Abstract

Background: Communication skills (CSs) and evidence-based practice (EBP) are key foundations for offering personalized and quality nursing care. CS that results in open communication is fundamental for applying EBP. Objectives: The objective of the study was to assess the relationship between the acquisition of EBP competencies and learning CS after taking an EBP course in the nursing degree. Methods: Pre-test-post-test quasi-experimental study without a control group. The study population comprised fourth-year undergraduate nursing students. Before and after an EBP course, students completed a questionnaire on CS competence (HP-CSS) and EBP competence (EBP-COQ). The EBP course lasts for 15 weeks, with 150 h of work for the student. Out of these 150 h, 60 are conducted in person, while the remaining 90 h are for independent or group work, utilizing the flipped classroom methodology. Bivariate, correlational, pairwise Student's t-test, and linear regression multivariate analyses were performed. Results: The sample was composed of 153 students. After completing the EBP course, there was a statistically significant improvement in informative communication (p = 0.046) and assertiveness (p = 0.018). However, there were no observed changes in empathy, respect, and the total score of the CS competence. The results from the multivariate analysis showed that the dimensions of attitude towards EBP and EBP knowledge had a positive relation to three of the four dimensions of the CS and overall competence. Regarding the students' variables and characteristics, admission through special programs for athletes and older students (p < 0.001) and being female (p = 0.004) were positively statistically associated with empathy. Conclusions: EBP training positively affects the development of CS in nursing students, as shown by significant improvements in the total scores of the CS and the dimensions of informative communication and assertiveness after the intervention. 16. Formal Quality and Compliance of Informed Consent Forms in Critical Care and Surgical Areas in Spain: An Observational Study.

Abstract

(1) Background: The informed consent form must contain all the relevant information about the procedure to be performed to guarantee the patient's freedom to choose. (2) Objective: To analyze the formal quality of, and compliance with informed consent forms in critical care and surgical areas in a county hospital in Spain. (3) Methods: The formal quality of informed consent forms in critical care and surgical areas from the hospital were analyzed, following the established formal quality criteria for informed consent forms. The compliance with specific criteria for each of the operated patients during the period of study was also evaluated. (4) Results: The formal quality of 224 informed consent forms was analyzed from 8 disciplines observing a median of non-compliances of 4 with a minimum of 1 and a maximum of 5, with the most breaches being in verifying the delivery of a copy to the patient and showing contraindications. The compliance of 376 documents from 188 operated patients were assessed, highlighting that the non-complied items were: the personalized risks and complete identification of the patient and the physician. A significant association was found between disciplines analyzed and the identification of the physician and personalized risks, with anesthesia and critical care showing the best compliance. (4) Conclusions: The informed consent forms in critical care and surgical areas were shown to have a deficient formal quality and an inadequate compliance. These deficiencies should be corrected to improve the information received by the patients and to guarantee their freedom to choose. As nurses have a responsibility to ensure that patients are adequately informed about both nursing interventions and care, as well as the surgical treatments they receive, consideration should be given to the possibility of nursing professionals taking the lead in obtaining informed consent. Abstract

OBJETIVOS El objetivo principal del estudio fue describir y cuantificar las complicaciones obstétricas asociadas al sobrepeso y la obesidad en mujeres españolas de la Región de Murcia durante el embarazo, parto y puerperio inmediato. METODOLOGÍA Para dar respuesta a los objetivos del estudio se llevó a cabo un estudio observacional de cohortes retrospectivo. El estudio se realizó en el hospital de referencia para patología obstétrica de la Región de Murcia de diciembre de 2014 a febrero de 2015, con una muestra (N=331) de gestantes de bajo riesgo de nacionalidad española. Las variables del estudio incluyen datos sociodemográficos y clínico-obstétricos que comprenden complicaciones asociadas al exceso de peso durante la gestación el parto y el puerperio inmediato. Los datos se recogieron a partir de los registros clínicos, la cartilla del seguimiento del embarazo y una entrevista a las mujeres tras el parto. Se realizaron análisis descriptivos, bivariados (T-Student, ANOVA, U de Mann-Whitney y Kruskall-Wallis) y multivariante (regresión logística y regresión lineal). También se estimaron los costes sanitarios asociados al sobrepeso y obesidad en las mujeres gestantes. Se empleó el programa estadísticos IBM SPSS 21.0. Abstract

La situación actual en el mundo occidental refleja un cambio gradual de la figura del padre en la crianza de su descendencia; según refleja Madeleine Leininger en su teoría, debemos conocer las características de este fenómeno para aplicar cuidados culturalmente congruentes. Basándonos en el concepto de Mercer “los roles no están fijados; por tanto, carece de importancia qué persona se encarga de realizar los distintos roles” (Marriner, 2007) podemos asemejar el vínculo materno-filial al paterno-filial. Proponemos una investigación de corte cualitativo basada en la entrevista en profundidad al objeto de estudio: padres (hombres) noveles; y grupos de discusión a matronas como figura profesional más representativa y que está directamente implicada con la salud reproductiva de la mujer y del cuidado del recién nacido. Así podemos afirmar que la idea de familia típica tal y como es aceptada entra en crisis entorno a los años noventa, emergiendo así la figura de los Nuevos Padres, concepto introducido por Bonino (2003). Nos encontramos con un padre cuya presencia en el cuidado de los hijos tiene una gran importancia en el buen desarrollo de los mismos (Alberdi & Escario, 2007). Por tanto debemos trabajar sobre el modelo de Nuevos Padres para definir la implicación de éste en la crianza. Baseado no conceito de Mercer “papéis não são fixos, então não importa onde a pessoa é responsável por executar os diferentes papéis” (Marriner, 2007) podem imitar a subsidiária materno-paterno-filial. Propomos uma pesquisa qualitativa com base em entrevistas em profundidade a fim de estudar: os pais (homens) novato, e grupos de discussão parteiras profissionais como a figura mais representativa e está diretamente envolvido com a saúde reprodutiva das mulheres e cuidados o recém-nascido. Assim, podemos dizer que a idéia típica família é aceito como está em crise em torno da década de noventa, emerge a figura de os novos pais, um conceito introduzido por Bonino (2003). Encontramos um pai cuja presença no cuidado das crianças é de grande importância no desenvolvimento adequado desses (Alberdi & Escario, 2007). Portanto, devemos trabalhar com o modelo para definir o envolvimento dos pais em Nova elevando-o. A parteira é o profissional mais qualificado para servir como agentes de mudança.
