28. Influence of socieoconomic status on prognosis of colorectal cancer: a population-based study in Cote D'Or, France

Abstract

Background. The prognostic significance of the socioeconomic status of patients treated for colorectal cancer, although stated in some studies, remains controversial. Methods. The authors studied a population-based series of 771 patients with colorectal cancer diagnosed in Cote D'Or, France, between January 1976 and December 1980. Survival was studied for the first 5 years after diagnosis. The relationship between socioeconomic status, assessed by comfort of housing, and the main clinical prognostic factors was examined. The influence of comfort of housing on prognosis was studied with the Cox model, while controlling for these factors. Interactions between type of housing and other prognostic factors also were tested. Results. Compared with patients living in a comfortable house, patients in the medium or no-comfort categories were more likely to receive diagnosis at an advanced stage (P = 0.03) and be treated by palliative therapy (P = 0.0005). After adjustment for these factors and age, sex, place of residence, and tumor site, patients living in no comfort had a twofold higher risk of dying during the follow-up period compared with patients living in comfortable housing, with the relative risk being 1.5 for the medium comfort category (95% confidence intervals, 1.3-3.2 and 1.2-1.7, respectively). This effect was more remarkable in patients with early-stage tumors than in patients with advanced tumors. Conclusions. Such findings should prompt public health measures for earlier access to care structures for people in a lower social class and research for a better understanding of the host-tumor relationship.

29. Macroprudential policy and quantitative instruments: a European historical perspective

Abstract

From 1945 to the 1970s, rather than steering interest rates, European central banks used a variety of quantitative instruments to attain their targets in terms of monetary aggregates and credit, and as part of selective credit policies. A number of the tools currently being referred to as macroprudential (loan to value ratio, reserve requirements, liquidity ratio, etc.) thus have historical precedents, which this article sets out to examine. The objectives pursued in the past were not always the same as those being used to justify the implementation of macroprudential policies today. However, the instruments were used in part to modulate the expansion of credit and direct it towards specific sectors, as well as to control bank liquidity levels in an effort to secure financial stability. A historical perspective can thus help to shed some light on how prudential instruments can complement or, on the contrary, conflict with monetary policy. But this article also argues that historical comparisons need to be regarded with caution, as the financial and economic sectors have undergone radical changes, as have the institutional framework and objectives of monetary policy.

Abstract

Background In chronic hepatitis C, higher ribavirin (RBV) concentrations are associated with sustained virological response (SVR); target concentration cutoffs have been proposed. As RBV displays interindividual variability, monitoring of RBV plasma levels appears relevant. The impact of RBV therapeutic drug monitoring (TDMRBV) on SVR has not been explored in current practice. Our study aimed to assess this impact. Methods Three patient groups were defined as RBV cutoffs achieved at week 12 (group A1), not achieved (group A2), and one without RBV concentration assessment (group B). A predictive model assessed the group impact on SVR in multivariate analysis, while adjusting for additional predictive factors. A specific evaluation of HIV–HCV-coinfected patients was performed. Results A total of 122 patients were included. In group A1 ( n=30, HIV-positive =18), SVR, relapse and non-response rates were 60%, 17% and 23%, respectively; in group A2 ( n=32, HIV-positive =18), 25%, 19% and 56%, respectively; and in group B ( n=60, HIV-positive =3), 52%, 33% and 15%, respectively ( P=0.0004). The patient group was an independent predictor of SVR ( P=0.01), along with baseline viral load and HCV genotype. HIV coinfection did not impede the SVR rate. The cutoffs were achieved in 62% and 28% ( P=0.008) of patients, when TDMRBV was performed or not, respectively. Conclusions The achievement of RBV cutoffs is a predictive factor of SVR independent of HIV coinfection. It makes it possible to reach high SVR rates, avoid relapse and obtain the same SVR rates in HIV–HCV-coinfected as in HCV-monoinfected patients. TDMRBV enables RBV concentration cutoffs to be reached more frequently and could thus be a useful tool to optimize hepatitis C treatment.
