Aberrant preterm infant gut microbiota assembly predisposes to early-life disorders and persistent health problems. Here, we characterize gut microbiome dynamics over the first 3 months of life in 236 preterm infants hospitalized in three neonatal intensive care units using shotgun metagenomics of 2,512 stools and metatranscriptomics of 1,381 stools. Strain tracking, taxonomic and functional profiling, and comprehensive clinical metadata identify Enterobacteriaceae , enterococci, and staphylococci as primarily exploiting available niches to populate the gut microbiome. Clostridioides difficile lineages persist between individuals in single centers, and Staphylococcus epidermidis lineages persist within and, unexpectedly, between centers. Collectively, antibiotic and non-antibiotic medications influence gut microbiome composition to greater extents than maternal or baseline variables. Finally, we identify a persistent low-diversity gut microbiome in neonates who develop necrotizing enterocolitis after day of life 40. Overall, we comprehensively describe gut microbiome dynamics in response to medical interventions in preterm, hospitalized neonates. BackgroundAcute gastroenteritis develops in millions of children in the United States every year, and treatment with probiotics is common. However, data to support the use of probiotics in this population are limited.MethodsWe conducted a prospective, randomized, double-blind trial involving children 3 months to 4 years of age with acute gastroenteritis who presented to one of 10 U.S. pediatric emergency departments. Participants received a 5-day course of Lactobacillus rhamnosus GG at a dose of 1×1010 colony-forming units twice daily or matching placebo. Follow-up surveys were conducted daily for 5 days and again 14 days after enrollment and 1 month after enrollment. The primary outcome was moderate-to-severe gastroenteritis, which was defined as an illness episode with a total score on the modified Vesikari scale of 9 or higher (scores range from 0 to 20, with higher scores indicating more severe disease), within 14 days after enrollment. Secondary outcomes included the duration and frequency of diarrhea and vomiting, the duration of day-care absenteeism, and the rate of household transmission (defined as the development of symptoms of gastroenteritis in previously asymptomatic household contacts).ResultsAmong the 971 participants, 943 (97.1%) completed the trial. The median age was 1.4 years (interquartile range, 0.9 to 2.3), and 513 participants (52.9%) were male. The modified Vesikari scale score for the 14-day period after enrollment was 9 or higher in 55 of 468 participants (11.8%) in the L. rhamnosus GG group and in 60 of 475 participants (12.6%) in the placebo group (relative risk, 0.96; 95% confidence interval, 0.68 to 1.35; P=0.83). There were no significant differences between the L. rhamnosus GG group and the placebo group in the duration of diarrhea (median, 49.7 hours in the L. rhamnosus GG group and 50.9 hours in the placebo group; P=0.26), duration of vomiting (median, 0 hours in both groups; P=0.17), or day-care absenteeism (median, 2 days in both groups; P=0.67) or in the rate of household transmission (10.6% and 14.1% in the two groups, respectively; P=0.16).ConclusionsAmong preschool children with acute gastroenteritis, those who received a 5-day course of L. rhamnosus GG did not have better outcomes than those who received placebo. The purpose of this study was to evaluate the acceptability and usability of an online training to prepare Expanded Food and Nutrition Education Program (EFNEP) educators to collect 24-hr dietary recalls using the Automated Self-Administered 24-Hour Dietary Assessment Tool (ASA24). Fifty-eight educators in 17 states were recruited to take the training; 29 completed the online training, assisted two individuals in completing a 24-hr recall using ASA24, and completed a survey about their experiences. The sample included 26 respondents. The majority (n = 16; 61.5%) of the sample was EFNEP educators with college education. The majority of the respondents indicated that they found the readings and videos acceptable for learning (n = 21; 80.7%). Half of the participants (n = 13) felt prepared to collect data using ASA24. The majority (n = 22; 84.6%) had positive feedback about the training content. The training may be acceptable for preparing EFNEP educators with at least some college education to collect 24-hr recalls using ASA24. Because of potential bias due to self-selection and nonresponse, the training has been revised and is now intended to be used to train trainers of EFNEP paraprofessionals.

Intimate partner violence (IPV), an international public health matter, is linked to psychological distress including suicidal behavior. Despite the noted sequelae associated with IPV, many individuals display resilience in the face of IPV exposure. This study examines two potential protective factors in the IPV-suicidal ideation link, namely existential and religious well-being, in a sample of African American women. Low-income, African American, female suicide attempters who were abused by a partner within the previous year (N = 111) were administered the Conflicts Tactics Scale-2, Spiritual Well-Being Scale, and Beck Scale for Suicide Ideation. Mediational models testing indirect effects through the bootstrapping method revealed that the total effect of IPV on suicidal ideation was positive and significant, confirming suicidal ideation as a consequential outcome of IPV among African American women. This effect was mediated by existential well-being; however, religious well-being was not a significant mediator, suggesting that existential well-being serves a unique protective role in the IPV-suicidal ideation relation. The results provide additional support for the well-known positive effects of existential well-being on mental health among African American samples. Focus groups were convened with 29 paraprofessional educators from two land grant universities (LGUs) to document their experiences delivering an adult nutrition education curriculum through the Expanded Food and Nutrition Education Program (EFNEP) and to solicit their suggestions for curriculum improvement. Emergent themes indicated that paraprofessional educators desired greater flexibility in curriculum delivery, more activities that employ active learning, and simplification of curriculum content. Educators also expressed concerns regarding professional competence, perceptions of contradictory information within the curriculum, and a disconnect between program leaders’ expectations and the realities of program delivery in the community setting. Findings highlight benefits of including educators in curriculum development and revision processes and support the development of future studies to investigate ways to incorporate more flexibility into curricula without compromising core content. Findings also support the need for additional studies that assess whether food-based recommendations rather than nutrient-based recommendations could improve outcomes in EFNEP and other health and nutrition education programs. Elastin gene transcription is cell type specific and developmentally regulated, but the promoter often exhibits relatively weak activity in transient transfections of cells that express elastin at high levels. To search for positive-acting regulatory sequences, we isolated genomic clones spanning the mouse elastin gene and extensive 5'- and 3'-flanking regions. Restriction fragments of potential regulatory regions were ligated 5' or 3' relative to the active promoter to test for enhancer activity in transient transfections of fetal rat lung fibroblasts, which express elastin at high levels, and distal lung epithelial cells, which do not express detectable elastin. Fragments of intron 1 did not exhibit significant enhancer activity. Inclusion of the 84-bp exon 1 and adjacent 5'-untranslated region increased activity of the elastin promoter approximately sixfold compared with parental constructs. Transfections with constructs of varying promoter length showed that as little as 40 bp of the 5' end of exon 1 confers enhanced activity in elastin-expressing rat lung fibroblasts, but these constructs had variable activity in lung epithelial cell lines. This region, localized between the transcription start site and extending into exon 1, binds Spl in nuclear extracts from elastin-expressing cells. Federal nutrition education programs use food demonstrations and tastings of healthy recipes to elicit behavior change among their target audiences, but acceptance and liking of such recipes has not been systematically evaluated. This study aimed to assess acceptability and key sensory attributes (appearance, flavor, texture, saltiness, and sweetness) of culturally tailored recipes using sensory evaluation methods in a nontraditional setting. Affective ballots were used to assess overall liking (OL), key attributes, and preparation intent of seven new recipes on a 9‐point hedonic scale. Open‐ended feedback was also collected. A convenience sample of 338 low‐income adults rated two out of seven recipes as meeting a standard acceptability rating ≥7. These ratings were significantly related to their preparation intent. Flavor and texture were revealed as attributes that significantly influenced OL. Findings support the practicality of sensory evaluation methodology in nontraditional settings for the development of recipes in Federal nutrition education programs. Practical applications: This novel application of sensory evaluation in nontraditional settings can provide curriculum developers' of federal nutrition education programs a holistic understanding of their target audience's recipe needs. Developers have the ability to analyze specific attributes, ask about preparation intent behaviors, and more accurately determine areas for improvement of each recipe through these targeted consumer sensory tests. Developers can thus ensure that recipes in their curricula (present and future) are well equipped to promote healthy behavior change. Additionally, this information can be built upon over the course of many sensory evaluations of more recipes to create a living record of the target audience's likes and dislikes for future recipe development.
