Las mordeduras y arañazos de animales domésticos, así como los envenenamientos por reptiles (serpientes) y artrópodos (arañas, escorpiones, abejas, avispas, avispones, hormigas, etc.), son frecuentes y a menudo graves, sobre todo en los países tropicales, donde constituyen un factor de mortalidad considerable. Se propone una visión general de los principales envenenamientos, así como una reseña de las acciones preventivas (contra la rabia, por ejemplo) y la conducta que debe seguirse en urgencias.

La donovanose est une infection bactérienne à Klebsiella granulomatis, de localisation principalement génitale et de transmission essentiellement sexuelle, prédominant en zone tropicale. Nous en rapportons un cas contracté en France métropolitaine.

Les morsures et griffures d’animaux domestiques, les envenimations par les reptiles (serpents) et les arthropodes (araignées, scorpions, abeilles, guêpes, frelons, fourmis, etc.) sont fréquentes et volontiers sévères, a fortiori dans les pays tropicaux où elles constituent un important facteur de mortalité. Un panorama des principales envenimations ainsi qu’un rappel des mesures préventives (contre la rage par exemple) et des conduites à tenir en urgence sont proposés.

Pseudomonas aeruginosa, microorganisme bacille Gram négatif ubiquitaire, caractérisé par sa couleur verdâtre et son odeur douceâtre, est à l’origine de dermatoses potentiellement sévères comme l’ecthyma gangrenosum qui signe une immunodépression ou révèle une septicémie notamment chez l’enfant. Cette bactérie colonise fréquemment les plaies chroniques ou brûlures graves, les dermatoses spongiotiques ou acantholytiques surtout lorsqu’elles sont étendues et localisées dans les plis. Elle nécessite une prise en charge particulière du fait de sa forte capacité de résistance aux antibiotiques et antiseptiques. Elle peut aussi se traduire par une folliculite favorisée par les sports aquatiques ou une atteinte unguéale (chloronychie).

Las lesiones por congelación constituyen una urgencia terapéutica, por lo que deben ser conocidas por el dermatólogo. En la patología cutánea ligada al frío en los países templados predominan los síndromes acrales, en particular el fenómeno de Raynaud. Los eritemas pernios, menos frecuentes, que suelen asociarse a un síndrome acral, no deben confundirse con un lupus-eritema pernio. La urticaria por frío y la paniculitis a frigore constituyen otras manifestaciones que aparecen con el frío, más infrecuentes.

Le dermatomyosite associée aux anticorps anti-MDA5 est une entité récemment décrite. Nous en rapportons deux observations compliquées de pneumocystose pulmonaire et d’évolution létale.

La enfermedad de Darier es una genodermatosis rara, de transmisión autosómica dominante, ligada a la mutación del gen ATP2A2(Ch12q23-q24.1) que codifica la proteína SERCA2. En la actualidad, están identificadas más de 187 mutaciones sin «punto caliente». Estas mutaciones provocan alteraciones en el funcionamiento de los principales dominios de la bomba de calcio de las paredes del retículo endoplásmico. Los trastornos de los flujos de calcio así inducidos van a provocar, desde el punto de vista clínico, lesiones papuloqueratósicas en la piel que predominan en las zonas seborreicas, sobre todo en las foliculares, pero que pueden ser confluyentes. En las uñas, la lesión característica consiste en una onicorrexis asociada a una onicosquicia, a una hiperqueratosis subungueal y a una alternancia de bandas longitudinales rojas y blancas de la lúnula al borde libre que les confiere un aspecto de «sándwich». Se distinguen diferentes formas clínicas, con formas localizadas (acrales, del cuero cabelludo), formas sistematizadas y formas atípicas (vesiculoampollosas, hipertróficas, comedonianas, hemorrágicas, discrómicas). En la histología, el trastorno de la queratinización se manifiesta por una acantólisis suprabasal y una disqueratosis que conduce a la formación de cuerpos redondos y de granos. Se han descrito casos asociados a otras enfermedades, sobre todo neuropsiquiátricas. Esta afección crónica evoluciona en accesos que pueden ser desencadenados por el estrés, el calor, la sudoración, la exposición solar y determinados medicamentos como el litio. También se puede complicar con sobreinfecciones virales o bacterianas y causar una discapacidad, sobre todo de tipo social, por su aspecto antiestético y maloliente. No existe en la actualidad un tratamiento curativo. Con independencia de la gravedad de la enfermedad, es necesario combinar una serie de medidas de higiene con un tratamiento tópico o sistémico, para limitar así las complicaciones y los accesos, que pueden tener un impacto importante en la vida cotidiana.

El género Mycobacterium consta de más de 60 especies, de las que sólo son patógenas un 25%; apenas unas 12 especies ocasionan manifestaciones cutaneomucosas. Las micobacterias ambientales (antes denominadas atípicas) se contraen accidentalmente en el medio exterior, al contrario que las micobacterias de transmisión interhumana, que son la tuberculosis y la lepra. En las regiones tropicales húmedas, la infección por Mycobacterium ulceranses una auténtica enfermedad emergente, prioritaria para la Organización Mundial de la Salud. En Francia, la afectación por Mycobacterium marinumde los acuariófilos es el modelo de contagio descrito con más frecuencia en personas inmunocompetentes. La inmunodepresión, sobre todo en el síndrome de inmunodeficiencia adquirida (SIDA), favorece las formas generalizadas de micobacteriosis, relacionadas en la mayoría de los casos con el complejo Mycobacterium avium-intracellulare. Desde la aparición de la triple terapia y de los factores de crecimiento, estas infecciones por micobacterias ambientales se han vuelto infrecuentes.

Lors d’un érythème pigmenté fixe (EPF) impliquant le cyclophosphamide et le mesna, il est difficile de déterminer le médicament formellement en cause. Nous rapportons une nouvelle observation dans laquelle les tests épicutanés, en démontrant la responsabilité du mesna, ont permis une reprise du cyclophosphamide sans récidive.

Le mélanome desmoplastique (MD) est une forme rare de mélanome. Son aspect souvent atypique et parfois trompeur rend son diagnostic difficile et tardif. Du fait de son fort potentiel de récidive locale, il est nécessaire de bien connaître cette tumeur.

We have synthesized a new series of hexakis (alkylthio) benzenes with C3 molecular symmetry. From these π donor molecules we have prepared a radical cation salt and several charge transfer complexes which have been physically characterized mainly by magnetic experiments. These results are presented and discussed within the scope of possible ferromagnetic systems.

We have studied the formation of aggregates of a TTF derivative (tetrakis(octadecylthio)tetrathiafulvalene) in solution and in Langmuir−Blodgett films. It has been found that the monolayer formation of the TTF derivative was dependent upon the concentration of the spreading solution and also on the maximum spreading area. The floating monolayers were not stable at the air−water interface. However, stable mixed monolayers could be formed by adding behenic acid. The surface pressure vs area isotherms at several molar fractions of the components were measured. A molecular reorganization of the molecules during the compression may be observed. The mixed films were formed on quartz and CaF2 substrates for UV−vis and IR studies.

The cations of the new series of hexakis(alkylthio)benzenes with C3 symmetry have been studied in solution as well as in solid state. Dications are more stable in acidic medium and the second redox potential appears to be higher than for hexamethoxytriphenylene (HMT). Electronic spectra of monocations show molecular excitation bands without evidence for dimer species. ESR spectra present sometimes an hyperfine structure indicating a mean C3 symmetry. Dications are chemically less stable and we have not been able to detect a triplet state. We suspect that a Jahn-Teller distorsion could cause the dication to have a singlet ground state. In solid state crystals of triethylenedithiobenzene-tetrafluoroborate (tEDT-B)2BF4 have a charge transfer band corresponding to a mixed valence state. The complexation with tetrafluorotetracyanoquinodimethane gives crystals of (tEDT-B)2TCNQF4 which present stacking columns of alternating donor dyads and acceptor monads (DAD units) with an intense charge transfer band at 10 000 cm-1.

A series of 2,4-dioxo-5-(2-naphthylmethylene)-3-thiazolidineacetic acids and 2-thioxo analogues have been prepared as aldose reductase inhibitors. In vitro inhibitory activities of bovine lens aldose reductase were determined by a conventional method. 1-Naphthyl-substituted derivatives of the 2-thioxo series were the more potent inhibitors (IC50 ≅ 10 nM) with similar activity to that of Epalrestat. Structural analysis, especially by X-ray crystallography of two selected compounds, and molecular modeling comparisons with Zopolrestat were performed. These results provide explanations of the good activity of the inhibitor, the preference for 1-naphthyl-substituted compounds, and the nature of molecular interactions in these systems.

Treatment of Chinese hamster ovary cells with phospholipase C was previously shown to stimulate the CDP-choline pathway for phosphatidylcholine biosynthesis, and to cause activation of the CTP:phosphocholine cytidylyltransferase with a concomitant change in subcellular location of the enzyme (Sleight, R., and Kent, C. (1983) J. Biol. Chem. 258, 831-835). This paper presents a detailed analysis of the early events in the phospholipase C treatment, and provides evidence that the increased cytidylyltransferase activity causes the increased flux through the pathway. The time courses for the increase in cytidylyltransferase activity, increase in amount of membrane-associated enzyme, decrease in phosphocholine levels, and increase in phosphatidylcholine synthesis were similar, with all changes occurring within 30 min after addition of phospholipase C. These events preceded a decrease in cellular choline levels which correlated with a decreased capacity for choline uptake. The rate at which radioactive label was lost from pulse-labeled phosphocholine was the same as the rate at which label was incorporated into phosphatidylcholine, and these rates were stimulated 2.2-fold by phospholipase C treatment. We have also shown that the association of cytidylyltransferase with membranes was rapidly reversible when phospholipase C was removed from the cultures, and that the rate of decrease in phosphatidylcholine synthesis paralleled the rate of decrease in cytidylyltransferase activity. Cytidylyltransferase became reassociated with membranes when phospholipase C was added back to cultures from which it was previously removed. These results represent the first detailed account of the time frame involved in regulating phosphatidylcholine synthesis by the reversible association of cytidylyltransferase with cellular membranes.
