Objectives: This study aimed to assess whether awake bruxism and masticatory muscle activity could be related to external root resorption (ERR) in second molars adjacent to impacted mandibular third molars., Materials and Methods: Sixty patients, with requests for a cone-beam, computed tomography, were divided into two groups: ERR (patients with ERR in the second molar, n = 30), and control group (n = 30). Awake bruxism was assessed through the Oral Behaviors Checklist (OBC) and an ecological momentary assessment (EMA). Surface electromyography (EMG) was used to assess the masseter and anterior temporal muscle function. Normality and homogeneity of variances were demonstrated. Descriptive analysis was performed, using the T-test and Chi-square test to compare the characteristics of the groups. A multiple regression model was performed., Results: The ERR group presented more non-functional oral activities related to awake bruxism than the control group, according to OBC (p = 0.027) and EMA (p = 0.035). In addition, the ERR group had higher EMG activity than the control group in rest and isotonic protocols (p < 0.05)., Conclusions: Awake bruxism and greater masticatory muscle activity seem to be related to the presence of ERR in second molars adjacent to impacted mandibular third molars., Clinical Relevance: The results of the present study can reinforce the theory that triggering ERR in the second molars adjacent to impacted mandibular third molars may be related to mechanical forces coming from the masticatory function., (© 2024. We describe a case of a 71-year-old otherwise healthy man who presented to the dental clinic with the chief complaint of mobility involving his upper left molar teeth. The patient was a febrile, and clinical oral examination revealed localised grade II mobility and absence of gingival swelling, erythema or sinus tract. Orthopantogram revealed a poorly defined radiolucency involving the upper left second and third molar teeth. Surgical exploration of the involved area was performed and revealed the presence of a 'jelly like' brown tissue that fragments easily. Introduction: This study aimed to identify significant factors affecting the spontaneous angular changes of impacted mandibular third molars as a result of second molar protraction. Temporary skeletal anchorage devices in the missing mandibular first molar (ML-6) or missing deciduous mandibular second molar (ML-E) with missing succedaneous premolar spaces provided traction., Methods: Forty-one mandibular third molars of 34 patients (10 male and 24 female; mean age 18.3 ± 3.7 years) that erupted after second molar protraction were included in this study. They were classified into upright (U) and tilted (T) groups. Linear and angular measurements were performed at the time of treatment initiation (T1) and of ML-6 or ML-E space closure (T2). Regression analyses were used to identify significant factors related to third molar uprighting., Results: Nolla stage (odds ratio [OR] 4.1), sex (OR 0.003 for male), third molar angulation at T1 (OR 1.1), missing tooth space (OR 0.006), rate of third molar eruption (OR 23.3), and rate of second molar protraction (OR 0.2) significantly affected third molar uprighting. Age, third molar angulation at T1, rate of third molar eruption, and rate of second molar protraction were significant factors for predicting third molar angulation at T2., Conclusions: Available space for third molar eruption before and after second molar protraction is not associated with uprighting of erupting third molars. Older patients whose third molars are in greater Nolla stage, are in a more upright position at T1, and have a greater eruption rate have a greater chance for third molar uprighting. A growing body of literature demonstrates that genetic patterning mechanisms underlie the relative proportions of the mammalian postcanine dentition with the third molar being key to understanding variation within the molar row. With this relatively recent insight, there has been renewed interest in mammalian taxa that have lost the third molars. Within platyrrhines, the marmosets and tamarins (Callitrichidae family) are characterized by small body size, claw-like nails, twinning, and reduced molar number. Small body size is hypothesized to have resulted in the third molar being crowded out of the jaws leading to its evolutionary loss in this family. To further explore this hypothesis, we measured the cranium and dentition of 142 individuals spanning all five platyrrhine families. These data reveal that callitrichids have a significantly smaller proportion of mandibular postcanine tooth row length relative to other platyrrhines, refuting the "crowding out" hypothesis. However, postcanine tooth row length is significantly correlated with mandibular length and cranial length (P < 0.01) across all platyrrhines providing evidence for a strong allometric association between postcanine tooth row length and body size more generally. The small body size that characterizes callitrichids results in part from slower prenatal growth rates. Given the allometric relationship between postcanine tooth row length and body size, reported here and in previous studies, we hypothesize that the evolutionary loss of the third molars in callitrichids results from the inhibition of third molar development as a consequence of the slower prenatal growth rates associated with small body size in this family. Background and Objectives: Impacted third molars (ITM) are the most commonly-impacted teeth. There is a risk for ITM to cause a number of pathological conditions, and external root resorption (ERR) of adjacent teeth is one of the most prevalent. Retaining or prophylactic extraction of ITM is a polemic topic. External root resorption of adjacent teeth is one of possible indications for prophylactic removal of ITM. The aim of this study was to assess the relationship between external root resorption (ERR) on the distal aspect of second molars' roots and positional parameters of ITM. Methods : Cone beam computed tomography scans of 109 patients (41 males, 68 females; mean age 26.4 ± 7.9 years) with 254 ITM (131 in the maxilla and 123 in the mandible) were retrospectively analyzed. Positional parameters of ITM (mesio-distal position, angulation, impaction depth, and available eruption space) were evaluated. The presence, location, and depth of ERR of adjacent second molars were assessed. Results: Analysis showed a relationship between ITM impaction depth, mesial inclination angle, and the presence of ERR. Mesial inclination angle of more than 13.6° increased the odds of ERR occurrence by 5.439 (95% CI, 2.97-9.98). ITM presence at the level of ½ of roots of the adjacent second molar or more apically increased the odds of ERR occurrence by 2.218 (95% CI, 1.215-4.048). No significant correlation was detected between the occurrence of ERR and patient age, gender, or the available eruption space in the mandible. Depth of ERR did not depend on its location. Conclusions: Incidence of ERR in second molars is significantly associated with mesial inclination and a deep position of ITM.

The relationship between mandibular third molar (M3) angulation and mandibular angle fragility is not well established. The aim of this study was to evaluate the impact of M3 angulation on the mandibular angle fragility when submitted to a trauma to the mandibular body region. A three-dimensional (3D) mandibular model without M3 (Model 0) was obtained by means of finite-element analysis (FEA). Four models were generated from the initial model, representing distoangular (Model D), horizontal (Model H), mesioangular (Model M) and vertical (Model V) angulations. A blunt trauma with a magnitude of 2000 N was applied perpendicularly to the sagittal plane in the mandibular body. Maximum principal stress (P max ) (tensile stress) values were calculated in the bone. The lowest P max stress values were noted in Model 0. When the M3 was present extra stress fields were found around marginal bone of second molar and M3. Comparative analysis of the models with M3 revealed that the highest level of stress was found in Model V, whereas Model D showed the lowest stress values. The angulation of M3 affects the stress levels in the mandibular angle and has an impact on mandibular fragility. The mandibular angle becomes more fragile in case of vertical impaction when submitted to a trauma to the mandibular body region.

Purpose: The current data suggest that the presence of lower third molars predisposes the patient to a greater risk of mandibular angle fracture. Thus, the present review sought to determine whether an association exists between the presence of a lower third molar and the occurrence of a mandibular angle fracture in adults and to assess the influence of third molar position according to the Pell and Gregory classification., Materials and Methods: The present study was a systematic review and meta-analysis of analytical observational studies. The present review included all reports of the relationship between mandibular angle fractures and lower third molars. No restriction regarding year, language, or publication status was used. The review protocol was registered at the PROSPERO database (registration no. CRD42016047057). Electronic searches unrestricted for publication period and language were performed in the PubMed, Scopus, SciELO, and Latin American and Caribbean Health Sciences databases. Google Scholar and OpenGrey databases were used to search the "gray literature," avoiding selection and publication biases. The entire search was performed by 2 eligibility reviewers. Association and proportion meta-analyses were planned for the studies with sufficient data. The primary predictor variable was the relationship between the presence of a lower third molar and the development of mandibular angle fractures. The secondary outcome variables were the vertical and horizontal positions of the lower third molar, according to the Pell and Gregory classification and their relationship to the susceptibility to developing a mandibular angle fracture., Results: The search strategies resulted in 411 studies, from which 16 were selected for qualitative and quantitative review. The association meta-analysis included all the selected studies and showed that patients with lower third molars are 3.16 times more likely to develop mandibular angle fractures. The proportion meta-analysis included 5 studies and showed that the overall rate of mandibular angle fractures was 51.58% and that positions III and C are more likely to result in fracture, with a rate of 59.84 and 63.67%, respectively., Conclusions: The results of the present study have shown that the presence of impacted third molars increases by 3.16 times the risk of mandibular angle fractures in adults, with the greatest risk present when third molars are classified as IIIC according to Pell and Gregory. Objective: To investigate the stress distribution to the mandible, with and without impacted third molars (IM3s) at various orientations, resulting from a 2000-Newton impact force either from the anterior midline or from the body of the mandible., Materials and Methods: A 3D mandibular virtual model from a healthy dentate patient was created and the mechanical properties of the mandible were categorized to 9 levels based on the Hounsfield unit measured from computed tomography (CT) images. Von Mises stress distributions to the mandibular angle and condylar areas from static impact forces (Load I-front blow and Load II left blow) were evaluated using finite element analysis (FEA). Six groups with IM3 were included: full horizontal bony, full vertical bony, full 450 mesioangular bony, partial horizontal bony, partial vertical, and partial 450 mesioangular bony impaction, and a baseline group with no third molars., Results: Von Mises stresses in the condyle and angle areas were higher for partially than for fully impacted third molars under both loading conditions, with partial horizontal IM3 showing the highest fracture risk. Stresses were higher on the contralateral than on the ipsilateral side. Under Load II, the angle area had the highest stress for various orientations of IM3s. The condylar region had the highest stress when IM3s were absent., Conclusions: High-impact forces are more likely to cause condylar rather than angular fracture when IM3s are missing. The risk of mandibular fracture is higher for partially than fully impacted third molars, with the angulation of impaction having little effect on facture risk.

Background: Through a systematic literature review, the authors assess the effect of premolar extractions on third-molar (M3) eruption considering eruption rate, retromolar space, and molar angulation., Types of Studies Reviewed: The authors performed a systematic search using MEDLINE and Web of Science databases up through April 2017 to identify quality studies available comparing M3 eruption between a group with premolar extraction and a group without premolar extraction., Results: Twelve comparative retrospective cohort studies met all the inclusion criteria. The authors found in 5 studies comparing the rate of M3 eruption that there were significantly higher results in the group with extractions. They found in 5 studies comparing the evolution of the retromolar space significantly higher results in the group with extractions. Lastly, concerning the uprighting of the M3 during treatment, the authors found only 2 studies showing significant differences between the 2 groups, each time in favor of the group with extractions., Conclusions and Practical Implications: The dental literature on premolar extraction related to the eruption of the M3 is composed of average-quality retrospective studies. Premolar extraction significantly improves the chances of M3 eruption, but the level of evidence of comparative retrospective cohort studies is low. Clinicians must continue to rely on their judgment regarding premolar extraction on a case-by-case basis until the evidence is stronger. Food impaction after impacted mandibular third molar extraction is a serious problem that should not be ignored. Incomplete suturing of the distal incision in the conventional method is the main cause of food impaction and delayed wound healing. The present study introduces a novel suture and drainage technology that requires hermetic suturing of the distal incision and rubber drainage for buccal drainage. 76 patients with horizontally/mesially impacted third molars (bilateral) were enrolled in this prospective study. An impacted tooth on one side of each patient was extracted by occlusal drainage using the conventional method, whereas the other side tooth was extracted by buccal drainage using the novel method. The differences in wound healing, facial swelling, bleeding and dry socket between the two sides of each patient were compared postoperatively, and the trends for patient selection of the surgical method were also compared. The results indicated that buccal drainage had obvious advantages in wound healing and reduced the risk of postoperative bleeding, and most patients preferred this technique; there were no significant differences in postoperative facial swelling or pain. Thus, buccal drainage can solve the problem of long-term food impaction induced by traditional incision postoperatively and is worthy of clinical promotion.

Objectives: To investigate the effects of orthodontic non-extraction treatment with or without headgear on the position of and the space available for upper third molars in growing children with class II malocclusions., Materials and Methods: The sample consisted of pre- and post-treatment panoramic radiographs and lateral cephalograms of 294 class II orthodontic patients; 160 were treated with headgear and 134 were treated without headgear. The space available for the upper third molar was measured on the lateral cephalogram as the distance from pterygoid vertical (PTV) to the distal surface of the upper first molar crown (PTV-M1). Angulation, vertical position and tooth development stage of the upper third molars were evaluated on panoramic radiographs. All measurements were evaluated statistically., Results: In both groups PTV-M1 increased, but the increase in PTV-M1 was significantly higher for patients treated without headgear. A linear model for repeated measures revealed that this difference was still significant after correction for age, gender and molar occlusion. Further, there is no evidence that the change in angulation, vertical position and development stage of the upper third molars during orthodontic treatment is influenced by headgear therapy., Conclusion: This study indicates that the use of headgear in growing patients significantly affects the space available for upper third molars. However, orthodontic treatment with headgear does not influence the angulation, vertical position and development stage of upper third molars. The aim of this systematic review was to investigate the influence of the presence and position of mandibular third molars in mandibular condyle fractures. An electronic search was conducted in PubMed, Scopus, Web of Science, Cochrane Library, and VHL, through January 2016. The eligibility criteria included observational studies. The search strategy resulted in 704 articles. Following the selection process, 13 studies were included in the systematic review and 11 in the meta-analysis. In terms of the risk of bias analysis, six studies presented ≤6 stars in the Newcastle-Ottawa scale assessment. The presence of a mandibular third molar decreased the probability of condylar fracture (cross-sectional and case-control studies: odds ratio (OR) 0.26, 95% confidence interval (CI) 0.17-0.40, I 2 =87.8%; case-control studies: OR 0.30, 95% CI 0.16-0.58, I 2 =91.6%). The third molar positions most favourable to condylar fracture according to the Pell and Gregory classification are class A (OR 1.32, 95% CI 1.09-1.61, I 2 =0%) and class I (OR 1.37, 95% CI 1.05-1.77, I 2 =32.8%). Class B (OR 0.69, 95% CI 0.49-0.97, I 2 =56.0%) and class II (OR 0.71, 95% CI 0.57-0.87, I 2 =0%) act as protective factors for condylar fracture. The results suggest that the presence of a mandibular third molar decreases the chance of condylar fracture and that the positions of the third molar most favourable for condylar fracture are classes A and I, with classes B and II acting as protective factors., (Copyright © 2017 International Association of Oral and Maxillofacial Surgeons. Objectives: To analyse the effect of first and second premolar extractions on eruption space for upper and lower third molars and on third molar position and angulation during orthodontic treatment., Methods: The sample consisted of 296 patients of which 218 patients were orthodontically treated without extraction and 78 patients with extraction of first or second premolars. The eruption space for third molars was measured on pre- and posttreatment lateral cephalograms, whereas the angulation, vertical position, the relation with the mandibular canal and the mineralization status of third molars were evaluated using pre- and posttreatment panoramic radiographs. All data were statistically analyzed., Results: The increase in eruption space and the change in vertical position of upper and lower third molars significantly differed between patients treated with and without premolar extractions, whereas the change in angulation, relationship with the mandibular canal and mineralization status of the third molars did not significantly differ between patients treated with and without premolar extractions., Conclusions: The retromolar space and the position of third molars significantly change during orthodontic treatment in growing patients. Premolar extractions have a positive influence on the eruption space and vertical position of third molars, whereas they do not influence the angular changes of third molars. Due to the retrospective character of the study, these conclusions should be carefully considered. The aim of this systematic review was to investigate the influence of the presence and position of mandibular third molars on angle fractures. An electronic search was conducted in the PubMed, Scopus, Web of Science, Cochrane Library, and VHL databases, through January 2016. The eligibility criteria included observational studies. The search strategy resulted in 704 articles. Following the selection process, 35 studies were included in the systematic review and 28 in the meta-analysis. Twenty studies presented a score of ≤6 stars in the Newcastle-Ottawa scale assessment, indicating a risk of bias in the analysis. The presence of a mandibular third molar increases the chance of an angle fracture (case-control and cross-sectional studies: odds ratio (OR) 3.83, 95% confidence interval (CI) 3.02-4.85, I 2 =83.1%; case-control studies: OR 3.27, 95% CI 2.57-4.16, I 2 =81.3%). The third molar positions most favourable to angle fracture according to the Pell and Gregory classification are class B (OR 1.44, 95% CI 1.06-1.96, I 2 =87.2%) and class II (OR 1.67, 95% CI 1.36-2.04, I 2 =72.4%). Class A (OR 0.60, 95% CI 0.45-0.81, I 2 =87.1%) and class I (OR 0.51, 95% CI 0.37-0.71, I 2 =89.4%) act as protective factors for angle fracture. The results suggest that the presence of the third molar increases the chance of angle fracture by 3.27 times and that the most favourable positions of the third molar for angle fracture are classes B and II, whilst classes A and I act as protective factors., (Copyright © 2017 International Association of Oral and Maxillofacial Surgeons. A 22-year-old woman came with a unilateral missing mandibular first molar and buccal crossbite. The open space was closed by protraction of the mandibular left second molar and uprighting and protraction of the horizontally impacted third molar using temporary skeletal anchorage devices, and her buccal crossbite was corrected with modified palatal and lingual appliances. The total active treatment time was 36 months. Posttreatment records after 9 months showed excellent results with a stable occlusion., (Copyright © 2016 American Association of Orthodontists. Tooth autotransplantation is a viable treatment option for tooth replacement when a suitable donor tooth is available. These case reports described significant vertical bone growth after autotransplantation of a mature third molar. The left mandible third molars (n = 2) were transplanted to the missing tooth in the left mandible. The patient follow-up period was 10 years after transplantation. Clinical examination revealed no mobility of the transplanted tooth. Radiographic examination indicated that bone regeneration occurred around the transplanted tooth. Vertical bone growth was observed in the cervical area of the root surface and the recipient bone. In autotransplantation of mature teeth, long-term follow-up results indicate that vertical bone growth can be expected if viability of the periodontal ligament cells is maintained., (Copyright © 2015 American Association of Endodontists. The aim of the present study was to investigate the influences of the presence and position of a lower third molar (M3) on the fragility of mandibular angle and condyle, using finite element analysis. From computed tomographic scans of a human mandible with normally erupted M3, two additional virtual models were generated: a mandibular model with partially impacted M3 and a model without M3. Two cases of impact were considered: a frontal and a lateral blow. The results are based on the chromatic analysis of the distributed von Mises and principal stresses, and calculation of their failure indices. In the frontal blow, the angle region showed the highest stress in the case with partially impacted M3, and the condylar region in the case without M3. Compressive stresses were dominant but caused no failure. Tensile stresses were recorded in the retromolar areas, but caused failure only in the case with partially impacted M3. In the lateral blow, the stress concentrated at the point of impact, in the ipsilateral and contralateral angle and condylar regions. The highest stresses were recorded in the case with partially impacted M3. The aim of this systematic review was to evaluate the clinical effectiveness of the surgical technique of coronectomy for third molars extraction in close proximity with the inferior alveolar nerve.A literature survey carried out through PubMed, SCOPUS and the Cochrane Library from inceptions to the last access in January 31, 2014, was performed to intercept randomised clinical trials, controlled clinical trials, prospective cohort studies or retrospective studies (with or without control group) that examined the clinical outcomes after coronectomy. The following variable were evaluated: inferior alveolar nerve injury, lingual nerve injury, postoperative adverse effects, pulp disease, root migration and rate of reoperation. Ten articles qualified for the final analysis. The successful coronectomies varied from a minimum of 61.7% to a maximum of 100%. Coronectomy was associated with a low incidence of complications in terms of inferior alveolar nerve injury (0%-9.5%), lingual nerve injury (0%-2%), postoperative pain (1.1%-41.9%) and swelling (4.6%), dry socket infection (2%-12%), infection rate (1%-9.5%) and pulp disease (0.9%). Migration of the retained roots seems to be a frequent occurrence (2%-85.3%).Coronectomy appears to be a safe procedure at least in the short term, with a reduced incidence of postoperative complications. Therefore, a coronectomy can be indicated for teeth that are very close to the inferior alveolar nerve. If a second operation is needed for the remnant roots, they can be removed with a low risk of paresthesia, because the roots are generally receded from the mandubular nerve.

Background: There is no consensus regarding the prophylactic removal of mandibular third molars (TM) in fracture lines to facilitate healing. Recent evidence suggests that poor healing is attributed to the limited use of antimicrobials, delayed care and semi-rigid fixation as a treatment method, favoring retention of TM., Study Design: A retrospective cohort study of all patients presenting with mandibular angle fractures at the Hippokration General Hospital of Athens (2006-2011) was designed to examine the association between the presence versus absence of TMs in the line of mandibular fractures and the fracture healing process. Development of complications during the healing process was the outcome of interest. Additional factors considered were patient age, sex, and fracture etiology., Materials and Methods: Data were extracted from a retrospective chart review, including information from clinical and radiological examinations. The analytical sample included 112 patients with 121 angle fractures. The main objective of this study was to determine to what extent a detailed oral instruction about treatment after surgical removal of a lower wisdom tooth affects postoperative quality of life (QoL). The research on QoL after removal of a lower wisdom tooth was conducted with 108 patients. Depending on the type of information given to each respondent individually, the examinees were divided into two groups: a test group which was given detailed written and oral instructions, and a control group which received only written instructions about treatment after the surgery. In this research the QoL was examined using modified Oral Health Impact Profile-14 (OHIP-14) criterion four, seven and thirty days after the operation depending on the type of information previously provided to the respondents. In order to reduce the dimensionality of the obtained data sets, as well as in order to explain the relationship between the examined variables that are interrelated the principal component analysis (PCA) was applied. Both groups expressed satisfaction with the postoperative period for the individual variables investigated in modified OHIP-14 questionnaire, with the intensity and the order of the major components of satisfaction determined by the PCA differing between the two groups of the patients. On the fourth postoperative day, the test group expressed the highest level of satisfaction with sleep, physical appearance and the ability to eat. In the later postoperative period the test group (on the seventh and thirtieth day) had the highest level of satisfaction with the absence of discomfort during removal of sutures, satisfaction with performed treatment, and the ability to eat. Detailed preoperative oral instructions to patients can significantly improve the quality of life after operative removal of a lower wisdom tooth.

Several studies have reported the mechanism of crack propagation with aging. Although structural modifications of dentinal microcracks with aging have been evaluated by observing the cracked surface using scanning electron microscope (SEM), very few attempts have been made at sectional observation of the microcracks inside dentine using transmission electron microscopy (TEM). The objectives of this study were to: (1) examine the process of dentinal microcrack formation using TEM and (2) to morphologically evaluate the relation between dentinal microcrack propagation and human aging. Molars from 'young' (16-28 years) and 'aged' (62-76 years) subjects were evaluated. Dentine blocks were cracked with an indenter and sectioned using a diamond knife and ultramicrotome after embedding in epoxy resin. Microcracks were observed by TEM and ultra-high-voltage electron microscope tomography to determine the characteristics of crack propagation in the young and aged teeth. The results show that, in young teeth, crack propagation tended to pass through the dentinal tubules, while in aged teeth, it tended to deflect to the outer side of peritubular dentine (PTD), especially in coronal dentine. The advantage of this method is that it allows visualization and evaluation of the ultrastructural propagation of microcracks in dentine. The differences in crack propagation between young and aged dentine could be explained by differences in PTD thickness.

Objective: To compare the angular changes in the developing mandibular third molars in the mandibular second premolar extraction and non-extraction cases and to determine whether the mandibular second premolar extraction causes favorable rotational changes in the angulations of the developing mandibular third molars and benefits its later eruption., Methods: Pretreatment and posttreatment panoramic radiographs were taken from 45 subjects who had been treated by the extraction of the mandibular second premolars and 48 subjects who had been treated without extraction. The horizontal reference plane was used to measure and compare the changes in the angulations of the developing mandibular third molars., Results: The mean uprighting of the mandibular third molars seen in the extraction group was (5.9±2.5) degrees on the right side and (8.0±3.4) degrees on the left side following treatment. For the nonextraction group the mean uprighting was (1.8±2.4) degrees on the right side and (1.5±2.1) degrees on the left side. There was a statistically significant difference between the groups (P<0.05)., Conclusion: Mandibular second premolar extraction might be a helpful procedure on the angulations of the developing mandibular third molars.

Objective: To compare the root development and the growth rate of the mandibular third molar (M3 inf) in individuals where the M3 inf erupted vs individuals exhibiting M3 inf impaction., Materials and Methods: Serial standardized intraoral radiographs (Eggen technique) were taken annually of the mandibular third molar region from 132 subjects (71 male and 61 female) from 15 to 20 years of age. Based on the films, 264 lower third molars were classified into an eruption and an impaction group. Root development was recorded according to a quantitative method described by Haavikko (1970), and the eruption status was analyzed using logistic regression., Results: In total, 155 (59%) of the M3 inf erupted, and 109 (41%) were impacted at age 20. In 44 (33%) patients both M3 inf were impacted, in 21 (16%) patients one tooth was erupted and the contralateral tooth impacted, and in 67 (51%) patients both M3 inf were erupted. The more mature a tooth was at age 15, the higher was the probability of eruption (odds ratio: 3.89, P < .001). The growth rate of the root development stage was statistically significantly associated with the probability of eruption (odds ratio: 10.50, P  =  .041)., Conclusions: Delayed mandibular third molar root development is associated with impaction. Radiographs taken at age 15 may predict the risk of impaction and thereby guide decision making for the orthodontist or the oral and maxillofacial surgeon.

One of the main criteria used in dental age diagnostics in living adolescents and young adults is assessment of the mineralisation stage of the third molars. In the case of Europid populations, it has been established that impaction status has an influence on the rate of mineralisation of the third molars. In view of this, a study was undertaken to determine whether the chronological process of wisdom tooth mineralisation is dependent upon impaction status in black Africans too. Orthopantomograms (553) of 437 male and 116 female black South Africans with verified birth dates in the age group between 10 and 26 years were studied. Mineralisation stage and impaction status were determined for all third molars. Statistical measures were calculated for the mandibular wisdom teeth at stages F, G and H and for the maxillary wisdom teeth at stage H in the male gender for both impacted and non-impacted third molars. It was ascertained that the minimum age in persons with impacted third molars, depending on the wisdom tooth observed, was 0.19-2.57 years higher than in those with non-impacted wisdom teeth. Test persons with impacted mandibular wisdom teeth at stage F or G were on average between 0.32 and 1.88 years older than those with non-impacted mandibular wisdom teeth. The 50 % probability values of impacted wisdom teeth at stage H were 1.85-3.31 years higher than those in non-impacted wisdom teeth. The conclusion was drawn that in male black Africans, impacted mandibular wisdom teeth mineralise more slowly than non-impacted lower third molars. The presence of impacted mandibular wisdom teeth in mineralisation stage H in male black Africans does not, however, furnish proof of completion of the 18(th) year of life beyond reasonable doubt.

Background: The space available for an unerupted mandibular third may depend on the choice of premolar extracted., Aims: To investigate the effects of orthodontic treatment and premolar extractions on the inclinations of the mandibular third molars and the space available for their eruption, and to compare these changes with a nonextraction group., Methods: The pre- and post-treatment panoramic radiographs of 54 subjects (20 males, 34 females) were used. Eighteen of these subjects had the four first premolars extracted, 16 subjects had four second premolars extracted and 20 subjects were treated nonextraction. Changes in the inclinations and spaces available for the unerupted third molars were compared., Results: In the nonextraction group the third molars uprighted approximately 1 degree and in the second premolar extraction group the third molars uprighted 10 degrees. The spaces available for the third molars increased significantly in the first and second premolar extraction groups as compared with the space available in the nonextraction group., Conclusions: Orthodontic treatment and extraction of the second premolars improved the inclinations of unerupted third molars and the space available for their eruption into the arch. The changes in inclination and eruption space were less marked following first premolar extractions.

Introduction: The purpose of this study was to clarify the frontal chewing patterns of various crossbite malocclusions., Methods: A mandibular kinesiograph was used to record the masticatory movements of 106 subjects (ages, 12-35 years) with crossbite malocclusion and 22 subjects (ages, 16-30 years) with normal occlusion. The chewing patterns were classified into 8 chewing types according to the cycle shape of the frontal incisor point movement. The crossbite subjects were divided into 5 groups by the anteroposterior position of the crossbite, and then the subjects with posterior crossbite were divided into 3 groups by the transverse position of the crossbite. The Mann-Whitney U test was used to compare the frequency of each chewing type between any crossbite group and the control group; and between the various crossbite groups., Results: In the crossbite groups, normal chewing occurred much less often than in subjects with normal occlusion. In the posterior crossbite group, reverse chewing was greater (P = 0.002), and normal chewing was less frequent (P = 0.001) compared with the anterior crossbite group. When accompanied by mandibular shift, mandibular prognathism, arch crossbite, in the crossbite or shift side, reverse type, and reverse-crossing type occurred more often than in contralateral side., Conclusions: In the frontal plane, patients with posterior crossbite might have more abnormal chewing types than those with anterior crossbite, and posterior crossbite could contribute to the high frequency of reverse and reverse-crossing chewing types, especially when accompanied by mandibular shift, mandibular prognathism, or arch crossbite., (2010 American Association of Orthodontists. This study investigated the change in interproximal force (IPF) in mandibular anterior teeth during retention and the relationship between the irregularity index before orthodontic treatment and the IPF. The effect that an erupting third molar had on IPF was also examined. Forty treated patients (40 with extraction of four bicuspids) were followed for 18 months during the retention phase. The irregularity index was determined from initial plaster casts. The total IPF was determined by measuring the interdental frictional forces at the mandibular anterior teeth by withdrawing a metal strip. The total IPF increased during the retention phase until 18 months, and there was a positive correlation between the irregularity index and total IPF 6 to 18 months after active treatment. An erupting third molar did not affect the total IPF. An increase in the total IPF may be an indication of relapse in mandibular anterior crowding. In conclusion, orthodontists should pay special attention to potential relapse in the lower anterior teeth 6 months after active treatment in cases with severe anterior crowding before treatment.

Bone formation below the crown of mandibular horizontal incompletely impacted third molar is frequently seen in the middle-aged and elderly. The phenomenon shows lamina dura loss without radiolucency and we hypothesized the participation of mature enamel without any influence on the environmental oral status. In order to investigate the characteristics of the phenomenon based on the presence/absence of the lamina dura and radiolucency below the crown, we studied the relationship between 58 men and 43 women with a lamina dura without radiolucency, 12 men and 8 women without a lamina dura with radiolucency, 34 men and 16 women without a lamina dura without radiolucency, and the status of teeth in the ipsilateral mandible. Subjects without a lamina dura without radiolucency were significantly older than those with a lamina dura without radiolucency in both men (P < 0.0001) and women (P <0.01), indicating different chronological causes. Men without lamina dura with radiolucency showed significantly more tooth loss than those with a lamina dura without radiolucency (P < 0.00001) and those without a lamina dura without radiolucency (P < 0.0001), indicating the influence of poor oral health. Thus, the phenomenon without a lamina dura without radiolucency may show the clinical importance of bone formation in the elderly.

Introduction: Extraction of second permanent molars is an option for providing space in orthodontic treatment. Although many articles have described its impact on the outcome, there are few data on the prognosis of the eruption of the adjacent third molars. The aims of this investigation were to provide predictive models of eruption of third molars after second permanent molar extraction and to validate them., Methods: A total of 48 patients (ages, 11-23 years) who had 128 second permanent molars (54 maxillary, 74 mandibular) extracted during orthodontic treatment were followed until eruption of the third molars was complete., Results and Conclusions: A lineal regression model predicted the final angle of the third molars with the permanent first molar by using the variables of initial angle, jaw, and the developmental stage of the third molar. A logistic regression model predicted the probability of correct eruption by using the variables of initial angle, jaw, sex, age, and the developmental stage of the third molar., (2010 American Association of Orthodontists. Objective: To compare the opinions of Swedish orthodontists and American orthodontists regarding the association between third molar eruption and dental crowding., Materials and Methods: A survey was distributed to Swedish orthodontists (n = 230) asking their views on the force exerted by erupting third molars, its relationship to crowding, and their recommendations for prophylactic removal. Results were compared with those from a similar study conducted in the United States. Chi square analysis was used to determine differences in responses to questions between Swedish and American orthodontists. P < or = .05 was considered significant., Results: Both Swedish and American orthodontists believed that lower third molars were more likely than upper third molars to cause force (65% and 58% for Swedish and American orthodontists, respectively) and crowding (42% and 40%, respectively). No statistically significant differences were seen between the answers of American and Swedish orthodontists regarding the role of upper and lower third molars in causing crowding. Although only 18% of Swedish orthodontists "generally" or "sometimes" recommended prophylactic removal of mandibular third molars, 36% of American orthodontists "generally" or "sometimes" recommended removal (P < .0001)., Conclusions: Most orthodontists in the United States and Sweden do believe that erupting lower third molars exert an anterior force; however, they also believe that these teeth "rarely" or "never" cause crowding of the dentition. The reason that more American orthodontists recommend prophylactic removal of mandibular third molars remains unexplained.

Rotational drift of mandibular third molar teeth is a challenge for clinicians to predict and manage. Evidence on the incidence and degree of rotation is sparse. As the factors influencing rotation are not defined, prediction is impossible. A series of four cases with lower third molar rotation are presented and discussed. Significant rotation can alter the degree of difficulty for an extraction. A lack of well-documented cases has hindered investigation of this phenomenon. Further research is required to identify the aetiology, incidence and increase in risk factors caused by such rotational drift.

Enamel dissolution occurs when it contacts with acids produced by plaque bacteria, foods or drinks. There have been numerous and varied studies quantifying and characterizing the rate, extent and chemical aspects of enamel erosion; however, there is still hot debate about the amounts of enamel softening and loss. The objective of this study was to measure the enamel erosion process with high accuracy. Native third molars were partly eroded in citric acid (pH = 3.8) with some domains protected by a Ti coating layer. The surface morphology and structures before and after exposure to citric acid for different time periods were studied by AFM, SEM and nanoindentation, respectively. Based on this, the functions between the amounts of enamel softening, loss and erosion time were established for the first time. It was demonstrated that AFM, SEM and nanoindentation were suitable for measuring the early stages of enamel demineralization qualitatively and quantitatively.

Although radiolucency has been shown as a risk of infection, the poorly understood effects of aging on radiolucency correlate with acute pericoronitis, which has a high risk of infection extending any complications. We reviewed the records of 346 consecutive patients aged more than 41 years to evaluate whether pericoronal radiolucency below the crown in mandibular horizontal incompletely impacted third molars is related to acute inflammation. The frequency of acute inflammation in teeth with pericoronal radiolucency below the crown was similar to that in teeth without; however, the odds ratio of acute inflammation exhibited in women aged more than 61 years compared to women aged 41-50 years was 9.77 (95% confidence interval [CI]: 1.67-57.29; P < <0.05), and in women aged more than 61 years compared to women aged 51-60 years was 26.25 (95% CI: 2.94-234.38; P < 0.01). The odds ratio of severe acute inflammation exhibited in men aged more than 61 years compared to men aged 41-50 years was 16.67 (95% CI: 1.76-158.27; P < 0.01). These odds ratios indicate an association of acute pericoronitis, including the severe forms of acute inflammation that result from pericoronitis, with pericoronal radiolucency below the crown in the elderly.

Objective: Previous in vitro studies have described the wear characteristics of specimens in which enamel has been opposed to enamel and dentine opposed to dentine. The aim of this study was to assess the characteristics of wear between specimens in which enamel was opposed to dentine at loads simulating attrition and at pH values simulating different erosive environments. It was hypothesized that enamel would wear more slowly than dentine under all conditions., Design: Opposing enamel and dentine specimens from 57 human third molar teeth were worn in electromechanical machines with various loads (32, 62 and 100 N) and lubricants (pH 1.2, 3.0 and 6.1). Tooth wear was quantified by measuring reduction in dentine volume over time using a 3D profilometer. Qualitative assessment was also carried out using scanning electron microscopy., Results: Dentine wear increased with increasing load, and dentine wear was faster at pH 1.2 than at pH 3.0 or 6.1 for all loads tested. Interestingly, enamel wore more rapidly than dentine at pH 1.2 under all loads. At pH values of 3.0 and 6.1, enamel wear rates were not measurably different from zero and they were less than wear rates for opposing dentine specimens at all loads. Micrographic assessment showed extensive surface destruction of dentine wear facets due to erosion at pH 1.2. Dentine wear facets were smoother at pH 3.0 that at pH 6.1., Conclusions: When enamel wears against dentine in an acidic environment enamel will wear more rapidly at very low pH, while under less acid conditions dentine will wear faster than enamel.

Background: The occurrence of ossifying fibromas (OFs) in childhood and adolescence has been described in the literature, along with different courses of the disease due to different growth rates., Case Report: In the case of the 15-year-old female patient presented here, an OF resulted in displacement of a maxillary third molar far into the maxillary sinus. It is assumed that the tumour originated coronal to the affected tooth 18. Radiographs document an initial rapid growth of this tumour over a period of 2 years, while its growth almost completely ceased in the next 2 years immediately prior to diagnosis and surgical treatment. The operation was complicated by unexpected profuse bleeding from the tumour tissue., Conclusion: The peculiarity of the OF in the case presented here is its similarity, in terms of clinical and radiological appearances, with a follicular cyst, its unusual place of origin that resulted in the migration of the tooth 18 into the maxillary sinus, its different growth dynamics, and the pronounced haemorrhage encountered as the tumour was surgically removed.

Purpose: The aim of the study was to assess changes in the angular position and eruption status of third molars in students of an Asian Indian population during a 4-year follow-up period., Patients and Methods: The series consisted of 43 students (11 males, 32 females). Standardized panoramic radiographs were taken at baseline (mean age, 19.1 +/- 0.7 years) and at the end of study (mean age 23.1 +/- 0.7 years). Angulation and degree of impaction of third molars were determined by their sagittal relationship to the adjacent second molar and eruption to the occlusal plane. Root formation was also assessed., Results: During the 4-year follow-up period, 11 of 72 of the mandibular teeth (15%) and 17 of 74 of the maxillary teeth (23%) changed their sagittal inclination. Thirty-one of 118 impacted teeth (26%; level B + C) achieved level A eruption more so in the mandible, ie, 15 of 52 (29%), than in the maxilla, ie, 16 of 66 (24%). In the mandible, 9 of 24 impacted vertical teeth (37.5%), 3 of 6 distoangular teeth (50%), and 3 of 22 mesioangular impacted teeth (13.6%) erupted to the occlusal plane. In the maxilla, 9 of 36 impacted vertical teeth (25%) and 7 of 27 distoangular teeth (26%) erupted to the occlusal plane during the follow-up. The number of teeth with complete root formation increased in mandible from 61% to 93% and in maxilla from 62% to 94.5%., Conclusions: Unpredictable changes in the position and angulation of teeth continued to occur even after the age of 19 years, more so in vertical and distoangular teeth than in mesioangular teeth.

Objectives: Ectopic mandibular third molar is a rare condition, and information is limited about its causes and characteristics. This article reports a case of ectopic mandibular third molar and reviews the literature on the clinical signs and symptoms and management of this condition., Study Design: We report a case of an impacted mandibular third molar dislocated high on the right side of the mandibular ramus. Thirteen cases of ectopic mandibular third molars reported in the English-language literature over the past 25 years, identified from Pubmed and Medline databases are also reviewed., Results: Among the 13 case reports identified, 10 occurred in women. Pain and swelling on the ispilateral side of the mandible or the preauricular region were the most common symptoms. Seven cases involved an ectopic mandibular third molar in the condylar or subcondylar region. Eight of the 11 cases included the description of a radiolucent image around the ectopic molar on the radiograph and described diagnosis of a dentigerous cyst. Seven of the ectopic third molars were extracted through intraoral access, and 3 were extracted through extraoral access., Conclusions: The etiology of ectopic mandibular third molars has not yet been completely clarified. Annual follow-up visits with panoramic radiographs are required for patients with symptom-free highly aberrant wisdom teeth. Treatment should be carefully planned according to the position of the ectopic tooth and the potential for trauma caused by the surgery.

Dental emergencies can significantly affect the mission of the deployed unit. As many as 22% of all emergency department visits at a deployed Expeditionary Medical Support facility can be attributed to dental problems and many of these problems are caused by impacted third molar teeth. Currently, there is no reliable method of predicting when asymptomatic, partially erupted third molar teeth will become painful. Therefore, it is imperative that Air Force dental providers carefully consider the consequences of giving patients a clean bill of health when asymptomatic, partially erupted third molar teeth are present in the oral cavity. Recommendations for future study are presented.

Introduction: Despite a substantial body of literature refuting an association between third molar eruption and crowding of the anterior dentition, the issue continues to be controversial., Methods: A survey was developed to evaluate and compare the current opinions of orthodontists (n = 393) and oral and maxillofacial surgeons (n = 458) regarding the link between third molars and the development of anterior crowding., Results: A smaller percentage of orthodontists than surgeons believed that maxillary (P <.0001) and mandibular (P <.0001) third molars produce anterior forces during eruption. Similarly, orthodontists were less likely to think that maxillary (P <.0001) and mandibular (P <.0001) third molars cause anterior crowding and were therefore less likely to recommend prophylactic removal of maxillary (P <.0001) and mandibular (P <.0001) third molars to prevent crowding. Surgeons were more likely to "generally" or "sometimes" (56.9%) recommend prophylactic removal of mandibular third molars to prevent crowding, whereas orthodontists more often said that they "rarely" or "never" (64.4%) recommend it. Differences in orthodontists' and oral and maxillofacial surgeons' beliefs about the association between third molar eruption and the development of crowding were significantly related to graduation year. More recently graduated orthodontists were less likely to recommend prophylactic removal of third molars to prevent crowding, and surgeons were more likely to recommend removal if they graduated in the 1970s or 1980s., Conclusions: Significant disagreement exists among practitioners, including both orthodontists and oral and maxillofacial surgeons, regarding the fundamental issues underlying the role of third molars in dental crowding.

Objective: Pain associated with third molar (wisdom) teeth is a common cause of morbidity for soldiers in the United Kingdom and on operational deployments. This study compared the incidence of third molar symptoms between soldiers serving in Iraq and soldiers stationed in barracks in Northern Ireland and assessed if pre-deployment screening could be improved., Method: Data was collected in a prospective cohort study over five consecutive months. Dental officers recorded each time an Army soldier presented with third molar related symptoms., Results: 1% of soldiers in Iraq had third molar related symptoms in this time compared to 1.4% of those stationed in Northern Ireland. The range of pathologies and teeth affected were similar between locations. In both locations approximately 40% of teeth that caused problems had been symptomatic before and 13 -16% had untreated decay., Conclusions: This study suggests that soldiers experience a lower incidence of symptoms related to third molars when in Iraq compared to Northern Ireland (P < 0.033) possibly due to pre-deployment treatment. If ideal pre-deployment screening of third molars was carried out and National Institute of Clinical Excellence guidelines applied 38% of the Operation Telic group of patient's problems could have been prevented.

Background: The paradental cyst is an odontogenic lesion of inflammatory origin that has few clinical signs and symptoms apart from recurring acute episodes. A well-defined radiolucency associated with the roots or distal to the crown may be seen radiographically. The purpose of this article is to report on different aspects of two cases involving paradental cysts. In the first case, the patient complained about recurring pericoronitis. A semilunar-shaped radiolucency on the distal aspect of the mandibular third molar was noted on the periapical radiograph. In the second case, the patient's main complaint was chronic trauma of the overlying mucosa. Radiographs revealed an enlarged pericoronal space., Methods: In both cases, the mandibular third molar was extracted due to a lack of space. Lesional samples were sent for histopathologic analysis., Results: In the first case, the drainage of cystic fluid and a regular concavity were found during tooth removal. In the second case, a nodular lesion was found adhering to the disto-buccal surface of the tooth arising from the distal wall of a periodontal pocket. The histopathologic analysis revealed a hyperplastic stratified squamous epithelium with arcading lining a fibrous capsule with inflammatory infiltrate, resulting in a final diagnosis of a paradental cyst., Conclusions: The presence of a paradental cyst should be considered when recurrent inflammatory periodontal processes are associated with partially erupted vital teeth, even when characteristic radiographic findings are absent. Definitive diagnosis requires a clinicopathologic correlation incorporating surgical, radiographic, and histologic findings.

The mandibular angle is one of the areas of the mandible that are prone to bone fractures, and the presence of an impacted third molar has been found to be associated with increased risk of angle fractures. The factors involved in bone fractures are the amount and direction of load, and the biomechanical and anatomical properties of bone. In the present study, micro-focused X-ray computed tomography was performed to observe and analyze the three-dimensional (3D) bone microstructure of the mandibular angle, and finite element analysis was conducted to examine the relationship between angle fractures and the presence or absence of mandibular third molars. 3D bone microstructure showed no marked difference between mandibles with and without third molars. Finite element analysis showed that, in the mandible with a third molar, stress was concentrated around the root apex of the third molar, and was transmitted in a direction matching the clinical findings of angle fractures. The results obtained in this study suggest that the presence of an impacted third molar changes the concentration and transmission of stress in the mandible, thus increasing the risk of an angle fracture.

A retrospective study, using panoramic radiographs, was conducted on 152 Taiwanese (72 males and 80 females) to investigate mandibular third molar eruption and impaction. The following measurements were made: inclinations and mesiodistal crown widths of the mandibular molars, vertical and horizontal spaces between the distal surface of the second molar and the anterior surface of the ramus, lengths and widths of the mandibular ramus and body, the ramus inclination, the mandibular plane angle, and the mandibular gonial angle. Differences between non-impaction and impaction groups were studied, and the variables were analyzed with multivariate discriminatory analysis. Significant differences between the two groups were found; variables describing spaces between the anterior of the ramus and the distal of the mandibular second molar and tooth size appeared to be the primary contributors to the differences observed.

The aim of the present study was to measure the dental arches in order to assess the potential influence of third molars on lower incisor crowding. Changes in dental arch dimensions were assessed in 47 patients (36 females, 11 males) three years following either removal of the third molars or deciding to leave them in situ. A dental pantomogram (DPT) was taken at the start of the study and plaster study models were obtained at both the beginning and end of the observation period. The study models were used to measure crowding, and to prepare photocopies for linear measurements of the dental arch (length and width). Using the DPT, the ratio of retromolar space to the width of the crown of the third molar was calculated using the method originally described by Olive and Basford and later modified by Ganss (Ganss ratio). The results showed that the measurements of crowding and arch length and width had changed in 12 lower and two upper dental arches. The relationship between these results and the Ganss ratio was statistically significant. Calculation of the Ganss ratio may therefore assist investigations into the development of dental arch crowding and also help determine the indications for third molar removal.

Bone formation is seen around the third molar even when the tooth is exposed to the oral environment due to overeruption. To determine if overeruption of the third molar with or without root exposure is related to the status of the exposure of other teeth in the mandible, using orthopantomographs, 424 third molars were studied in 371 patients who were over 41 years of age. The rate of overeruption and root exposure in third molars was measured, and its relationship to the number of teeth lost and the rate of root exposure in other teeth in the mandible was analyzed. Tooth loss in the group of third molars with overeruption without root exposure was greater than in that without overeruption or root exposure in men, whereas the relationship was not seen in women. We found that root exposures of other teeth in the group of third molars with overeruption without root exposure were significantly smaller than in those with root exposure in both genders. Third molars with overeruption without root exposure, in which bone formation was easy to observe for radiographic diagnosis, were correlated with periodontal health in the mandible, suggesting a component of precision determination for predicting resistance to periodontitis.

Ectopic eruption of teeth into regions other than the oral cavity is rare although there have been reports of teeth in the nasal septum, mandibular condyle, coronoid process, palate, chin, and maxillary sinus. Occasionally, a tooth may erupt in the maxillary sinus and present with local sinonasal symptoms attributed to chronic sinusitis. We present a case of an ectopic maxillary third molar tooth that caused chronic sinusitis in the maxillary sinus.
