1. Dual anticancer activity of Aspergillus nidulans pigment and Ionizing γ-Radiation on human larynx carcinoma cell line

Abstract

Abstract Background Fungi are a readily available source of naturally generated colored compounds. These compounds might be used as radiosensitizers for treating cancer cells. Methods Aspergillus nidulans was examined for its color-producing ability in Potato dextrose agar (PDA) broth medium. The pigment was characterized by Ultraviolet (UV) spectrophotometer and Gas Chromatography Mass Spectrometry (GC/MS). Pigment extracts from A. nidulans were studied for their cytotoxic effects on the growth of human larynx carcinoma cell line (HEp-2) with or without exposure to γ-radiation at three different doses (5, 10, and 15 Gy). A. nidulans pigment cytotoxic activity was tested against normal Vero cells. Cell apoptosis was studied using flow cytometry. Gene expression of P53, Caspase 3 and Bcl-2 were quantified. Results Ultraviolet spectrum and GC/MS revealed the ability of Aspergillus nidulans to produce Rhodopin pigment. HEp-2 cells treated with A. nidulans pigment only give IC50 about 208 µg/ml. In contrast, when treated with the pigment +10 Gy γ-radiation, it give about 115 µg/ml. However, for normal cells, lower cytotoxic activity was detected. Treatment with pigment (208 g/mL) caused about 50% ± 1.0 total apoptosis level and gene expression of P53: 2.3 fold and Caspase 3: 1.84 fold in respect to untreated HEp-2), while Bcl-2 was decreased (Bcl-2: 0.63 fold in respect to untreated HEp-2). Furthermore, treated with pigment (115 µg/mL) + 10Gy caused about 47.41% ± 1.7 total apoptosis level and P53: 2.53 fold and Caspase 3: 2.0 fold in respect to untreated HEp-2, while Bcl-2 was downregulated (Bcl-2: 0.61 fold in respect to untreated HEp-2). Conclusion This study concluded that the anti-cancer activity of Aspergillus nidulans pigment was enhanced by ionizing radiation at 10 Gy, as well as its low cytotoxic activity against normal Vero cells.

Abstract

Selenium is a fundamental trace element of the living system. Microorganisms play a crucial role in the selenium cycle, both in the environment and in life. Biogenic selenium nanoparticles have shown promising prospects for use in medicine as an antioxidant and anticancer agent. In this study, SeNPs were biosynthesized by Penicillium citrinum . The spore suspension which was previously prepared was exposed to different doses of gamma radiation (10, 20, 30, 50, and 60 Gy). SeNPs were then produced by an irradiated P citrinum . UV spectroscopy, transmission electron microscopy, X-ray diffraction, and GSH content were assayed to evaluate the probability of P citrinum synthesizing SeNPs. In conclusion, irradiation of P citrinum by gamma ray enhances the mycosynthesis of SeNPs.

Abstract

– Liver cancer is the sixth common cancer and the second leading cause of cancer death worldwide. Hepatocellular carcinoma (HCC) accounts for ~90% of cases of liver cancer and is the fourth most common cause of cancer-related death in the world. Up to now, 4 oral multityrosine kinase inhibitors (sorafenib, lenvatinib, regorafenib and cabozantinib), 1 anti-angiogenic antibody (ramucirumab) and 5 immune checkpoint inhibitors, alone or in combination (atezolizumab in combination with bevacizumab, ipilimumab in combination with nivolumab, tremelimumab in combination with durvalumab, nivolumab and pembrolizumab in monotherapy) have been commercialized for advanced HCC patients’ treatment. The aim of this editorial is to provide an insight in current hepatocellular carcinoma systemic pharmacological treatment options.

Abstract

Hepatocellular carcinoma (HCC) is among the most prevalent and lethal cancers worldwide. Chitosan-coated iron oxide nanocomposite (Fe3O4/Cs) is a promising bio-nanomaterial for many biological applications. The objective of this research was to evaluate the anticancer efficacy of Fe3O4/Cs against HCC in animal models. Fe3O4 nanoparticles were prepared and added to chitosan solution; then, the mixture was exposed to gamma radiation at a dose of 20 kGy. Rats have received diethylnitrosamine (DEN) orally at a dose of 20 mg/kg body weight 5 times per week during a period of 10 weeks to induce HCC and then have received Fe3O4/Cs intraperitoneal injection at a dose of 50 mg/kg body weight 3 times per week during a period of 4 weeks. After the last dose of Fe3O4/Cs administration, animals were sacrificed. DEN induced upregulation of PI3K/Akt/mTOR and MAPK (ERK, JNK, P38) signaling pathways and inflammatory markers (TLR4, iNOS, and TNF-α). DEN also decreases cleaved caspase-3 and increases liver enzymes (ALT, AST, and GGT) activities. Administration of Fe3O4/Cs significantly ameliorated the above-mentioned parameters.

Abstract

Introduction: Neuropeptides are frequently identified as potential drugs for the development of new therapeutics or for the study of a variety of human cerebral diseases. Due to their instability when injected proteins and peptides need specific delivery systems, to stretch the biodisponibility. Nanocarriers can be a solution to solve the problem of transport to the central nervous system. Material and method: We performed a data literature analysis from PubMed. We analyze data from multiple studies, reporting that nanovectors are a possible drug delivery system (DDS) in the central nervous system (CNS). Results: We found that several studies are developing nanocarriers for neuropeptides delivery through intranasal route or injected. With this strategy it is possible to deliver efficiently several drugs with different natures such as small polar molecules, peptides, and proteins, and also the large proteins, polysaccharides, and even neuropeptides entering the CNS. Conclusions: We can conclude that the developments of the nanocarriers as DDS for neuropeptides and the transport through the nasal passages are a promising strategy for the delivery of neuropeptides in CNS.

Abstract

Background: Plant-based diets have spread widely in the general population and among athletes too, both for well-documented health benefits and for eco-sustainability and ethical concerns. Despite appropriate nutrition is essential for satisfying training, performance, and recovery needs in athletes, little is known about the effects of plant-based diets on body composition in athletes. Appropriate training and nourishment strategies are a daily priority to arrive at victory and achieving an optimal body composition helps achieve success. Our study aimed to shed light on the adequacy of a vegetarian or vegan diet on the body composition of the athlete, in comparison with an omnivorous diet. In particular, we had tried to clarify the effects of well-structured plant-based nutrition in competitive athletes, who, previously, had never followed a diet under specialist supervision. Methods: The groups were followed for 8 months and subjected to a well-planned diet under medical supervision. Anthropometric parameters and body composition evaluation through Bioelectrical Impedance Analysis (BIA) were performed in each participant. Results: All subjects, especially vegans, showed an improvement in cellular efficiency, muscle mass, and a re-distribution of body water thanks to a better distribution of macronutrients with dietary plans. These changes demonstrate improvement in the athlete's body composition thanks to well-structured plant-based diets that meet the nutrient requirements, without a change in physical training, especially in vegan athletes. Conclusions: The administration of well-structured dietetic schemes for each group has reduced the differences between omnivores, vegetarians, and vegans, underling their more difficult autonomous management and the important role of the nutrition specialist. Given the evidence collected, we demonstrate that plant-based diets are sustainable for athletes and that the role of nutritionist is central to sports and food choices to achieve an optimal body composition.

Abstract

Introduction: Metabolic syndrome (MS) includes a various factors such as hyperglycemia, high blood pressure and hypercholesterolemia that predispose individuals to the development of cardiovascular disease and diabetes. Furthermore, MS is strongly associated with obesity, indeed, it is well know that abdominal and/or visceral adipose tissue accumulation is a contributory cause of MS development. These diseases are associated with multiple metabolic toxicities resulting from the oxidative stress, which are the product of reactive oxygen species. In this scenario, a corrected nutrition and adequate physical activity are important measures against MS, obesity, aging and prevention of cardiovascular disease and cancer. In this regards, the healthy lifestyle, and in particular correct physical activity, represents one the epigenetic strategies against MS, having beneficial and anti-inflammatory effects, reducing the production of free radicals and the risk to development cardiovascular diseases and diabetes. The aim of this review is to discuss on physical activity and its role in modification of lyfestile inducing beneficial effect on health status. Material and method: We performed a data literature analysis from PubMed. We analyze data from multiple studies reporting that physical activity decreases body adiposity and the physical exercise reduces the fat mass in a dose dependent. Physical activity decreases abdominal visceral and subcutaneous fat indipendently of changes in dietary energy intake in healthy, overweight and obese man and women, suggesting that training is critical in the treatment, or prevention, of MS. Also, it induces the expression of a many genes involved in the repair of cellular damage and reduces the expression of genes involved in the mechanisms of oxidative stress and inflammation, activating numerous metabolic pathways and down-regulating the production of mediators of inflammation and ROS production. Conclusion: the physical activity can be an efficient therapeutic approach in MS, promoting weight loss in obesity and reducing cardiovascular disease and diabetes risks.

Abstract

Objective To examine the potential therapeutic effect of mesenchymal stem cells (MSCs) for experimental scleroderma. Materials and methods Fifty-four mice six-week-old (30−35 g) were studied. Hypochlorous acid (HOCl) induced scleroderma was considered. Mice were divided into 3 groups: (I) Control: Six mice did not receive any treatment and were sacrificed at the end of the experiment; (II) HOCl mice (induced scleroderma as a positive control): (III) MSCs-treated HOCl mice: Thirty six HOCl-induced mice were injected with MSCs (7.5 × 105) intravenous every week for 3 weeks. Skin pieces were taken from the backs of mice and lung tissue pieces. a smooth muscle actin (α-SMA) and transforming growth factor-β (TGF-β1) were analysed or fixed in 10 % formalin for skin and lung tissue histopathological analysis. Plasma nitric oxide (NO) was also assayed. Results There was a significant rise in the NO level and of the cutaneous and lung tissue α-SMA and TGF-β1 in untreated scleroderma-induced mice. The values significantly normalized after MSC therapy over the 7 weeks duration of the study. The altered histopathology of the skin and lung tissues in the scleroderma-induced mice showed a remarkable tendency to normalization of the skin and lung parenchyma and vasculature. Conclusion There was a significant rise in the level of NO and skin and lung tissue α-SMA and TGF-β1 in untreated scleroderma-induced mice and values were significantly normalized after MSC therapy over the 7 weeks duration of the study. Altered histopathology of the skin and lung appeared nearly normal after MSC therapy.

Abstract

Eating disorders (ED) are a group of psychopathological disorders that affect the patient's relationship with food and his own body and that are manifested mainly in adolescence and in young-adult age. ED include anorexia nervosa (AN), bulimia nervosa (BN) and other eating disorders as classified in the DMS-V. ED can result in several oral and dental manifestations that often occur in the early stages of ED and may allow early detection. The aim of the study is to describe the different oral and dental manifestations in patients with ED in order to offer a classification for their identification during an extra/intra-oral examination.A search on PubMed, Medline and Cochrane Library data bases has been performed.Oral manifestations in ED patients include a variety of signs and symptoms, which involve the oral mucosa and perioral tissues (exfoliative cheilitis, labial erythema, atrophic glossitis, glossodynia, yellow-orange colouration of the soft palate, cheek/lip biting, candidiasis), the teeth (dental erosion, tooth hypersensitivity, dental caries), periodontal diseases, and salivary manifestations (sialoadenosis, alterations in salivary flow). The oral signs are caused by a number of factors, including nutritional deficiencies and consequent metabolic changes, poor personal hygiene, altered eating habits and pharmacological therapies. There is a very specific link between oral manifestations and ED in the presence of self-induced vomiting.The paediatric dentist may be the first professional to detect the clinical signs thus improving the interception, early diagnosis, characterisation and prognosis of ED. In addition, the oral manifestations of ED can cause alterations of the oral function, discomfort, oral pain, and worsen aesthetics of the face and the quality of life.

Abstract

Rheumatoid arthritis (RA) is one of the major causes of disability that aggrieve a financial burden for the healthcare system as well as for patients. Both stem cell therapy and low-dose radiotherapy (LD-RT) have been recognized as alternative approaches to alleviate pain, inflammation, and bone in the inflamed joints. Accordingly, the present study was designed to compare the efficacy of both modalities in the treatment of RA in experimental rats. Rheumatoid arthritis was induced in forty female rats using Complete Freund’s adjuvant (CFA), divided into four groups (arthritic induction group and three treatment groups) compared to controls: one of the treatment groups received Human’s Wharton Jelly Mesenchymal Stem Cells (WJMSC), and the other two groups received individual doses of 0.5 and 1 Gy of Gamma irradiation once a week for six weeks. After six weeks animals were sacrificed and examined for joint histology, paw swelling, mRNA gene expression for TNF-α, IL-1β and IL-6 together with serum anticyclic citrullinated peptide (anti-CCP) and nitric oxide (NO). Results showed that both doses of (0.5 Gy and 1 Gy) and WJMSC individually modulated RA effects by significantly decreasing the measured biochemical and molecular parameters. However, at the histology level it was evident that irradiation has a better pronounced effect compared to WJMSC, which showed aggravated effects on joint erosion. The present study reports that both IR and WJMSC significantly decreased Anti-CCP, which is a specific marker for RA, however, WJMSC might have deliberate effects on the synovitis and arthritis.

Abstract

Introduction: Microparticles (MPs) are a heterogeneous population of small cell-derived vesicles. The composition is influenced by the cellular origin and the cellular processes starting their formation. The MPs have various biochemical composition, depending on the site in which they originate: from platelets or endothelium, MPs are involved in many disease, in particular in cardiovascular disease but also in cancer and in central nervous system disease. They are involved in many cellular processes and in particular in apoptosis and in cellular rearrangement. MPs could promote the recruitment of more immune cells in vascular inflammatory areas. Material and method: We performed a data literature analysis from PubMed. We analyze data from multiple studies, reporting that in central nervous system (CNS), the MPs have an important biological role, they can be derived from platelets, endothelial cells, neurons or glial cells. RESULT: We found that the CNS cells in the neurovascular unit, including the endothelial lining and neural cells are also subject to stress by a variety of stimuli such as oxygen radicals and inflammation. Indeed, it is induced membrane shedding in vascular cells. For these reasons, it is possible that the shedding in the neurovascular network may be linked not only to the development and progression of various CNS diseases, but also to the physiological development of the nervous system. In fact, the MPs attend, also, to a regeneration and repair of nerve injury. The MPs are involved in neuroimmune response, indeed, during the various negative stimuli, the microglial release a MPs containing pro-inflammatory cytokines. The MPs can be considered as a biomarkers from a specific tissue undergoing activation or damage. The plasma membrane is considerate as a sensor of cell interactions with the microenvironment. Conclusion: Given these evidences, we can conclude that the releasing of MPs is an indicator of a physiological and/ or pathophysiological activation in CNS, and then these vesicles are strongly involved in various CNS disease such as stoke, multiple sclerosis and Alzheimer’s disease.

Abstract

Background: Albuminuria has been classically considered a marker of kidney damage progression in diabetic patients and it is routinely assessed to monitor kidney function. However, the role of a mild GFR reduction on the development of stage ≥3 CKD has been less explored in type 1 diabetes mellitus (T1DM) patients. Aim of the present study was to evaluate the prognostic role of kidney disease measures, namely albuminuria and reduced GFR, on the development of stage ≥3 CKD in a large cohort of patients affected by T1DM. Methods: A total of 4284 patients affected by T1DM followed-up at 76 diabetes centers participating to the Italian Association of Clinical Diabetologists (Associazione Medici Diabetologi, AMD) initiative constitutes the study population. Urinary albumin excretion (ACR) and estimated GFR (eGFR) were retrieved and analyzed. The incidence of stage ≥3 CKD (eGFR < 60 mL/min/1.73 m2) or eGFR reduction > 30% from baseline was evaluated. Results: The mean estimated GFR was 98 ± 17 mL/min/1.73m2 and the proportion of patients with albuminuria was 15.3% (n = 654) at baseline. About 8% (n = 337) of patients developed one of the two renal endpoints during the 4-year follow-up period. Age, albuminuria (micro or macro) and baseline eGFR < 90 ml/min/m2 were independent risk factors for stage ≥3 CKD and renal function worsening. When compared to patients with eGFR > 90 ml/min/1.73m2 and normoalbuminuria, those with albuminuria at baseline had a 1.69 greater risk of reaching stage 3 CKD, while patients with mild eGFR reduction (i.e. eGFR between 90 and 60 mL/min/1.73 m2) show a 3.81 greater risk that rose to 8.24 for those patients with albuminuria and mild eGFR reduction at baseline. Conclusions: Albuminuria and eGFR reduction represent independent risk factors for incident stage ≥3 CKD in T1DM patients. The simultaneous occurrence of reduced eGFR and albuminuria have a synergistic effect on renal function worsening.

Abstract

Pregnancy is a physiological state during which the woman's body undergoes profound modifications. During this time, every woman must be nourished in such a way as to maintain herself in good health and at the same time allow the fetal tissues to form and develop in the best way. Proper nutrition is one of the fundamental prerequisites for the normal evolution of both pregnancy and fetal growth: the quantity and quality of food and drinks must be scrupulously controlled. Contrariwise, it is important to avoid the excess calories and macronutrients. In fact, during pregnancy, it may be just as harmful as their shortage, especially in overweight and obese women, for which the risk of miscarriage, gestational diabetes, fetal growth disorders and preeclampsia increases. The pasta constitutes the foundation of the Mediterranean diet food. The dough can be seen, therefore, so it represents an excellent ally of the diet from a nutritional point of view. The nutritional values of the pasta change depending on the type, based on the fact that the dough is traditional or integral semolina. It also contains Vitamin B and has an abundance of minerals, among which potassium. The pasta is an easily digestible food, ensuring energy ready to use (such as all carbohydrates); moreover, it possesses a great satiating power. In case of overweight, however, it is possible to reduce the number of carbohydrates in the diet, taking care not to make them ever miss. In conclusion, the consumption of pasta is very important during pregnancy: a diet without carbohydrates can be dangerous for all physiological pathways.

Abstract

Background: Albuminuria has been classically considered a marker of kidney damage progression in diabetic patients and it is routinely assessed to monitor kidney function. However, the role of a mild GFR reduction on the development of stage ≥3 CKD has been less explored in type 1 diabetes mellitus (T1DM) patients. Aim of the present study was to evaluate the prognostic role of kidney disease measures, namely albuminuria and reduced GFR, on the development of stage ≥3 CKD in a large cohort of patients affected by T1DM. Methods: A total of 4284 patients affected by T1DM followed-up at 76 diabetes centers participating to the Italian Association of Clinical Diabetologists (Associazione Medici Diabetologi, AMD) initiative constitutes the study population. Urinary albumin excretion (ACR) and estimated GFR (eGFR) were retrieved and analyzed. The incidence of stage ≥3 CKD (eGFR < 60 mL/min/1.73 m2) or eGFR reduction > 30% from baseline was evaluated. Results: The mean estimated GFR was 98 ± 17 mL/min/1.73m2 and the proportion of patients with albuminuria was 15.3% (n = 654) at baseline. About 8% (n = 337) of patients developed one of the two renal endpoints during the 4-year follow-up period. Age, albuminuria (micro or macro) and baseline eGFR < 90 ml/min/m2 were independent risk factors for stage ≥3 CKD and renal function worsening. When compared to patients with eGFR > 90 ml/min/1.73m2 and normoalbuminuria, those with albuminuria at baseline had a 1.69 greater risk of reaching stage 3 CKD, while patients with mild eGFR reduction (i.e. eGFR between 90 and 60 mL/min/1.73 m2) show a 3.81 greater risk that rose to 8.24 for those patients with albuminuria and mild eGFR reduction at baseline. Conclusions: Albuminuria and eGFR reduction represent independent risk factors for incident stage ≥3 CKD in T1DM patients. The simultaneous occurrence of reduced eGFR and albuminuria have a synergistic effect on renal function worsening.

Male Sprague-Dawley rats with lateral hypothalamic (LH) lesions were analyzed to determine the effects of intraperitoneal injections of lysine acetylsalicylate (L-ASA) on food intake. Inhibition of prostaglandin synthesis by L-ASA in rats altered the sympathetic and thermogenic changes caused by lesions in the LH. Furthermore, rats with LH lesions exhibited altered feeding behavior and high body temperatures due to sympathetic discharge.

Intracerebroventricular injection of the nitric oxide synthesis inhibitor L-NAME decreases food intake and in enhances the oxygen consumption, the firing rate of sympathetic nerves innervating interscapular brown adipose tissue and temperature. However, intraperitoneal injection decreased all the factors. Different effects of the molecule on sympathetic output are most likely responsible for the oppositional effects when the different injection routes were used.

The involvement of the cortex in fever generationwas investigated in rats intracerebroventricularly injected with either prostaglandin E1 (PGE1) or corticotropin-releasing hormone (CRH) or intravenously injected with Escherichia coli toxin. Frontal cortical depressionthrough potassium chloride application significantly decreased hyperthermia in the PGE1- and endotoxin-treated rats. No effect was observed with CRH-treated subjects. These results confirm cortical involvement in hyperthermia development.

Abstract

Diabetes mellitus type 1 is an autoimmune disorder that leads to β cell destruction and lowered insulin production. The approach of using stem cells in the clinical field has refocused the objectives of islet transplantation. Accordingly, in the present study experimental rats pancreatic β cells damage was induced using alloxan. Mesenchymal stem cells (MSCs) were separated from bone marrow and differentiated into insulin-producing pancreatic cells. Both undifferentiated mesenchymal stem cells or early differentiated insulin-producing pancreatic cells were injected into the alloxan-treated female albino rats. In addition, alloxan-treated rats were then exposed to 6 Gy γ-radiation prior to cells injection by 24 hours for immunosupression. The rats were monitored for blood glucose levels along the period of the experiment. The results showed that the mean of postprandial blood glucose levels of the diabetic rats with undifferentiated mesenchymal stem cell transplantation or early differentiated insulin-producing pancreatic cells, either exposed to ionizing radiation before cell injection or not, were significantly lower than those of the diabetic rats without cell transplantation. In addition the histological observations showed an amelioration after cell therapy either with or without radiation, as the necrotic cells were just few after 15 days and were absent after 30 days in case of treatment with MSCs. Treatment with early differentiated insulin-producing pancreatic cells after 30 days revealed blood capillary enclosing erythocytes in association with presumably early differentiated pancreatic cells and the migration of these cells into the neighbouring islet tissue. It is concluded that undifferentiated mesenchymal stem cells or early differentiated insulin-producing pancreatic cell therapy have the potential to successfully repair the damage induced by alloxan in rats pancreatic β-cells.

Abstract

To study mobility in older populations it can be advantageous to use portable gait analysis systems, such as inertial measurement units (IMUs), which can be used in the community. To define a normal range, 136 active subjects were recruited with an age range of 18 to 97. Four IMUs were attached to the subjects, one on each thigh and shank. Subjects were asked to walk 10 m at their own self-selected speed. The ranges of motion of thigh, shank, and knee in both swing and stance phase were calculated, in addition to stride duration. Thigh, shank, and knee range of movement in swing and stance were significantly different only in the > 80 age group. Regressions of angle against age showed a cubic relationship. Abstract

Zingerone (ZO) is an ingredient of ginger (Zingiber officinale) which has different pharmacological properties. The objective of this research was to evaluate the protective effect of ZO against Cisplatin (Cis) or γ-Irradiation (IR)-induced hepatotoxicity in rats. ZO was given orally for consecutive 14 days prior to the treatment with Cis or exposure to IR at 15th day. Animals were sacrificed at the 23rd day. Cis or IR induced a marked increase in MAPK signal transduction as evidenced by increased p38 MAPK, JNK and ErK1/2. CYP2E1 and NADPH oxidase were significantly up-regulated. Inflammatory markers (TLR4, iNOS, COX-2 and MPO) and liver enzymes (AST, ALT and ALP) activities were also increased. Administration of ZO significantly ameliorated the above mentioned parameters.

Abstract

Orexin system regulates sleep/wake states and its deficiency result in narcolepsy thus indicating the crucial role of orexins in maintaining wakefulness. There are two types of orexin peptides: the orexin-A (OXA or hypocretin 1) and orexin-B (OXB or hypocre- tin 2). The Majority of the central nervous system orexin peptides are synthesized in neurons located in the lateral and back hypotha- lamus and send projections throughout the brain regions Orexin neurons are “multi-tasking” hence regulating also energy homeosta- sis, reward systems and feeding behaviour through connection with hypothalamic nuclei and through responsiveness to leptine and glucose. It has recently been found a connection with lymbic system suggesting a further possible role of orexins in regulating emo- tions. All the studies conducted confirm that orexin system regulates vigilance states, energy homeostasis, reward system, and emo- tions. These crucial role might be the target to develope treatments of narcolepsy, obesity, emotional stress, and drug addiction.

Abstract

Background Albuminuria has been classically considered a marker of kidney damage progression in diabetic patients and it is routinely assessed to monitor kidney function. However, the role of a mild GFR reduction on the development of stage ≥3 CKD has been less explored in type 1 diabetes mellitus (T1DM) patients. Aim of the present study was to evaluate the prognostic role of kidney disease measures, namely albuminuria and reduced GFR, on the development of stage ≥3 CKD in a large cohort of patients affected by T1DM. Methods A total of 4284 patients affected by T1DM followed-up at 76 diabetes centers participating to the Italian Association of Clinical Diabetologists (Associazione Medici Diabetologi, AMD) initiative constitutes the study population. Urinary albumin excretion (ACR) and estimated GFR (eGFR) were retrieved and analyzed. The incidence of stage ≥3 CKD (eGFR 30% from baseline was evaluated. Results The mean estimated GFR was 98 ± 17 mL/min/1.73m2 and the proportion of patients with albuminuria was 15.3% (n = 654) at baseline. About 8% (n = 337) of patients developed one of the two renal endpoints during the 4-year follow-up period. Age, albuminuria (micro or macro) and baseline eGFR 90 ml/min/1.73m2 and normoalbuminuria, those with albuminuria at baseline had a 1.69 greater risk of reaching stage 3 CKD, while patients with mild eGFR reduction (i.e. eGFR between 90 and 60 mL/min/1.73 m2) show a 3.81 greater risk that rose to 8.24 for those patients with albuminuria and mild eGFR reduction at baseline. Conclusions Albuminuria and eGFR reduction represent independent risk factors for incident stage ≥3 CKD in T1DM patients. The simultaneous occurrence of reduced eGFR and albuminuria have a synergistic effect on renal function worsening.
