ObjectiveTo investigate the contribution of duration and temporal dispersion (TD) of the distal compound muscle action potential (CMAP) in discriminating chronic inflammatory demyelinating polyneuropathy (CIDP) from diabetic sensorimotor polyneuropathy (DSP) and from CIDP+DSP.MethodsWe performed a retrospective review of patients diagnosed with CIDP, DSP and CIDP+DSP (responsive to immunotherapy) and examined differences in CMAP duration and TD at baseline.ResultsWe included 59 subjects: 17 CIDP, 21 DSP and 21 CIDP+DSP. Of these, 16 (94.1%) CIDP, 18 (85.7%) CIDP+DSP and 1 (4.7%) DSP fulfilled the 2010 EFNS/PNS criteria for definite CIDP. There was no difference in CMAP duration or TD in all nerves (compound outcome) or in individual motor nerves. Patients with CIDP/CIDP+DSP had more conduction blocks, slower conduction velocities and more prolonged F wave latencies than those with DSP.ConclusionMeasures of CMAP duration and TD were not helpful in distinguishing CIDP, DSP or CIDP+DSP patients; however, parameters such as F-wave latencies, conduction blocks or the number of demyelinating parameters were useful in this separation.SignificanceThere are no definite nerve conduction criteria to distinguish patients with CIDP+DSP from DSP alone. Further studies focusing on measures of demyelination may provide stronger evidence to guide treatment decisions in CIDP + DSP patients.

Introduction: Endolymphatic hydrops is the pathophysiological substrate of Ménière’s disease. The changes in the inner ear, transmitted to the middle ear through changes in the ossicular chain mobility, can be quantified by wideband tympanometry, through the measurement of the acoustic absorbance at multiple frequencies, represented by the sound energy absorbed by the middle ear, even at its early stages. Studying the behavior of the middle ear through the absorbance in patients with endolymphatic hydrops under ambient pressure and under peak pressure can be useful for detecting Ménière's disease. Objective: To characterize acoustic absorbance behavior in subjects with symptomatic and asymptomatic Ménière's disease compared to controls, in order to verify the ability of wideband tympanometry to detect Ménière's disease. Methods: We carried out a cross-sectional study with a diagnostic approach comparing the findings of wideband tympanometry at ambient pressure and peak pressure between the ears of the control group (n = 30), the asymptomatic group (n = 21) and the symptomatic group (n = 9). Results: Different peak pressure values were found between the ears of the control group (0 daPa), the asymptomatic group (−11 daPa) and the symptomatic group (−192 daPa), with p < 0.05 by the Kruskal-Wallis test, Mann Whitney test and Bonferroni correction. Different absorbance values were found between the ears of the symptomatic group and the asymptomatic group compared to the control group for low frequencies at ambient pressure and peak pressure, with p < 0.05 by the Kruskal-Wallis test, Mann Whitney test and Bonferroni correction. Conclusions: The Wideband Tympanometry test was capable of identifying the presence of Ménière´s disease, and to differentiate between asymptomatic and symptomatic patients, when comparing them with healthy individuals.

RESUMO Objetivo avaliar se a posturografia, exame que avalia a habilidade de manter o equilíbrio em condições sensoriais conflitantes, pode identificar risco de queda em idosos com tontura. Métodos comparar os resultados posturográficos de idosos com e sem história de quedas, pareados por gênero, idade e diagnóstico etiológico da tontura. Resultados dezoito idosos com quedas – 4 com único episódio e 14 com história de 2 ou mais quedas no último ano - foram comparados com 18 idosos sem quedas, pareados por gênero, idade e diagnóstico etiológico. Pacientes com quedas apresentaram resultados piores para as análises de dependência visual (p=0,04, p=0,01, p=0,03). Pacientes com quedas recorrentes (2 ou mais episódios) apresentaram piores resultados em diversas condições sensoriais: somatossensorial, vestibular, dependências visuais e índice de equilíbrio composto. Conclusão a posturografia mostrou-se útil na identificação de idosos com quedas, principalmente em indivíduos com quadros recorrentes.

Introduction: Neurotology is a rapidly expanding field of knowledge. The study of the vestibular system has advanced so much that even basic definitions, such as the meaning of vestibular symptoms, have only recently been standardized. Objective: To present a review of the main subjects of neurotology, including concepts, diagnosis and treatment of Neurotology, defining current scientific evidence to facilitate decision-making and to point out the most evidence-lacking areas to stimulate further new research. Methods: This text is the result of the I Brazilian Forum of Neurotology, which brought together the foremost Brazilian researchers in this area for a literature review. In all, there will be three review papers to be published. This first review will address definitions and therapies, the second one will address diagnostic tools, and the third will define the main diseases diagnoses. Each author performed a bibliographic search in the LILACS, SciELO, PubMed and MEDLINE databases on a given subject. The text was then submitted to the other Forum participants for a period of 30 days for analysis. A special chapter, on the definition of vestibular symptoms, was translated by an official translation service, and equally submitted to the other stages of the process. There was then a in-person meeting in which all the texts were orally presented, and there was a discussion among the participants to define a consensual text for each chapter. The consensual texts were then submitted to a final review by four professors of neurotology disciplines from three Brazilian universities and finally concluded. Based on the full text, available on the website of the Brazilian Association of Otorhinolaryngology and Cervical-Facial Surgery, this summary version was written as a review article. Result: The text presents the official translation into Portuguese of the definition of vestibular symptoms proposed by the Bárány Society and brings together the main scientific evidence for each of the main existing therapies for neurotological diseases. Conclusion: This text rationally grouped the main topics of knowledge regarding the definitions and therapies of Neurotology, allowing the reader a broad view of the approach of neurotological patients based on scientific evidence and national experience, which should assist them in clinical decision-making, and show the most evidence-lacking topics to stimulate further study. Resumo: Introdução: A otoneurologia é uma área de conhecimento que tem se expandido muito rapidamente. O estudo do sistema vestibular tem avançado tanto que mesmo definições básicas, como o significado dos sintomas vestibulares, foram apenas recentemente padronizadas. Objetivo: Apresentar uma revisão dos principais assuntos da otoneurologia, inclusive conceitos, diagnóstico e tratamento da otoneurologia, definir a evidência científica atual para facilitar a tomada de decisões e demonstrar as áreas mais carentes de evidência para estimular novas pesquisas. Método: Este texto é fruto do I Fórum Brasileiro de Otoneurologia, que reuniu os principais pesquisadores brasileiros dessa área para uma revisão da literatura. Serão feitos três trabalhos de revisão a serem publicados. Este primeiro abordou as definições e as terapias, o segundo abordará as ferramentas diagnósticas e o terceiro definirá os principais diagnósticos. Cada autor fez um levantamento bibliográfico na base de dados da Lilacs, SciELO, Pubmed e Medline de um determinado assunto. O seu texto foi então submetido aos demais participantes do Fórum por 30 dias para análise. Um capítulo especial, da definição dos sintomas vestibulares, foi traduzido por serviço de tradução oficial e igualmente submetido às demais etapas do processo. Houve então uma reunião presencial em que todos os textos foram apresentados oralmente e houve uma discussão entre os participantes para a definição de um texto consensual para cada capítulo. Os textos consensuais foram então submetidos a uma revisão final por quatro professores de otoneurologia de três universidades brasileiras e, por fim, finalizado. A partir do texto completo, publicado no site da Associação Brasileira de Otorrinolaringologia e Cirurgia Cérvico-Facial, foi escrita esta versão-resumo como artigo de revisão. Resultado: O texto apresenta a tradução oficial para o português da definição dos sintomas vestibulares propostos pela Barany Society e agrupa as principais evidências científicas para cada um das principais terapias existentes para as doenças otoneurológicas. Conclusão: Este texto agrupou de forma racional os principais tópicos de conhecimento a respeito das definições e terapias da otoneurologia, permite ao leitor uma visão ampla da abordagem dos pacientes otoneurológicos baseada em evidências científicas e experiência nacional, que deverá auxiliá-lo na tomada de decisões clínicas, e mostra os assuntos mais carentes de evidência para estimular novos estudos. Few studies have evaluated the influence of idiosyncrasies that may influence the judgment of space-time orientation after passive motion. We designed a study to assess the influence of anxiety/depression (which may distort time perception), motion sickness susceptibility (which has been related to vestibular function, disorientation, and to the velocity storage mechanism), and personal habits on the ability to update orientation, after passive rotations in the horizontal plane. Eighty-one healthy adults (22–64 years old) accepted to participate. After they completed an in-house general health/habits questionnaire, the short Motion Sickness Susceptibility Questionnaire, the Hospital Anxiety and Depression Scale (HADS), the Pittsburgh Sleep Quality Index, and the short International Physical Activity Questionnaire, they were exposed to 10 manually driven whole-body rotations (45°, 90°, or 135°), in a square room, with distinctive features on the walls, while seated in the normal upright position, unrestrained, with noise-attenuating headphones and blindfolded. After each rotation, they were asked to report which wall or corner they were facing. To calculate the error of estimation of orientation, the perceived rotation was subtracted from the actual rotation. Multivariate analysis showed that the estimation error of the first rotation was strongly related to the results of the orientation test. The magnitude and the frequency of estimation errors of orientation were independently related to HADS anxiety sub-score and to adult motion sickness susceptibility, with no influence of age, but a contribution from the interaction of the use of spectacles, the quality of sleep and sex. The results suggest that idiosyncrasies may contribute to the space-time estimation of passive self-motion, with influence from emotional traits, adult motion sickness susceptibility, experience, and possibly sleep quality.

Croton urucurana Baillon, espécie conhecida popularmente como “sangria d’água”, pertence à família Euphorbiaceae, possui grande importância na medicina popular, a qual lhe atribui propriedades antibacterianas, anti-inflamatórias, antissépticas, antivirais, antidiarreicas, antissecretória intestinal e cicatrizante. Os objetivos deste estudo foram verificar a presença, nos extratos aquoso, etanólico e hidroalcoólico, de metabolitos secundários (alcaloides, antraquinonas, esteroides/triterpenoides, flavonoides, glicosídeos, saponinas e taninos), analisar a inibição do crescimento das bactérias Staphylococcus aureus ATCC 25923 e Salmonella thyphimurium ATCC 14028, e verificar a atividade antioxidante através do método DPPH. Para o perfil fitoquímico o melhor extrato foi o aquoso, quanto à susceptibilidade bacteriana, houve inibição com o extrato etanólico e o extrato com a maior atividade antioxidante foi o hidroalcóolico, apresentando IC50 1,3 μg/mL. Considerando os resultados obtidos no estudo, foi possível concluir que os extratos da Croton urucurana Baillon apresentam resultados interessantes e que mais investigações devem ser realizadas para a elucidação aprimorada dos seus benefícios.

En enero de 1979, a unos meses de iniciado el pontificado de Juan Pablo II, jóvenes universitarios identificados con el catolicismo intransigente y vinculados a organizaciones transnacionales de derechas, fundaron en la Ciudad de México el Consejo Hispanoamericano de Estudiantes (CHE). La organización estableció una agenda regional común y se propuso coordinar las actividades de los jóvenes en sus diferentes países. Un año después, en medio de las celebraciones en Argentina por el año Mariano, los integrantes del consejo se reunieron en la ciudad de Córdoba para dar continuidad a sus objetivos. En las actividades llevadas a cabo, contaron con la asistencia de autoridades eclesiásticas y destacados intelectuales católicos. Este artículo se propone demostrar la presencia y protagonismo juvenil entre los grupos de católicos laicos integristas y de la fortaleza de las alianzas transnacionales católicas en un contexto de intensificación del anticomunismo durante la Guerra Fría. La reconstrucción y análisis de los congresos ha sido posible a partir de la revisión hemerográfica y de archivos de inteligencia de Argentina y México. Estas fuentes han sido contrastadas con el testimonio de un militante inactivo de las organizaciones católicas mexicanas. Su narración ofreció numerosos elementos para repensar el papel de los jóvenes católicos en el periodo, además, expone las tensiones y disputas en torno a los significados y disputas sobre la juventud y la definición de los jóvenes en la sociedad.

Tontura é muito frequente em idosos. OBJETIVOS: Avaliar a qualidade de vida (QV) em idosos vestibulopatas e relacioná-la com o gênero e a faixa etária. MATERIAL E MÉTODO: Estudo prospectivo que incluiu série de 120 casos de idosos com tontura que foram avaliados por meio das versões brasileiras dos questionários Whoqol-bref e Dizziness Handicap Inventory (DHI). Os resultados obtidos foram submetidos à Análise Fatorial (AF) e aos testes de Mann Whitney, Kruskal Wallis e correlação de Spearman. RESULTADOS: O domínio físico avaliado pelo DHI, e o físico e o ambiental do Whoqol-bref foram os mais comprometidos. A AF resultou em 3 fatores no DHI e 5 no Whoqol-bref: Houve moderada correlação (-0,596) entre o escore total de ambos os instrumentos. Os homens apresentaram melhor QV nos fatores "percepção ambiental e introspectividade" e "percepção da saúde" do Whoqol-bref. As idosas obtiveram melhor QV no fator "percepção da funcionalidade" do Whoqol-bref. Não houve diferenças significantes entre os fatores dos dois instrumentos por faixa etária. CONCLUSÕES: Idosos vestibulopatas apresentam QV prejudicada. As idosas vestibulopatas apresentam pior QV em relação aos fatores "percepção ambiental e introspectividade" e "percepção da saúde" e melhor QV em relação ao fator "percepção da funcionalidade" que os homens.Dizziness is frequent in elderly people. AIMS: To evaluate the Quality of Life (QoL) in elderly subjects with dizziness, relate it with gender and age. MATERIAL AND METHOD: A prospective study comprising 120 elderly patients with dizziness evaluated with Brazilian versions of the Whoqol-bref and the dizziness handicap inventory (DHI). The factor analysis (FA), the Mann Whitney and Kruskal Wallis tests, and the Spearman correlation were applied to study the results. RESULTS: The most compromised domains were the DHI physical domain and the Whoqol-bref physical and environment domains. FA resulted in 3 factors in the DHI and 5 factors in the Whoqol-bref. There was a moderate correlation (-0.596) in the total scores of both instruments. Males had a better QoL in the "environment perception and introspectivity" and "health perception" factors of the Whoqol-bref test. Females had a better QoL in the "functionality perception" factor of the Whoqol-bref test. There were no significant age differences. CONCLUSIONS: Elderly patients with dizziness have a worse QoL. Elderly females with dizziness have worse QoL scores in "environment perception and introspectivity" and "health perception" and better QoL in the "functionality perception" factor compared to elderly males.

Se investigó, en escolares de tercer grado de primaria de Lima Metropolitana y Piura, si existían diferencias significativas en el desarrollo de los procesos cognitivos implicados en la lectura y escritura, considerando las variables género, nivel socioeconómico, gestión de la institución educativa y localidad de residencia. Para la recolección de los datos se adaptó previamente la Batería de Evaluación Cognitiva de Lectura y Escritura, estableciéndose su confiabilidad y validez. Los resultados indicaron diferencias en los procesos cognitivos examinados en razón de las variable, nivel socioeconómico, gestión de la institución educativa y localidad de residencia.

Embora a paralisia de Bell seja o tipo mais frequente de paralisia facial periférica,sua causa ainda é objeto de inúmeros questionamentos. A reativação do vírus varicela zoster tem sido considerada uma das principais causas da paralisia de Bell, porém, os poucos trabalhos que estudam a prevalência do VVZ como agente etiológico da PB são japoneses, o que determina características geográficas e populacionais bastante díspares de nossa população. OBJETIVOS: Verificar a frequência do vírus varicela zoster em saliva de indivíduos com PB, pela técnica de PCR. MATERIAL E MÉTODO: Estudo prospectivo com 171 pacientes com PFP, sendo 120 pacientes portadores de paralisia de Bell, com até uma semana de evolução, sem uso prévio de drogas antivirais. O grupo controle foi composto de 20 adultos sadios. Nestes indivíduos foram coletadas três amostras de saliva em semanas consecutivas, para pesquisa de DNA viral pela técnica de PCR. RESULTADOS: O vírus varicela zoster foi encontrado em amostras de saliva de dois pacientes com paralisia de Bell (1,7%). Nenhum vírus foi identificado no grupo controle. CONCLUSÃO: Foi verificada frequência de 1,7% para vírus varicela zoster em amostras de saliva de pacientes com paralisia de Bell, pela técnica de PCR.Although Bell's palsy is the major cause of acute peripheral facial palsy, its pathogenesis remains unknown. Reactivation of the varicella zoster virus has been implicated as one of the main causes of Bell's palsy, however, studies which investigate the varicella zoster virus reactivation in Bell's palsy patients are mostly Japanese and, therefore, personal and geographic characteristics are quite different from our population. AIMS: To determine varicella zoster virus frequency in saliva samples from patients with Bell's palsy, using PCR. MATERIAL AND METHOD: One hundred seventy one patients with acute peripheral facial palsy were prospectively enrolled in this study. One hundred twenty were clinically diagnosed with Bell's palsy, within one week of onset of the disease and no previous anti-viral therapy. We had 20 healthy adults as controls. Three saliva samples were collected from patients and controls at initial examination and at one and two weeks later. The detection of the varicella zoster virus DNA was performed using PCR. RESULTS: Varicella zoster virus was detected in two patients (1.7%). The virus was not identified in saliva samples from the controls. CONCLUSIONS: Varicella zoster virus was detected in 1.7% of saliva samples from patients with Bell's palsy, using PCR.

ABSTRACT Facial nerve hemangioma is a rare and benign vascular tumor, and accounts for 0.7% of intratemporal tumors. We report the second case described in the literature of a facial nerve hemangioma in its tympanic segment. A 14-year-old male patient presented with a history of progressive right ear hearing loss with preserved facial mimicry. Pure tone audiometry showed a right ear moderate conductive hearing loss. Magnetic resonance imaging demonstrated an expansive lesion involving the tympanic segment of the right facial nerve, suggestive of hemangioma. Watchful waiting was chosen as management. In the first case of middle ear facial hemangioma described in the literature, facial palsy was the symptom that led the patient to seek medical care. In the present case, it can be inferred that the first symptom was conductive hearing loss ipsilateral to the lesion. Facial palsy may not be present and the clinical presentation may resemble otosclerosis, ossicular chain disruption, and third window abnormalities, among other differential diagnoses of conductive hearing loss. The second case of tympanic portion facial nerve hemangioma is reported, describing the specificity of conductive hearing loss as its only clinical manifestation.
