Abstract

Estudios de cohortes; VIH; Pérdida de seguimiento Cohort studies; HIV; Lost to follow-up Estudis de cohorts; VIH; Pèrdua del seguiment Background Despite remarkable achievements in antiretroviral therapy (ART), losses to follow-up (LTFU) might prevent the long-term success of HIV treatment and might delay the achievement of the 90–90-90 objectives. This scoping review is aimed at the description and analysis of the strategies used in high-income countries to reengage LTFU in HIV care, their implementation and impact. Methods A scoping review was done following Arksey & O′Malley’s methodological framework and recommendations from Joanna Briggs Institute. Peer reviewed articles were searched for in Pubmed, Scopus and Web of Science; and grey literature was searched for in Google and other sources of information. Documents were charted according to the information presented on LTFU, the reengagement procedures used in HIV units in high-income countries, published during the last 15 years. In addition, bibliographies of chosen articles were reviewed for additional articles. Results Twenty-eight documents were finally included, over 80% of them published in the United States later than 2015. Database searches, phone calls and/or mail contacts were the most common strategies used to locate and track LTFU, while motivational interviews and strengths-based techniques were used most often during reengagement visits. Outcomes like tracing activities efficacy, rates of reengagement and viral load reduction were reported as outcome measures. Conclusions This review shows a recent and growing trend in developing and implementing patient reengagement strategies in HIV care. However, most of these strategies have been implemented in the United States and little information is available for other high-income countries. The procedures used to trace and contact LTFU are similar across reviewed studies, but their impact and sustainability are widely different depending on the country studied. The project leading to these results (PISCIS Cohort) has received funding from “la Caixa” Banking Foundation under the project code LCF/PR/PR17/51120008. This work is supported by a grant from the Foundation Marató TV3 (project code 239/C/2018) aimed at the analysis of the LTFU patients of the PISCIS Cohort. Abstract

We report controlled silicon doping of Ga2O3 grown in plasma-assisted molecular beam epitaxy. Adding an endplate to the Si effusion cell enables the control of the mobile carrier density, leading to over 5-orders of magnitude change in the electrical resistivity. Room temperature mobilities > 100 cm 2 / V s are achieved, with a peak value > 125 cm 2 / V s at a doping density of low- 10 17 / cm 3 . Temperature-dependent Hall effect measurements exhibit carrier freeze out for samples doped below the Mott criterion. A mobility of 390 cm 2 / V s is observed at 97 K. [ABSTRACT FROM AUTHOR]

Abstract

La dendrocronología es el método de datación absoluta de madera más exacto que existe hoy en día. Esta ciencia estudia los anillos anuales de crecimiento de especies leñosas para determinar las condiciones ambientales y datar eventos del pasado. La datación cruzada con cronologías de referencia ancladas en la escala temporal permite asignar años exactos de calendario a las series de anillos extraídas de maderas históricas y prehistóricas, lo que a su vez permite establecer la procedencia de la madera. En este capítulo se presentan los conceptos básicos de esta ciencia, ilustrando su potencial y limitaciones y apuntando algunos usos de la dendrocronología más allá de la datación de la madera.

Abstract

Objectives The aim of the study was to assess the rates of discontinuation of integrase inhibitor regimens because of any neuropsychiatric adverse event (NPAE) and the factors associated with discontinuation. Methods A population-based, prospective, multicentre cohort study was carried out. Treatment-naive subjects starting therapy with a regimen containing integrase inhibitors, or those switching to such a regimen, with plasma HIV-1 RNA < 50 HIV-1 RNA copies/mL in 14 hospitals in Catalonia or the Balearic Islands (Spain) were included in the study. Every discontinuation because of adverse events (AEs) was double-checked directly with treating physicians. Multivariable Cox models identified factors correlated with discontinuation. Results A total of 4165 subjects (37% treatment-naive) started regimens containing dolutegravir (n = 1650; 91% with abacavir), raltegravir (n = 930) or elvitegravir/cobicistat (n = 1585). There were no significant differences among regimens in the rate of discontinuation because of any AE. Rates of discontinuation because of NPAEs were low but higher for dolutegravir/abacavir/lamivudine [2.1%; 2.9 (95% confidence interval (CI) 2.0, 4.2) discontinuations/100 patients/year] versus elvitegravir/cobicistat (0.5%; 0.8 (95% CI 0.3, 1.5) discontinuations/100 patients/year], with significant differences among centres for dolutegravir/abacavir/lamivudine and NPAEs (P = 0.003). We identified an association of female gender and lower CD4 count with increased risk of discontinuation because of any AE [Incidence ratio (IR) 2.3 (95% CI 1.4, 4.0) and 1.8 (95% CI 1.1, 2.8), respectively]. Female gender, age > 60 years and abacavir use were not associated with NPAE discontinuations. NPAEs were commonly grade 1-2, and had been present before and improved after drug withdrawal. Conclusions In this large prospective cohort study, patients receiving dolutegravir, raltegravir or elvitegravir/cobicistat did not show significant differences in the rate of discontinuation because of any toxicity. The rate of discontinuations because of NPAEs was low, but was significantly higher for dolutegravir than for elvitegravir/cobicistat, with significant differences among centres, suggesting that greater predisposition to believe that a given adverse event is caused by a given drug of some treating physicians might play a role in the discordance seen between cohorts.

Abstract

In this paper, the Special Interest Group on Attention Deficit Hyperactivity Disorder (GEITDAH, from its name in Spanish) presents a consensus reached by experts from all over Spain on conduct disorders in children and adolescents. Following the initial work by the team at the Pedopsychiatry Unit at the Quiron-Teknon Hospital in Barcelona, agreements have been reached on a number of basic aspects that could be the starting point for future consensuses. A top priority aim of the work was also to update the criteria in the Diagnostic and statistical manual of mental disorders, fifth edition, for conduct disorders in children and adolescents, together with their comorbidity with attention deficit hyperactivity disorder.Consenso del GEITDAH sobre los trastornos de conducta en niños y adolescentes.El Grupo de Especial Interes en el Trastorno por Deficit de Atencion/Hiperactividad (GEITDAH) presenta en este articulo un consenso de expertos de toda España sobre los trastornos de conducta en niños y adolescentes. A partir del trabajo inicial del equipo de la Unidad de Paidopsiquiatria del Hospital Quiron-Teknon de Barcelona, se han consensuado aspectos basicos que podrian ser el punto de partida para futuros consensos. Ha sido tambien objetivo prioritario del trabajo actualizar en los trastornos de conducta en niños y adolescentes los criterios del Manual diagnostico y estadistico de los trastornos mentales, quinta edicion, y su comorbilidad con el trastorno por deficit de atencion/hiperactividad.

Abstract

Endoluminal vacuum therapy (EVT) is an accepted treatment for anastomotic leakage (AL) after esophagectomy. A novel concept is to use this technology in a preemptive setting, with the aim to reduce the AL rate and postoperative morbidity. Preemptive EVT (pEVT) was performed intraoperatively in 19 consecutive patients undergoing minimally invasive esophagectomy, immediately after completion of esophagogastrostomy. Twelve patients (63%) were high-risk cases with severe comorbidity. The EVT device was removed routinely three to six (median 5) days after esophagectomy. The endpoints of this study were AL rate and postoperative morbidity. There were 20 anastomoses at risk in 19 patients. One patient (5.3%) experienced major morbidity (Clavien–Dindo grade IIIb) unrelated to anastomotic healing. He underwent open reanastomosis at postoperative day 12 with pEVT for redundancy of the gastric tube and failure of transition to oral diet. Mortality after 30 days was 0% and anastomotic healing was uneventful in 19/20 anastomoses (95%). One minor contained AL healed after a second course of EVT. Except early proximal dislodgement in one patient, there were no adverse events attributable to pEVT. The median comprehensive complication index 30 days after surgery was 20.9 (IQR 0–26.2). PEVT appears to be a safe procedure that may have the potential to improve surgical outcome in patients undergoing esophagectomy. [ABSTRACT FROM AUTHOR]

Abstract

Introduccion. El deficit dopaminergico y noradrenergico en los circuitos frontoestriatales es considerado como la base bioquimica del trastorno por deficit de atencion/hiperactividad (TDAH). Estas alteraciones parecen explicar parte de sus deficit cognitivos, entre los cuales estan las funciones ejecutivas. El metilfenidato incrementa la biodisponibilidad dopaminergica y noradrenergica en la corteza prefrontal y los ganglios basales. Este hecho supone beneficios inmediatos en el incremento de la atencion y un descenso de la impulsividad. Menos documentado esta el efecto del metilfenidato sobre las funciones ejecutivas y, en concreto, sobre la memoria de trabajo. Objetivos. Evaluar si el metilfenidato-OROS incrementa el rendimiento de los TDAH en diversos parametros atencionales y en tareas de memoria de trabajo, y estudiar si las diferencias entre el grupo con TDAH y el grupo control desaparecen tras un mes de tratamiento con metilfenidato-OROS. Sujetos y metodos. Se seleccionaron 11 pacientes con TDAH y 11 sujetos control equiparados en edad, cociente intelectual, anos de escolaridad y nivel socioeconomico. Se administro un protocolo neuropsicologico en tres momentos: antes del tratamiento, tras una sola dosis y tras un mes de tratamiento con metilfenidato-OROS. Se administro el mismo protocolo neuropsicologico al grupo control. Resultados. Se encontraron diferencias en memoria de trabajo tras un mes de tratamiento y en parametros atencionales tras una sola toma en el grupo con TDAH. Las diferencias iniciales entre el grupo con TDAH y el grupo control en memoria de trabajo dejaron de ser significativas tras un mes de tratamiento. Conclusion. El metilfenidato-OROS mejora el rendimiento atencional desde la primera dosis y la memoria de trabajo verbal tras un mes de administracion diaria.

Abstract

Este trabajo se adentra en las circunstancias ―vitales y pedagógicas― que animaron la labor literaria de Alberta Giménez Adrover y abre una vía de investigación encaminada a analizar la producción poética y dramática de una de las voces más destacadas de la educación en Baleares. Asimismo, este primer acercamiento a su obra busca reseñar algunas de las funciones que tanto lo poético como lo teatral cumplieron en su actividad docente al frente del colegio de La Pureza y de la Escuela Normal de Maestras de Palma. Para ello, la documentación aportada por sus principales biógrafos ha resultado de especial ayuda, sobre todo los testimonios y documentos acopiados por Margarita Juan.
