Although the etiology of obsessive-compulsive disorder (OCD) is largely unknown, it is accepted that OCD is a complex disorder. There is a known bi-directional interaction between the gut microbiome and brain activity. Several authors have reported associations between changes in gut microbiota and neuropsychiatric disorders, including depression or autism. Furthermore, a pediatric-onset neuropsychiatric OCD-related syndrome occurs after streptococcal infection, which might indicate that exposure to certain microbes could be involved in OCD susceptibility. However, only one study has investigated the microbiome of OCD patients to date. We performed 16S ribosomal RNA gene-based metagenomic sequencing to analyze the stool and oropharyngeal microbiome composition of 32 OCD cases and 32 age and gender matched controls. We estimated different alpha- and beta-diversity measures and performed LEfSe and Wilcoxon tests to assess differences in bacterial distribution. OCD stool samples showed a trend towards lower bacterial alpha-diversity, as well as an increase of the relative abundance of Rikenellaceae, particularly of the genus Alistipes, and lower relative abundance of Prevotellaceae, and two genera within the Lachnospiraceae: Agathobacer and Coprococcus. However, we did not observe a different Bacteroidetes to Firmicutes ratio between OCD cases and controls. Analysis of the oropharyngeal microbiome composition showed a lower Fusobacteria to Actinobacteria ratio in OCD cases. In conclusion, we observed an imbalance in the gut and oropharyngeal microbiomes of OCD cases, including, in stool, an increase of bacteria from the Rikenellaceae family, associated with gut inflammation, and a decrease of bacteria from the Coprococcus genus, associated with DOPAC synthesis.

Low back pain (LBP) is the leading cause of disability and one of the most common reasons for physician visits in primary care, with a 33% rate of recurrence during the first year. However, the most optimal exercise program in this context remains unknown. The objective was to evaluate the effectiveness of a group-based progressive strength training program in non-specific chronic LBP (CLBP) patients in primary care on pain recurrence and physical function. Eighty-five patients with non-specific CLBP were separated into two groups (Intervention group: completed a progressive strength training program 3 days per week for 8 weeks; Control group: received the usual care). The intervention group showed a recurrence rate of 8.3%, while the control group had a recurrence rate of 33.3% and a shorter time until the first recurrent episode. The intervention group showed increased lumbar extensor strength, left-hand handgrip strength, and reduced the number of pain sites compared with the control group. Results also showed greater odds for reducing LBP intensity and disability in the intervention group. In conclusion, a group-based progressive strength training program is a more effective and efficient alternative than Back-School programs and can easily be carried out in the primary health care context.

Trabajo presentado al 35º Encuentro GEEFSM (Groupe d’Etudes sur l’Eco-pathologie de la Faune Sauvage de Montagne), celebrado en Cofrentes, Muela de Cortes (España) del 1 al 4 de junio de 2017., [ES]: La tuberculosis es una enfermedad bacteriana causada por agentes del complejo Mycobacterium tuberculosis (CMTB), que puede afectar a un amplio rango de mamíferos y que produce graves pérdidas económicas. El carácter re‐emergente de la enfermedad y los repetidos fallos en conseguir su erradicación se han asociado a la presencia de fauna silvestre, que actuaría como reservorio. Las mayores prevalencias en la Península se han detectado en el centro y suroeste. En la Región de Murcia la enfermedad no había sido descrita en ungulados silvestres y, dadas las características especiales de la zona, como la ganadería extensiva presente, escasa y conformada por pequeños rumiantes, su epidemiología podría diferir con respecto a otras áreas del país. En el marco del Programa de Vigilancia Sanitaria en Fauna Silvestre, se asistió a eventos cinegéticos (temporadas 2012/2013 y 2013/2014), obteniéndose muestras de 187 jabalíes. Estas se sometieron a un diagnóstico combinado mediante ELISA indirecto, cultivo y PCR sobre tejido, y en algunos casos se consiguió llegar a la caracterización molecular (DVR‐Spoligotyping) del CMTB. En total se han detectado 9 animales positivos, todos ellos en el cuadrante noroeste de la Región (Moratalla, Cehegín y Mula), aunque sólo 4 de ellos pudieron ser caracterizados molecularmente: 3 M. bovis y 1 M. caprae. Considerando todas las técnicas de diagnóstico, la prevalencia el CMTB en estos 3 municipios es del 10,1 ± 3,2 % (n=87) y, por edades, encontramos un 9.1 % en 2 años (adultos). Atendiendo solamente a las poblaciones donde se detectó el CMTB o contacto frente a él, se observan elevadas tasas de prevalencia que alcanzan su máximo entre los adultos (66,7%). En estas poblaciones positivas la prevalencia en los machos alcanzó un 62,5% (n=8) y un 30% en hembras (n=10). Además, en la única finca muestreada vallada y con un cierto régimen de manejo intensivo la prevalencia total alcanzó un 50% (n=10)., [IT[: La tubercolosi è una malattia batterica causata da agenti appartenenti al complesso Mycobactrium tuberculosis (CMTB), che può infettare un’ampia varietà di mammiferi e produce gravi perdite economiche. Il carattere reemergente della malattia e i ripetuti fallimenti nell’ottenere la sua eradicazione sono associati alla presenza di fauna silvestre, che ne è il reservoir. La maggiore prevalenza nella Penisola è stata riscontrata nel centro e nel sud-est del paese. Nella Regione di Murcia la malattia non era stata descritta in ungulati silvestri e, date le caratteristiche particolari della zona, come la presenza di allevamento estensivo, scarsa e composta da piccoli ruminanti, la sua epidemiologia potrebbe essere differente rispetto a quella di altre aree del paese. La direttiva del Programma di Vigilanza Sanitaria relativa alla Fauna Silvestre, in seguito a battute di caccia (stagioni 2012/2013 e 2013/2014), sono stati ottenuti campioni da 187 cinghiali. I campioni sono stati sottoposti a test diagnostici combinati quali ELISA indiretto, coltura e PCR sui tessuti, e in alcuni casi si è potuti giungere all’identificazione molecolare (DVR-Spoligotyping) del CMTB. In totale sono stati individuati 9 animali positivi, tutti nel quadrante nord-est della Regione (Moratalla, Chegin e Mula), anche se solo in 4 di essi è stata possibile l’identificazione molecolare: 3 in M. bovis e 1 M. caprae. Considerando tutte le tecniche impiegate nella diagnosi, la prevalenza del CMTB in questi tre municipi è del 10,1 ± 3,2 % (n=87) e, per età, del 9.1% 2 anni (adulti). Considerando solo le popolazioni in cui è stato individuato il CMTB o contatto con esso, si osservano alti tassi di prevalenza che raggiungono il loro massimo tra gli adulti (66,7%). In queste popolazioni positive la prevalenza nei maschi ha raggiunto il 62,5% (n=8) e un 30% nelle femmine (n=10). Inoltre, nell’unica tenuta recintata e con una gestione semi-intensivo la prevalenza totale ha raggiunto il 50% (n=10)., [FR]: La tuberculose est une maladie bactérienne causée par les agents du complexe Mycobacterium tuberculosis (CMTB), pouvant affecter un grand éventail de mammifères et causant des pertes économiques graves. Le caractère re-émergent de la maladie et les échecs répétés de son éradication se sont associés à la présence de faune sylvestre comme réservoir. Les prévalences majeures de la péninsule se sont détectées dans le centre et le sud-est. Dans la région de Murcia, la maladie n’a pas été décrite chez les ongulés sylvestres, et étant donné les caractéristiques spéciales de la zone, comme l’élevage extensif présent, faible et formé par les petits ruminants, son épidémiologie pourrait être différente du reste du pais. Dans le cadre du Programme de Vigilance Sanitaire de la Faune Sylvestre, les échantillons de 187 sangliers ont étés obtenus lors des périodes chasse (saisons 2012/2013 et 2013/2014). Ces échantillons furent soumis à un diagnostic combiné avec ELISA indirect, mise en culture et PCR sur tissus, et dans certains cas il fut possible une caractérisation moléculaire (DVR- Spoligotyping) du CMTB. Au total, 9 animaux positifs furent détectés, tous dans le cadrant nord-ouest de la région Moratalla, Cehegín et Mula), bien que seulement 4 d’entre eux purent être caractérisés moléculairement: 3 M. bovis et 1 M. caprae. Considérant toutes les techniques de diagnostic, la prévalence du CMTB dans ces 3 communes est de 10,1 ± 3,2 % (n=87) et, par âge, nous trouvons un 9.1% de 2 ans (adultes). Si l’on fait seulement attention aux populations où le CMTB fut détecté ou un contact avec celui-ci, on observe des taux élevés de prévalence, qui atteignent un maximum chez les adultes (66,7%). Dans ces populations positives la prévalence des mâles atteint un 62,5% (n=8) et un 30% chez les femelles (n=10). De plus, les échantillons de l’unique ferme clôturée et avec un certain régime de gestion intensive, la prévalence totale atteignit un 50% (n=10).
