- Abstract
Introducción. Actualmente, el éxito del tratamiento con implantes no solo se consigue con la osteointegración; si no con la respuesta favorable de la restauración definitiva con las estructuras adyacentes, a nivel de tejidos duros y blandos. Los tejidos que rodean los implantes dentales reciben el nombre de tejidos periimplantarios y están formados por tejido blando y tejido duro. La pérdida ósea marginal se considera un parámetro biológico y clínico importante a la hora de evaluar el éxito de las rehabilitaciones fijas sobre implantes. Tras la colocación de implantes dentales comienza un proceso de cicatrización del hueso alveolar, en el que el tejido óseo se adapta a los estímulos mecánicos recibidos mediante un proceso de reabsorción - aposición ósea. La pérdida ósea marginal progresiva puede estar relacionada con factores que influyen o afectan en la remodelación ósea, como el diseño y posición del implante, la técnica quirúrgica empleada, el diseño protésico, factores biomecánicos y biológicos, los antecedentes del paciente y factores sistémicos y locales. Los tejidos periimplantarios están constantemente expuestos a las bacterias del medio oral. La enfermedad periimplantaria aparece debido al desequilibrio entre la carga bacteriana y la defensa del huésped. Dentro de la enfermedad periimplantaria se distinguen la mucositis y la periimplantitis. La mucositis se define como una lesión inflamatoria de la mucosa que rodea a un implante. Presenta una prevalencia de 32-54% y , al igual que la gingivitis, se considera una inflamación reversible. La periimplantitis es una condición patológica que afecta a la mucosa y al tejido óseo que rodea el implante. Se caracteriza por la inflamación de la mucosa periimplantaria y por la pérdida progresiva del hueso de soporte, por lo que se considera irreversible. Presenta una prevalencia del 14-30%. - Abstract
Clinical implant dentistry has evolved significantly during last decades. The development of diagnostic, surgical and prosthetic aspects has improved the predictability of osseointegration and long-term clinical outcomes of the implant treatment. Moreover, several criteria of clinical and radiographic conditions have been incorporated in the concept of implant success, as implant level, peri-implant tissues, prosthetic le vel, and patient satisfaction. The scientific evidence of the literature demonstrates that implant treatment is an excellent option for the rehabilitation of patients with partial and total edentulism. In fact, long-term studies are reported the success of fixed total rehabilitation, overdentures, fixed bridges and single crowns. However, some biological and technical complications are defined the different concepts of success and survival of prosthetic and dental implants. Conclusions: This paper indicates that restorations with dental implants in the rehabilitation of partially and totally edentulous patients constitute a dental treatment with long-term clinical outcomes with a high success.

- Abstract
The influence of the surface topography of dental implants has been studied to optimize titanium surfaces in order to improve osseointegration. Different techniques can be used to obtain rough titanium, however, their effect on wettability, surface energy, as well as bacterial and cell adhe sion and differentiation has not been studied deeply. Two-hundred disks made of grade 4 titanium were subjected to different treatments: machined titanium (MACH), acid-attacked titanium (AE), titanium sprayed with abrasive alumina particles under pressure (GBLAST), and titanium that has been treated with GBLAST and then subjected to AE (GBLAST + AE). The roughness of the different treatments was determined by confocal microscopy, and the wettability was determined by the sessile drop technique; then, the surface energy of each treatment was calculated. Osteoblast-like cells (SaOs-2) were cultured, and alkaline phosphatase was determined using a colorimetric test. Likewise, bacterial strains S. gordonii, S. oralis, A. viscosus, and E. faecalis were cultured, and proliferation on the different surfaces was determined. It could be observed that the roughness of the GBLAST and GBLAS + AE was higher, at 1.99 and 2.13 µm of Ra, with respect to the AE and MACH samples, which were 0.35 and 0.20 µm, respectively. The abrasive treated surfaces showed lower hydrophilicity but lower surface energy. Significant differences could be seen at 21 days between SaOS-2 osteoblastic cell adhesion for the blasted ones and higher osteocalcin levels. However, no significant differences in terms of bacterial proliferation were observed between the four surfaces studied, demonstrating the insensitivity of bacteria to topography. These results may help in the search for the best topographies for osteoblast behavior and for the inhibition of bacterial colonization.

- Abstract
Background: This study aimed to report the outcomes of the immediate loading of implants with fixed rehabilitations in edentulous geriatric patients. Methods: Edentulous geriatric patients were diagnosed with an oral examination, radiographic evaluation, and intermaxillary relations and treated with fixed rehabilitation over several implants. After immediate surgery, the implants were immediately loaded with a fully fixed prosthesis. Results: Twenty-four patients (20 females and 4 males) were treated using a total 210 implants. All patients (100%) had a previous history of periodontitis. Eleven patients (45.8%) were smokers. Eleven patients (45.8%) suffered from chronic medical diseases (i.e., diabetes, cardiovascular diseases). The study’s clinical follow-up period extended for three years, during which thirty-three fixed prostheses were installed over the implants in 24 patients. The average marginal bone loss measured was 1.33 ± 0.17 mm. The success rate of the implants and prosthodontics being placed in this study yielded 98.5% and 97%, respectively. One patient (4.2%) showed some kind of technical complications. Eleven patients (45.8%) showed mucositis, and 25 implants (11.9%) in 10 patients (41.7%) were associated with peri-implantitis. Conclusions: This study shows that the treatment of edentulous geriatric patients by immediate loading of implants with fixed rehabilitations is a clinically successful protocol but with a high prevalence of peri-implant diseases.

- Abstract
Introducción: El objetivo del presente estudio era mostrar los resultados clínicos del tratamiento del maxi lar superior posterior con implantes colocados mediante la técnica de elevación transalveolar del seno maxilar en pacientes geriátricos. Pacientes y Métodos: 24 pacientes geriátricos (15 hombres y 9 mujeres) con pérdidas dentales maxilares fueron tratados con 28 implantes diferentes colocados mediante la técnica de elevación sinusal transal veolar para su posterior rehabilitación prostodóncica. Los implantes fueron cargados después de un perio do de cicatrización de 6 meses. Resultados: Los hallazgos clínicos indican una supervivencia y éxito de los implantes del 96,4%. En un pacien te de los 24 tratados (4,2%) se perdió un implante (3,6%). La altura ósea residual media fué de 6,38 ±1,10 mm (rango:5-10 mm). La ganancia media de hueso vertical fué de 4,40 ±1,15 mm (rango: 2-7 mm). 67,8% de los implantes se insertaron en localización molar y el 32,2% en la localización premolar. Se utilizaron bio materiales en el 97,8% de los implantes. Se realizaron coronas unitarias en 20 pacientes (83,3%), puentes fijos en 3 pacientes (12,5%) y una sobredentadura con bolas en un paciente (4,2%). El seguimiento medio de los pacientes estudiados ha sido de 64,2 ±8,6 meses con un rango entre 50 y 82 meses. Conclusiones: Este estudio indica que el tratamiento con implantes dentales mediante la elevación tran salveolar del seno maxilar superior en pacientes geriátricos, constituye una terapéutica implantológica con una elevada tasa de éxito.

Los implantes fueron cargados inmediatamente en el mismo día de la cirugía. El seguimiento clínico medio fue de 37,1 ±14,6 meses. Resultados: Los hallazgos clínicos indican una supervivencia y éxito de los implantes del 99,5%. Los pacien tes fueron rehabilitados con 33 prótesis totales fijas atornilladas. Complicaciones biológicas fueron observa das en el 50% de los pacientes y las complicaciones prostodóncicas en el 4,2% de los pacientes. La pérdida ósea marginal fue de 1,33± 0,77 mm. La pérdida ósea marginal no estuvo relacionada significativamente con la edad, sexo, hábito de fumar, antecedentes médicos, tipos de prótesis y seguimiento clínico. Conclusiones: Este estudio indica que la rehabilitación fija mediante implantes dentales en pacientes edéntulos totales constituye una terapéutica con éxito con una pérdida ósea marginal frecuente., Introduction: The aim of this study was to report the outcome of marginal bone loss in the treatment of fully edentulous patients with fixed prostheses supported with dental implants. Material & Methods: 24 edentulous patients were treated with 210 KTX ® sandblasted and acid surface implants for fixed maxilla/mandible rehabilitation. Implants were immediately loaded in the same day of the surgery. The mean follow-up was 37.1 ±14.6 months. Results: Clinical results indicate a survival and success rate of implants of 99.5%. One implant was lost during the follow-up period. The patients were restored with 33 screw fixed rehabilitations. Biological com plications were observed in 50% of patients and technical complications in 4.2% of patients. Mean marginal bone loss was 1.33 ± 0. 77 mm. Marginal bone loss are not significantly related with age, gender, smoking, medical background, types of prostheses, and clinical follow-up. Conclusions: This study indicates that fixed rehabilitations by dental implants in totally edentulous patients constitute a successful treatment with a frequent marginal bone loss.

11. Early loading of two implants supporting mandibular overdentures in geriatric edentulous patients: a 12-year follow-up study

- Abstract
Background: This study aims to show the clinical outcomes of implants supporting mandibular overdentures in edentulous patients. Methods: Mandibular edentulous patients were diagnosed with an oral examination, panoramic radiograph, and diagnostic casts for intermaxillary relations and treated with overdentures over two implants. After two-stage surgery, implants were early loaded with an overdenture at 6 weeks. Results: Fifty-four patients (28 females and 24 males) were treated with 108 implants. Thirty-two patients (59.2%) had a previous history of periodontitis. Twenty-three patients (46%) were smokers. Forty patients (74.1%) suffered from systemic diseases (i.e., diabetes, cardiovascular diseases). The clinical follow-up of the study was 147.8 ± 10.4 months. The clinical outcomes showed a global success of 94.5% of implants. Fifty-four overdentures were placed in the patients over the implants. The mean marginal bone loss was 1.12 ± 0.34 mm. Nineteen patients (35.2%) showed some kind of mechanical prosthodontic complication. Sixteen implants (14.8%) were associated with peri-implantitis. Conclusions: Based on the clinical results obtained, we can determine that the treatment of elderly edentulous patients with mandibular overdentures through the early loading of two placed implants is a successful implant protocol.

At the beginning of the COVID-19 pandemic, strict measures of confinement and social distancing were taken. Dentists were considered essential personnel and their activity was restricted to emergency treatment. The present observational study aims to determine the situation of oral implantology practice in Spain during the initial period of the COVID-19 pandemic. This is a crosssectional observational study based on the STROBE (Strengthening the Reporting of Observational Studies in Epidemiology) guidelines. An electronic survey consisting of three blocks of questions was sent to all members of the Spanish Society of Implants. The data were analyzed using descriptive analysis. A total of 237 participants (14.3%) responded to the questionnaire. The majority of participants (60.8%) only attended emergencies during the first 9 months of the pandemic. Despite this, 77.2% reported having performed dental implant surgeries and 75.5% indicated that they performed non-essential treatments. The activity was fully recovered by 64.1% of the surveyed dentists. The majority of dentists (90.7%) considered that sufficient/adequate preventive measures were carried out at their workplace, which possibly contributed to the fact that 49.3% were not afraid of becoming infected. This concern was significantly and directly proportionally associated with the age of the surveyed dentists. The oral implant practice was affected to a greater extent during the first 9 months of the pandemic, especially in urban areas, with a greater impact on the workload of professionals with less specialised training in oral implantology.

- Abstract
Immediate implants present a high risk of early failure. To avoid this, preventive antibiotics (PAs) are prescribed; however, their inappropriate administration leads to antimicrobial resistance. The present study aims to clarify whether the prescription of PAs reduces the rate of early failure of immediate implants and to establish guidelines to avoid the overprescription of these drugs. An electronic search of the MEDLINE database (via PubMed), Web of Science, Scopus, LILACS and OpenGrey was carried out. The criteria described in the PRISMA® statement were used. The search was temporarily restricted from 2010 to 2021. The risk of bias was analysed using the SIGN Methodological Assessment Checklist for Systematic Reviews and Meta-Analyses and the JBI Prevalence Critical Appraisal Tool. After searching, eight studies were included that met the established criteria. With the limitations of this study, it can be stated that antibiotic prescription in immediate implants reduces the early failure rate. Preoperative administration of 2–3 g amoxicillin one hour before surgery followed by 500 mg/8 h for five to seven days is recommended. It is considered prudent to avoid the use of clindamycin in favour of azithromycin, clarithromycin or metronidazole in penicillin allergy patients until further studies are conducted.

- Abstract
Implant dental therapy is a clinical procedure used for treating patients with tooth loss with known clinical success. This clinical study aimed to evaluate the long-term clinical outcomes of dental implants in partially and totally edentulous patients. A total of 544 Microdent (Microdent SU, Implant Microdent System®, Santa Eulàlia de Ronçana Barcelona, Spain) screw implants were placed in 111 patients using a two-stage surgical technique and a conventional loading protocol (lasting 3 months). Implant and prosthetic clinical findings were evaluated during a 15-year follow-up. A total of 6 implants were lost during the healing period, and 124 prostheses were placed over the 538 implants that remained: 20 single crowns, 52 partially fixed bridges, 45 full-arch fixed restorations, and 7 overdentures. A total of 20 of these were lost during the follow-up period. The cumulative survival rate for all implants was 96.4%. The data underwent statistical analysis (significance level: p < 0.05). The mean marginal bone loss was 1.82 ± 0.54 mm, ranging from 1.2 to 3.1 mm. The most frequent complications were mechanical prosthodontic complications (16.2%). In all, 11.8% of implants showed periimplantitis as the primary biological complication. Dental implants inserted in both the maxillary and mandibular areas produce long-term favorable outcomes and stable tissue conditions when a delayed loading protocol is followed.

- Abstract
Current patterns of preventive antibiotic prescribing are encouraging the spread of an timicrobial resistance. Recently, the Spanish Society of Implants (SEI) developed the first clinical practice guidelines published to date, providing clear guidelines on how to prescribe responsible and informed preventive antibiotic therapy (PAT) based on the available scientific evidence on dental implant treatments (DIs). The present document aims to summarise and disseminate the recom mendations established by this expert panel. These were based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. Studies were analysed using the Scottish Intercollegiate Guidelines Network (SIGN) checklist templates and ranked according to their level of evidence. They were then assigned a level of recommendation using the Grading of Recommendations, Assessment, Development and Evaluation system (GRADE). Guidelines were established on the type of PAT, antibiotic and dosage of administration in the placement of DIs without anatomical constraints, in bone augmentation with the placement of DIs in one or two stages, placement of immediate DIs, sinus elevations, implant prosthetic phase, as well as recommendations in patients allergic to penicillin. Therefore, the PAT must be adapted to the type of implant procedure to be performed.

- Abstract
Background. The immediate functional loading of implants is a clinical procedure used for treating periodontal edentulous patients. This clinical study aimed to evaluate the clinical outcomes of the immediate functional loading of implants with fully fixed rehabilitations in compromised periodontal patients. Methods. Three hundred and five implants IPX screw implants were placed in 27 periodontal patients using an immediate functional loading protocol with fixed rehabilitations. All patients had a previous history of periodontitis, four patients (14.8%) were smokers and seven patients (25.9%) suffered from chronic medical conditions. Results. Implant and prosthetic clinical findings were evaluated during a mean period of 41.3 ± 19.6 months. No implants were lost during the clinical follow-up. The cumulative survival rate for all implants was 100%. Regarding the prostheses designed, a total of 54 fixed prostheses were placed in the 27 patients immediately after the surgery. Forty-four hybrid fixed prostheses (81.5%) and 10 fixed rehabilitations (18.5%) were placed in the patients. The mean marginal bone loss was 1.51 ± 1.16 mm, ranging from 0 to 3.5 mm during the follow-up evaluation. Thirty-one implants (10.2%) in 10 patients (37%) were associated with peri-implantitis. Five patients (18.5%) showed some kind of technical complications (loss/fracture of the prosthetic screw, acrylic resin fracture, ceramic chipping). Conclusions. The clinical outcomes of this study demonstrate that fixed rehabilitation by immediate functional loading of implants is considered a predictable procedure.

La plataforma reducida es considerada un factor importante para preservar la estabilidad del hueso crestal y de los tejidos blandos y asegurar el éxito de los implantes dentales a largo plazo. La plataforma redu- cida reduce las fuerzas oclusales y la contaminación bacteriana en la interfase entre el hueso crestal y el implante. Los estudios experimentales en animales y clínicos en pacientes muestran su eficacia para prevenir la pérdida ósea y de los tejidos blandos periimplantarios Conclusiones. La introducción de la plataforma reducida constituye un importante campo en la investi- gación experimental en implantología oral y en el tratamiento clínico con implantes para mantener los tejidos periimplantarios., The evolution of designs of implant systems and abutments configurations has developed the concept of platform-switching that involves the connection of a narrower abutment to the platform implant to allow horizontal distance of the interface bone-implant. Platform-switching is considered an important factor to preserve the stability of crestal bone and soft tissue ensuring the success of dental implants in the long-term follow-up. Platform-switching reduces the forces of occlusal loading and bacterial conta- mination in the interface between the crestal bone and the implant. Experimental studies in animals and clinical studies in patients showed that implants with platform-switching have demonstrated the effecti- veness to prevent peri-implant bone loss and subsequent soft tissue loss. Conclusions. The platform-switching constitute an important field for research of experimental implant dentistry and clinical implant treatment in the maintenance of peri-implant tissues

Background: This study aims to show the clinical outcomes of implants supporting mandibular overdentures in edentulous patients. Methods: Mandibular edentulous patients were diagnosed with an oral examination, panoramic radiograph, and diagnostic casts for intermaxillary relations and treated with overdentures over two implants. After two-stage surgery, implants were early loaded with an overdenture at 6 weeks. Results: Fifty-four patients (28 females and 24 males) were treated with 108 implants. Thirty-two patients (59.2%) had a previous history of periodontitis. Twenty-three patients (46%) were smokers. Forty patients (74.1%) suffered from systemic diseases (i.e., diabetes, cardiovascular diseases). The clinical follow-up of the study was 147.8 ± 10.4 months. The clinical outcomes showed a global success of 94.5% of implants. Fifty-four overdentures were placed in the patients over the implants. The mean marginal bone loss was 1.12 ± 0.34 mm. Nineteen patients (35.2%) showed some kind of mechanical prosthodontic complication. Sixteen implants (14.8%) were associated with peri-implantitis. Conclusions: Based on the clinical results obtained, we can determine that the treatment of elderly edentulous patients with mandibular overdentures through the early loading of two placed implants is a successful implant protocol. - Abstract
The aim of this study was to show the clinical outcomes of the immediate loading of implants inserted by guided surgery in edentulous mandible patients. Edentulous mandible patients were diagnosed with oral examination, cone beam computerized tomography and diagnostic casts for intermaxillary relations and treated with 8–10 implants for rehabilitation with guided surgery and immediate loading. After flapless surgery, implants were loaded with an immediate acrylic temporary prosthesis. After a period of six months, a ceramic definitive full-arch prosthesis was placed. A total of 22 patients (12 females and 10 males) were treated with 198 implants. Eleven patients (50%) had a previous history of periodontitis. Six patients (27.3%) were smokers. The follow-up was 84.2 ± 4.9 months. Clinical outcomes showed a global success rate of 97.5% of implants. Five implants were lost during the healing phase with provisional prosthesis. Twenty-two fixed full-arch rehabilitations were placed in the patients over the 193 remaining implants. Mean marginal bone loss was 1.44 mm ± 0.45 mm. Six patients (27.3%) showed some kind of mechanical prosthodontic complication. Eighteen (9.3%) of the 193 remaining implants were associated with peri-implantitis. The antecedents of peri-implantitis are critical elements for the survival of the implants. The loss of implants was significant in patients who smoked up to 10 cigarettes, compared to non-smokers. Peri-implantitis is one of the key elements in the long-term follow-up of implants and it was more manifest in smoking patients, and in those with a history of peri-implantitis. Marginal bone loss was more significant in smokers. Full-arch rehabilitation is presented as a predictable alternative with minor fatigue problems that are easily solvable.

- Abstract
Introducción. La implantología oral representa una opción importante en el tratamiento de los pacientes con pérdida dental total y parcial. El estudio a largo plazo muestra la evaluación de los pacientes tratados mediante carga precoz de implantes con conexión interna y superficie arenada y grabada. Métodos. 40 pacientes con pérdidas dentales fueron tratados con implantes Frontier GMI ® con conexión interna y superficie arenada y grabada. Los implantes fueron cargados tras un periodo de tiempo de 6 semanas en la mandíbula y 8 semanas en el maxilar superior. Los hallazgos clínicos (implantológicos y prostodóncicos) se han seguido durante al menos 6 años. Resultados. 103 implantes fueron insertados en ambos maxilares, 46 implantes (44,7%) en el maxilar superior y 57 implantes (55,3%) en la mandíbula, para su rehabilitación prostodóncica. 37 implantes (35,9%) fueron insertados en el sector anterior y 66 implantes (64,1%) en el sector posterior. 70 implantes (68%) de forma sumergida (2 cirugías) y 33 implantes (32%) de forma no sumergida (una cirugía). Des pués de un seguimiento clínico de 92,2 meses, los resultados indican una supervivencia de los implantes del 96,2%; ya que se perdieron 4 implantes. La pérdida ósea marginal fué de 0,99 ± 0,84 mm. Las restau raciones prostodóncicas incluyeron 31 coronas unitarias, 15 puentes fijos, 5 sobredentaduras con bolas, 2 rehabilitaciones totales fijas y una rehabilitación completa híbrida. Las complicaciones tardías fueron 11 implantes (10,7%) con periimplantitis y 6 prótesis (11,1%) con complicaciones protésicas. Conclusiones. Los hallazgos clínicos del estudio indican que el tratamiento con carga precoz mediante prótesis de los implantes con conexión interna y superficie arenada y grabada, representa una terapéutica odontológica con éxito., Introduction. Implant dentistry constitute an important option in the prosthodontic treatment of patients with partial and total tooth loss. This long-term study reports the evaluation of patients treated by early loading of implants with internal connection and sandblasted-acidetched surface. Methods. 40 patients with tooth loss were treated with Frontier GMI ® sandblasted and acid-etched surfa ce implants. Implants were loaded after a healing free-loading period of 6 weeks (mandible) and 8 weeks (maxilla). Clinical findings (implants and prosthodontics) were followed during at least 6 years. Results. 103 implants were inserted (46 maxillary (44.7%), and 57 mandibular (55.3%)) for prosthodontic rehabilitation. 37 implants (35.9%) were inserted in anterior sites and 66 implant (64.1%) in posterior sites. Seventy implants (68%) were placed submerged (two stages) while that 33 implants (32%) were placed nonsubmerged (one stage). After at a mean follow-up of 92.2 months, clinical results indicate a survival rate of implants of 96.2%. Four implants were lost during the treatment. Mean marginal bone loss were 0.99 ± 0.84 mm. Prosthodontic restorations included 31 single crowns, 15 fixed bridges, 5 overdentures, 2 fixed totally rehabilitation and an hybrid full rehabilitation. Delayed complications include 11 implants (10.7%) with peri-implantitis and 6 prostheses (11.1%) with technical complications. Conclusions. Clinical results of this study indicate that prosthodontic rehabilitation by early loading of internal connection and sandblasted and etched- implants can be a successful dental treatment.

- Abstract
Titanium surface is an important factor in achieving osseointegration during the early wound healing of dental implants in alveolar bone. The purpose of this study was to evaluate sandblasted-etched surface implants to investigate the osseointegration. In the present study, we used two different types of sandblasted-etched surface implants, an SLA™ surface and a Nanoblast Plus™ surface. Roughness and chemical composition were evaluated by a white light interferometer microscope and X-ray photoelectron spectroscopy, respectively. The SLA™ surface exhibited the higher values (Ra 3.05 µm) of rugosity compared to the Nanoblast Plus™ surface (Ra 1.78 µm). Both types of implants were inserted in the femoral condyles of ten New Zealand white rabbits. After 12 weeks, histological and histomorphometric analysis was performed. All the implants were osseointegrated and no signs of infection were observed. Histomorphometric analysis revealed that the bone–implant contact % (BIC) ratio was similar around the SLA™ implants (63.74 ± 13.61) than around the Nanoblast Plus™ implants (62.83 ± 9.91). Both implant surfaces demonstrated a favorable bone response, confirming the relevance of the sandblasted-etched surface on implant osseointegration.

- Abstract
Introduction: This study aimed to show the clinical outcomes of implants inserted by guided surgery supporting mandibular overdentures in edentulous patients. Patients and methods: Mandibular edentulous patients were diagnosed with an oral examination, cone-beam computerized tomography, and diagnostic casts for intermaxillary relations and treated with overdentures over two implants by guided surgery. After flapless surgery, implants were early loaded with an overdenture at 6 weeks. Results and discussion: Fourteen patients (nine females and five males) were treated with 28 implants. Four patients (28.6%) had a previous history of periodontitis. Five patients (35.7%) were smokers. Nine patients (64.3%) suffered from systemic diseases (i.e., diabetes, cardiovascular diseases). The clinical follow-up of the study was 44.7 ± 31.4 months. Clinical outcomes showed a global success of 100% of implants. Fourteen overdentures were placed in the patients over the implants. Mean marginal bone loss was 1.25 mm ± 0.95 mm. Four patients (28.6%) showed some kind of mechanical prosthodontic complications. Six implants (21.4%) were associated with peri-implantitis. Conclusions: This study indicates that treatment of mandibular edentulous patients with overdentures by guided surgery and early loading of implants placed appears to be a successful implant protocol.

- Abstract
The prescription of preventive antibiotics in dental implant treatments reduces the incidence of early failures. This study has focused mainly on the influence of amoxicillin, which is contraindicated in penicillin-allergic patients. The present systematic review aimed to determine whether penicillin-allergic patients have a higher risk of implant failure compared to non-allergic patients. An electronic search was performed on Medline and Web of Science using the following MeSH terms: (penicillin allergy OR clindamycin OR erythromycin OR azithromycin OR metronidazole) AND (dental implant OR dental implant failure OR dental implant complications). The criteria employed were those described in the PRISMA® Declaration. Only five articles were included that analyzed the failure rates of implants placed in penicillin-allergic patients who were prescribed clindamycin compared to non-allergic patients who were prescribed amoxicillin. With the limitations of this study, it is not possible to state that penicillin allergy per se constitutes a risk factor for early dental implant failure as most of the studies included self-reported allergic patients. Clindamycin has been associated with a significantly elevated risk of failure and an up to six times increased risk of infection. Immediate implants also have a 5.7 to 10 times higher risk of failure.

Introducción. La conexión implante-pilar ha sido sugerida como crucial para el éxito a largo plazo de las restauraciones sobre implantes oseointegración y para prevenir futuras complicaciones biológicas y me cánicas. El objetivo de este estudio fué evaluar la influencia del test de fatiga cíclica en el comportamiento de las conexiones internas implante-pilar. Métodos. 36 pilares mutiposición de implantes de titanio de conexión interna fueron divididos en 2 gru pos: 18 pilares slim o curvos y 10 pilares rectos con diferentes alturas. Los pilares fueron apretados con una llave de torque a 30 Ncm. Una carga cíclica entre 175 N y 100 N fué aplicada con 30º de inclinación axial al sistema de implantes durante 5 millones de ciclos. Resultados. Los tests biomecánicos muestran una fractura de los implantes y de los tornillos de retención a una carga límite de 100 N de los pilares slim curvos y de 130 N en los pilares rectos. La fracturas apa recieron en la zona de unión entre el cuello y el cuerpo del implante y en los tornillos protéscios. No se han encontrado fracturas en los pilares multiposición. Conclusiones. Este estudio indican que los tests de fatiga cíclica son importantes para analizar la respues ta biomecánica de los diferentes pilares en las conexiones implante-pilar de los sistemas de implantes., Introduction. The connection implant-abutment has been suggested to be crucial for the long-term success of implant restorations and to prevent future biological and mechanic complications. The aim of this study was to evaluate the influence of fatigue cyclic test in the behavior of internal connection implant-abutments. Methods. Thirty six titanium abutments of internal connection implants were divided in two groups: 18 slim and 18 right multiunit abutments with different length. Abutments were tightened to 30 Ncm with a torque controller. A cyclic load between 175 N and 100 N according to different implant abutments at a 30-degree angle to the long axis was applied to the implants for a 5 million cycles. Results. Biomechanical testing showed implant and screw retention fracture in a limit load of 100 N of slim implant abutments and 130 N of right implant abutments. Fracture cracks were located in the area between neck and body of implants and screw retention. No abutment fractures were found. Conclusions. This study indicate that fatigue cyclic test are very important to analize the biomechanical be havior of different abutments in connection implant-abutment of implants system.

Introducción. El objetivo del presente estudio era mostrar los resultados del tratamiento con implantes dentales insertados mediante la técnica de elevación transcrestal en el maxilar superior. Pacientes y Metodos. 47 pacientes (18 hombres y 29 mujeres) con pérdidas dentales maxilares fueron tratados con 60 implantes IPX ® Galimplant con conexión interna y superficie arenada y grabada fueron insertados mediante la técnica de elevación sinusal transcrestal para la rehabilitación del maxilar posterior. Los implantes fueron cargados después de un periodo de cicatrización de 6 meses. Resultados. Los hallazgos clínicos indican una supervivencia y éxito de los implantes del 100%. La ga nancia media de hueso vertical fué de 4,8 mm (rango: 2-6,5 mm). 46,7% de los implantes se insertaron en localización molar y el 53,3% en la localización premolar. Biomateriales fueron utilizados en el 91,7% de los implantes. Después de un periodo medio de carga funcional de 37,6 meses (rango: 24-52 meses), no ha habido complicaciones tardías. El 70% de los implantes fueron rehabilitados con coronas unitarias y el 30% con puentes fijos. Conclusiones. Este estudio indica que el tratamiento con implantes dentales mediante su inserción con elevación transcrestal del seno maxilar superior constituye una terapéutica implantológica con éxito., Introduction. The aim of this study was to report the outcome of treatment with maxillary dental implants inserted by transcrestal sinus elevation. Patients and Methods. 47 patients (18 males and 29 females) with maxillary tooth loss were treated with 60 IPX ® Ga limplant internal connection and sandblasted and acid-etched surface implants inserted by transcrestal sinus elevation for rehabilitation of posterior maxilla. Implants were loaded after a healing free-loading period of 6 months. Results. Clinical results indicate a survival and success rate of implants of 100%. The mean elevation height was 4.8 mm (range: 2-6.5 mm). 46.7% of implants were inserted in molar and 53.3% in premolar localization. Bone substitutes were used in 91.7% of implants. After a mean functioning period of 37.6 months (range: 24-52 months), no late com plications were reported. 70% of implants were restored with single crowns and 30% with fixed bridges. Conclusions. This study indicate that treatment with dental implants inserted in maxilla by transcrestal sinus elevation constitute a successful implant treatment.

- Abstract
La superficie de los implantes es muy importante para conseguir la oseointegración. Existen muchos tipos de superficies en los implantes dentales. La investigación en superficie de implantes comienza con la superficie mecanizada. La superficie rugosa incrementa la adherencia celular y muestra una mayor área de contacto hueso-implante. El recubrimiento con plasma de titanio e hidroxiapatita fueron méto dos comunes para modificar la rugosidad de la superficie. Más recientemente, el arenado (ej. alúmina) y el grabado ácido (ej. chorhídrico, sulfúrico, nítrico) son otros procesos que pueden incrementar la superficie rugosa de los implantes. Más recientemente, la tecnología ha comenzado en la implantología oral con el desarrollo de superficies porosas que manifiestan una mejor respuesta osteogénica e incre menta la unión mecánica hueso-titanio. La oseointegración parece estar influenciada por los modelos de especies animales. En el conejo el proceso es más rápido que en el perro, y en el perro es más rápido que en el hombre. La incorporación de nuevos materiales y procesos técnicos en la investigación sobre superficies de implantes ofrecen nuevas posibilidades de mejorar su respuesta tisular y su potencial óseo regenerativo. Conclusiones. El tratamiento de la superficie de los implantes constituye un importante campo en futuras investigaciones de la implantología oral mediante los estudios de experimentación.

Resultado: Fueron insertados 216 implantes en ambos maxilares (168 en el maxilar superior y 48 en la mandíbula) para su rehabilitación prostodóncica con coronas implantosoportadas. 81 implantes fueron insertados en el sector anterior y 135 implantes en el sector posterior. Después de 5 años de seguimiento clínico, los resultados indican una supervivencia y éxito de los implantes del 95,8%. Durante el periodo de cicatrización, se perdieron 4 implantes por movilidad, mientras que 5 implantes se perdieron por periimplantitis. Se presentaron complicaciones prostodóncicas en 8 coronas con fractura de cerámica. Conclusiones: Los hallazgos clínicos del presente estudio indican que la rehabilitación prostodóncica con coronas unitarias mediante la inserción de implantes con superficie arenada y grabada, representa una terapéutica odontológica con éxito. Introducción: La cirugía guiada y la carga precoz de los implantes constituye un enfoque integral de la rehabilitación de los pacientes edéntulos. El objetivo del presente trabajo era presentar el protocolo diagnóstico, quirúrgico y protésico del tratamiento con implantes mediante la técnica de cirugía guiada y carga precoz en el tratamiento con sobredentadura con implantes. Caso clínico: Paciente varón de 68 años que acude a consulta para tratamiento con implantes de la mandíbula edéntula. La tomografía computarizada de haz cónico (CBCT) muestra que el paciente presenta un adecuado volumen óseo para la inserción de implantes. La información obtenida por la CBCT es utilizada para la evaluación y el plan de tratamiento. Se planificaron dos implantes interforaminales. Los datos digitales de imagen tomográfica fueron enviados a un centro especializado para la realización de la férula quirúrgica. De acuerdo a la férula guiada se insertaron los implantes sin colgajo con un fresado preciso. A las 6 semanas de la cirugía, una sobredentadura fue realizada y ajustada sobre los ataches, evaluando su oclusión. El periodo de seguimiento ha sido de 5 años. Conclusiones: Este caso clínico indica que la cirugía guiada puede constituir una terapéutica exitosa en el tratamiento del paciente edéntulo mandibular con sobredentaduras., Introduction: Guided surgery and early functional loading of dental implants constitute a comprehensive approach for rehabilitation of edentulous patients. The aim of this paper was to present the diagnosis, surgical and prosthetic protocol of implants by guided technique and early loading in the treatment of edentulous mandible. Case report: A 68-year-old-man, requested an evaluation regarding implant treatment for the edentulous mandible. The cone beam computarized tomography (CBCT) showed that the patient have an adequate bone volume for mandibular implant placement. The data from CBCT were used for evaluation and treatment planning. Two implants were treatment planned. Digital data were sent to a software manufacturer for surgical template. According surgical template implants were inserted for a flapless surgical technique with an accuraced drilling. After a 6 weeks-period of time an overdenture were realized and finally, occlusion is evaluated. The follow-up were 5 years. Conclusions: This paper indicated that guided implant dentistry may constitute a successful treatment of edentulous mandible patients treated with overdentures

- Abstract
Introducción: El objetivo del presente estudio era mostrar los resultados del tratamiento con implantes dentales insertados mediante la técnica de expansión ósea en el maxilar superior. Pacientes y métodos: Setenta y tres pacientes con pérdidas dentales maxilares fueron tratados con 149 implantes IPX Galimplant® con conexión interna y superficie arenada y grabada para la rehabilitación mediante la técnica de expansión ósea con osteótomos roscados. Los implantes fueron cargados después de un periodo de cicatrización de 3 meses. Resultados: Los hallazgos clínicos indican una supervivencia y éxito de los implantes del 97,4%. Se perdieron 4 implantes durante el periodo de cicatrización. El 57,7% de los implantes se insertaron en el sector anterior maxilar y el 42,3%, en el sector posterior maxilar. Después de un periodo medio de carga funcional de 36,4 meses no ha habido complicaciones tardías. El 88,6% de los pacientes fueron rehabilitados con puentes fijos y el 11,5%, con coronas unitarias. Conclusiones: Este estudio indica que el tratamiento con implantes dentales mediante su inserción con expansión ósea en el maxilar superior constituye una terapéutica implantológica con éxito., Introduction: The aim of this study was to report the outcome of treatment with maxillary dental implants inserted by ridge expansion technique. Patients and Methods: 73 patients with maxillary tooth loss were treated with 149 IPX Galimplant® internal connection and sandblasted-acid-etched surface implants for rehabilitation by bone expansion technique. Implants were loaded after a healing free-loading period of 3 months. Results: Clinical results indicate a survival and success rate of implants of 97.4%. 4 implants were lost during the healing period. 57.7% of implants were inserted in anterior maxilla and 42.3% in posterior maxilla. After a mean functioning period of 36.4 months, no late complications were reported. 88.6% of patients were restored with fixed bridges and 11.5% with single crowns. Conclusions: This study indicate that treatment with dental implants inserted in maxilla by ridge expansion constitute a successful implant treatments

- Abstract
En las últimas décadas, la implantología oral ha representado una técnica creciente en gerodontología. En este sentido, la edad avanzada no es una contraindicación para la implantología oral. Desde un punto de vista integral, es necesario la valoración de las condiciones médicas del paciente mayor, un diagnóstico oral, y una correcta planificación de tratamiento, con un estricto protocolo quirúrgico y prostodóncico. Muchos pacientes candidatos para la cirugía de implantes son mayores, y su perfil médico puede ser complejo. La comunicación con su médico puede ser importante para minimizar los riesgos quirúrgicos. Después de la fase quirúrgica, el éxito a largo plazo de los implantes dentales exige una buena reacción de los tejidos blandos y duros periimplantarios. Prótesis fijas y removibles pueden estar indicadas en los pacientes mayores edéntulos, según diferentes protocolos de carga funcional. El mantenimiento por el paciente y las revisiones periódicas por el dentista son muy importantes. Las prótesis implantosoportadas mejoran la calidad de vida oral y el bienestar psicosocial de los pacientes ancianos edéntulos., During last decades, implant dentistry has been an increased technique in geriatric dentistry. Aging is not a contraindication for oral implantology. From a comprehensive point of view, are necessary a medical assessment of the aged patient, an oral diagnosis, a correct treatment planning, with a strict protocol of surgical and prosthetic procedures. Many patients for dental implant surgery are elderly, and their overall medical condition may be complex. Consultation with the patient´s physician is important to ensure that surgical risks are minimized. After the surgical step, a good reaction of hard and soft tissues is necessary for long-term success of dental implants. Fixed and removable prostheses supported by dental implants were been indicated in edentulous older patients, according several functional loading protocols. Maintenance by the patient and periodical follow-up by the dentist are very important. Implant-supported prostheses improve oral quality of life and psychosocial well-being of edentulous aged patients

This paper studies the properties of the continuous double auction trading mechanishm using an artificial market populated by heterogeneous computational agents. In particular, we investigate how changes in the population of traders and in market microstructure characteristics affect price dynamics, information dissemination and distribution of wealth across agents. In our computer simulated market only a small fraction of the population observe the risky asset's fundamental value with noise, while the rest of agents try to forecast the asset's price from past transaction data. In contrast to other artificial markets, we assume that the risky asset pays no dividend, so agents cannot learn from past transaction prices and subsequent dividend payments. We find that private information can effectively disseminate in the market unless market regulation prevents informed investors from short selling or borrowing the asset, and these investors do not constitute a critical mass. In such case, not only are markets less efficient informationally, but may even experience crashes and bubbles. Finally, increased informational efficiency has a negative impact on informed agents' trading profits and a positive impact on artificial intelligent agents' profits.

Immediate implants present a high risk of early failure. To avoid this, preventive antibiotics (PAs) are prescribed; however, their inappropriate administration leads to antimicrobial resistance. The present study aims to clarify whether the prescription of PAs reduces the rate of early failure of immediate implants and to establish guidelines to avoid the overprescription of these drugs. An electronic search of the MEDLINE database (via PubMed), Web of Science, Scopus, LILACS and OpenGrey was carried out. The criteria described in the PRISMA
® statement were used. The search was temporarily restricted from 2010 to 2021. The risk of bias was analysed using the SIGN Methodological Assessment Checklist for Systematic Reviews and Meta-Analyses and the JBI Prevalence Critical Appraisal Tool. After searching, eight studies were included that met the established criteria. With the limitations of this study, it can be stated that antibiotic prescription in immediate implants reduces the early failure rate. Preoperative administration of 2–3 g amoxicillin one hour before surgery followed by 500 mg/8 h for five to seven days is recommended. It is considered prudent to avoid the use of clindamycin in favour of azithromycin, clarithromycin or metronidazole in penicillin allergy patients until further studies are conducted.
