30. Right ventricular hypertrophy and polycythaemia in rats after intermittent exposure to hypoxia

Abstract

1. Six groups of 20 male adult rats were maintained in an environmental chamber, each group for a period of 28 days. One group breathed air throughout its experimental period, and a second group breathed a normobaric atmosphere of 12% oxygen. The other four groups were exposed to this hypoxic atmosphere for only a proportion of each 24 h cycle: 2, 4 and 12 h daily, and eight periods of 30 min daily. 2. After 28 days, measurement was made, in each rat, of right ventricule (RV) weight and of red cell mass (RCM) by using 51Cr-labelled rat erythrocytes. 3. In the normoxic control group, RV weight corrected for log body weight in grams was 63.2 ± 1 mg/log body wt. and RCM was 2.02 ± 0.05 ml/100 g body wt. This was significantly less than in the group hypoxic for only 2 h each day for 28 days: RV weight 66.6 ± 0.8 mg/log body wt. (P < 0.05) and RCM 2.27 ± 0.05 ml/100g body wt. (P < 0.05). Greater increases compared with control were observed in all the other hypoxic groups. There was no significant difference in the increases in RV weight and RCM produced by daily hypoxia in a 4 h continuous period and daily hypoxia in eight 30 min periods. 4. The possible role of intermittent hypoxia in producing polycythaemia and pulmonary hypertension has been the subject of much speculation. Our results show that intermittent hypoxia is a potent stimulus to erythropoiesis and to pulmonary hypertension, reflected in RV hypertrophy. They support the view that abnormalities of respiration during sleep may be responsible for the polycythaemia and cor pulmonale seen in some patients with sleep apnoea syndromes and with chronic obstructive airways disease.

Abstract

Background The guidelines on chemotherapy and management of tuberculosis in the United Kingdom have been reviewed and updated. Methods A subcommittee was appointed by the Joint Tuberculosis Committee (JTC) of the British Thoracic Society to revise the guidelines published in 1990 by the JTC. In preparing the revised guidelines the authors took account of new published evidence and graded the strength of evidence for their recommendations. The guidelines have been approved by the JTC and the Standards of Care Committee of the British Thoracic Society. Recommendations(1) Patients with tuberculosis should be notified. (2) In view of the rising incidence of drug resistance, bacteriological confirmation and drug susceptibility testing should be sought whenever possible. (3) A six month short course regimen, with four drugs in the initial phase, should be used for all forms of tuberculosis, except meningitis, in both adults and children. (4) The fourth drug (ethambutol) in the initial phase can be omitted in certain circumstances. (5) Treatment of all patients should be supervised by physicians with full training in the management of tuberculosis and with direct working access to tuberculosis nurse specialists or health visitors. (6) Advice is given on (a) management in special situations and patient groups, (b) drug interactions, and special precautions and pretreatment screening, (c) chemoprophylaxis for different groups, and (d) the management of single and multiple drug resistance. (7) Advice is given on follow up after treatment and the organisational framework for tuberculosis services. (8) The role of directly observed therapy is discussed. (9) The management of multidrug resistant tuberculosis is explained in outline: such patients should be managed by physicians with special experience and in close liaison with the Mycobacterium Reference Units, and in hospitals with appropriate isolation facilities. (10) Infection control and segregation for such patients and for patients with dual infection with human immunodeficiency virus (HIV) and tuberculosis are covered in an appendix.

Abstract

Oxygen administration via a nasal cannula incorporating a small collapsible reservoir (Oxymizer, Chad Therapeutics Inc, California) was compared with delivery via a standard nasal cannula. Twelve patients with chronic, stable hypoxaemia (arterial oxygen tension less than 60 mm Hg (8.0 kPa)) were studied. Transcutaneous oxygen and carbon dioxide tensions were recorded by skin electrodes and oxygen saturation by ear oximetry. Baseline measurements during the breathing of air were compared with those made during the breathing of oxygen at flow rates of 0.5, 1.0, and 2.0 l/min via each device. Increases in saturation and transcutaneous oxygen tension were significantly greater at each flow rate with the reservoir device than with the conventional cannula. To produce similar improvements in oxygenation the reservoir device required an oxygen flow rate about half that of the conventional cannula. Use of the reservoir device may reduce the inconvenience and perhaps the cost of supplying domiciliary oxygen, and prolong the time during which patients may rely on a portable cylinder. Abstract

The sensitivity of the Enhanced Tuberculosis Surveillance (ETS) scheme for monitoring tuberculosis in children is unknown. We used the British Paediatric Surveillance Unit (BPSU) reporting scheme to conduct a prospective observational study of tuberculosis in children aged <16 yrs in the UK. Reported cases were then matched with records from the ETS database. A total of 320 cases were reported to the BPSU between January and December 2004. We estimated that there were 557 paediatric cases in England, Wales and Northern Ireland in 2004: 222 (40%) cases reported to both BPSU and ETS, 98 (18%) reported to BPSU but not ETS and 237 (42%) reported to ETS but not BPSU. Children aged <5 yrs were significantly less likely to be reported to ETS compared with older children (p<0.01). There is substantial under-reporting of childhood tuberculosis, especially of children aged <5 yrs. ETS provides a representative picture of the demographics but may miss approximately 20% of cases. This should be taken into account when planning training and resource requirements for tuberculosis. Increased efforts are needed to ensure that all paediatric cases are reported to ETS.

Abstract

The costs of multidrug-resistant tuberculosis (MDR TB) reach far beyond the cost of the clinical treatment of the patient. The first impact of the discovery of MDR TB in a population is the need to recognize that all TB patients have the potential of being MDR. Public health measures to prevent the spread of MDR TB, or to control or reverse the problem where spread has already occurred, can be extremely expensive to implement. At the level of the individual patient, the second-line drugs used to treat MDR TB are more expensive, and the remaining first-line drugs will have to be used for a longer time than in drug-susceptible TB. Negative-pressure units add to the costs of treatment, as does the increased nursing intensity required. The cost to the wider economy includes lost productivity and lost tax revenue to the state as well as the cost of supporting the family if the patient is the breadwinner.
