Hereditary neuropathy with liability to pressure palsies could be diagnosed in two families. This little-known, dominantly inherited disorder is clinically characterised by recurring, spontaneously regressive palsies of peripheral nerves, mostly after minimal mechanical compression of the nerve concerned. It can be clearly differentiated from mononeuropathies of different pathogenesis even in clinically not involved nerves, by means of pathological neurographic findings and by the detection of pathognomonic myelinic thickenings in nerve biopsies. Prognosis is favourable if recurrences are avoided. This means that counselling of the patients and their families is of prophylactic significance.

This paper reports results from a survey-based study among eighty-six airline pilots investigating their willingness to report safety-relevant events and incidents. Pilots have been asked to report how many events they have experienced in thirty-five different contextual areas and how often they have reported these cases. Thus, underreporting rates, respectively dark figures, were calculated and listed. These results and the willingness to report are discussed within an aviation operation’s background. Most of these surveyed underreporting rates are very high, which means a substantial source of uncertainty in airlines’ safety reporting databases, and thus for airlines’ safety management systems.

Ein 36-jähriger Portugiese wurde wegen eines diffusen Krankheitsgefühls, eines Gewichtsverlusts von 25 kg im letzten halben Jahr sowie wegen Nachtschweißes auf Zuweisung eines Hausarztes bei uns im Spital abgeklärt. Im CT des Thorax und Abdomens fand sich nebst einer bis nach retrosternal reichenden Struma eine Raumforderung im vorderen oberen Mediastinum. Gleichzeitig wurde die Diagnose einer Hyperthyreose vom Typ eines Morbus Basedow gestellt. Diese wurde mittels Carbimazol thyreostatisch behandelt. Aufgrund der unklaren Dignität wurde die Raumforderung im vorderen oberen Mediastinum thorakoskopisch entfernt. Histologisch handelte es sich um eine Thymushyperplasie mit Begleitthymitis ohne Zeichen der Malignität. Die Assoziation von Morbus Basedow und Thymushyperplasie ist in der Literatur beschrieben, bei Internisten und auch Endokrinologen jedoch wenig bekannt. Die Thymushyperplasie ist unter thyreostatischer Therapie regredient. Rückblickend hätten wir bei unserem Patienten auf die Thymektomie verzichten sollen.
