2. Utilizing a Data-Driven Approach to Address Social Vulnerability in Outpatient Dialysis: A Quality Improvement Project

Abstract

Through a quality improvement project, we developed an initiative that leveraged patient- and community-level data to address health disparities and social vulnerability among patients receiving outpatient dialysis, including both incenter and home modalities. Using the Area Deprivation Index, we identified patients living in areas with the highest levels of deprivation and developed targeted interventions to help address adverse social determinants of health to improve patient health outcomes. Our quality improvement project demonstrates the potential of data-driven approaches to identify and address health disparities in outpatient dialysis, and highlights the importance of addressing social determinants of health in improving patient outcomes. 3. From Education to Empowerment: Redesigning the Role of Students in College Health Promotion

Abstract

College health promotion departments frequently employ peer health educators to disseminate relevant education and conduct outreach to their student body. While there are certainly benefits to these programs, this approach is outdated and does little to empower students or engage them in the process of health promotion. In this viewpoint article, I describe the Community Health Organizer model, which expands students' role in peer engagement and advocacy beyond the traditional peer educator or peer counseling programs. Finally, I provide recommendations for college health practitioners interested in implementing a similar model on their campuses.

Abstract

The response of the hydrological cycle to anthropogenic climate change, especially across the tropical oceans, remains poorly understood due to the scarcity of long instrumental temperature and hydrological records. Massive shallow-water corals are ideally suited to reconstructing past oceanic variability as they are widely distributed across the tropics, rapidly deposit calcium carbonate skeletons that continuously record ambient environmental conditions, and can be sampled at monthly to annual resolution. Climate reconstructions based on corals primarily use the stable oxygen isotope composition (δ18O), which acts as a proxy for sea surface temperature (SST), and the oxygen isotope composition of seawater (δ18Osw), a measure of hydrological variability. Increasingly, coral δ18O time series are paired with time series of strontium-to-calcium ratios (Sr/Ca), a proxy for SST, from the same coral to quantify temperature and δ18Osw variability through time. To increase the utility of such reconstructions, we present the CoralHydro2k database, a compilation of published, peer-reviewed coral Sr/Ca and δ18O records from the Common Era (CE). The database contains 54 paired Sr/Ca–δ18O records and 125 unpaired Sr/Ca or δ18O records, with 88 % of these records providing data coverage from 1800 CE to the present. A quality-controlled set of metadata with standardized vocabulary and units accompanies each record, informing the use of the database. The CoralHydro2k database tracks large-scale temperature and hydrological variability. As such, it is well-suited for investigations of past climate variability, comparisons with climate model simulations including isotope-enabled models, and application in paleodata-assimilation projects. The CoralHydro2k database is available in Linked Paleo Data (LiPD) format with serializations in MATLAB, R, and Python and can be downloaded from the NOAA National Center for Environmental Information's Paleoclimate Data Archive at https://doi.org/10.25921/yp94-v135 (Walter et al., 2022).

22. CDC-Funded HIV Testing and Linkage to HIV Medical Care Among American Indian and Alaska Native People in the United States, 2014-2020.

Abstract

Objective: An estimated 1 in 5 American Indian and Alaska Native (AI/AN) adults living with HIV are unaware of their status. We investigated HIV testing among AI/AN people receiving a Centers for Disease Control and Prevention (CDC)–funded test from 2014 through 2020. Methods: We analyzed data on CDC-funded HIV tests reported by health departments and community-based organizations in the United States. We described the number of CDC-funded HIV tests, the percentage of people with newly and previously diagnosed HIV, and linkage to HIV medical care within 90 days of diagnosis. Results: CDC-funded health departments and community-based organizations provided 99 227 HIV tests to AI/AN people during 2014-2020. Seven hundred thirty-five (0.7%) AI/AN people were diagnosed with HIV; 361 (0.4%) were newly diagnosed, 319 (0.3%) had a previous HIV diagnosis, and 55 (0.1%) had a previously unknown HIV status. Positivity for new diagnoses was highest among the following population groups tested in non–health care settings: men who had sex with men (MSM; n = 72, 1.2%), MSM who inject drugs (n = 12, 1.8%), and transgender people (n = 12, 1.5%). The percentage of linkage to HIV medical care was 80.6% for newly diagnosed people and 78.2% for previously diagnosed people. Conclusions: MSM AI/AN, including those who inject drugs, and transgender AI/AN may benefit from prioritized HIV testing. All AI/AN people with HIV, whether newly or previously diagnosed, should rapidly link to HIV medical care and receive support throughout the continuum of care. Abstract

'Hyphy,' a genre of rap and lifestyle associated with Bay Area hip hop evolved into a counter-cultural social movement for marginalized youth in early 2000. Hyphy originated from Black youth as a musical protest in response to their historical lack of social power, economic resources and systematic institutional oppression. Hyphy provided a space to release tension, celebrate life and freedom of expression, primarily as a means of resisting hegemonic perceptions of Black youth and their cultural productions. Applying a cultural studies theoretical approach, this ethnographic research examines literature and media coverage pre and post Hyphy highlighting the ways in which it fostered a personal and political agenda, attracting organizations that employed hip hop to provide direct services to youth as a means of advocating for social justice. This article argues that the Hyphy Movement although dated, supported the foundation of the Black Lives Matter movement in the Bay Area by playing a pivotal role in shifting the revolutionary consciousness of young people when addressing police violence during a pivotal social upheaval in 2009, the unjust murder of Oscar Grant III. 29. Executive Order 12898 and Social, Economic, and Sociopolitical Factors Influencing Toxic Release Inventory Facility Location in EPA Region 6: A Multi-Scale Spatial Assessment of Environmental Justice

Abstract

Toxic Release Inventory facilities are among the many environmental hazards shown to create environmental inequities in the United States. This project examined four factors associated with Toxic Release Inventory, specifically, manufacturing facility location at multiple spatial scales using spatial analysis techniques (i.e., O-ring statistic and geographically weighted regression (GWR)). The first project in this study examined the influence of Executive Order 12898 on the spatial distribution of Toxic Release Inventory facilities and whether the influence or ideologies of environmental agency administrators, the Presidential, and Congress (political climate) played a role in the placement of facilities post-enactment in EPA Region 6. The results suggest that neither EO 12898 nor political climate influence facility location uniformly and that county and local level factors may be of importance. The second study examined three competing environmental inequity hypotheses. Discrimination, economic, and sociopolitical factors from environmental inequity literature were used to assess statewide effects at the local (census block) level in Oklahoma. Discrimination factors explained a greater proportion of the variability; however, the model including factors from all three models was superior. The third portion of this work combined the results and concepts from the first study with a real life situation to create a case-based teaching unit. The case study used environmental justice policy and environmental law in the context of protecting a small, Native American group in northern Oklahoma from persistent exposure to carbon black, a known carcinogen. The case study was designed to fill a gap in environmental justice curriculum materials and provide a resource for instructors to teach students about environmental justice concepts, Executive Order 12898 and existing environmental laws in environmental studies, sociology, and political science courses. This work contributes to environmental justice literature by evaluating efforts to achieve environmental justice, recognizing the role of the state in producing environmental justice outcomes, advancing our understanding of the spatial scale(s) of environmental justice, incorporating new methodology (from ecology) to address environmental justice research methodological concerns, examining trends in the socio-ecological landscape, and adding to curriculum materials on environmental justice. Abstract

Background: The ProtecT trial reported intention-to-treat analysis of men with localised prostate cancer (PCa) randomly allocated to active monitoring (AM), radical prostatectomy, and external beam radiotherapy. Objective: To determine report outcomes according to treatment received in men in randomised and treatment choice cohorts. Design, Setting, and Participants: This study focuses on secondary care. Men with clinically localised prostate cancer at one of nine UK centres were invited to participate in the treatment trial comparing AM, radical prostatectomy, and radiotherapy. Intervention: Two cohorts included 1643 men who agreed to be randomised; 997 declined randomisation and chose treatment. Outcome Measurements and Statistical Analysis: Health-related quality of life impacts on urinary, bowel, and sexual function were assessed using patient-reported outcome measures. Analysis was carried out based on treatment received for each cohort and on pooled estimates using meta-analysis. Differences were estimated with adjustment for known prognostic factors using propensity scores. Results and Limitations: According to treatment received, more men receiving AM died of PCa (AM 1.85%, surgery 0.67%, radiotherapy 0.73%), whilst this difference remained consistent with chance in the randomised cohort (p = 0.08); stronger evidence was found in the exploratory analyses (randomised plus choice cohort) when AM was compared with the combined radical treatment group (p = 0.003). There was also strong evidence that metastasis (AM 5.6%, surgery 2.4%, radiotherapy 2.7%) and disease progression (AM 20.35%, surgery 5.87%, radiotherapy 6.62%) were more common in the AM group. Compared with AM, there were higher risks of sexual dysfunction (95% at 6 mo) and urinary incontinence (55% at 6 mo) after surgery, and of sexual dysfunction (88% at 6 mo) and bowel dysfunction (5% at 6 mo) after radiotherapy. The key limitations are the potential for bias when comparing groups defined by treatment received and outdating of the interventions being evaluated during the lengthy follow-up required in trials of screen-detected PCa. Conclusions: Analyses according to treatment received showed increased rates of disease-related events and lower rates of patient-reported harms in men managed by AM compared with men managed by radical treatment, and stronger evidence of greater PCa mortality in the AM group. Patient Summary: More than 90 out of every 100 men with localised prostate cancer do not die of prostate cancer within 10 yr, irrespective of whether treatment is by means of monitoring, surgery, or radiotherapy. Side effects on sexual and bladder function are much better after active monitoring, but the risks of spreading of prostate cancer are more common.
