Background Medicinal cannabis has been legalised for use for a range of specified medical conditions in Australia since 2016. However, the nature of the government regulations and the subsequent complexity of prescribing, as well as doctors’ safety uncertainties and the stigma of the plant, remain contributing barriers to patient access. Media representations can offer insights into the nature of the discourse about new medical products and therapies and how ideas and understandings about social phenomena become constructed. Focusing on professional medical publications, this study sought to investigate how medicinal cannabis is being represented in professional medical publications. Methods Using a content analysis approach, we investigated articles about medicinal cannabis from 2000 to the end of 2019 in the Medical Journal of Australia, Australian Doctor, Medical Observer, Australian Journal of General Practice, Australian Family Physician, and Australian Medicine. Articles were coded according to article type, framings of cannabis, headline and article tone, and key sources used in the article. We also used manifest textual analysis to search for word frequencies, and specific conditions referred to in the articles retrieved. Results A total of 117 articles were retrieved for analysis, the majority of which were news stories for a physician audience. Across the longitudinal period, we found that most reports carried a positive tone towards medicinal cannabis. Cannabis is most frequently framed as a legitimate therapeutic option that is complex to prescribe and access, does not have a strong evidence base to support its use, and also carries safety concerns. At the same time, the outlook on cannabis research data is largely positive. Primary sources most frequently used in these reports are peer-reviewed journals or government reports, voices from medical associations or foundations, as well as government and university researchers. Chronic pain or pain were the conditions most frequently mentioned in articles about cannabis, followed by epilepsy, cancer or cancer pain, and nausea and chemotherapy. Conclusions This analysis offers evidence that medicinal cannabis is being framed as a valid medicine advocated by the community, with potential for addressing a range of conditions despite the lack of evidence, and a medicine that is not free of risk.

Scholarly knowledge about the communication dimensions of the COVID-19 pandemic contributes to interdisciplinary lessons for future public health emergencies. This collection nurtures further thinking about the ‘messiness’ that comes with communication during an infectious disease outbreak such as COVID-19, and how we can embrace and learn from it. The chapter highlights the importance of communication preparedness and the urgency to make meaningful efforts to improve communication knowledge, especially during times when public health measures are highly reliant on communication, without which health messages remain unheard and unattended to. It further discusses the need for partnerships to address communication inequalities between health authorities and trusted community organisations; and the mobilisation of community-led organisations as key partners for the people most severely impacted by COVID-19. The chapter concludes by suggesting areas of future research to contribute to a nuanced understanding of the diverse pandemic experiences of specific sub-groups and populations with varying cultural and ethnic influences.

The COVID pandemic has overwhelmed many countries in their attempts at tracking and tracing people infected with the disease. Our paper examines how tracking and tracing is done looking at manual and technological means. It raises the issues around efficiency and privacy, etc. The paper investigates more closely the approaches taken by two countries, namely Taiwan and the UK. It shows how tracking and tracing can be handled sensitively and openly compared to the bungled attempts of the UK that have led to the greatest number of dead in Europe. The key messages are that all communications around tracking and tracing need to open, clear, without confusion and delivered by those closest to the communities receiving the messages.This occurred in Taiwan but in the UK the central government chose to close out local government and other local resources. The highly centralised dirigiste approach of the government alienated much of the population who came to distrust government. As local government was later brought into the COVID fold the messaging improved. Taiwan always remained open in its communications, even allowing citizens to participate in improving the technology around COVID. Taiwan learnt from its earlier experiences with SARS, whereas the UK ignored its pandemic planning exercises from earlier years and even experimented with crude ideas of herd immunity by letting the disease rip through the population--an idea soon abandoned. We also derive a new type of citizen from the pandemic, namely the R citizen. This unfortunate archetype is both a blessing and a curse. If the citizen scores over 1 the disease accelerates and the R citizen is chastised, whereas if the citizen declines to zero it disappears but receives no plaudits for their behaviour. The R citizen can neither exist or die, rather like Schrodinger's cat. R citizens are of course datafied individuals who are assemblages of data and are treated as distinct from humans. We argue they cannot be so distinguished without rendering them inhuman. This is as much a moral category as it is a scientific one. We suggest one approach to the handling of data through blockchain as a means of ensuring the integrity and transparency of the data.

This chapter introduces the diverse communication dimensions of the COVID-19 pandemic that are analysed and discussed in this book, which brings together scholars from around the world. Their disciplinary insights illuminate a wide range of communication challenges and opportunities. The chapter articulates the book’s contribution to knowledge about the pandemic through the detailed analyses and perspectives of scholars from communication, public health, journalism, sociology, psychology, and political science. In introducing each of the chapters, we identify key findings and themes as they variously relate to the roles of journalists, governments, public health agencies, scientists, health professionals, and citizens in communicating and responding to the medical, scientific, social, cultural, political, and technological dimensions of this profoundly mediatised public health crisis.

The objective of the research was to map the discourse about herbal medicine in Australia’s primary medical journal, the Medical Journal of Australia, over 4 decades. Manifest content analysis of 148 articles about herbal or complementary and alternative medicine (CAM) in journal articles from 1966 to 2008 was undertaken. Issues of risk dominated the discourse about herbal medicine in the journal (70%), with a focus on adverse events (42%) as the most significant risk of herbal and other complementary medicines. Toxicity (23%) and drug interactions (18%) were the most frequently mentioned specific risk items. Items that acknowledged efficacy (32%) outweighed those suggesting ineffectiveness (11%). Case reports, reviews, research articles, and letters all had risk frequencies of more than 70%. Review articles and research articles had a high rate of reference to both risk and benefit. Review articles were found to have greater reference to benefit than risk. Possible reasons for these findings are discussed.

Evidence-based medicine (EBM) has been advocated as a new paradigm in orthodox medicine and as a methodology for natural medicines, which are often accused of lacking an adequate scientific basis. This paper presents the voices of tradition-sensitive naturopathic practitioners in response to what they perceive as an ideologic assault by EBM advocates on the validity and integrity of natural medicine practice. Those natural medicine practices, which have tradition-based paradigms articulating vitalistic and holistic principles, may have significant problems in relating to the idea of EBM as developed in biomedical contexts. The paper questions the appropriateness of imposing a methodology that appears to minimize or bypass the philosophic and methodological foundations of natural medicine, and that itself seems primarily driven by political considerations.

Accelerated solvent extraction (ASE) is a new sample extraction method offering a number of advantages such as low pe -extraction cost, reduced solvent and time consumption, and simplified extraction protocols. In this study, the ASE method was applied to the extraction of polycyclic aromatic hydrocarbons (PAHs) from biological samples. For recovery studies, fish tissues and ground pork were used as sample matrices. Sample aliquots fortified with 16 PAHs were extracted by ASE, and the extracts were treated with sulfuric acid and Florisil, followed by gas chromatography-mass spectrometry analysis. The PAH recoveries by the ASE method were found to be comparable with or better than those by Soxhlet extraction. The extraction and quantitation method was then applied to the determination of PAHs in several smoked meat samples obtained from a local market. Up to 12 PAHs were found to be present at concentrations ranging from 3 to 52 ng/g wet sample.

The evaluation described in this article compared the Therevac SB "mini-enema" with bisacodyl suppositories in the bowel management programs of patients with spinal cord injury (SCI). Of particular interest were (a) determining whether the additional costs of Therevac SB could be justified and (b) identifying the degree of bowel program improvement possible. Fourteen SCI patients were selected from the SCI inpatient unit and the clinic of a Department of Veterans Affairs facility. All patients experienced a delay of at least 45 minutes between insertion of a suppository and the beginning of stool evacuation. Each patient used bisacodyl suppositories for five bowel programs, then Therevac SB for five programs, finally repeating the bisacodyl suppositories for five more programs. Each patient maintained a bowel program log. Ten patients completed the evaluation. Using a MANOVA, the authors found a significant difference between bisacodyl and Therevac SB mean evacuation times for this group. An analysis of direct and indirect costs related to bowel care with the two regimens is also presented.

Complementary and alternative medicine (CAM) has developed into a complex and formidable commercial, sociocultural and political force in Australia, and given its influence, it is a relevant subject for scholars, health practitioners, health communicators, journalists, policy-makers, and consumers of healthcare products and services. This research will consider a relative newcomer to the claims-making space about CAM in the Australian health media-scape; the Friends of Science in Medicine (FSM), an activist group of medical practitioners, researchers, and scientists, founded in late 2011. Using content analysis supported by NVivo, I searched for articles specifically referring to FSM and measured the patterns and frequencies of media frames, intonation and sources that are featured in Australian mainstream news reports between December 2011 and April 2017. The negative headlining and intonation of reports predominated, along with framing CAM as part of a lucrative, undisciplined and unethical industry as well as an illegitimate healthcare approach, more broadly. The findings offer insight into how journalists respond, replicate or reconstruct the framings that are provided by an influential and elite group of medical practitioners and scientists, and readdresses issues surrounding the need for more critical health reporting in Australia.
