Abstract

Objective: The aim of the current article was to present a radiographic method to determine the surface area of newly formed periodontal attachment, as well as to analyze volumetric and morphologic changes after regenerative periodontal treatment. Method and materials: In this retrospective study, 11 singular intrabony periodontal defects were selected for minimally invasive surgical treatment and 3D evaluation. 3D virtual models were acquired by the segmentation of pre- and postoperative CBCT scans. This study determined the surface area of baseline periodontal attachment (RSA-A) and defect-involved root surface (RSA-D) on the preoperative 3D models, and the surface area of new periodontal attachment (RSA-NA) on the postoperative models. Finally, cumulative change of periodontal attachment (ΔRSA-A) was calculated and Boolean subtraction was applied on pre- and postoperative 3D models to demonstrate postoperative 3D hard tissue alterations. Results: The average RSA-A was 84.39 ± 33.27 mm², while the average RSA-D was 24.26 ± 11.94 mm². The average surface area of RSA-NA after regenerative periodontal surgery was 17.68 ± 10.56 mm². Additionally, ΔRSA-A was determined to assess the overall effects of ridge alterations on periodontal attachment, averaging 15.53 ± 12.47 mm², which was found to be statistically significant (P = .00149). Lastly, the volumetric hard tissue gain was found to be 33.56 ± 19.35 mm³, whereas hard tissue resorption of 26.31 ± 38.39 mm³ occurred. Conclusion: The proposed 3D radiographic method provides a detailed understanding of new periodontal attachment formation and hard tissue alterations following regenerative surgical treatment of intrabony periodontal defects. Abstract

The aim of this case series was the histologic evaluation of guided tissue regeneration utilizing deproteinized bovine bone mineral (DBBM) when regenerative surgery was combined with (test) or without (control) early orthodontic tooth movement. Core biopsy samples were harvested from previously defected sites after 9 months. The histologic section showed integration of DBBM particles in newly formed bone in the apical and middle thirds of the defect, while in the coronal part, graft materials were mainly embedded in connective tissues in the control patient. Abstract

INTRODUCTION: To overcome the limitations of collagen membranes, which are most used membranes for guided bone regeneration (GBR), a synthetic magnesium membranes has been developed as alternatives. METHODS: Eighteen Beagle dogs were divided into three cohorts of six animals each at different time points (1 week, 8 weeks, 16 weeks), and two animals were assigned to a 52-week cohort. Experimental bone defects were filled with bovine xenograft and covered with either magnesium membrane or collagen membrane. The health status of the animals was regularly monitored and recorded. Following sacrifice, the hemimandibles were prepared for micro-CT (μ-CT) analysis. The following outcomes were observed - the new bone volume/total defect volume (BV/TV), soft tissue volume, void volume and residual magnesium metal and histological evaluation. The surface area and volume of the magnesium metal were also determined. RESULTS: The μ-CT analysis and histology evaluation showed that the greatest degradation of the magnesium membranes occurred between 1 and 8 weeks and continued until week 16, when only one membrane had visible remnants. Complete degradation of all the magnesium membranes had occurred at a time point of 52 weeks. DISCUSSION & CONCLUSIONS: The in vivo performance study demonstrated that the magnesium membrane has a comparable healing response and tissue regeneration to that of a resorbable collagen membrane. Overall, the magnesium membrane demonstrated all of the ideal qualities for a barrier membrane used in GBR treatment.

Abstract

INTRODUCTION: The most commonly used fixation systems in dentistry are non-resorbable titanium screws. Alternative resorbable magnesium WZM211 alloy screws with a MgF2 surface are presented and compared to standard titanium screws for membrane fixation in a guided bone regeneration model in beagle dogs, data previously published. Examples of the magnesium screw’s application to clinical cases will also be shown. METHODS: Twenty Beagle dogs had four 3-wall defect sites treated in the mandible (2 sites on the left and 2 on the right). Defects were filled with bovine bone granules, covered with a collagen membrane and secured with either 4 magnesium screws or 4 titanium screws. Animal wellbeing was regularly checked by a veterinary team. Cohorts of six animals were assigned to 1 week, 8 week and 16 week timepoints, with 2 spare animals that were later assigned to a 52 week timepoint. Micro-CT and histological evaluation were performed for defect analysis at each timepoint. DISCUSSION & CONCLUSIONS: The expected observations of transient inflammation and pain were observed postoperatively. A good healing response was demonstrated in both groups. Micro-CT analysis showed no significant difference between groups in terms of relative bone volume or soft tissue volume. Initially void space was observed in the magnesium screw group caused by the release of hydrogen gas, however, the void volume continued to decrease on average between each timepoint. Volumetric measurement of the magnesium screws showed a gradual progression of the degradation process until no remnants were visible at the 52 week timepoint. Overall, the magnesium screws and titanium screws showed equal performance in tissue regeneration according to the principles of guided bone regeneration. Yet the magnesium screws had the additional advantage of being completely resorbable. The success of the in-vivo performance study has since been replicated in a clinical setting.
