14. Assessing universal coverage : Mexico's Seguro Popular

Abstract

In the second half of the 20th century, a series of reforms in Mexico's health system with the goal of expanding coverage gave rise to Seguro Popular de Salud, or Popular Insurance, a financial protection mechanism designed to guarantee the right for healthcare for every citizen. The intention of Seguro Popular was to expand access to health services for over 50 million uninsured individuals by eliminating out-of-pocket health payments with the ultimate aim to improve the population's health. The objective of this thesis is to assess the impact of Seguro Popular on health outcomes, healthcare access, inequality and financial protection. The first chapter introduces the study and describes the programme. The second chapter is an assessment of the impact of Seguro Popular on Mexican population's health status and on utilisation of healthcare. Exploiting the time and space variation in the introduction of Seguro Popular, treatment effects are estimated using difference-in- differences specifications with variations in identification strategies. Results show that the programme had significant effects on reducing infant mortality rates. However, there were no clear effects on the adult health outcomes examined and on healthcare utilisation. The aim of the third chapter is to find the effect of Seguro Popular on children's nutritional status; and to estimate the change in pure health inequality derived from the programme. Taking advantage of the staggered roll-out of Seguro Popular, children's height-for-age z-scores are assessed using a changes-in-changes approach. Results show that of Seguro Popular had a slightly positive effect on children's height. Moreover, the benefits were marginally larger for children in the lowest tail of the height distribution which represents a reduction of pure inequality in health. The fourth chapter is an assessment of the antipoverty effect of Seguro Popular. Using instrumental variables, this study presents local average treatment effects of Seguro Popular on out-of-pocket healthcare expenditures, catastrophic expenditures and impoverishing. Results show a small reduction in out-of-pocket healthcare payments, although no clear effect on catastrophic payments or on impoverishing. However, the programme slightly reduced poverty overall and had a positive effect on households' welfare.

Abstract

BerryPortraits (Phenotyping of Ripening Traits) is open source Python-based image-analysis software that rapidly detects and segments berries and extracts morphometric data on fruit quality traits such as berry color, size, shape, and uniformity. Utilizing the YOLOv8 framework and community-developed, actively-maintained Python libraries such as OpenCV, BerryPortraits software was trained on 512 postharvest images (taken under controlled lighting conditions) of phenotypically diverse cranberry populations (Vaccinium macrocarpon Ait.) from the two largest public cranberry breeding programs in the U.S. The implementation of CIELAB, an intuitive and perceptually uniform color space, enables differentiation between berry color and berry brightness, which are confounded in classic RGB color channel measurements. Furthermore, computer vision enables precise and quantifiable color phenotyping, thus facilitating inclusion of researchers and data analysts with color vision deficiency. BerryPortraits is a phenotyping tool for researchers in plant breeding, plant genetics, horticulture, food science, plant physiology, plant pathology, and related fields. BerryPortraits has strong potential applications for other specialty crops such as blueberry, lingonberry, caneberry, grape, and more. Abstract

When incentives fail to encourage enough owners to make their buildings climate-proof, the State has to impose obligations to retrofit buildings if climate targets are to be reached. This contribution examines whether the State can employ obligations based upon agreement with third-party effect, to enforce higher energy-efficiency standards for buildings and an improved energy mix. It presents and compares the responses to two cases from eleven European jurisdictions. It shows that the majority of examined jurisdictions will allow for the State to impose positive obligations with third-party effect through a burden registered in favour of the State or a property right for the buyer. Only Dutch law would be likely to invalidate the obligation because the obligation circumvents public-law safeguards. Abstract

We consider a central system which is coupled via dephasing to an open system, i.e. an intermediate system which in turn is coupled to another environment. Considering intermediate and far environment as one composite system, the coherences in the central system are given in the form of fidelity amplitudes for a certain perturbed echo dynamics in the composite environment. On the basis of the Born-Markov approximation, we derive a master equation for the reduction of that dynamics to the intermediate system alone. In distinction to an earlier paper [arXiv: 1502.04143 (2015)] where we discussed the stabilizing effect of the far environment on the decoherence in the central system, we focus here on the possibility to use the measurable coherences in the central system for probing the open quantum dynamics in the intermediate system. We illustrate our results for the case of chaotic dynamics in the near environment, where we compare random matrix simulations with our analytical result., Comment: 15 pages, 2 figures

Abstract

Fruit texture is a priority trait that guarantees the long‐term economic sustainability of the cranberry industry through value‐added products such as sweetened dried cranberries (SDCs). To develop a standard methodology to measure texture, we conducted a comparative analysis of 22 textural traits using five different methods under both harvest and postharvest conditions in 10 representative cranberry cultivars. A set of textural traits from the 10%‐strain compression and puncture methods were identified that differentiate between cultivars primarily based on hardness/stiffness and elasticity properties. The complementary use of both methodologies allowed for a detailed evaluation by capturing the effect of key texture‐determining factors such as structure, flesh, and skin. Furthermore, the high effectiveness of this approach in different conditions and its ability to capture high phenotypic variation in cultivars highlights its great potential for applicability in various areas of the value chain and research. Abstract

Introduction: For nearly two centuries, cranberry (Vaccinium macrocarpon Ait.) breeders have improved fruit quality and yield by selecting traits on fruiting stems, termed "reproductive uprights." Crop improvement is accelerating rapidly in contemporary breeding programs due to modern genetic tools and highthroughput phenotyping methods, improving selection efficiency and accuracy. Methods: We conducted genotypic evaluation on 29 primary traits encompassing fruit quality, yield, and chemical composition in two full-sib cranberry breeding populations--CNJ02 (n = 168) and CNJ04 (n = 67)--over 3 years. Genetic characterization was further performed on 11 secondary traits derived from these primary traits. Results: For CNJ02, 170 major quantitative trait loci (QTL; R²= 0.10) were found with interval mapping, 150 major QTL were found with model mapping, and 9 QTL were found to be stable across multiple years. In CNJ04, 69 major QTL were found with interval mapping, 81 major QTL were found with model mapping, and 4 QTL were found to be stable across multiple years. Meta-QTL represent stable genomic regions consistent across multiple years, populations, studies, or traits. Seven multi-trait meta-QTL were found in CNJ02, one in CNJ04, and one in the combined analysis of both populations. A total of 22 meta-QTL were identified in cross-study, cross-population analysis using digital traits for berry shape and size (8 meta-QTL), digital images for berry color (2 meta-QTL), and three-study cross-analysis (12 meta-QTL). Discussion: Together, these meta-QTL anchor high-throughput fruit quality phenotyping techniques to traditional phenotyping methods, validating state-of-the-art methods in cranberry phenotyping that will improve breeding accuracy, efficiency, and genetic gain in this globally significant fruit crop. Abstract

We present a time-lapse 4-D high-resolution synchrotron imaging study of the morphological evolution of methane gas bubbles and hydrate film growth on these bubbles. Methane gas and partially water-saturated sand were used to form hydrate with a maximum hydrate saturation of 60%. We investigated the transient evolution of gas bubble size distribution during hydrate formation and observed three distinct stages: a) nucleation and hydrate film formation, b) rapid bubble break-up, c) gas bubble coalescence and hydrate framework formation. Our results show that the average gas bubble size distribution decreases from 34.17 μιτι (during hydrate nucleation) to 8.87 μιτι (during secondary bubble formation). The small-size methane bubble population (mean diameter below 10 μm) initially increases at the expense of the larger methane bubble population (mean diameter above 50 μη) due to breakage of the larger bubbles and coalescence of the smaller bubbles. We quantified that the average hydrate film thickness increases from 3.51 to 14.7 μ!! by tracking the evolution of a particular gas bubble. This thickness increase agrees with an analytical model with an average deviation error of 3.3%. This study provides insights into gas bubble distribution and hydrate film growth during hydrate formation, both of which impact the geophysical and mechanical properties of hydrate-bearing sediments. Abstract

Submarine channels are conduits for sediment-laden flows called turbidity currents, which play a globally significant role in the offshore transport of sediment and organic carbon and pose a hazard to critical seafloor infrastructure. Time-lapse repeat surveys of active submarine channels have recently shown that upstream-migrating knickpoints can dominate channel evolution. This finding contrasts with many studies of ancient outcrops and subsurface geophysical data that inferred channel bends migrate laterally, as occurs in meandering rivers. Here, we aim to test these two contrasting views by analysing two high-resolution repeat seafloor surveys acquired 13 years apart across the entirety of an active submarine channel in Knight Inlet, British Columbia. We find that two main mechanisms control channel evolution, with the normalised channel radius of curvature (specifically, R* - channel radius of curvature normalised to channel width) explaining which of these mechanisms dominate. Pronounced outer bend migration only occurs at tight bends (R*<1.5). In contrast, at broader bends and straighter sections (R*>1.5), erosion is focused within the channel axis, where upstream-migrating knickpoints dominate. High centrifugal accelerations at tight bends promote super-elevation of flows on the outer channel flank, thus, enhancing outer bend erosion. At R*>1.5, flow is focused within the channel axis, promoting knickpoints that migrate upstream at an order of magnitude faster than the rate of outer bend erosion at tight bends. Despite the dominance of knickpoints in eroding the channel axis, their stratigraphic preservation is very low. In contrast, the lateral migration of channel bends results in much higher preservation via lateral accretion of deposits on the inner bend. Abstract

Artificial caverns in salt rock formations play an important role in the net-zero energy transition challenge, both for covering short-term fluctuations in energy demand and serving as safe locations for long-term underground gas storage both for hydrogen and natural gas. Geophysical tools can serve for monitoring geomechanical changes in the salt cavern during selection and development, and during gas storage/extraction activities, but the use of common geophysical monitoring techniques has been very limited in this area. Here, we present experimental work on physical and transport properties of halite rocks within the energy storage context and assess the potential of seismic and electromagnetic data to monitor gas storage activities in salt formations. First, we analysed the stress-dependency of the elastic and transport properties of five halite rocks to improve our understanding on changes in the geological system during gas storage operations. Second, we conducted two dissolution tests, using cracked and intact halite samples, monitored with seismic (ultrasonic P- and S-waves velocities and their attenuation factors) and electromagnetic (electrical resistivity) sources to evaluate (i) the use of these common geophysical sensing methods to remotely interpret caverning development and (ii) the effect of structural discontinuities on rock salt dissolution. Elastic properties and permeability showed an increasing trend towards rock sealing and mechanical enhancement with increasing pressure for permeabilities above 10−21 m2, with strong linear correlations up to 20 MPa. In the dissolution tests, the ultrasonic waves and electrical resistivity showed that the presence of small structural discontinuities largely impacts the dissolution patterns.
