Abstract

New evidence from 2023 has slightly shifted some perspectives on rheumatoid arthritis (RA) management. Glucocorticoids have reaffirmed their role as bridging therapy, while novel studies on JAK inhibitors have examined efficacy, mechanism of action, and their potential in high-risk populations, bolstering our understanding with real-world data. Additionally, among treatment strategies, achieving low disease activity has emerged as comparable to achieving remission in the long term, and new insights have been gained regarding tapering both biological and conventional synthetic DMARDs. Furthermore, novel approaches have been proposed for managing difficult-to-treat RA and pre-RA. In this paper, the reviewers aim to present the most relevant studies published during the last year in the field of RA management.

Abstract

Disorders of consciousness are neurological conditions characterized by impaired arousal and awareness of self and environment. Behavioural responses are absent or are present but fluctuate. Disorders of consciousness are commonly encountered as a consequence of both acute and chronic brain injuries, yet reliable epidemiological estimates would require inclusive, operational definitions of the concept, as well as wider knowledge dissemination among involved professionals. Whereas several manifestations have been described, including coma, vegetative state/unresponsive wakefulness syndrome and minimally conscious state, a comprehensive neurobiological definition for disorders of consciousness is still lacking. The scientific literature is primarily observational, and studies-specific aetiologies lead to disorders of consciousness. Despite advances in these disease-related forms, there remains uncertainty about whether disorders of consciousness are a disease-agnostic unitary entity with a common mechanism, prognosis or treatment response paradigm. Our knowledge of disorders of consciousness has also been hampered by heterogeneity of study designs, variables, and outcomes, leading to results that are not comparable for evidence synthesis. Abstract

The convergence of an interdisciplinary team of neurocritical care specialists to organize the Curing Coma Campaign is the first effort of its kind to coordinate national and international research efforts aimed at a deeper understanding of disorders of consciousness (DoC). This process of understanding includes translational research from bench to bedside, descriptions of systems of care delivery, diagnosis, treatment, rehabilitation, and ethical frameworks. The description and measurement of varying confounding factors related to hospital care was thought to be critical in furthering meaningful research in patients with DoC. Interdisciplinary hospital care is inherently varied across geographical areas as well as community and academic medical centers. Access to monitoring technologies, specialist consultation (medical, nursing, pharmacy, respiratory, and rehabilitation), staffing resources, specialty intensive and acute care units, specialty medications and specific surgical, diagnostic and interventional procedures, and imaging is variable, and the impact on patient outcome in terms of DoC is largely unknown. The heterogeneity of causes in DoC is the source of some expected variability in care and treatment of patients, which necessitated the development of a common nomenclature and set of data elements for meaningful measurement across studies. Guideline adherence in hemorrhagic stroke and severe traumatic brain injury may also be variable due to moderate or low levels of evidence for many recommendations. This article outlines the process of the development of common data elements for hospital course, confounders, and medications to streamline definitions and variables to collect for clinical studies of DoC.

Abstract

The electronic energy levels of cyclo(glycine-phenylalanine), cyclo(tryptophan-tyrosine) and cyclo(tryptophan-tryptophan) dipeptides are investigated with a joint experimental and theoretical approach. Experimentally, valence photoelectron spectra in the gas phase are measured using VUV radiation. Theoretically, we first obtain low-energy conformers through an automated conformer-rotamer ensemble sampling scheme based on tight-binding simulations. Then, different first principles computational schemes are considered to simulate the spectra: Hartree-Fock (HF), density functional theory (DFT) within the B3LYP approximation, the quasi-particle GW correction, and the quantum-chemistry CCSD method. Theory allows assignment of the main features of the spectra. A discussion on the role of electronic correlation is provided, by comparing computationally cheaper DFT scheme (and GW) results with the accurate CCSD method.

Abstract

Antimalarials have been associated with QT prolongation in COVID-19 patients but are generally safe in systemic lupus erythematosus (SLE).We compared the prevalence of QTc prolongation between COVID-19 and SLE patients treated with hydroxychloroquine (HCQ).We included patients with SARS-CoV-2 infection confirmed by nasopharyngeal swab and patients taking HCQ for SLE. A prolonged QTc was defined as an increase in QTc intervals60 ms (compared with baseline) or as a QTc of ≥500 ms. We performed the univariate and multivariate logistic regression to investigate the risk factors for QTc prolongation in COVID-19 patients.We enrolled 58 COVID-19 patients (median age 70.5 years, IQR 25), grouped into group A (patients with HCQ) group B (patients with HCQ + azithromycin) and group C (not received either drug). Fifty (26%) COVID-19 patients presented a QTc prolongation (12 QTc≥500 ms, 3 patients ΔQTc60 ms). We did not find any differences in QTc prolongation among the three treatment groups. Baseline QTc (OR 111.5) and D-dimer (OR 78.3) were independently associated to QTc prolongation. Compared to the 50 SLE patients (median age 38.5 years, IQR 22), chronically treated with HCQ, COVID-19 patients showed significantly longer QTc (p0.001).This is the first study demonstrating that, unlike COVID-19 patients, patients with SLE are not susceptible to HCQ-induced long QT syndrome and arrhythmia. The combined arrhythmogenic effect of SARS-CoV-2 infection and HCQ could account for the excess of QTc prolongation and fatal arrhythmias described in patients with COVID-19.

IMPORTANCE: Overall, immunotherapy has been shown to improve outcomes and reduce relapses in individuals with N-methyl-d-aspartate receptor (NMDAR) antibody encephalitis (NMDARE); however, the superiority of specific treatments and combinations remains unclear. OBJECTIVE: To map the use and safety of immunotherapies in individuals with NMDARE, identify early predictors of poor functional outcome and relapse, evaluate changes in immunotherapy use and disease outcome over the 14 years since first reports of NMDARE, and assess the Anti-NMDAR Encephalitis One-Year Functional Status (NEOS) score. DATA SOURCES: Systematic search in PubMed from inception to January 1, 2019. STUDY SELECTION: Published articles including patients with NMDARE with positive NMDAR antibodies and available individual immunotherapy data. DATA EXTRACTION AND SYNTHESIS: Individual patient data on immunotherapies, clinical characteristics at presentation, disease course, and final functional outcome (modified Rankin Scale [mRS] score) were entered into multivariable logistic regression models. MAIN OUTCOMES AND MEASURES: The planned study outcomes were functional outcome at 12 months from disease onset (good, mRS score of 0 to 2; poor, mRS score greater than 2) and monophasic course (absence of relapse at 24 months or later from onset). RESULTS: Data from 1550 patients from 652 articles were evaluated. Of these, 1105 of 1508 (73.3%) were female and 707 of 1526 (46.3%) were 18 years or younger at disease onset. Factors at first event that were significantly associated with good functional outcome included adolescent age and first-line treatment with therapeutic apheresis, corticosteroids plus intravenous immunoglobulin (IVIG), or corticosteroids plus IVIG plus therapeutic apheresis. Factors significantly associated with poor functional outcome were age younger than 2 years or age of 65 years or older at onset, intensive care unit admission, extreme delta brush pattern on electroencephalography, lack of immunotherapy within the first 30 days of onset, and maintenance IVIG use for 6 months or more. Factors significantly associated with nonrelapsing disease were rituximab use or maintenance IVIG use for 6 months or more. Adolescent age at onset was significantly associated with relapsing disease. Rituximab use increased from 13.5% (52 of 384; 2007 to 2013) to 28.3% (311 of 1100; 2013 to 2019) (P < .001), concurrent with a falling relapse rate over the same period (22% [12 of 55] in 2008 and earlier; 10.9% [35 of 322] in 2017 and later; P = .006). Modified NEOS score (including 4 of 5 original NEOS items) was associated with probability of poor functional status at 1 year (20.1% [40 of 199] for a score of 0 to 1 points; 43.8% [77 of 176] for a score of 3 to 4 points; P = .05). CONCLUSIONS AND RELEVANCE: Factors influencing functional outcomes and relapse are different and need to be considered independently in development of evidence-based optimal management guidelines of patients with NMDARE.

Abstract

The current gold standard for diagnosis of attention deficit/hyperactivity disorder (ADHD) includes subjective measures, such as clinical interview, observation, and rating scales. The significant heterogeneity of ADHD symptoms represents a challenge for this assessment and could prevent an accurate diagnosis. The aim of this work was to investigate the ability of a multi-domain profile of measures, including blood fatty acid (FA) profiles, neuropsychological measures, and functional measures from near-infrared spectroscopy (fNIRS), to correctly recognize school-aged children with ADHD. To answer this question, we elaborated a supervised machine-learning method to accurately discriminate 22 children with ADHD from 22 children with typical development by means of the proposed profile of measures. To assess the performance of our classifier, we adopted a nested 10-fold cross validation, where the original dataset was split into 10 subsets of equal size, which were used repeatedly for training and testing. Each subset was used once for performance validation. Our method reached a maximum diagnostic accuracy of 81% through the combining of the predictive models trained on neuropsychological, FA profiles, and deoxygenated-hemoglobin features. With respect to the analysis of a single-domain dataset per time, the most discriminant neuropsychological features were measures of vigilance, focused and sustained attention, and cognitive flexibility; the most discriminating blood FAs were linoleic acid and the total amount of polyunsaturated fatty acids. Finally, with respect to the fNIRS data, we found a significant advantage of the deoxygenated-hemoglobin over the oxygenated-hemoglobin data in terms of predictive accuracy. These preliminary findings show the feasibility and applicability of our machine-learning method in correctly identifying children with ADHD based on multi-domain data. The present machine-learning classification approach might be helpful for supporting the clinical practice of diagnosing ADHD, even fostering a computer-aided diagnosis perspective.

Abstract

The investigation of neural networks underlying emotion perception and recognition has received considerable attention during the past few years within the affective neuroscience domain. Neuroimaging studies revealed that the prefrontal cortex (PFC) is involved during emotional stimuli comprehension and evaluation, based on stimulus valence and arousal. Nevertheless, the way in which these emotional parameters affect hemodynamic variations has to be clarified. The present study investigated the changes in prefrontal hemodynamic activation by means of an event-related near-infrared spectroscopy (fNIRS) paradigm during the presentation of emotional patterns taken from the International Affective Picture System (IAPS). During the task, 20 subjects (12 females and 8 males) passively viewed neutral, positive and negative emotional images, which could also be low or high arousing (20 stimuli for each category). Stimuli were presented for 6 s in a randomized order, with 12s inter- stimulus interval. Hemodynamic and electrocortical (EEG) activities were simultaneously recorded from 6 NIRS and 16 EEG channels, in the same session. Changes in oxygenated (oxyHb) and deoxygenated (deoxyHb) hemoglobin were measured with respect to the different experimental conditions. Results showed that the emotional valence of the picture modulates the activation of the PFC, with increased activity for negative patterns and decreased activity for positive ones, if compared to neutral condition. Moreover, preliminary analyses revealed a consonance between EEG (frequency band oscillations) and hemodynamic responses. In conclusion, the present study revealed that prefrontal blood oxygenation and brain oscillations are differently modulated by the emotional content mainly related to the stimulus valence, and demonstrated that the PFC is involved in emotional processing.

Abstract

Il recente interesse per le neuroscienze affettive ha permesso di individuare, attraverso studi di neuroimmagine funzionale, il ruolo della corteccia prefrontale (PFC) nei processi di elaborazione e regolazione emotiva. Nonostante le numerose evidenze a supporto dei dati di imaging, rimane ancora da chiarire il rapporto che sussiste tra manifestazioni emodinamiche, elettrofisiologiche ed autonomiche periferiche legate al processamento emotivo, secondo un’ottica integrata. In questo contesto è stato sviluppato un paradigma fNIRS (functional Near-Infrared Spectroscopy) evento-relato, in coregistrazione EEG e rilevazione autonomica (arousal-correlati), che prevedeva la presentazione di immagini emotive (IAPS) selezionate in base ai punteggi di valenza e arousal. A conclusione del compito era richiesta la valutazione degli stimoli e la compilazione della scala BIS/BAS (indici psicometrici) per la misura di componenti personologiche. I dati emodinamici sono stati acquisiti mediante un sistema di rilevazione “in continua” (CW: continuous-wave), con un montaggio prefrontale a 6 canali. Dai risultati è emerso che la presentazione d’immagini emotive si associa a una maggiore attivazione della PFC rispetto alle immagini neutre, e che il profilo di risposta emodinamica varia in base alla valenza dello stimolo. In particolare, durante la presentazione di immagini negative, si osserva una maggiore attivazione della PFC, con un aumento nel livello di O2Hb e un decremento nel livello di HHb. Un pattern opposto emerge, invece, per le immagini a valenza positiva. Queste evidenze sono state interpretate in relazione ai dati neurofisiologici, autonomici e psicometrici. I risultati hanno confermato la presenza di un coinvolgimento prefrontale durante la presentazione di immagini emotive e suggeriscono il vantaggio di un approccio metodologico integrato per lo studio di fenomeni complessi come quelli legati al processamento emotivo.

Abstract

Background. Upper motoreuron lesions (UML) affect people of all ages and are a major cause of disability in the young. Lower limb rehabilitation is a fundamental part of post-acute care in neurological disease. In this contest passive mobilisation could be of help for acute patients in a very early stage of their recovery to safeguard tissue properties and prevent the worsening of neural damages that cause cognitive and motor impairment, when paresis prevent the beginning of active workout and so physical treatment may be delayed. This research investigates the potential role of early passive motion in stimulating cortical areas of the brain dedicated to the control of the lower limb in UML. Methods. Seven pediatric patient (aged 15.35±4.36) took part in the study. They were treated for two weeks (2 daily sessions) with a robotic passive ankle mobiliser (Toe-Up!).The device was implemented using specially-designed shape-memory-alloy-based actuators. Range of motion, muscle length and Ashworth score were measured before and after this treatment. At the same time-points brain activity was recorded by 64-channels electroencephalography (EEG) under four different conditions: rest, active dorsiflexion of the ankle, assisted and passive mobilisation of the same joint. The acquired data were processed to obtain cortical ERD/ERS (Event Related Desynchronisation/ Synchronisation) maps, which were then compared. Results. Preliminary results show that this therapy is very well tolerated and that its application specifically improves ankle PROM (+4.71°, p=0.063) and plantarflexor muscle length (+7.57°, p=0.007). EEG data for the passive/assistive condition showed hints of improved desynchronisation in at least one frequency band. Conclusions. The results of this pilot study suggest that passive mobilisation by the Toe-Up! device produces valuable clinical effects and set a basis for the study of cortical re-organisation in UML patients treated with ankle passive/assistive mobilisation.

47. Electronic and optical properties of hexathiapentacene in the gas and crystal phases

Abstract

Using density functional theory (DFT) and its time-dependent (TD) extension, the electronic and optical properties of the hexathiapentacene (HTP) molecule, a derivative of pentacene (PNT) obtained by symmetric substitution of the six central H atoms with S atoms, are investigated for its gas and solid phases. For the molecular structure, all-electron calculations are performed using a Gaussian localized orbital basis set in conjunction with the Becke three-parameter Lee-Yang-Parr (B3LYP) hybrid exchange-correlation functional. Electron affinities, ionization energies, quasiparticle energy gaps, optical absorption spectra, and exciton binding energies are calculated and compared with the corresponding results for PNT, as well as with the available experimental data. The DFT and TDDFT results are also validated by performing many-body perturbation theory calculations within the GW and Bethe-Salpeter equation formalisms. The functionalization with S atoms induces an increase of both ionization energies and electron affinities, a sizable reduction of the fundamental electronic gap, and a redshift of the optical absorption onset. Notably, the intensity of the first absorption peak of HTP falling in the visible region is found to be nearly tripled with respect to the pure PNT molecule. For the crystal structures, pseudopotential calculations are adopted using a plane-wave basis set together with the Perdew-Burke-Ernzerhof exchange-correlation functional empirically corrected in order to take dispersive interactions into account. The electronic excitations are also obtained within a perturbative B3LYP scheme. A comparative analysis is carried out between the ground-state and excited-state properties of crystalline HTP and PNT linking to the findings obtained for the isolated molecules.
