnoses, possibly resulting in ineffective therapies. The outcomes of our research significantly contribute to the field of sEV-DNA-based liquid biopsy, providing valuable insights into tumor tissue heterogeneity and establishing it as a promising tool for detecting GB biomarkers. These results have substantial implications for advancing predictive and therapeutic approaches in the context of GB and warrant further exploration and validation in clinical settings.

Brazilian jalap root, Operculina hamiltonii (G. Don) D.F. Austin & Staple, Convolvulaceae, belongs to the morning glory family and is used as a purgative or laxative medicinal plant. After hydrolysis and peracetylation of the EtOH-soluble extract, the known operculinic acids A and B and turpethic acid C, in addition to three undescribed glycosidic acids, operculinic acids L-N with a tetrasaccharide or pentasaccharide moieties with unusual 12-hydroxy fatty acid aglycones of different chain lengths (C17 and C18), were isolated from a commercial sample of pulverized roots by preparative recycling HPLC. Analysis of sixteen samples, including crude drugs and commercial phytopharmaceuticals based on jalap root, led to the generation of distinctive chromatographic profiles for each sample. The major isolated glycosidic acids were used as diagnostic peaks for HPLC-ESIMS fingerprinting by selected ion monitoring mode using target ions at [M + Na]+.

The aim was to identify effective surgical treatments for patients with rheumatoid arthritis in the foot and/or ankle. A systematic review of the literature was conducted via a data search of the PubMed, Scopus, Cumulative Index to Nursing and Allied Health Literature (CINAHL), and PROSPERO databases, from their inception until June 2019. Only non-randomized controlled trials and cohort studies were included in this review. Two of the present authors independently assessed the quality of each study and extracted the relevant data. A quality assessment of all articles was performed using the methodological index for non-randomized studies criteria. In addition, the Newcastle Ottawa scale was used for cohort studies. Thirteen studies met the inclusion criteria (five cohort studies and eight NRCTs). The total population considered was 923 patients (570 patients had RA), with a mean age of 58.8 years. Regarding the risk of bias, both the NRCTs and the cohort studies had a moderate level of quality. Despite the relatively low quality of these studies, surgical treatment for the foot and/or ankle is shown to reduce pain and improve functionality in patients with RA, in the short term (6−12 months).

This thesis seeks to identify and define strategic actions that help to turn water a important way to resolve conflicts and convert it into an indispensable tool for building peace in the region of Urabá Antioqueño. It is based on the premise that there can be no peace in any territory if the population does not have access to drinking water, if the aquifers are contaminated or if their use is limited by the territorial control of agents other than state institutions and the communities that inhabit them. And this is the reason why, after the signing of the Havana Peace Accords in 2016, water is highlighted as a structuring and articulating element of the projects identified by the communities to guarantee peace in the territory. But, what does it mean to live in peace? This work establishes a second premise: peace construction cannot be separated from territorial planning and management. Living in peace means guaranteeing living conditions that provide health and hospital resources, economic opportunities, food security and a physical environment free of threats. With this approach, the different instruments, plans and projects for which water is an essential element for the development of the region are considered, and emphasis is placed on the initiatives prioritized in Planes de Desarrollo con Enfoque Territorial (PDET), taking as a case study El Tres, Turbo - Antioquia. This document is composed of four chapters that build a knowledge route around water. The first chapter analyzes the role of water as a tool for the implementation of the Peace Agreements, in the framework of the creation of the PDETs. The second chapter delves into the geographical and historical characterization of the study area and details the different management and planning instruments in order to develop a methodological roadmap to address the multiplicity of actions and intentions that converge in the same territory. The third chapter is a synthesis of the different components that make up the network of strategies for water management, from the environmental, social and institutional perspectives. Finally, the fourth chapter examines six enabling projects of small and large scale framed in six strategic lines. This journey takes us to a new beginning where water becomes the enabling element of the different initiatives and projects prioritized by the communities within the framework of the Peace Agreements and the articulating element of the different instruments operating in the territory. The work seeks, among other things, to show that such an essential element as water is not only a resource, but the key to the development of those communities seeking to build a stable and lasting peace.

Cobalt toxicity in patients with hip arthroplasty is a rare complication, but it should be considered in those patients who, after a ceramic fracture, were implanted with a metal-on-polyethylene prosthesis. The complete removal of ceramic particles during revision surgery can be complicated. If the bearing surface is replaced with a metal-on-polyethylene prosthesis, these residual ceramic particles may wear down the chrome-cobalt head, producing localised metallosis. This can trigger blood metal ion levels to rise, causing systemic toxicity. Visual and auditory alterations, cognitive deterioration, hypothyroidism, neuropathy, cardiomyopathy, anorexia, fatigue, diabetes, polycythemia, and respiratory and cutaneous symptoms are some of the clinical manifestations of prosthetic cobaltism.A young patient presented with multiorgan failure secondary to cobalt toxicity after a ceramic fracture and revision with a metal-on-polyethylene prosthesis; his serum cobalt and chromium levels were 652 μg/L and 270 μg/L, respectively. The patient needed a heart transplant after presenting with cobalt-induced cardiogenic shock. In a patient with a ceramic fracture who is subjected to revision surgery with a metal-on-polyethylene bearing, it is necessary to rule out the possibility of cobalt intoxication. Serum cobalt levels20 μg/L are inadmissible; in these cases, surgical treatment should be considered in the short term. A wide synovectomy and replacement of components should be performed with hard friction options, preferably with a ceramic-on-ceramic prosthesis.

The regional elections in Catalonia held on 21 December 2017 received wide media coverage, far beyond Spanish media, due to separatist tension in that territory and was one of the main topics in most of the world’s media. Within this process social networks, especially Twitter, obtained crucial relevance given the interest aroused by the political leaders’ publications, since in those elections the debate transcended the usual ideological divisions of right and left and became a struggle between constitutionalists and separatists. This paper analyses the presence and influence of the main candidates of the Catalan political parties on Twitter. To achieve this, a mainly quantitative, mixed methodology based on big data was carried out where all the tweets issued by the candidates during the electoral campaign were analysed.

Background: The influence of vocation on success can be studied through various surveys, but a model is needed that will validate the results of such surveys with a high probability of accurately representing the reality under study. Bayesian networks meet this requirement Purpose: Students who enroll on an engineering degree may have to choose engineering disciplines that differ from their initial choice. In some cases, these differences can undermine their vocational commitment and leave them with the feeling that their final choice was mistaken. This study is intended to gauge engineering student perceptions of success in terms of vocation and to remedy weaknesses that may discernible in their attitudes. Method: Bayesian networks, by choosing a variable, proper graphs, probabilistic calculation and survey validations with the ROC curve (Receiver Operating Characteristic) can identify variable-dependent survey results and filter them accordingly. Results: Issues relating to professional expectations and learning have more influence on students whether they are or are not in the specific field of engineering that reflected their vocation. Success rates increase considerably when students perceive variables such as "find useful work", "find a job easily" or "enjoy learning" in a positive light. Conclusions: What helps students to feel that they are not wrong is to think that they will find employment and that their jobs will make them feel useful. It is therefore essential to talk about professional life from the very first year through to graduation and students should do so believing fully in their choice of degree.

International audience; The main purpose of the present study was to analyse the internal structure and to test the measurement invariance of the Strengths and Difficulties Questionnaire (SDQ), self-reported version, in five European countries. The sample consisted of 3012 adolescents aged between 12 and 17 years (M = 14.20; SD = 0.83). The five-factor model (with correlated errors added), and the five-factor model (with correlated errors added) with the reverse-worded items allowed to cross-load on the Prosocial subscale, displayed adequate goodness of-fit indices. Multi-group confirmatory factor analysis showed that the five-factor model (with correlated errors added) had partial strong measurement invariance by countries. A total of 11 of the 25 items were non-invariant across samples. The level of internal consistency of the Total difficulties score was 0.84, ranging between 0.69 and 0.78 for the SDQ subscales. The findings indicate that the SDQ's subscales need to be modified in various ways for screening emotional and behavioural problems in the five European countries that were analysed.

[EN] The progressive advancement of the family law towards recognition of its autonomy runs parallel to the development of its limits; constituted mainly by the limits of the constitutional principles of equality of the spouses in the marriage and the limits deriving from the configuration of marriage as an institution, covering the mandatory regulations regarding the duties between spouses and the causes of separation. Expressions of autonomy of the will before the marriage crisis include premarital agreements. It is pacts between the spouses reached before and after marriage, but before the crisis and which are conditional, i.e. they depend on the existence of a marital crisis; and prevention, that is, they anticipate problems or conflicts that may arise before the eventual crisis., Tesis Univ. The development of the planning of academic units is an important activity within the undergraduate teaching process defined in the Quality Management System at ESPOL, since the registration process for each academic term depends directly on this planning. Until now, academic planning of the Faculty of Electrical and Computer Engineering (FIEC) has been developed using basic utility programs that do not offer enough support to the dean, area coordinators and career coordinators, in terms of collaboration among them, reference to historical data, editing of class and activity planning, and correction of schedule conflicts. This article presents a software solution based on web technologies and business process management, which increases the productivity of the actors involved in the planning of classes, schedules and activities of teachers in

- Abstract

- Abstract
Operculina hamiltoniiis a vine native to the north and northeast region of Brazil, where its roots are traded as a depurative and laxative remedy with the name of Brazilian jalap in traditional medicine. Procedures for the isolation, purification by recycling HPLC, and structure elucidation of three undescribed resin glycosides are presented herein. Hamiltonin I (1) represents a macrocyclic structure of a tetrasaccharide of (11S)-hydroxyhexadecanoic acid. Additionally, two acyclic pentasaccharides, named hamiltoniosides I (2) and II (3), were also isolated, which are related structurally to the known compounds 4and 5, macrocyclic lactone-type batatinosides. The tetrasaccharide core of 1was diacylated by n-decanoic acid and the unusual n-hexadecanoic acid moiety, while the pentasaccharides 2–5were esterified by one unit of n-decanoic or n-dodecanoic acid. All the isolated compounds were found to be inactive as cytotoxic agents. However, when they were evaluated (1–25 μM) in combination with a sublethal concentration of the anticancer agent vinblastine (0.003 μM), a significant enhancement of the resultant cytotoxicity was produced, especially for multidrug-resistant breast carcinoma epithelial cells. Such combined synergistic potency may be beneficial for chemotherapy, making resin glycosides potential candidates for drug repurposing of conventional chemotherapeutic drugs to reduce their side effects.
