Purpose: To implement a SARS-CoV-2 wastewater surveillance system in Valencia and study the correlation between SARS-CoV-2 concentration levels and epidemiological indicators. Methods & Materials: The working strategy developed in the municipality of Valencia consisted of monitoring wastewater effluents collected by 790,000 people. The city was divided into 24 different hydraulic sectors. 3,023 samples of untreated wastewater were collected and analysed between May 2020 and May 2021. RNA extraction from sewage material was carried out using the NucleoSpin RNA virus Kit. SARS-CoV-2 RNA detection was performed by RT-qPCR using One-Step PrimeScript™ RT-PCR Kit (Perfect Real Time), targeting the nucleoprotein (N), N1 and N2 fragments, and envelope protein (E) gene. Mengovirus RNA recovery rates were used as quality assurance parameters according to ISO 15216-1:2017. Results: A total of 2,169 samples were positive for SARS-CoV-2 RNA material. SARS-CoV-2 variations were detected throughout the entire study evidencing trends during the first, second and third wave. As the National State of Emergency ended (June 2020), SARS-CoV-2 values began to increase reaching the first and most significant concentration spike of the study (20th-26th of July 2020), with a weekly average aggregate concentration of 34,5M GC/L (an increase of 2 units in the log scale). This translated in the worsening of epidemiological indicators (number of cases, hospitalizations, deaths and cumulative incidence (CI)), which maintained a stable increase until a second spike was detected during an important bank holiday in October (9th-12th), where the concentration changed from 28M GC/L to 345M GC/L in a 3-day period while the city was registering 1,100 new weekly cases and a CI of 200 cases/100,000 residents. Finally, a third wave placed Valencia with the worst historical epidemiological data (6,545 new cases; 1,000 new hospitalizations; CI= 1,318,04 cases/100,000 inhabitants) with a weekly SARS-CoV-2 average aggregate concentration of 967M GC/L, leading health officials to implement new restrictive measures. Conclusion: Wastewater surveillance could be used as a complimentary tool to estimate the presence and prevalence of COVID-19 in communities and can be used for preventive purposes, as an increasing SARS-CoV-2 trend in wastewater could be a signal of the possible re-emergence of the pandemic.

Abstract Introduction Colorectal cancer treatment requires a complex, multidisciplinary approach. Because of the potential variability, monitoring through clinical audits is advisable. This study assesses the effects of a quality improvement action plan in patients with locally advanced rectal cancer and treated with radiotherapy. Methods Comparative, multicentre study in two cohorts of 120 patients each, selected randomly from patients diagnosed with rectal cancer who had initiated radiotherapy with a curative intent. Based on the results from a baseline clinical audit in 2013, a quality improvement action plan was designed and implemented; a second audit in 2017 evaluated its impact. Results Standardised information was present on 77.5% of the magnetic resonance imaging (MRI) staging reports. Treatment strategies were similar in all three study centres. Of the patients whose treatment was interrupted, just 9.7% received a compensation dose. A visible-light photoredox functionalization of 3,4-dihydro-quinoxalin-2-ones with pyrazolones using an inexpensive organophotoredox catalyst is described. The reaction uses 9,10-phenanthrenedione as photocatalyst, a very simple and cheap photocatalyst, a HP single Blue LED (455 nm) as visible-light source and oxygen from air as terminal oxidant, obtaining the corresponding acetylated pyrazoles with good yields. The reaction can be extended to other nitrogen heterocycles such as 5-aminopyrazoles with good results.

Covalent adaptable networks (CANs) represent a pioneering advance in polymer science, offering unprecedented versatility in materials design. Unlike conventional adhesives with irreversible bonds, CAN-based polyurethane adhesives have the unique ability to undergo chemical restructuring through reversible bonds. One of the strategies for incorporating these types of reactions in polyurethanes is by functionalisation with Diels–Alder (DA) adducts. By taking advantage of the reversible nature of the DA chemistry, the adhesive undergoes controlled crosslinking and decrosslinking processes, allowing for precise modulation of bond strength. This adaptability is critical in applications requiring reworkability or recyclability, as it allows for easy disassembly and reassembly of bonded components without compromising the integrity of the material. This study focuses on the sustainable synthesis and characterisation of a solvent-based polyurethane adhesive, obtained by functionalising a polyurethane prepolymer with DA diene and dienophiles. The characterisation of the adhesives was carried out using different experimental techniques: nuclear magnetic resonance spectroscopy (NMR), Brookfield viscosity, differential scanning calorimetry (DSC), thermogravimetric analysis (TGA) and T-peel strength testing of leather/adhesive/rubber joints to determine the adhesive properties, both before and after the application of external stimuli. The conversion of both the DA and retro-Diels–Alder (r-DA) reactions was confirmed by 1H-NMR. The adhesive properties were not altered by the functionalisation of the adhesive prepolymer, showing similar thermal resistance and good rheological and adhesive properties, even exceeding the most demanding technical requirements for upper-to-sole joints in footwear. After the application of an external thermal stimuli, the bonded materials separated without difficulty and without damage, thus facilitating their separation, recovery and recycling.

La seguridad, así como el acceso a la información y formación relativa a la prevención es un derecho que todas las personas deben tener garantizado. Este no es el caso de la mayoría de las personas con discapacidad intelectual (DI), quienes pueden presentar numerosas dificultades para desenvolverse en situaciones de emergencia. El proyecto SIT (Seguridad, Inclusión y Tecnología)se centra en el diseño y desarrollo de una solución tecnológica que permita la identificación, atención y formación preventiva de personas con DI en situaciones de emergencia con el fin de mejorar así su autodeterminación y calidad de vida. Para ello hemos utilizado la metodología de la Investigación Basada en el Diseño (DBR) como método de investigación por su característica cíclica de refinamiento de los productos diseñados. Esto nos ha permitido validar el contenido de la aplicación SOSDI y la web del proyecto SIT en sus versiones teóricas finales. En este artículo podrán encontrar el flujo de uso, la definición detallada de cada una de las partes de los productos, así como los aspectos de accesibilidad que se han tenido en cuenta para garantizar la accesibilidad cognitiva.

Desde que el clarinetista alemán Johann Christoph Denner le añadiera la primera llave al antiguo chalumeau francés en el siglo XVII, inventando así el antecesor del clarinete, el peculiar sonido del instrumento ha sido objeto de estudio. A lo a lo largo de la historia, son muchos los que han intentado analizar el comportamiento acústico y las características particulares del tubo de este instrumento que, ya en su forma más primitiva, llegó a entusiasmar a compositores como Stamitz, Mozart, Weber, Brahms, ... Seguidamente revisaremos los fundamentos de la sonoridad que lo caracteriza, y, siguiendo la evolución del instrumento, trataremos de ofrecer nuestra visión de cuáles fueron las cualidades del mismo que inspiraron a los distintos compositores en los principales estilos musicales. Para ello nos fijaremos en obras de algunos de los composi- tores más representativos de cada época (Barroco, Clasicismo, Romanticismo y siglo XX), para mostrar así cómo el grado de evolución del instrumento fue determinante para su utilización.

Abstract: Staphylococcus aureus is an important pathogen involved in a series of infections whose impact is increased by its multiple factors of virulence and antimicrobial resistance. Erythromycin induced clindamycin resistance is a growing problem in various parts of the world. This study was retrospective and analyzed the behavior in response to antimicrobials of 4307 strains of Staphylococcus aureus isolated in a hospital in the city of Maracaibo between January 2006 and December 2013. The frequency of erythromycin-induced clindamycin resistance, its association with resistance to oxacillin and the biological origin of the samples from which the microorganism was isolated were determined. Susceptibility to oxacillin was checked by diffusion method with disk agar and the induced clindamycin resistance was evaluated using the D-test. 60 D-Test positive strains were detected (1.39%), of which 38 (63.33%) were sensitive to methicillin and 22 strains were resistant (36.67%. The total resistance to clindamycin (constitutive and induced) represented 31.43% (1354) of the total number of strains tested. The frequency of induced resistance to clindamycin in Staphylococcus aureus strains in the locality is still low for both methicillin sensitive and resistant strains. Resumen: Staphylococcus aureus es un importante patógeno involucrado en una serie de infecciones cuyo impacto se incrementa por sus múltiples factores de virulencia y su resistencia a los antimicrobianos. La resistencia a clindamicina inducida por eritromicina constituye un problema creciente en diversas partes del mundo. Este estudio fue de tipo retrospectivo y se analizó el comportamiento frente a los antimicrobianos de 4307 cepas de Staphylococcus aureus aisladas en un hospital de la ciudad de Maracaibo entre enero 2006 y diciembre de 2013. Se determinó la frecuencia de resistencia a clindamicina inducida por eritromicina, su asociación con la resistencia a oxacilina y el origen biológico de las muestras a partir de las cuales se aisló el microorganismo. La susceptibilidad a oxacilina se comprobó mediante el método de difusión con discos en agar y la resistencia inducida a clindamicina usando la prueba D-test. Se detectaron 60 cepas D-Test positivo (1,39%), de las cuales 38(63,33%) fueron sensibles a meticilina y 22 cepas fueron resistentes (36,67%). La resistencia total a clindamicina (constitutiva e inducida) representó el 31,43% (1354) del total de cepas evaluadas. La frecuencia de resistencia inducida a clindamicina en cepas de Staphylococcus aureus en la localidad es aún baja tanto en cepas sensibles como resistentes a meticilina.

The access to COVID vaccines by millions of human beings and their high level of protection against the disease, both in its mild and severe forms, together with a plausible decrease in the transmission of the infection from vaccinated patients, has prompted a series of questions from the members of the College of Physicians of Madrid (ICOMEM) and the society. The ICOMEM Scientific Committee on this subject has tried to answer these questions after discussion and consensus among its members. The main answers can be summarized as follows: The occurrence of new SARS-CoV-2 infections in both vaccinated and previously infected patients is very low, in the observation time we already have. When breakthrough infections do occur, they are usually asymptomatic or mild and, purportedly, should have a lower capacity for transmission to other persons. Vaccinated subjects who have contact with a SARS-CoV-2 infected patient can avoid quarantine as long as they are asymptomatic, although this decision depends on variables such as age, occupation, circulating variants, degree of contact and time since vaccination. In countries with a high proportion of the population vaccinated, it is already suggested that fully vaccinated persons could avoid the use of masks and social distancing in most circumstances. Systematic use of diagnostic tests to assess the immune response or the degree of protection against reinfection after natural infection or vaccination is discouraged, since their practical consequences are not known at this time. The existing information precludes any precision regarding a possible need for future revaccination. This Committee considers that when mass vaccination of health care workers and the general population is achieved, SARS-CoV-2 screening tests could be avoided at least in outpatient care and in the case of exploratory procedures that do not require hospitalization. post-print 156 KB

Since the beginning of the SARS-CoV-2 epidemic, virus isolation in the infected patient was only possible for a short period of time and it was striking that this occurred constantly and did not provide guidance on the clinical course. This fact led to confusion about the efficacy of some of the drugs initially used, which seemed to have a high efficiency in viral clearance and proved ineffective in modifying the course of the disease. The immune response also did not prove to be definitive in terms of evolution, although most of the patients with very mild disease had a weak or no antibody response, and the opposite was true for the most severe patients. With whatever the antibody response, few cases have been re-infected after a first infection and generally, those that have, have not reproduced a spectrum of disease similar to the first infection. Among those re-infected, a large number have been asymptomatic or with very few symptoms, others have had a moderate picture and very few have had a poor evolution. Despite this dynamic of rapid viral clearance, laboratory tests were still able to generate positive results in the recovery of genomic sequences and this occurred in patients who were already symptom-free, in others who were still ill and in those who were very seriously ill. There was also no good correlate. For this reason and with the perspective of this year and the half of pandemic, we compiled what the literature leaves us in these aspects and anticipating that, as always in biology, there are cases that jump the limits of the general behavior of the dynamics of infection in general. post-print 116 KB

En este trabajo se lleva a cabo un estudio exploratorio cuyo objetivo general es analizar el comportamiento sexual, y el nivel de conocimientos y actitudes relativas a las prácticas de sexo seguro en una muestra de personas con discapacidad intelectual para prevenir la violencia sexual hacia este colectivo. La muestra está compuesta por 78 personas (39 hombres y 39 mujeres) con discapacidad intelectual leve o moderada. Los resultados obtenidos mediante una entrevista estructurada indican que en el nivel de conocimientos sobre prácticas sexuales destaca que el 73.1% había mantenido relaciones sexuales, el 87.4% conocían los diferentes tipos de métodos anticonceptivos y el 56.3% confiaba en los profesionales y educadores para tratar temas relativos a la sexualidad. Estos resultados subrayan la necesidad de desarrollar programas de educación afectiva-sexual adecuados a este colectivo.

One of the difficulties in choosing the families who come to foster care is related to the lack of studies that indicate the features needed to support optimal integrated development of children in foster care and also help in the process both the family as the child to generate the necessary resources. The sample consisted of 53 families with 80 children in foster care for three years. The aim was to identify characteristics that allow promote the development of the child. The results underline the importance of establishing secure links between the child and the foster family, the integration of the child in the foster family (the emergence of a sense of belonging), the existence of children in the foster family and a sense of satisfaction with welcoming, by the foster family.

La calidad del desarrollo de los miembros de la familia depende de la calidad de las relaciones que han establecido. Desde esta perspectiva evolutiva, las interacciones tempranas padres-hijos van a constituir un aspecto central y básico de la teoría del apego de Bowlby. Mary Ainsworth nos permite establecer una clasificación de las conductas de apego y son Main y Solomon quienes nos aportan bajo la denominación de apego desorganizado los perfiles más complejos y perturbadores de las relaciones entre padres e hijos, relaciones que deben ser asimétricas y tienen un único sentido. En este trabajo ponemos de manifiesto las aportaciones relacionadas con el apego desorganizado su relación con el desarrollo de la personalidad y la necesidad de recoger criterios diagnósticos cuando las experiencias traumáticas son repetidas y acumulativas y tuvieron lugar muchos tiempo antes del reconocimiento de los síntomas.

The emergence and spread of new variants of SARS-CoV-2 has produced enormous interest due to their possible implication in the improved transmissibility of the virus, their consequences in the individual evolution of the infection, as well as in the possible escape from the immunity generated by the current vaccines. This list is extended by the variants of interest that emerge and are expanding in certain countries but are found sporadically in others, such as B.1.427 and B.1.429 (Californians) or B.1.617 (Indian). Whole genome sequencing or strategies specifically targeting the spicule gene are used in the microbiology laboratories for characterization and detection. The number of infected individuals, the sanitary situation of each country, epidemiological measures and vaccination strategies influence its dispersion and new variants are expected to emerge. This emergence can only be avoided today by increasing the vaccinated population in all countries and by not relaxing epidemiological containment measures. Objetivo: Las zonas de alta montaña en la región mediterránea europea son particularmente ricas en plantas vasculares endémicas. Nuestro objetivo es comparar los modelos altitudinales para la riqueza de plantas vasculares y la proporción de endemismos en dos regiones mediterráneas: Lefka Ori en la isla de Creta (Grecia) y Sierra Nevada en la Península Ibérica. Localización: Sierra Nevada, Granada (España); Lefka Ori, Creta (Grecia). Método: Los datos proceden de un muestreo estandarizado en varias cimas situadas a diferentes altitudes (GLORIA Multi-Summit approach; www.gloria.ac.at). El número de especies, tasas de endemicidad, y temperatura del suelo se compararon por medio de ANCOVA y regresión lineal. Resultados: Las dos regiones objeto de análisis, aunque similares climáticamente, muestran patrones llamativamente diferentes: en Sierra Nevada, la proporción de plantas vasculares endémicas (especies restringidas a Sierra Nevada) muestra un incremento gradual desde la cima más baja a la más alta. En contraste, la proporción de endemismos restringidos a Creta no fue significativamente diferente entre las cuatro cimas de Lefka Ori. Las tendencias observadas en ambas regiones fueron en gran parte consistentes con la distribución de las especies endémicas obtenida de las floras para cada región. Conclusiones principales: La posición geográfica de ambas regiones, por ejemplo. isla frente a continente, y la mayor elevación de Sierra Nevada se sugieren como las principales causas de las diferencias observadas. El alto grado de endemicidad en los ambientes fríos de las zonas bioclimáticas superiores de las montañas mediterráneas evidencia una marcada vulnerabilidad a los impactos del cambio climático. Por lo tanto, el seguimiento continuado e intensivo de las especies de montaña alrededor de la cuenca mediterránea, debería considerarse como una tarea investigadora prioritaria.

Los líquenes son organismos comunes en las zonas de alta montaña donde juegan un importante papel en el equilibrio de los ecosistemas. En los últimos años, el creciente interés por entender más acerca de sus interacciones con los factores abióticos ha motivado diversas investigaciones, algunas de las cuales han demostrado su valor como bioindicadores de las condiciones climáticas. En este contexto, centrándonos en los efectos del cambio climático en plantas vasculares de alta montaña y respaldados por el proyecto “Iniciativa para la investigación y el seguimiento global de los ambientes alpinos (GLORIA)”, se ha monitorizado por primera vez la diversidad de líquenes en Sierra Nevada con la intención de estudiar las posibles alteraciones que esta pueda sufrir causadas por el proceso de cambio climático. El objetivo de este artículo es el de dar a conocer la experiencia de seguimiento en el macizo y aportar los primeros resultados procedentes del análisis, tanto de la biodiversidad como estadístico, de los datos de muestreo.

BACKGROUND:Colorectal cancer is one of the most common cancers worldwide, and is influenced by the interplay of various factors, including a very strong genetic component. For instance, incorrect mitochondrial biogenesis is correlated with increased risk of developing colorectal cancer. Thus, it is important to understand the consequences of changes in both the expression and the correct function of the transcription factors that regulate mitochondrial biogenesis, namely NRF2. OBJECTIVES:The main objective of this paper is to characterise the relationship between NRF2 and colorectal cancer by compiling data from an exhaustive literature search. METHODS:Information was obtained by defining specific search terms and searching in several databases. After a strict selection procedure, data were tabulated and the relationships between articles were assessed by measuring heterogeneity and by constructing conceptual maps. RESULTS AND DISCUSSION:We found a general consensus in the literature that the presence of oxidizing agents as well as the inhibition of the NRF2 repressor Keap1 maintain NRF2 expression at basal levels. This predominantly exerts a cytoprotective effect on cells and decreases risk of colorectal cancer. However, if NRF2 is inhibited, protection against external agents disappears and risk of colorectal cancer increases. Interestingly, colorectal cancer risk is also increased when NRF2 becomes overexpressed. In this case, the increased risk arises from NRF2-induced inflammation and resistance to chemotherapy. CONCLUSION:The proper basal function of NRF2 and Keap1 are essential for preventing oncogenic processes in the colon. Consequently, any disruption to the expression of these genes can promote the genesis and progression of colon cancer.

En esta investigación parte de una hipótesis previa que pone en relación las distintas semiosferas de la cultura que circulan en el contexto globalizado del cine de Román Chalbaud. Se plantea como objetivo la explicación y reflexión de la semiosis textual lotmaniana del fenómeno de la globalización en la puesta en escena fílmica del cine chalbaudeano, a partir de la noción de identidad y de cambio en las mediaciones técnicas y tecnológicas. Se concluye que en esta puesta en escena fílmica con determinadas actividades situadas y el uso de las mediaciones técnicas y tecnológicas, el autor pone en circulación el concepto de lo propio identitario respecto a su relación con lo ajeno que es diverso y globalizado respecto al mundo y al otro social, permitiendo al espectador intérprete reflexionar acerca de las distintas semiosferas de la cultura venezolana y sus respectivos campos de sentido.

El hallazgo reciente de tres poblaciones del complejo Euphorbia esula-E. virgata (Euphorbiaceae) en el nordeste de la Península Ibérica (centro y sur de Cataluña) nos ha inducido a realizar un estudio macro y micromorfológico para establecer su identidad taxonómica. En esta zona tan sólo existían dos datos previos de plantas de este complejo, correspondientes a recolecciones de los años 1908/1909 (previamente identificadas como E. esula subsp. saratoi) y 1930 (E. esula s. l.). Según nuestros resultados, todo el material examinado (recolecciones recientes y antiguas) se puede atribuir a E. virgata, un taxon de área principal en el este de Europa, que encuentra en el nordeste de la Península Ibérica su límite suroccidental de distribución. Los caracteres macromorfológicos presentan un cierto grado de variación interpoblacional, y en algún caso los individuos se parecen mucho a las formas que suelen denominarse E. ×pseudovirgata, supuestamente híbridas entre E. virgata y E. esula, pero no es posible confirmar la presencia regional de este híbrido sin la realización de estudios complementarios. Con los resultados de este estudio, debe excluirse la presencia de E. esula subsp. esula de Cataluña, Valencia y Aragón. Las poblaciones actuales de E. virgata, muy localizadas pero densas, se sitúan en hábitats herbáceos con clara influencia antrópica (campos abandonados y bordes de carreteras y caminos), en suelos profundos bastante secos; estos hábitats son similares a los típicos de E. virgata en Europa oriental y central. Aparentemente se trata de poblaciones de carácter temporal, y en dos de las tres localidades recientes se ha confirmado que se han establecido con posterioridad a 2005. Los datos actuales sugieren que puede tratarse de una especie alóctona, pero considerando que se detectó ya hace un siglo, no se puede excluir que sea una autóctona rara con poblaciones itinerantes. [ct] La troballa recent de tres poblacions del complex Euphorbia esula-E. virgata (Euphorbiaceae) al nord-est de la península Ibèrica (centre i sud de Catalunya) ens ha induït a realitzar-ne un estudi macro i micromorfològic, per tal d’establir la seva identitat taxonòmica. En aquesta zona només existien dues dades prèvies de plantes d’aquest complex, corresponents a recol·leccions dels anys 1908/1909 (prèviament identificades com a E. esula subsp. saratoi) i 1930 (E. esula s. l.). Segons els nostres resultats, tot el material examinat (recol·leccions recents i antigues) es pot atribuir a E. virgata, un tàxon d’àrea principal a l’est d’Europa, que troba al nord-est de la península Ibèrica el seu límit sud-occidental de distribució. Els caràcters macromorfològics presenten un grau limitat de variabilitat interpoblacional, i en algun cas s’assemblen molt a les formes que solen anomenar-se E. ×pseudovirgata, suposadament híbrides entre E. virgata i E. esula, però sense estudis addicionals no és possible confirmar la presència regional d’aquest híbrid. Després d’aquest estudi cal excloure la presència d’E. esula subsp. esula de Catalunya, el Pais Valencià i l’Aragó. Les poblacions actuals d’E. virgata, molt localitzades però denses, es fan en hàbitats herbacis amb influència antròpica clara (camps abandonats i marges de carreteres i camins), en sòls profunds força secs; aquests hàbitats són similars als típics d’E. virgata a l’Europa central i oriental. Aparentment es tracta de poblacions de caràcter temporal, i en dues de les tres localitats s’ha confirmat un establiment posterior a 2005. Les dades actuals suggereixen que pot tractar-se d’una planta al·lòctona, però considerant que ja va ser detectada fa un segle, tampoc no es pot excloure que sigui una autòctona rara i amb poblacions itinerants.

Seseli farrenyi es un endemismo estrictamente catalán, que cuenta con un número muy reducido de poblaciones en la franja litoral norte del cabo de Creus (Alt Empordà). La especie ha sido catalogada como “En Peligro” (EN) según los criterios de la UICN basándose en una serie de estudios realizados a finales de los años noventa, incluyendo censos y cartografía detallada. Tras poco más de una década, la realización de un nuevo censo por parte del mismo equipo de investigadores revela una pérdida de más del 90% del total de individuos, así como la desaparición de la localidad clásica de la especie, justificando elevar su categoría de amenaza a “En Peligro Crítico” (CR). Una de las prioridades del plan de recuperación para esta especie (la redacción y aplicación del cual se deriva de la inclusión de S. farrenyi en el Catàleg de Flora Amenaçada de Catalunya) debería ser la realización de estudios demográficos que permitieran conocer las causas de este declive poblacional. [ct] Seseli farrenyi és un endemisme estrictament català, que compta amb un nombre molt reduït de poblacions a la franja litoral nord del cap de Creus (Alt Empordà). L’espècie ha estat catalogada com a “En Perill” (EN) segons els criteris de la UICN en base a una sèrie d’estudis realitzats a finals dels anys noranta, que van incloure un cens i una cartografia detallada. Després de poc més d’una dècada, un nou cens realitzat pel mateix equip d’investigadors revela una pèrdua de més de 90% del nombre total d’individus, junt amb la desaparició de la localitat clàssica de l’espècie, justificant elevar la seva categoria d’amenaça a “En Perill Crític” (CR). Una de les prioritats del pla de recuperació per a l’espècie (la redacció i aplicació del qual es deriva de la inclusió de S. farrenyi al Catàleg de Flora Amenaçada de Catalunya) hauria d’ésser la realització d’estudis demogràfics que permetin conèixer la causa del seu declivi poblacional.

Several health organizations, mainly in Western countries, have recently authorized the use of a booster dose of the COVID-19 vaccine for patients previously vaccinated with mRNA vaccines, with criteria that do not always coincide. The COVID Scientific Committee of the Illustrious College of Physicians of Madrid (ICOMEM) has received and asked several questions about this situation, to which the group has tried to give answers, after deliberation and consensus. The efficacy of the vaccines administered so far is beyond doubt and they have managed to reduce, fundamentally, the severe forms of the disease. The duration of this protection is not well known, is different in different individuals and for different variants of the virus and is not easily predictable with laboratory tests. Data on the real impact of a supplementary or “booster” dose in the scientific literature are scarce for the moment and its application in large populations such as those in the state of Israel may be associated with a decrease in the risk of new and severe episodes in the short observation period available. We also lack sufficient data on the safety and potential adverse effects of these supplementary doses and we do not know the ideal time to administer them in different situations. In this state of affairs, it seems prudent to administer supplemental doses to those exposed to a higher risk, such as immunocompromised individuals and the elderly. On the other hand, we consider that this is not the time to accelerate, on the spur of the moment, a massive administration of a third dose to other population groups that are less exposed and at lower risk, without waiting for adequate scientific information, which will undoubtedly arrive gradually. We do not believe that this position is incompatible with the practical and ethical warnings made by the World Health Organization in this respect. post-print 162 KB

The association between PS from food groups and homeostasis model assessment of insulin resistance (HOMA-IR) index, VO2 max, and metabolic risk score was assessed by multilinear regression analysis adjusting for several confounders. Analysis of covariance (ANCOVA) was used to determine the mean differences of food PS from food groups by HOMA-IR cutoff categories by using maternal education as a covariable., [Results] Larger PS from vegetables in both gender and milk, yoghurt, and milk beverages in males were associated with higher VO2 max, while larger PS from margarines and vegetable oils were associated with lower VO2 max (p < 0.05). Males who consumed larger PS from fish and fish products; meat substitutes, nuts, and pulses; cakes, pies, and biscuits; and sugar, honey, jams, and chocolate have a higher metabolic risk score (p < 0.05). Males with lower HOMA-IR cutoff values consumed larger PS from vegetables, milk, yoghurt, and milk beverages (p < 0.05). Females with lower HOMA-IR cutoff values consumed larger PS from breakfast cereals, while those with higher HOMA-IR cutoff values consumed larger PS from butter and animal fats (p = 0.018)., [Conclusion] The results show that larger PS from dairy products, cereals, and high energy dense foods are a significant determinant of IR and VO2 max, and larger PS from food with higher content of sugar were associated with higher metabolic risk score., HELENA study received funding from the European Community Sixth RTD Framework Program (Contract FOODCT-2005-007034).
