2. Development of a Personalised Device for Systemic Magnetic Drug Targeting to Brain Tumours

Abstract

Delivering therapies to deeply seated brain tumours (BT) is a major clinical challenge. Magnetic drug targeting (MDT) could overcome this by rapidly transporting magnetised drugs directly into BT. We have developed a magnetic device for application in murine BT models using an array of neodymium magnets with a combined strength of 0.7T. In a closed fluidic system, the magnetic device trapped magnetic nanoparticles (MNP) up to distances of 0.8cm. In mice, the magnetic device guided intravenously administered MNP (<50nm) from the circulation into the brain where they localised within mouse BT. Furthermore, MDT of magnetised Temozolomide (TMZmag+) significantly reduced tumour growth and extended mouse survival to 48 days compared to the other treatment groups. Using the same principles, we built a proof of principle scalable magnetic device for human use with a strength of 1.1T. This magnetic device demonstrated trapping of MNP undergoing flow at distances up to 5cm. MDT using our magnetic device provides an opportunity for targeted delivery of magnetised drugs to human BT.

Abstract

There is a requirement to ensure that in-vitro studies that use demineralised human dentine models are reliable and clinically relevant. The literature reports several strategies for these studies with a lack of consensus on the mode of action of the different demineralising acids on human dentine. \ud \ud This in-vitro study aims to characterise the effect of clinically-relevant acids on human dentine, using standardised substrates and complementary analytical techniques. The study focuses on an analysis of the mineral content and the integrity of the collagen following partial demineralisation.\ud \ud Samples of human dentine were exposed to a range of acids commonly encountered in the oral cavity. Characterisation of the mineral content used Vickers micro-hardness, Energy dispersive spectroscopy and X-ray fluorescence. Characterisation of the collagen integrity was undertaken by means of Scanning electron microscopy and Hydroxyproline assay. \ud \ud The following conclusions were reached: (i) Each demineralising agent tested had a unique effect on the mineral levels; (ii) Chelating agents, strong acids and weak acids affect the mineral and organic phases of dentine in significantly different ways with no correlation between them; (iii) The demineralising agents caused some degree of collagen denaturation, citric acid causing the most damage. Overall, there is no clear link between the type of demineralising agent and the effect on the organic and inorganic dentine. The choice of demineralising agent should be aligned to the experiment objectives so that the selected dentine (caries or erosion) model is fit for purpose.

24. Design and Properties of Novel Substituted Borosilicate Bioactive Glasses and Their Glass-Ceramic Derivatives

Abstract

Three novel borosilicate bioactive glasses (BBGs) of general formula of 0.05Na2O·0.35x·0.20B2O3·0.40SiO2 (molar ratio, where x = MgO or CaO or SrO) were prepared and used to investigate the effect of crystallization on their properties including cytotoxicity. The three postmelt compositions were determined using X-ray fluorescence spectroscopy, and crystallization events were studied using differential thermal analysis and X-ray diffraction. This information was used to determine heat treatments to prepare glass-ceramics by controlled crystallization. X-ray diffraction analysis and Fourier transform infrared spectroscopy showed that, after higher heat treatment temperatures (800–900 °C), borosilicate bioactive glass-ceramics (BBGCs) contained mainly borate and silicate crystalline phases. Specifically, BBG-Mg, BBG-Ca, and BBG-Sr glass-ceramics detected the presence of magnesium silicate-Mg2(SiO3)2 and magnesium borate-Mg2B2O5; wollastonite-2M-CaSiO3 and calcium borate-Ca(BO2)2; and strontium silicate-SrSiO3 and strontium borate-Sr2B2O5, respectively. In vitro cytotoxicity tests were performed using the mouse fibroblast cell line (L929). Glass and glass ceramic at concentrations lower than 50 mg/mL did not exhibit any level of cytotoxicity when compared with the control. However, quantitative evaluation indicated that greater cell growth occurred in the presence of materials with crystalline phases. Control of BBGs crystallization may therefore be used to influence the biocompatibility of these glass-ceramic systems.

25. Process optimisation to control the physico-chemical characteristics of biomimetic nanoscale hydroxyapatites prepared using wet chemical precipitation

Abstract

Hydroxyapatite nanoscale particles (nHA) were prepared by wet chemical precipitation using four different synthesis methods. Differences in physico-chemical properties including morphology, particle-size, and crystallinity were investigated following alteration of critical processing parameters. The nanoparticles were also studied using X-ray diffraction (XRD), Fourier Transform infrared spectroscopy in attenuated total reflectance mode (FTIR-ATR), and transmission electron microscopy (TEM) with energy dispersive X-ray (EDS) spectrometry. The results showed that the particles obtained were composed of nHA, with different morphologies and aspect ratios (1.5 to 4) and degrees of crystallinity (40% to 70% following calcination) depending on the different process parameters of the synthesis method used, such as temperature, ripening time and pH. This study demonstrated that relatively small adjustments to processing conditions of different wet chemical preparation methods significantly affect the morphological and chemical characteristics of nHA. For the predicable preparation of biomimetic nHA for specific applications, the selection of both production method and careful control of processing conditions are paramount.

Abstract

A total of 303 patients underwent attempted laparoscopic cholecystectomy (LC) over a four-year period by two consultant surgeons or a senior trainee under their supervision. The procedure was completed in 291 with a conversion rate to open cholecystectomy of 3.9% and a median postoperative length of stay of two days, range zero to nine days. In eighteen patients the indication for LC was failure of symptoms to settle, two of whom required conversion (11.1%). Diathermy dissection was avoided in Calot's triangle and dissection started at the junction of Hartmann's pouch and cystic duct with full mobilisation of this area prior to clip application. Pre-operative endoscopic retrograde cholangiopancreatography ERCP was performed in patients suspected of having common bile duct stones without routine intra-operative cholangiography. There was one death in this series (0.3%) and an overall complication rate of 6.3 %. There was no incidence of either bile duct injury or leak. LC can be performed with a low complication rate with attention to careful dissection technique in the region of Calot's triangle.

Abstract

Survival from colorectal cancer has not improved over the last four decades despite advances in surgery and anaesthesia. The answer to the question whether adjuvant chemotherapy and radiotherapy will improve survival from the disease can only come from randomised, controlled trails. In the future, immunotherapy and gene therapy may be of benefit but these are still many years from the clinical arena. We believe that current evidence suggests that patients with Dukes B and C colorectal cancer should be entered into trials of adjuvant therapy. This evidence is reviewed below among with estimates of the impact that adjuvant therapy would have on the outcome from this disease in Northern Ireland.

Abstract

Epithelial ovarian cancer (EOC) is the most lethal gynecologic cancer and also one of the most poorly understood. Other health issues that are affecting women with increasing frequency are obesity and diabetes, which are associated with dysglycemia and increased blood glucose. The Warburg Effect describes the ability of fast-growing cancer cells to preferentially metabolize glucose via anaerobic glycolysis rather than oxidative phosphorylation. Recent epidemiological studies have suggested a role for hyperglycemia in the pathogenesis of a number of cancers. If hyperglycemia contributes to tumour growth and progression, then it is intuitive that antihyperglycemic drugs may also have an important antitumour role. Preliminary reports suggest that these drugs not only reduce available plasma glucose, but also have direct effects on cancer cell viability through modification of molecular energy-sensing pathways. This review investigates the effect that hyperglycemia may have on EOC and the potential of antihyperglycemic drugs as therapeutic adjuncts.

Abstract

Tumor development is a complex process that relies on interaction and communication between a number of cellular compartments. Much of the mass of a solid tumor is comprised of the stroma which is richly invested with extracellular matrix. Within this matrix are a host of matricellular proteins that regulate the expression and function of a myriad of proteins that regulate tumorigenic processes. One of the processes that is vital to tumor growth and progression is angiogenesis, or the formation of new blood vessels from preexisting vasculature. Within the extracellular matrix are structural proteins, a host of proteases, and resident pro- and antiangiogenic factors that control tumor angiogenesis in a tightly regulated fashion. This paper discusses the role that the extracellular matrix and ECM proteins play in the regulation of tumor angiogenesis.

Abstract

A twelve year prospective wound audit was undertaken in an academic surgical unit. Data from 10,000 operations were analysed. Overall, wound infection rates decreased during this time. Infection rates in contaminated wounds in particular fell from 19.2% to 4.7%. This decrease in wound infection may be related in part to a change in the antibiotic prophylactic regimen and in part to the institution of the wound sepsis audit which provided regular information on the unit infection rates. This audit permitted early detection of adverse trends, and may have had a direct influence on surgical techniques.

38. Re-Staging Following Long-Course Chemoradiotherapy For Rectal Cancer: Does It Influence Management?

Abstract

Background: In patients with locally advanced or low rectal cancers, long-course chemoradiotherapy (LCCRT) is recommended prior to surgical management.1 The need for restaging afterwards has been questioned as it may be difficult to interpret imaging due to local tissue effects of chemoradiotherapy. The pupose of this study was to determine if restaging affected the management of patients receiving long-course chemoradiotherapy for rectal cancer. Methods: A retrospective review of patients with rectal cancer discussed at the South Eastern Health and Social Care Trust Lower Gastrointestinal Multi-Disciplinary Team Meeting (LGIMDT) in 2013 who had received long-course chemoradiotherapy was performed. Patients were identified from the Trust Audit Department, LGIMDT notes and patient records. Imaging results and outcomes from meetings were obtained through the Northern Ireland Picture Archiving and Communications System® (NIPACS) and Electronic Care Record® (ECR). Data including patient demographics, initial radiological staging and LGIMDT discussion, restaging modality and result, outcome from post-treatment LGIMDT discussion and recorded changes in management plans were documented using a proforma. Results: Seventy-one patients with rectal cancer were identified as having LCCRT in 2013 (M:F 36:35; age range 31 - 85 years). Fifty-nine patients were restaged following long-course treatment with computed tomography (CT) and magnetic resonance imaging (MRI). Twelve patients did not undergo restaging. Data was not available for 6 patients, one patient underwent emergency surgery, two patients were not fit for treatment, one failed to attend for restaging and two patients died prior to completion of treatment. Of the 59 patients restaged, 19 patients (32%) had their management plan altered from that which had been proposed at the initial LGIMDT discussion. The most common change in plan was not to operate. Ten patients had a complete clinical and radiological response to treatment and have undergone intensive follow-up. Nine patients had disease progression, with 3 requiring palliative surgery and 6 referred for palliative care. Conclusion: Of those patients who were restaged, 32% had their management plan altered from that recorded at the initial LGIMDT discussion. Seventeen per cent of patients in this group had a complete clinical and radiological response to treatment. Fifteen percent demonstrated disease progression. We recommend, therefore, that patients with rectal cancer be restaged with CT and MRI following long-course chemoradiotherapy as surgery may be avoided in up to 27% of cases. [ABSTRACT FROM AUTHOR]

42. Intraperitoneal Rupture of Ectopic Varices—a Rare Complication of Portal Hypertension

A 50 year old man presented with sudden abdominal pain, abdominal distension and shock. At emergency laparotomy a large amount of blood was found in the peritoneal cavity. There was micronodular cirrhosis of the liver and the spleen was enlarged. The bleeding was traced to distended veins in the right paracolic gutter which were oversewn and the abdomen closed. A coagulopathy was diagnosed and treatment including high dose aprotinin commenced. However, he continued to bleed and at a second laparotomy the area of previous haemorrhage was packed. Further deterioration continued until death 12 hours later. Intraperitoneal haemorrhage from ectopic varices is a rare occurrence. There is a high mortality rate usually due to an advanced coagulopathy. This is the first report of aprotinin being used in an attempt to treat this. On the basis of this report aprotinin would not seem to be of benefit for this condition.

A highly efficient, all-digital communications signal design employing convolutional coding and PN spectrum spreading is described for two-way transmission of voice and data between a manned spacecraft and ground. Variable-slope delta modulation is selected for analog/digital conversion of the voice signal, and a convolutional decoder utilizing the Viterbi decoding algorithm is selected for use at each receiving terminal. A PN spread spectrum technique is implemented to protect against multipath effects and to reduce the energy density (per unit bandwidth) impinging on the earth's surface to a value within the guidelines adopted by international agreement. Performance predictions are presented for transmission via a TDRS (tracking and data relay satellite) system and for direct transmission between the spacecraft and earth. Hardware estimates are provided for a flight-qualified communications system employing the coded digital signal design.

Concepts involved in determining the performance of coded digital communications systems are introduced. The basic concepts of convolutional encoding and decoding are summarized, and hardware implementations of sequential and maximum likelihood decoders are described briefly. Results of parametric studies of the Viterbi decoding algorithm are summarized. Bit error probability is chosen as the measure of performance and is calculated, by using digital computer simulations, for various encoder and decoder parameters. Results are presented for code rates of one-half and one-third, for constraint lengths of 4 to 8, for both hard-decision and soft-decision bit detectors, and for several important systematic and nonsystematic codes. The effect of decoder block length on bit error rate also is considered, so that a more complete estimate of the relationship between performance and decoder complexity can be made.
