1. Trends, over 14 years, in the ground cover on an unimproved western hill grazed by sheep, and associated trends in animal performance

Abstract

The frequency of individual plant species at ground level and the species composition of the unimproved vegetation on a western hill farm, stocked with Scottish Blackface sheep, were monitored from 1995 to 2008. Performance criteria of the flock that relied totally, or almost totally, on this vegetation for sustenance from 1994 to 2011 were evaluated. The frequency of vegetation increased over time (from 65% to 82% of the surface area; P < 0.05), with a corresponding decline in the frequency of bare soil, thus reducing vulnerability to soil erosion. This increased incidence of vegetation cover reflected increases in ‘other forbs’(P < 0.01), heather(P < 0.05)and grass (P < 0.08).A significant change (P < 0.05) also occurred in the species composition of the vegetation, reflecting an increase in the proportions of ‘other forbs’ (P < 0.05) and heather (P = 0.14), and a decline in the proportion of sedges (P = 0.14). A similar pattern occurred in the two main habitats: blanket bog and wet heath. Annual stocking rate (ewes per hectare, based on actual ewe grazing days) on the unimproved hill grazing averaged 0.9 (0.13 livestock units) per hectare prior to 1999 and 0.78 (0.11 livestock units) per hectare subsequently. There was no trend in weight gain of replacement females while confined to the unimproved hill area between weaning (14 weeks old) and first joining at 18 months of age. A negative trend (P < 0.01) occurred in the pre-weaning growth rate of lambs on the hill. The average number of lambs reared per ewe joined (reflecting fertility, litter size and ewe/lamb mortality) was 1.0, and this showed no evidence of change over time. The study flock performed 10% to > 60% better, depending on the variable, than similar flocks in the National Farm Survey at comparable stocking rates. A well-defined rational management system can sustain a productive sheep enterprise on unimproved hill land without negative consequences for the frequency or composition of the vegetation.

Abstract

The role of pharmacists caring for patients with chronic kidney disease has been documented in the international literature. This study aims at compiling all pharmaceutical services provided to these patients in order to adapt them to the Portuguese reality. Medline, International Pharmaceutical Abstracts, Pharmacy Abstracts and The Cochrane Library were searched for studies describing pharmacist interventions in patients with chronic kidney disease. Ultimately, and after applying the inclusion and exclusion criteria, 37 studies (corresponding to 38 articles) were included and analyzed. Pharmacist interventions reported were: performing medication reviews to identify drug-related problems, adjusting and optimizing drug therapy, identifying and correcting drug records discrepancies, evaluating admission and discharge medication appropriateness, performing lab- oratory monitoring of specific parameters, implementing anaemia managing, phosphate-managing and lipid-managing protocols, performing patient education, improving compliance, participating in medical rounds and multidisciplinary patient care meetings, communicating and interacting with other health care professionals, and providing information about drug selection and therapeutic regimes. In conclusion, a multitude of potential pharmacist interventions in patients with chronic kidney disease was reported in the literature, where pharmacists played a key role in contributing to improve the process of use of medicines and clinical outcomes in these patients.

However, predictors of MRB-QoL were not reported in greater detail in the validation study. This study aimed to explore factors associated with MRB-QoL to see whether there is any new information that calls for further research.Methods: Analysis of data from the MRB-QoL validation study was undertaken. Outcome variables were domains of the MRB-QoL (Routine and Regimen Complexity, Psychological Burden, Functional and Role Limitation, Therapeutic Relationship, and Social Burden). Explanatory variables were patient age; disease-related factors; and medication-related factors, such as number of medications, complexity of medication regimen (measured by the Medication Regimen Complexity Index [MRCI]), and exposure to medications with anticholinergic and sedative effects (measured by the Drug Burden Index [DBI]). Linear regression analyses were used to identify factors associated with the MRB-QoL.Results: The study included 367 participants (52.1% male), with a median age of 64 years. In multivariable regression analyses, an increase in the DBI was significantly associated with poorer Psychological wellbeing (β=− 0.15, p< 0.001) and Functional and Role Limitation (β=− 1.79, p< 0.001). Living with three or more medical conditions was significantly associated with poorer Psychological wellbeing (β=− 0.21, p< 0.001). Age was significantly associated with all domains of the MRB-QoL (β=0.28 to 0.55). Polypharmacy and MRCI were not associated with any of the MRB-QoL domains.Conclusion: In this sample of community-dwelling adults with multiple medications, the DBI was independently associated with the Psychological Burden and Functional and Role Limitation domains of the MRB-QoL. This study provides preliminary evidence on factors affecting medication-related quality of life outcomes from a patient perspective. Future longitudinal studies, along with further psychometric testing of the MRB-QoL measure, are warranted to better understand predictors of MRB-QoL.Keywords: patient-reported outcomes, medication-related burden, quality of life, community-dwelling adults

Abstract

Policy making to promote more sustainable development is a complex task due in part to the large number of both stakeholders and potential policies. Policy feasibility testing provides a guide to the viability and practicality of policy implementation and forms an important part of an evidence based policy making process. An extensive literature review has identified no standardized approach to feasibility testing. This paper addresses this knowledge gap by describing a novel method using Multi-Criteria Decision Analysis (MCDA) for feasibility testing of policies aimed at increasing the sustainability of towns and villages in Ireland. Feasibility results are provided for 40 frequently cited policy interventions tested for 18 settlements in Ireland. Policies were selected in the arenas of transport, food, housing and urban form, energy, waste and water. Policies are feasibility tested through analysis of operational evidence from both quantitative and qualitative data sources. Following testing, policies are ranked in terms of feasibility. This research examines the effectiveness of local and national level policies and the importance of both local community involvement and central government regulation in policy success. The inter-settlement variation in feasibility testing scores prioritises policy selection and aims to reduce cherry-picking of policies to support the viewpoints of the decision maker. Although developed for an Irish urban context the methods described here may have applicability elsewhere.
