Abstract

Peripheral ameloblastoma (PA) is a rare benign tumor that can occur mostly in the mandibular gingiva of the premolar area, originating from the residual odontogenic epithelium. The patient is usually asymptomatic and the lesion can be an occasional finding during routine intraoral examination. Due to the lack of clinical and radiographic pathognomonic features, the diagnosis is based on histopathological analysis, associated with 3D computed tomography (CT) imaging. Here, we report the case of a middle-aged man showing an asymptomatic, sessile, normochromic papillomatous mass of the lingual alveolar mucosa, in correspondence of tooth 4.4, which was histologically diagnosed as peripheral ameloblastoma. After the complete excision of the lesion, there were no recurrence and no symptoms during the 3-year follow-up. The treatment of choice for PA is a conservative surgical excision, which usually results in a good prognosis, together with a long-term follow-up, necessary to intercept possible recurrence or, more rarely, malignant transformation. Abstract

Salivary gland pleomorphic adenoma (SGPA) is the most common type of benign epithelial tumor; it is observed more commonly in females (with a female-to-male ratio of 1.43:1), and the age at diagnosis ranges between 40 and 59 years, with only 2% of cases diagnosed before age 18. Cri du Chat (CdC) is a rare syndrome caused by deletions of various sizes in the short arm of chromosome 5. Tumors in CdC patients are extremely rare: in Danish, Spanish, Australian, and Japanese groups of cases, no tumors have been reported, while a few cases have been described among 321 CdC patients collected in Italy and Germany. These cases all involve tumors that appear at a young age. We here report the case of a parotid pleomorphic adenoma in an 8-year-old boy with CdC. Exome analysis did not identify variants certainly significant for the development of SGPA. A CGH array, analyzed both in peripheral blood and tumor samples, failed to recognize anomalies previously associated with SGPA but identified a de novo duplication in 7p15.2, which contains part of a gene, SKAP2, in which the increased copy number is associated with the development of a different type of tumor such as pancreatic duct adenocarcinoma. The assumption that the duplication in 7p15.2 is relevant for the development of SGPA in our patient implies that CGH array studies must be included early in life in routine work-ups of CdC to identify CNVs with possible pathogenic roles for tumor development. This is particularly also relevant in relation to the severely impaired possibility for patients with CdC to report discomfort or pain related to tumor development. Constitutional CNVs in addition to the deletion in 5p should also be extensively studied to verify if their presence in some patients could explain why, in these cases, tumors develop at an age younger than expected. Abstract

The article presents a case study of a 59 year old HIV (Human immunodeficiency virus) positive man who has been presented with a persistent, painless, multi-lobulated gingival mass. It is reported that despite initial diagnostic considerations for conditions such as Kaposi's sarcoma and pyogenic granuloma, histopathological and immunohistochemical analyses confirmed a diagnosis of gingival extramedullary plasmacytoma (EMP).

Abstract

Introduction: Kaposi's sarcoma (KS) is a malignant mucocutaneous neoplasm caused by human herpesvirus 8 (HHV-8). Four types of KS exist and, in each of them, the patient could develop skin and visceral lesions. Surgical excision, radiotherapy, intralesional chemotherapy and systemic chemotherapy are widely accepted as treatment options. Observation: The aim of this paper is to present diode laser as minimally invasive procedure in management of oral KS. We report here a case of multiple oral lesions of acquired immunodeficiency syndrome (AIDS)-associated KS, which has been solely treated with diode laser. Discussion: There is no bibliography on local treatment of oral KS with diode laser and this clinical case appears to be the first regarding this technique. Conclusion: This conservative approach, in association with highly active antiretroviral therapy (HAART), is safe and effective, shows fewer side effects than chemotherapy, radiotherapy and surgical excision and may be evaluated as potential treatment for oral KS.
