ABSTRACT The collection of clinical data is an essential step for the development of any scientific research. Online digital data collection can optimize this step. Objective: To compare the response rate and the accuracy of the clinical data collection date through the online and physical digital questionnaire in orthopedic patients. Methods: Comparative study, level III of evidence, with forty patients who had ankle sprains were evaluated, followed up for a period of 12 weeks with the application of physical and digital Visual Analogue Scale, Foot Function Index and Cumberland Ankle Instability Tool questionnaires, and data were collected about the moment of collection of each questionnaire. Results: We obtained a response rate of 83.3% in the digital collection group and 60% in the physical collection group (p < 0.05), and the response rate in the digital collection group was higher at all times of collection (3, 6 and 12 weeks). Analysis of the time of collection shows greater variability in the larger physical collection group at all times of the study (2.8 vs 1.5; 4.0 vs 2.4; 8.6 vs 1.5). Conclusion: Digital data collection is effective for obtaining clinical data in patients with ankle sprains. Level of Evidence III, Comparative, Prospective, Longitudinal Study in Parallel Groups.

ABSTRACT Objective: To perform a systematic review and meta-analysis to compare clinical and surgical outcomes of posterior versus anterior approach to primary total hip arthroplasty (THA). Methods: This study followed the standard methodology established by the Cochrane Handbook and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Two independent reviewers searched for randomized controlled trials comparing posterior an anterior approach to primary THA with at least one quantifiable functional outcome published in the PubMed, Cochrane, and Virtual Health Library databases. Results: The analysis included ten randomized controlled trials conducted with 774 patients. The posterior approach was associated with shorter operative time (mean of 15.98 minutes shorter, 95% CI 11.21 to 20.76, p < 0.00001) while the anterior approach was associated with shorter length of hospital stay (0.31 days or about eight hours shorter, 95% CI 0.12 to 0.51, p = 0.002) and greater earlier improvement in functional outcomes up to six months from the procedure (mean Harris Hip Score of 4.06 points greater, 95% CI 2.23 to 5.88, p < 0.0001). Conclusion: Whereas the posterior approach to primary THA is associated with a shorter operative time, the anterior approach has the potential to decrease the length of stay and provide greater short-term functional restoration. Background: Muscle mass loss occurs following anterior cruciate ligament reconstruction surgery. Objective: To compare the gain in muscle strength in the quadriceps and hamstring muscles in patients following anterior cruciate ligament reconstruction surgery, using exercises with and without blood flow restriction. Methods: This is a randomized controlled trial, in which 50% (n = 14) of the participants were allocated to the intervention group and the remaining 50% (n = 14) to the control group. The study included the participation of postoperative patients, with reconstruction of the anterior cruciate ligament. Results: After comparing the rehabilitation of the groups, a statistical difference was observed in the quadriceps with an increase in muscle strength (p < 0.01) after 12 weeks and an increase in muscle strength hamstrings (p < 0.01) after 8 and 12 weeks in the injured legs of the intervention group compared to the control. In the analysis of the participants' physical function, there was an significant increase difference in the Lysholm questionnaire (p < 0.01) after 8 and 12 weeks, in the KOOS pain questionnaire (p < 0.01) after 4 weeks a decrease was observed, symptoms and daily activities (p < 0.01) after 8 and 12 weeks, quality of life (p < 0.01) after 12 weeks, and in the IKDC questionnaire (p < 0.01) after 8 and 12 weeks there was an significant increase difference of the intervention group compared to the control. Conclusion: After anterior cruciate ligament surgery, exercises with blood flow restriction proved more efficient for improving the muscle strength of the quadriceps and hamstrings, and the physical function of the knee than the same exercises without blood flow restriction. LAY ABSTRACT After anterior cruciate ligament reconstruction surgery patients need to undergo a gradual process of rehabilitation. This prospective study compared muscle strength gain between exercises with and without blood flow restriction (BFR) in the quadriceps and hamstring muscles. Regarding knee extension, there was an improvement in muscle strength after 12 weeks in the injured legs. For knee flexion an improvement was observed at 8 weeks, in addition to improvement in functionality. The use of blood flow restriction in rehabilitation exercises after anterior cruciate ligament reconstruction was shown to be more efficient for muscle strength gain in the quadriceps and hamstrings and improvement in knee physical function.

Abstract Background Osteoarthritis is the most common form of hand arthritis and arthritis of the carpometacarpal joint of the thumb is a potentially limiting disease. There is no homogeneity in the evaluation of outcomes for the rhizarthrosis treatment. In an attempt to standardize the evaluation of results, some subjective questionnaires, non-specific, were used to evaluate rhizarthrosis. Trapeziometacarpal Arthrosis Symptoms and Disability (TASD) was described by Becker et al.with the purpose of evaluating symptom intensity and degree of disability, as to compare results after treatment. Our objective is to translate, validate and do the cultural adaptation of the questionnaire TASD into the Brazilian Portuguese. Methods The questionnaire was translated, with reverse translation. The translations were evaluated and synthesized by a committee, arriving at TASD-BR. Thirty-one patients with a diagnosis of rhizarthrosis answered the questionnaire. We evaluated, the internal consistency, reliability, agreement and ceiling and floor effect for validation. Results The questionnaires were translated and adapted according to defined protocols. The internal consistency, through Cronbach's α coefficient for TASD-BR, was 0.927. The questionnaire's reliability, through the Intraclass Correlation Coefficient, was also shown to be quite high, with κ = 0.961 (0.954–0.967). The agreement, measured through the Standard Error Measurement, remained with standardized values below 5%. There was no ceiling and floor effect. Conclusion Through specific methodology we consider TASD-BR translated and valid for the Brazilian Portuguese.

Abstract Objective The aim of the present study was to quantify the urinary concentration of the C-terminal cross-linked telopeptide of type-II collagen (CTX-II) biomarker in patients who suffered an isolated ACL injury, and to compare the concentrations found in this population with a control group of patients with no metabolic changes in the knee that could lead to cartilage degeneration. Methods A cross-sectional pilot study was performed in two groups: patients with ACL tears and a control group (each group with 10 male subjects, with an age range between 18 and 35 years, and body mass index below 30 kg/m2). In both groups, urine concentrations of a biomarker related to the degradation of type-II collagen (CTX-II) was measured. For the group with ACL tears, a temporal relationship between the time after the injury and the amount of the biomarker was also examined. Results There were significant differences in the concentrations of urinary CTX-II between the ACL group and the control group (p = 0.009). No significant relationship was observed between the time after the injury and the quantity of the biomarker. Conclusions Patients with ACL injury had higher concentrations of urinary CTX-II biomarker than those with no ACL injury (p = 0.009). However, there was no correlation between the concentration of this biomarker and the elapsed time after the injury (p≥ 0.05).

Abstract Objective The present paper evaluates the resuming of physical activities by young, active patients who practiced some sport modality and underwent a high tibial osteotomy (HTO) using the opening wedge technique. Methods A total of 12 patients submitted to HTO using the opening wedge technique were prospectively analyzed. All patients were not playing sports at that time. Pre- and postoperative Lysholm and International Knee Documentation Committee (IKDC) scores, visual analog scale for pain and performance level were compared. The average follow-up time was of 12 months. Results One patient resumed sporting activities at a performance level significantly lower compared to the preoperative level, while eight patients returned at a slightly below level, two returned at the same level and one patient returned at a higher level in comparison with the preoperative period. Conclusion For isolated medial osteoarthrosis treatment, HTO using the opening wedge technique has favorable clinical and functional results, allowing patients to resume their sporting activities.

ABSTRACT Objective: This study aims to identify the epidemiology of orthopedic injuries in jiu-jitsu practitioners. Methods: Ninety-six jiu-jitsu practitioners aged between 18 and 45 years, male and female, answered a questionnaire addressing personal data and history of injuries related to the sport during the last 24 months. Results: In the period cover, 85% of the sample presented injuries related to the practice of jiu-jitsu, with an average of 60 days of absenteeism from sports practice. Fingers, shoulders, and knees were the joints most affected by orthopedic injuries. Conclusion: Orthopedic injuries are quite prevalent among jiu-jitsu practitioners, often distancing athletes from the sport. Level of Evidence IV, case series / cross sectional study.

Abstract Objective To examine the prevalence of carpal tunnel syndrome in powerlifting athletes with disabilities. Methods The present study evaluated the presence and intensity of pain (numerical scale), nocturnal paresthesia (self-report), and nerve compression (Tinel and Phalen signs) in wheelchair- and non-wheelchair-bound powerlifting athletes with disabilities. The clinical diagnosis of carpal tunnel syndrome was confirmed by the presence of two or more signs/symptoms. Results In total, 29 powerlifting athletes with disabilities were evaluated. None of the athletes reported the presence of pain or nocturnal paresthesia. The Tinel sign was present in 1 (3.45%) wheelchair-bound athlete. A positive Phalen test was present in 3 (10.35%) athletes (1 wheelchair-bound and 2 non-wheelchair-bound). Concurrent positive Tinel sign and Phalen sign tests were found in 2 (6.89%) athletes (1 wheelchair-bound and 1 non-wheelchair-bound). Conclusion Carpal tunnel syndrome was clinically diagnosed in 2 (6.89%) out of 29 powerlifting athletes with disabilities.

ABSTRACT Objective: To evaluate the fellow in knee surgery and the training offered by accredited programs in Brazil. Methods: This cross-sectional study administered a questionnaire to fellows in knee surgery. Results: Most fellows reported being assisted by the preceptor in theoretical, practical, and scientific activities. Conclusion: The training offered by accredited programs to fellows in knee surgery presented good results for their education. This study may propose educational measures for the Brazilian Society for Surgery of the Knee (SBCJ). Level of Evidence IIC, Cross-sectional study.

Abstract Purpose (1) To evaluate the biomechanical properties of a porcine flexor digitorum superficialis tendon graft with preserved muscle fibers and (2) to compare these results with the biomechanical properties of a porcine tendon graft after removal of associated muscle. Methods Eighty‐two porcine forelegs were dissected and the flexor digitorum superficialis muscle tendons were harvested. The study comprised of two groups: Group 1 (G1), harvested tendon with preserved muscle tissue; and Group 2 (G2), harvested contralateral tendon with removal of all muscle tissue. Tests in both groups were conducted using an electro‐mechanical material testing machine (Instron, model 23‐5S, Instron Corp., Canton, MA, USA) with a 500 N force transducer. Yield load, stiffness, and maximum load were evaluated and compared between groups. Results The behavior of the autografts during the tests followed the same stretching, deformation, and failure patterns as those observed in human autografts subjected to axial strain. There were no significant differences in the comparison between groups for ultimate load to failure (p = 0.105), stiffness (p = 0.097), and energy (p = 0.761). Conclusion In this porcine model biomechanical study, using autograft tendon with preserved muscle showed no statistically significant differences for yield load, stiffness, or maximum load compared to autograft tendon without preserved muscle. The preservation of muscle on the autograft tendon did not compromise the mechanical properties of the autograft. Level of evidence Level III Controlled laboratory study

ABSTRACT This is a case report of a previously healthy athlete who did not use oral anticoagulant, suffered a rupture of the distal biceps brachii tendon, and evolved with arm compartment syndrome. An emergency fasciotomy and the repair of the tendon were performed. After surgery the patient had a good recovery of the paresthesia and sensibility. This complication is rare and, when reported, is usually associated with patients who use anticoagulant therapy. Due to growth of rupture of distal biceps tendon cases, physicians should be aware that this complication must be treated as an emergency.

ABSTRACT Objective: To determine a relationship between smoking, alcohol abuse and anabolic steroids and meniscoligamentous injuries. Methods: A total of 239 patients underwent surgical treatment of isolated anterior cruciate ligament (ACL) lesion, ACL associated with meniscal injury and isolated meniscal injury, and were inquired by a single team of surgeons about their habits: smoking, alcoholism and use of artificial anabolic agents. In addition, quality of life was assessed using the SF-36 questionnaire. Results: It was not possible to establish a direct relationship between habits and meniscal and ligamentous injuries, despite the finding that patients with ACL-associated lesions were more frequently smokers, and that patients with isolated meniscal injuries used more artificial anabolic agents. Regarding quality of life, there were no statistical differences in the habits between the groups; however, except for smokers with isolated meniscal injury, all patients who did not have the habits analyzed presented a better score in the SF-36 questionnaire. Conclusion: There were no significant findings correlating smoking, alcohol abuse and artificial anabolic agents with the presence of injuries. In addition, six months after the surgical treatment, there was no difference in the quality of life between the groups evaluated. Level of Evidence III, Prospective case series.

Abstract Background The treatment of humeral fractures remains controversial. Systematic reviews demonstrate similar results between dynamic compression plating and locked intramedullary nailing in the surgical treatment of these fractures. However, it appears that antegrade intramedullary nailing causes higher residual pain in the shoulder. The proposal of this work is to evaluate through the WORC protocol (Western Ontario Rotator Cuff Index) the consequences in the quality of life of patients submitted to osteosynthesis of the humerus with antegrade locked intramedullary nailing. Methods This work is a cohort retrospective study in addition to the application of a questionnaire for self-rated quality of life with its 05 domains (WORC - Western Ontario Rotator Cuff Index) for patients (N = 26) classified in the Trauma Sector of the Department of Orthopedics and Traumatology of the Federal University of São Paulo (DOT/UNIFESP) submitted to Humerus Osteosynthesis with Antegrade Locked Intramedullary Nailing. There was also the inclusion of data related to the time since surgery, age, sex, surgical laterality, dominance among members and work leave, which were not considered in the original protocol. After, the data were statistically assessed to evaluate the association between numerical and categorical variables. Results The overall WORC score was 82.75 ± 17.00 (Mean ± SD) and was not different considering sex, age and postoperative period. Among the WORC domains, both Work and Sport / Recreation Protocols were the most unfavorable factors in the evaluation of patients. Although not statistically significant, those who had the procedure on the dominant side presented a lower quality of life score than those who had the surgery on the non-dominant side. Although non-significant again, those who were away from work had an overall lower quality of life score than those who were not. Conclusions The WORC Quality of Life Protocol shows good results for evaluating patients submitted to humerus osteosynthesis with antegrade locked intramedullary nailing. The data stratified by domains were good, however, Work and Sport/Recreation domains showed the lowest means compared to the other domains. ABSTRACT Objective: Several studies have already described the main injuries to soccer players during FIFA World Cup events; however, little is known about the main reasons spectators require medical assistance during these matches. The aim of this study is to assess the number of cases and main reasons spectators required medical assistance during the 2014 World Cup matches in Brazil. Methods: Data were collected from spectators who received medical assistance on all game days, and factors related to the assistance provided were analyzed. Results: Medical assistance was given to spectators in a total of 6,222 cases during the 64 games played in Brazil, an average of 97.2 times per game. The total number of spectators removed from the stadiums by ambulance was 167, a mean of 2.6 removals per game. The main reasons spectators required medical assistance during the World Cup games were headache, gastrointestinal problems, and trauma. Conclusions: Most spectators required medical assistance during the World Cup games for headache, gastrointestinal problems, and trauma; this information is fundamental to develop new prevention strategies and plan medical assistance for large-scale events. Introduction Basketball is a contact sport with complex movements that include jumps, turns and changes in direction, which cause frequent musculoskeletal injuries in all regions of the body.Objective This is an integrative systematic review of the epidemiology of musculoskeletal injuries in basketball.Methods This is an integrative review based on the following sources of information: PubMed/MEDLINE, Embase, LILACS, BBO-Biblioteca Brasileira de Odontologia, IBECS-Índice Bibliográfico Espanhol em Ciências da Saúde, nursing journals, dental journals and core clinical journals in the last 10 years with studies addressing the general epidemiology of sports injuries in basketball.Results In total, 268 articles were selected, of which 11 were eligible for the integrative review. A total of 12 960 injuries were observed, most of which occurred in the lower limbs (63.7%), with 2832 (21.9%) ankle injuries and 2305 (17.8%) knee injuries. Injuries in the upper limbs represented 12%–14% of the total injuries. Children and adolescents received head injuries more often compared with the other age and skill categories. In the adult category, there was an increased prevalence of injuries in the trunk and spine. In the upper limbs, hands, fingers and wrists were affected more frequently than the shoulders, arms and forearms. In the masters’ category, there was an increase in the incidence of thigh injuries.Conclusion The lower limbs were the most affected, with the ankle and knee joints having the highest prevalence of injuries regardless of gender and category. Further randomised studies, increased surveillance and epidemiological data collection are necessary to improve knowledge on sports injuries in basketball and to validate the effectiveness of preventive interventions.

ABSTRACT Objective: To determine the knowledge of Brazilian Orthopedic Surgeons on the costs of orthopedic surgical devices used in surgical implants . Methods: A questionnaire was applied to Brazilian Orthopedic Surgeons during the 46th Brazilian Congress on Orthopedics and Traumatology . Results: Two hundred and one Orthopedic Surgeons completely filled out the questionnaire. The difference between the average prices estimated by the surgeons and the average prices provided by the supplier companies was 47.1%. No differences were found between the orthopedic specialists and other subspecialties on the prices indicated for specific orthopedic implants. However, differences were found among orthopedic surgeons who received visits from representatives of implant companies and those who did not receive those visits on prices indicated for shaver and radiofrequency device. Correlation was found between length of orthopedic experience and prices indicated for shaver and interference screw, and higher the experience time the lower the price indicated by Surgeons for these materials . Conclusion: The knowledge of Brazilian Orthopedic Surgeons on the costs of orthopedic implants is precarious. Reduction of cost of orthopedics materials depends on a more effective communication and interaction between doctors, hospitals and supplier companies with solid orientation programs and awareness for physicians about their importance in this scenario. Objective:To evaluate the management, procedures and perspectives of sports physicians and orthopedists in Brazil with regard to diagnosing and treating muscle injuries.Methods:A questionnaire containing 20 questions relating to the topic of muscle injury was applied to sports physicians and orthopedists during the Second Brazilian Congress of Arthroscopy and Sports Traumatology, in 2013.Results:Completely answered questionnaires were received from 168 sports physicians and orthopedists. Doctors from all regions of Brazil with a mean of 11 years of experience of treating muscle injuries were interviewed. Lower limbs were affected in 97% of the cases, particularly the quadriceps, adductor and sural triceps. The injury occurred during the eccentric phase in 62% of the interviews; 39% underwent ultrasound examination and 37% magnetic resonance imaging (MRI) for the injury to be diagnosed. Medication, rest and cryotherapy during the acute phase (87.5%) and medication, rest and physiotherapy during treatment of the injury (56%) were the prevalent options. The criteria for returning to sports were very subjective and disparate among the options presented, and most of the interviewees had already used some therapy that was adjuvant to traditional methods.Conclusion:The number of muscle injuries treated per year was greater than 30, independent of whether in the public or private sector. These injuries occurred mainly at the muscle–tendon junction, in the lower limbs and during the eccentric phase of muscle contraction. Ultrasound was the examination most performed, while MRI was considered ideal. For most of the interviewees, the preferred treatment involved rest, medication and physiotherapy. In addition, 52% believed that platelet-rich plasma was an efficient treatment and 42% said that they had already used it.

Objective:To evaluate the expression of the genes COL1A1, COL1A2, COL3A1 and COL5A1 in the glenohumeral capsule of patients with traumatic anterior instability of the shoulder.Methods:Samples from the glenohumeral capsule of 18 patients with traumatic anterior instability of the shoulder were evaluated. Male patients with a positive grip test and a Bankart lesion seen on magnetic resonance imaging were included. All the patients had suffered more than one episode of shoulder dislocation. Samples were collected from the injured glenohumeral capsule (anteroinferior region) and from the macroscopically unaffected region (anterosuperior region) of each patient. The expression of collagen genes was evaluated using the polymerase chain reaction after reverse transcription with quantitative analysis (qRT-PCR).Results:The expression of COL1A1, COL1A2 and COL3A1 did not differ between the two regions of the shoulder capsule. However, it was observed that the expression of COL5A1 was significantly lower in the anteroinferior region than in the anterosuperior region (median ± interquartile range: 0.057 ±0.052 vs. 0.155 ±0.398; p = 0.028) of the glenohumeral capsule.Conclusion:The affected region of the glenohumeral capsule in patients with shoulder instability presented reduced expression of COL5A1.

Rotator cuff tear is a common orthopedic condition. Metalloproteinases (MMP) and their inhibitors (TIMP) seem to play a role in the development of joint injuries and in the failure of tissue healing. However, the mechanisms of regulation of gene expression in tendons are still unknown. Epigenetic mechanisms, such as DNA methylation and microRNAs regulation, are involved in the dynamic control of gene expression. Here, the mRNA expression and DNA methylation status of MMPs (MMP1, MMP2, MMP3, MMP9, MMP13, and MMP14) and TIMPs (TIMP1-3) and the expression of miR-29 family members in ruptured supraspinatus tendons were compared with non-injured tendons of individuals without this lesion. Additionally, the gene expression and methylation status at the edge of the ruptured tendon were compared with macroscopically non-injured rotator cuff tendon samples from the anterior and posterior regions of patients with tendon tears. Moreover, the possible associations between the molecular alterations and the clinical and histologic characteristics were investigated. Dysregulated expression and DNA methylation of MMP and TIMP genes were found across the rotator cuff tendon samples of patients with supraspinatus tears. These alterations were influenced at least in part by age at surgery, sex, smoking habit, tear size, and duration of symptoms. Alterations in the studied MMP and TIMP genes may contribute to the presence of microcysts, fissures, necrosis, and neovascularization in tendons and may thus be involved in the tendon healing process. In conclusion, MMPs and their inhibitors are regulated by epigenetic modifications and may play a role in rotator cuff tears.

Objective:To evaluate the approaches and procedures used by knee surgeons in Brazil for treating medial patellofemoral lesions (MPFL) of the knee in cases of acute traumatic dislocation of the patella.Materials and methods:A questionnaire comprising 15 closed questions on topics relating to treating MPFL of the knee following acute dislocation of the patella was used. It was applied to Brazilian knee surgeons during the three days of the 44th Brazilian Congress of Orthopedics and Traumatology, in 2012.Results:106 knee surgeons completely filled out the questionnaire and formed part of the sample analyzed. Most of them were from the southeastern region of Brazil. The majority (57%) reported that they perform fewer than five MPFL reconstruction procedures per year. Indication of non-surgical treatment after a first episode of acute dislocation of the patella was preferred and done by 93.4% of the sample. Only 9.1% of the participants reported that they had never observed postoperative complications. Intraoperative radioscopy was used routinely by 48%. The professionals who did not use this tool to determine the point of ligament fixation in the femur did not have a statistically greater number of postoperative complications than those who used it (p > 0.05).Conclusions:There are clear evolutionary trends in treatments and rehabilitation for acute dislocation of the patella due to MPFL, in Brazil. However, further prospective controlled studies are needed in order to evaluate the clinical and scientific benefit of these trends.

Objective:To determine the reference points for the exit of the tibial guidewire in relation to the posterior cortical bone of the tibia.Methods:Sixteen knees from fresh cadavers were used for this study. Using a viewing device and a guide marked out in millimeters, three guidewires were passed through the tibia at 0, 10 and 15 mm distally in relation to the posterior crest of the tibia. Dissections were performed and the region of the center of the tibial insertion of the posterior cruciate ligament (PCL) was determined in each knee. The distances between the center of the tibial insertion of the PCL and the posterior tibial border (CB) and between the center of the tibial insertion of the PCL and wires 1, 2 and 3 (CW1, CW2 and CW3) were measured.Results:In the dissected knees, we found the center of the tibial insertion of the PCL at 1.09 ± 0.06 cm from the posterior tibial border. The distances between the wires 1, 2 and 3 and the center of the tibial insertion of the PCL were respectively 1.01 ± 0.08, 0.09 ± 0.05 and 0.5 ± 0.05 cm.Conclusion:The guidewire exit point 10 mm distal in relation to the posterior crest of the tibia was the best position for attempting to reproduce the anatomical center of the PCL.

Background. Although Lactobacillus casei Shirota (LcS) can benefit the immune status, the effects of LcS in the immune/inflammatory responses of marathon runners has never been evaluated. Therefore, here we evaluated the effect of daily ingestion of fermented milk containing or not LcS in the systemic and upper airway immune/inflammatory responses before and after a marathon. Methods. Forty-two male marathon runners ingested a fermented milk containing 40 billion of LcS/day (LcS group, n = 20) or placebo (unfermented milk, n = 22) during 30 days pre-marathon. Immune/inflammatory parameters in nasal mucosa and serum, as well as concentrations of secretory IgA (SIgA) and antimicrobial peptides in saliva, were evaluated before and after fermented milk ingestion, immediately, 72 h, and 14 d post-marathon. Results. Higher proinflammatory cytokine levels in serum and nasal mucosa, and also lower salivary levels of SIgA and antimicrobial peptides, were found immediately post-marathon in the placebo group compared to other time points and to LcS group. In opposite, higher anti-inflammatory levels and reduced neutrophil infiltration on nasal mucosa were found in the LcS group compared to other time points and to the placebo group. Conclusion. For the first time, it is shown that LcS is able to modulate the systemic and airways immune responses post-marathon.

Objective: To study the insertion of the pectoralis major tendon to the humerus, through knowledge of its dimensions in the coronal and sagittal planes. Methods: Twenty shoulders from 10 cadavers were dissected and the pectoralis major tendon insertion on the humerus was identified and isolated. The dimensions of its "footprint" (proximal to distal and medial to lateral borders) and the distance from the top edge of the pectoralis major tendon to apex of the humeral head structures were measured. Results: The average proximal to distal border length was 80.8 mm (range: 70 -90) and the medial-to-lateral border length was 6.1 mm (5 -7). The average distance (and range) from the apex of the pectoralis major tendon to the humeral head was 59.3 mm. Conclusions: We demonstrate that the insertion of the pectoralis major tendon is laminar, and the pectoralis major tendon has an average footprint height and width of 80.8 mm and 6.1 mm, respectively.

OBJECTIVE: To evaluate and validate the radiographic measurement of patellar height with computerized tomography scans. METHODS: Measured the patellar height through the lateral radiographic image supported by one foot and sagittal tomographic view of the knee in extension, flexion of 20°, and quadriceps contraction of 40 patients (80 knees), asymptomatic and no history of knee injuries using Insall-Salvati index. There were 20 adult females and 20 adult males. RESULTS: The height patellar index was higher in women of all images taken, in proportion. There was no statistical difference of patellar height index between the radiographics and tomographics images. CONCLUSION: The Insall-Salvati index in females was higher in all cases evaluated. Furthermore, it is possible to measure the patellar height index during tomographic study without distorting the results obtained, using to define the presence of patella alta or patella baja.

OBJECTIVE: Evaluate a better moment by the medical team and patient to be discharged and relate to possible medical discharge criteria. METHODS: 31 anterior cruciate ligament reconstructed patients under similar conditions prospectively evaluated about the possibility of discharge with 24 and 48 hours after surgery and possibles discharges criteria such as pain, range of motion and capacity quadriceps contraction, besides the use of a validated scale to measure the patient's functional independence. RESULTS: 50% and 6.4% of patients prefer remain hospitalized after 24 and 48 hours of surgery, respectively. The average of the visual analogue scale of pain was 2.63 and 1.76 points, and the range of motion of 79º and 86,7º after 24 and 48 hours, respectively. 100% of patients were able to quadriceps contraction in every evaluated moments. CONCLUSION: In Brazil, possible discharged criteria as pain, range of motion, quad contraction and motor independence motor function scale show that anterior cruciate reconstruction reconstructed patients could be discharged after 24 hours of surgery. However, 50% of patients still prefer to remain hospitalized for longer periods.

OBJETIVO: Analisar os efeitos do esforço físico de partida de futebol na capacidade funcional e estabilidade dos membros inferiores em jovens jogadores de futebol. MÉTODOS: Foram analisados 10 atletas jogadores de futebol submetidos a avaliação da capacidade funcional dos membros inferiores através de um protocolo de Hop Test e avaliação do nível de estabilidade postural através do Biodex Stability System (Biodex, Inc., Shirley, Nova Iorque), antes e imediatamente após um tempo de jogo amistoso de futebol de campo com duração de 45 minutos. RESULTADOS: Após a partida, ocorreu uma diminuição no índice de estabilidade global (F(1,23) = 7,29 P = .024) e índice anteroposterior (APSI) (F(1,23) = 5,53 P = .043). A fadiga no membro dominante foi a responsável pelo déficit significativo no OSI (F(1,23) = 3.16, P = .047) e APSI (F(1,23) = 3.49, P = .029), enquanto o membro não-dominante não gerou qualquer alteração na condição pré e pós- jogo. CONCLUSÃO: Uma partida de futebol pode causar diminuição da estabilidade e capacidade funcional dos membros inferiores em jovens jogadores. OBJETIVO: O objetivo deste estudo é avaliar as condutas e procedimentos realizados pelos cirurgiões de joelho do Brasil no tratamento e reabilitação das lesões do ligamento cruzado anterior. MÉTODOS: Um questionário de 21 questões fechadas foi elaborado abordando tópicos relacionados ao tratamento e reabilitação após reconstrução do LCA. O questionário foi aplicado a cirurgiões brasileiros de joelho durante os três dias do 42º Congresso Brasileiro de Ortopedia e Traumatologia do ano de 2010. RESULTADOS: No total, 226 cirurgiões preencheram completamente o questionário e fizeram parte da amostra analisada. A maior parte destes era proveniente da região Sudeste do País. Os tipos de enxerto mais utilizados foram os tendões flexores e o terço central do tendão patelar ipsilateral à lesão, utilizados por 82,3% e 53,5% da amostra, respectivamente. A técnica de reconstrução com banda única transtibial é a preferida, sendo realizada por 66,4% dos participantes. O período de uma a quatro semanas entre a lesão e a realização do procedimento cirúrgico foi o considerado ideal pela maioria dos participantes (52,65%). Queixa de falseio/instabilidade do paciente e presença da manobra de pivot-shift positiva no exame físico foram os fatores considerados mais determinantes na decisão de operar o paciente. Já a satisfação do paciente e a ausência de queixas de instabilidade no pós-operatório foram os critérios julgados mais importantes para considerar a cirurgia um sucesso. CONCLUSÕES: Existem claras tendências em evolução no tratamento e reabilitação do LCA no Brasil. No entanto, mais estudos prospectivos controlados são necessários para avaliar o benefício clínico e científico destas tendências.OBJECTIVE: The aim of this study was to evaluate the procedures used by knee surgeons in Brazil for treating and rehabilitating anterior cruciate ligament injuries. METHODS: A questionnaire consisting of 21 closed questions was developed, addressing topics relating to treatment and rehabilitation after anterior cruciate ligament reconstruction. The questionnaire was applied to Brazilian knee surgeons during the three days of the 42nd Brazilian Congress of Orthopedics and Traumatology in 2010. RESULTS: A total of 226 surgeons filled out the questionnaire completely. The most commonly used types of graft were hamstrings tendons and the central third of the ipsilateral patellar tendon, which were used by 82.3% and 53.5% of the sample, respectively. The technique of reconstruction with a single transtibial band was the first preference and was used by 66.4% of the participants. A period of 1 to 4 weeks between injury and surgical procedure was considered ideal by most participants (52.65%). Complaints from patients that the knee was 'giving way' or unstable and presence of a positive pivot shift maneuver were the most decisive factors considered in making the decision to operate the patient. Patient satisfaction and absence of complaints of instability during the postoperative period were the criteria deemed to be most important for the surgery to be considered a success. CONCLUSIONS: There are clearly evolving trends in treating and rehabilitating the anterior cruciate ligament in Brazil. However, more prospective controlled studies are needed in order to evaluate the clinical and scientific benefits of these trends.

OBJETIVO: O objetivo primário deste estudo é avaliar a evolução clínica e funcional dos pacientes com lesão cartilaginosa de espessura total, sintomática da superfície articular da patela tratados com transplante osteocondral autólogo. MÉTODOS: Este estudo prospectivo envolveu 17 pacientes, sendo realizado no período de junho de 2008 a março de 2011. Foram preenchidos no pré-operatório e com um ano de pós-operatório, os questionários específicos de Lysholm, Kujala e Fulkerson para avaliação do joelho acometido e o SF-36 para avaliação da qualidade de vida geral dos pacientes. Foi utilizado o teste não paramétrico pareado de Wilcoxon na análise estatística dos valores pré e pós-operatórios dos questionários e os dados foram analisados no programa SPSS for Windows versão 16.0 e uma significância de 5% foi adotada. RESULTADOS: O Lysholm pré e pós-operatório médio foi de 54,59 e 75,76 pontos (p < 0,05). A pontuação do Fulkerson pré e pós-operatório médio foi de 52,53 e 78,41 pontos (p < 0,05). CONCLUSÕES: Consideramos o transplante osteocondral autólogo um bom método de tratamento para as lesões condrais de espessura total sintomáticas da superfície articular da patela.OBJECTIVE: The primary aim of this study was to assess the clinical and functional evolution of patients with total-thickness symptomatic cartilaginous injury of the patellar joint surface, treated by means of osteochondral autologous transplantation. METHODS: This prospective study was conducted from June 2008 to March 2011 and involved 17 patients. The specific questionnaires of Lysholm, Kujala and Fulkerson were completed preoperatively and one year postoperatively in order to assess the affected knee, and SF-36 was used to assess these patients' general quality of life. The nonparametric paired Wilcoxon test was used for statistical analysis on the pre and postoperative questionnaires. The data were analyzed using the SPSS for Windows software, version 16.0, and a significance level of 5% was used. RESULTS: The Lysholm preoperative and postoperative average scores were 54.59 and 75.76 points (p < 0.05). The Fulkerson pre and postoperative average scores were 52.53 and 78.41 points (p < 0.05). CONCLUSIONS: We believe that autologous osteochondral transplantation is a good treatment method for total-thickness symptomatic chondral lesions of the joint surface of the patella.

OBJETIVO: O objetivo deste estudo é avaliar as condutas e procedimentos realizados pelos cirurgiões de joelho do Brasil no tratamento da osteoartrose com artroplastia unicompartimental e osteotomia tibial alta do joelho. MÉTODOS: Um questionário de 14 questões fechadas foi elaborado e aplicado a cirurgiões brasileiros de joelho durante os três dias do 43º Congresso Brasileiro de Ortopedia e Traumatologia. RESULTADOS: Um total de 113 cirurgiões de joelho preencheram completamente o questionário e fizeram parte da amostra analisada. Neste estudo, a maioria dos cirurgiões realizava menos de cinco artroplastias unicompartimentais do joelho/ano (61,1%) e entre cinco e 15 osteotomias tibiais altas/ano (37,2%). A utilização de navegação computadorizada no intraoperatório é ainda infrequente em nosso meio, sendo realizada por apenas 0,9% dos especialistas. A opção pelo uso da artroplastia total do joelho em detrimento da parcial devido à falta de familiaridade com a técnica cirúrgica foi relatada por 65,5% dos cirurgiões. Quando arguidos sobre a possibilidade de crescimento no número de próteses unicompartimentais no Brasil com o aumento da familiaridade com a técnica pelos cirurgiões do País, 80,5% dos entrevistados responderam que acreditam nesta hipótese. Nesta amostra, constatamos que quanto maior a experiência do cirurgião maior o número de próteses unicompartimentais e osteotomias tibiais realizadas anualmente (r = 0,550 e r = 0,465, respectivamente, e p < 0,05). CONCLUSÕES: Existem claras tendências em evolução no tratamento da osteoartrose unicompartimental com artroplastia parcial do joelho no Brasil. No entanto, mais estudos prospectivos controlados são necessários para avaliar o benefício clínico e científico destas tendências.OBJECTIVE: The aim of this study was to evaluate the approaches and procedures used by Brazilian orthopedic surgeons for treating osteoarthrosis by means of unicompartmental knee arthroplasty and high tibial osteotomy of the knee. METHODS: A questionnaire with 14 closed questions was developed and applied to Brazilian knee surgeons during the three days of the 43rd Brazilian Congress of Orthopedics and Traumatology. RESULTS: A total of 113 surgeons filled out the questionnaire completely and became part of the sample analyzed. In this study, the majority of the surgeons performed fewer than five unicompartmental knee arthroplasty procedures/year (61.1%) and between 5 and 15 high tibial osteotomy procedures/year (37.2%). Use of computerized navigation systems during surgery remains uncommon in our environment, since only 0.9% of the specialists were using it. 65.5% of the surgeons reported that they had chosen to use total knee arthroplasty rather than partial arthroplasty due to lack of familiarity with the surgical technique. When asked about the possibility that the number of unicompartmental prostheses used in Brazil would grow as surgeons in this country become increasingly familiar with the technique, 80.5% of the respondents believed in this hypothesis. In this sample, we found that the greater the surgeon's experience was, the greater the numbers of unicompartmental prostheses and tibial osteotomies performed annually were (r = 0.550 and r = 0.465, respectively; p < 0.05). CONCLUSIONS: There is a clear evolutional trend towards treatment of unicompartmental osteoarthritis using partial knee arthroplasty in Brazil. However, further prospective controlled studies are needed in order to evaluate the clinical and scientific benefits of these trends.

OBJETIVO: Avaliar a capacidade do adesivo de fibrina em promover a fixação do menisco ao longo de duas, quatro e oito semanas comparando com a técnica convencional de fixação por sutura a tecidos moles. MÉTODOS: 36 meniscos mediais direitos de coelhos preservados a 73°C negativos por 30 dias foram transplantados para os animais da mesma amostra e fixados com sutura ou cola de fibrina. Após duas, quatro ou oito semanas a aparência dos meniscos e a qualidade da fixação foram verificadas macroscopicamente e avaliadas por um sistema de escores. Os achados foram submetidos a estudo estatístico de análise de variância por postos (p < 0,05%). RESULTADOS: A preservação por ultracongelamento manteve a integridade macroscópica dos meniscos transplantados. Após todos os períodos pós-transplante também não houve redução significante da extensão dos meniscos (p = 0,015). Os meniscos fixados com fibrina mostraram discretas alterações de coloração e rugosidade de superfície. Não houve sinais de rejeição ou infecção em ambos os grupos. A avaliação por escores da fixação pela sutura foi superior (p = 0,015) em todos os períodos (80% de fixação total) do que a fixação promovida pela fibrina (20% de fixação total). CONCLUSÃO: O transplante homólogo do menisco de coelho sofreu diferentes graus de integração ao joelho de acordo com a técnica de fixação; a técnica operatória com sutura aos tecidos moles mostrou-se superior na avaliação de escores em relação à fixação com adesivo de fibrina.OBJECTIVE: To evaluate the ability of fibrin adhesive in promoting the meniscus fixation within two, four and eight weeks compared to the conventional soft-tissue suture technique. MATERIALS AND METHODS: 36 right medial menisci of rabbits preserved at negative 73° Celsius for 30 days were transplanted to animals of the same sample and fixed with soft-tissue suture or fibrin glue. After 2, 4 or 8 weeks, the appearance of the menisci and the quality of fixation were macroscopically checked and evaluated by a scoring system. The findings were subjected to the statistical study of variance analysis (p < 0.05%). RESULTS: The deep-frozen meniscus preservation maintained the integrity of the meniscus transplant, and, macroscopically, there was no significant reduction of the length of the meniscus in all post-transplant periods (p = 0.015). The menisci fixed with fibrin showed slight changes in color and surface roughness. There were no signs of rejection or infection in both groups. Suture fixation scoring was superior (p = 0.015) in all periods (80% of total fixation) as compared to the setting promoted by fibrin (20% of total fixation). CONCLUSION: The homologous transplantation of the meniscus of rabbits experienced various degrees of integration to the knee according to the fixation method; the surgical soft tissues suturing technique was shown to be superior in the evaluation of scores compared to the fixation with fibrin adhesive.

Objective: To show the first Brazilian autologous chondrocyteimplantation. Methods: Young patients with localized lesions infemoral condyle and talus were selected. The clinical evaluationwas complemented with the imaginologic resources. Results: Theneoformed cartilage tissue occupies the defect. The patients showedimprove in the quality life, returning to the daily and sport activities. Conclusions: It was possible to concluded that the autologous chondrocyte implantation is a promising technique for the treatment of femoral condyle and talar condral lesions.

OBJETIVO: Realizar a tradução, adaptação cultural e validação do "Knee Outcome Survey- Activities of Daily Living Scale" (ADLS) para a língua portuguesa, bem como verificar suas propriedades de medida, reprodutibilidade e validade, para que o mesmo possa ser usado como um instrumento específico para avaliação de sintomas em pacientes brasileiros portadores de afecções no joelho durante as atividades de vida diária. MÉTODOS: Foram realizadas 2 entrevistas,na primeira o questionário foi aplicado em conjunto ao Lysholm, SF-36 e EAV (examinador 1). Após aproximadamente 30 minutos da primeira entrevista, os mesmos pacientes responderam apenas ao ADLS (examinador 2). Após um intervalo entre 7 a 10 dias, foi realizada a terceira entrevista constituída de nova aplicação do ADLS (examinador 2). RESULTADOS: A amostra consistiu de 53 entrevistados, sendo a média de idade de 33.0566 anos e patologia mais freqüente a lesão ligamentar (56,6%). O índice de Sperman obteve um valor de 0,986 (alfa=0,05, roOBJECTIVES: To perform the translation, cultural adaptation and validation of the "Knee Outcome Survey - Activities of Daily Living Scale" (ADLS) into Portuguese, as well as to check its measurement, reproducibility and validity properties in order to make it useful as a specific instrument for evaluating symptoms in Brazilian patients with knee injuries during their daily living activities. METHODS: Two interviews were performed; in the first interview, the questionnaire was applied along with Lysholm's, SF-36 and VAS (investigator 1). Approximately 30 minutes after the first interview, the same patients answered only the ADLS (investigator 2). After a break of 7 to 10 days, a third additional interview with the ADLS was applied (investigator 2). RESULTS: The sample was constituted of 53 subjects, with mean age of 33.0566 years, with the most frequent pathology being ligamentar injury (56.6%). OBJETIVOS: Realizar a tradução e adaptação cultural do "Sports Activities Scale" para a língua portuguesa, e verificar suas propriedades de medida, reprodutibilidade e validade. MÉTODO: baseado na revisão da literatura de Guillemin et al. foi realizado a tradução, adaptação cultural e validação. Para a análise estatística foi utilizado a análise descritiva, o teste t, o coeficiente de correlação intraclasse e o coeficiente de correlação de Spearman, com nível de significância igual a 5% (alfa = 0,05). RESULTADOS: O questionário traduzido apresentou: boa compreensão e equivalência cultural de 95% dos pacientes após algumas modificações; uma excelente reprodutibilidade (alfa=0,01, roOBJECTIVES: to translate and adapt the "Sports Activities Scale" as well as to check its measurement, reproduction and validity properties. METHOD: the translation, cultural adaptation and validation were made by reviewing the work by Guillemin et al. For statistical analysis, we used descriptive analysis, t-test, inter-class correlation coefficient and Spearman correlation coefficient, with significance level equal to 5% (alpha = 0.05). RESULTS: the translated questionnaire showed to be understandable and culturally equivalent for 95% of Brazilian patients after some modifications were made; an excellent reproducibility (alpha = 0.01, rho < 0.001) when correlating intra- and inter-interviewers; a significant correlation with the SF-36 questionnaire on the following items: functional capacity, pain, general aspects and social aspects, a weak association with SF-36 questionnaire on vitality, emotional aspects and mental health; a significant correlation with Lysholm (alpha = 0.01, rho > 0.001); and a moderate negative correlation with the visual analog scale for pain (alpha = 0.05, rho = 0.0013). CONCLUSION: The Brazilian version of the "Sports Activities Scale" questionnaire is an easily-manageable, reproducible and valid parameter for a specific evaluation of knee's symptoms and restraints during sports activities, in Brazilian patients.

The anterior cruciate ligament (ACL) is one of the most frequently injured structures during high-impact sporting activities. Gene expression analysis may be a useful tool for understanding ACL tears and healing failure. Reverse transcription-quantitative polymerase chain reaction (RT-qPCR) has emerged as an effective method for such studies. However, this technique requires the use of suitable reference genes for data normalization. Here, we evaluated the suitability of six reference genes (18S, ACTB, B2M, GAPDH, HPRT1, and TBP) by using ACL samples of 39 individuals with ACL tears (20 with isolated ACL tears and 19 with ACL tear and combined meniscal injury) and of 13 controls. The stability of the candidate reference genes was determined by using the NormFinder, geNorm, BestKeeper DataAssist, and RefFinder software packages and the comparative ΔCt method. ACTB was the best single reference gene and ACTB+TBP was the best gene pair. The GenEx software showed that the accumulated standard deviation is reduced when a larger number of reference genes is used for gene expression normalization. However, the use of a single reference gene may not be suitable. To identify the optimal combination of reference genes, we evaluated the expression of FN1 and PLOD1. We observed that at least 3 reference genes should be used. ACTB+HPRT1+18S is the best trio for the analyses involving isolated ACL tears and controls. Conversely, ACTB+TBP+18S is the best trio for the analyses involving (1) injured ACL tears and controls, and (2) ACL tears of patients with meniscal tears and controls. Therefore, if the gene expression study aims to compare non-injured ACL, isolated ACL tears and ACL tears from patients with meniscal tear as three independent groups ACTB+TBP+18S+HPRT1 should be used. In conclusion, 3 or more genes should be used as reference genes for analysis of ACL samples of individuals with and without ACL tears.

Rotator cuff tear is one of the most common causes of shoulder dysfunction. Gene expression analysis may be a useful tool for understanding tendon tears and the failure of cuff healing, and reverse-transcription quantitative polymerase chain reaction (RT-qPCR) has become an effective method for such studies. However, this technique requires the use of suitable reference genes for data normalization. Here, we evaluate the suitability of six reference genes (18S, ACTB, B2M, GAPDH, HPRT1 and TBP) using samples from the rotator cuff tendons of 28 individuals with tendon tears (3 tendons regions) and 8 controls (2 tendon regions); for the tear patients, we evaluated ruptured and non-ruptured tendon samples. The stability of the candidate reference genes was determined using the NormFinder, geNorm, BestKeeper and DataAssist software packages. Overall, HPRT1 was the best single reference gene, and HPRT1+TBP composed the best pair and HPRT1+TBP+ACTB composed the best trio of reference genes from the analysis of different groups, including the simultaneous analysis of all tissue samples. To identify the optimal combination of reference genes, we evaluated the expression of COL1A1 and COL3A1, and no obvious differences were observed when using 2, 3 or 4 reference genes for most of the analyses. However, COL3A1 expression differed between ruptured and non-ruptured (posterior superior region) tendons of patients only when normalized by HPRT1+TBP+B2M and HPRT1+TBP. On the other hand, the comparison between these two groups using the best trio of reference genes (HPRT1+TBP+ACTB) and 4 reference genes did not revealed a significant difference in COL3A1 expression. Consequently, the use of suitable reference genes for a reliable gene expression evaluation by RT-qPCR should consider the type of tendon samples investigated. HPRT1+TBP+ACTB seems to be the best combination of reference genes for the analysis of involving different tendon samples of individuals with rotator cuff tears.

Introduction: Quality of life can be defined as the expression of aconceptual model that tries to represent patient’s perspectivesand his/her level of satisfaction expressed by numbers. Theobjective of this study is to evaluate the parameters of quality oflife of 23 patients who underwent surgery for anterior cruciateligament reconstruction. Methods: We adopted SF-36, a generichealth-related evaluation questionnaire, to obtain informationregarding several aspects of patients’ health conditions, and theLysholm questionnaire, specific to evaluate the symptoms andfunction of the knee. The questionnaires were applied at two stagesof the treatment: pre- and postoperatively (after the rehabilitationprogram). Results: Before surgery, the Lysholm questionnairepresented the following results: excellent in 4% of the cases, goodin 22%, fair in 22%, and poor in 52%. After surgery (Lysholm e SF-36) the correlation level was approximately 44% (p = 0.041).Discussion: The correlation between the Lysholm and the SF-36questionnaires showed the following: the lower the level of pain,the higher the Lysholm score. The high scores presented by theLysholm questionnaire are directly proportional to physical andemotional aspects, and to functional capacity. Conclusion:Analysis of both questionnaires, as well as of their correlation,showed some improvement in patients´ quality of life. We werealso able to demonstrate the importance and usefulness of applyingthe two questionnaires at three different moments: before, duringand after physiotherapeutic intervention.

Shoulder instability is a common shoulder injury, and patients present with plastic deformation of the glenohumeral capsule. Gene expression analysis may be a useful tool for increasing the general understanding of capsule deformation, and reverse-transcription quantitative polymerase chain reaction (RT-qPCR) has become an effective method for such studies. Although RT-qPCR is highly sensitive and specific, it requires the use of suitable reference genes for data normalization to guarantee meaningful and reproducible results. In the present study, we evaluated the suitability of a set of reference genes using samples from the glenohumeral capsules of individuals with and without shoulder instability. We analyzed the expression of six commonly used reference genes (ACTB, B2M, GAPDH, HPRT1, TBP and TFRC) in the antero-inferior, antero-superior and posterior portions of the glenohumeral capsules of cases and controls. The stability of the candidate reference gene expression was determined using four software packages: NormFinder, geNorm, BestKeeper and DataAssist. Overall, HPRT1 was the best single reference gene, and HPRT1 and B2M composed the best pair of reference genes from different analysis groups, including simultaneous analysis of all tissue samples. GenEx software was used to identify the optimal number of reference genes to be used for normalization and demonstrated that the accumulated standard deviation resulting from the use of 2 reference genes was similar to that resulting from the use of 3 or more reference genes. To identify the optimal combination of reference genes, we evaluated the expression of COL1A1. Although the use of different reference gene combinations yielded variable normalized quantities, the relative quantities within sample groups were similar and confirmed that no obvious differences were observed when using 2, 3 or 4 reference genes. Consequently, the use of 2 stable reference genes for normalization, especially HPRT1 and B2M, is a reliable method for evaluating gene expression by RT-qPCR.

Although, it limits the range of motionit can achieve pain relief and function, when performed in the rightangle of fl exion and extension.

Abstract Objective To identify the incidence, the prevalence, the characteristics, and the possible risk factors for injuries occurring during the matches of the Brazilian Soccer Championship. Methods A prospective study was carried out to collect data on the injuries that occurred during the 2016 Brazilian Soccer Championship. Lesions were recorded by the physician responsible for each team through an online software. Results Among the 864 athletes included in the study, 231 (26.7%) of the players presented some injury during the tournament. In total, 312 injuries were recorded during the Brazilian Soccer Championship, with an average of 0.82injuries pergame. Theincidence of injuries was 24.9 injuries per 1,000 match hours. Midfielders and forwards presented, respectively, an injury risk 3.6 and 2.4 times higher than goalkeepers. Conclusion The prevalence and incidence of lesions were, respectively, 26.7% and 24.9 injuries per 1,000 match hours. The most frequently affected body segment was the lower limbs (76.3%), and the athletes acting in midfield and forward positions were themost affected. Moreover, the greater prevalence of injuries occurred in the first part of the championship.

ABSTRACT Objective: Perform the preoperative measurement of the hamstring tendons using ultrasound imaging, validating and correlating the measured value with that found during surgical reconstruction of the ligament. Methods: A cross-sectional study was carried out with 24 patients who underwent ultrasonographic measurement of the semitendinosus and gracilis muscle tendons and were subsequently submitted to surgical reconstruction of the ACL, with ipsilateral semitendinosus and gracilis tendon grafting. Results: The patients’ ages ranged from 16 to 43 years, with a mean of 24.8 years (SD = 8.4 years), 79.2% were men, and the distribution by side was 41.7% right knees and 58.3% left knees. A non-significant correlation coefficient was found between the area calculated by ultrasound (2 × semitendinosus area + 2 × gracilis area) and the intraoperative measurement (r = 0.16; p = 0.443). No evidence of a difference between intraoperative measurements

ABSTRACT OBJECTIVE: To analyze the incidence of ACL and meniscal injuries in a population of recreational and elite athletes from Brazil and the relation of these injuries with their sports activities. METHODS: This was a prospective observational study of 240 patients with ACL and/or meniscal injuries submitted to surgical treatment. Data of patients and sport modality, as well as Tegner score were registered in the first clinical evaluation. The patients were divided into three groups: (1) isolated rupture of the ACL; (2) ACL injury associated with meniscal injury; (3) isolated menisci injury. RESULTS: The majority of the patients belonged to group 1 (44.58%), followed by group 2 (30.2%) and 3 (25%). Most patients were soccer players. The mean time from sport practice to injury in group 1 was 17.81 years. In group 2, it was 17.3 years, and in group 3, 26.91 years. Soccer athletes presented ACL injury in 0.523/1000 h of practice and meniscal injury in 0.448/1000 h of practice. Before the injury, the mean Tegner score obtained for groups 1, 2, and 3 were 7.18, 7.34, and 6.53, respectively. After knee injury, those values were 3.07, 3.18, and 2.87, respectively. CONCLUSION: Soccer was the sport that caused the majority of lesions, regardless the group. Furthermore, patients from groups 1 and 2 had less time of practice prior to the injury (17.81 and 17.3 years) than the patients of group 3 (26.91 years). Women presented a higher risk to develop ACL and meniscal injuries in 1000 h of game/practice. Running, volleyball, and weightlifting are in ascending order of risk for ACL and/or meniscal injury. Regarding the return to sport practice, the efficiency of all athletes was impaired because of the injury.

CONTEXT AND OBJECTIVE Orthopedic surgery implies high costs for both public and private healthcare. The aim of this study was to better understand the differences between the public and private sectors regarding treatment of a damaged anterior cruciate ligament, which is a common knee injury. DESIGN AND SETTING Descriptive cross-sectional study conducted during the Brazilian Orthopedics Congress in Brasília. METHODS We applied questionnaires during the 2010 Brazilian Orthopedics Congress, with participation by 241 knee surgeons from 24 Brazilian states. This was followed by statistical analysis on the data that were obtained. RESULTS The orthopedic surgeons who were evaluated used different approaches and treatment options in different Brazilian states, comparing between the public and private systems. CONCLUSION Both in the public and in the private systems in Brazil, because of non-medical issues surrounding the treatment, the best medical decision is not always made. This may be harmful both to patients and to physicians.

Abstract Objective To describe the surgical technique of distal closing-wedge femoral osteotomy and a cases series submitted to this technique. Methods A total of 26 patients submitted to medial closing-wedge distal femoral osteotomy from 2002 to 2013 were evaluated. All of the patients had their medical files and imaging exams reviewed to evaluate the degree of correction and their current state. Results Out of the 26 patients, 12 weremale and 14 were female. Their mean age was 47.15 years old. In all of the cases, a neutral alignment related to the anatomical axis was achieved. Most of the patients presented bone healing at 6 weeks. There were no cases of bleeding during the surgery. One patient presented with delayed bone healing. One patient complained of plaque-related discomfort, requiring the removal of the device. One patient had a superficial infection, but no osteotomy revision was needed. Therewere no cases of deep venous thrombosis or of pulmonary thromboembolism. To date, there has been no conversion to total knee replacement. Conclusion Treatment with medial closing-wedge distal femoral osteotomy sustained the proposed correction in patients with up to 15 years of follow-up.

OBJECTIVES: We evaluated mRNA expression levels of genes that encode TGF-β1; the TGF-β1 receptor; the collagen-modifying enzymes LOX, PLOD1, and PLOD2; and the extracellular matrix proteins COMP, FN1, TNC and TNXB in synovial/capsule specimens from patients with idiopathic adhesive capsulitis. Possible associations between the measured mRNA levels and clinical parameters were also investigated. METHODS: We obtained glenohumeral joint synovium/capsule specimens from 9 patients with idiopathic adhesive capsulitis who had not shown improvement in symptoms after 5 months of physiotherapy. Adhesive capsulitis was confirmed in all patients by magnetic resonance imaging. We also obtained specimens from 8 control patients who had underwent surgery for acute acromioclavicular joint dislocation and who had radiological indication of glenohumeral capsule alteration based on arthroscopic evaluation. mRNA expression in the synovium/capsule specimens was analyzed by quantitative reverse transcription PCR. The B2M and HPRT1 genes were used as references to normalize target gene expression in the shoulder tissue samples. RESULTS: The synovium/capsule samples from the patients with adhesive capsulitis had significantly higher TNC and FN1 expression than those from the controls. Additionally, symptom duration directly correlated with expression of TGFβ1 receptor I. CONCLUSION: Elevated levels of TNC and FN1 expression may be a marker of capsule injury. Upregulation of TGFβ1 receptor I seems to be dependent on symptom duration; therefore, TGFβ signaling may be involved in adhesive capsulitis. As such, TNC, FN1 and TGFβ1 receptor I may also play roles in adhesive capsulitis by contributing to capsule inflammation and fibrosis.

ABSTRACT Objectives: To evaluate the clinical and functional results of patients diagnosed with full-thickness chondral defects on symptomatic knees who underwent a biological repair technique using autologous matrix-induced chondrogenesis. Methods: Seven patients who underwent surgical treatment due to chondral lesions in the knee by autologous matrix-induced chondrogenesis were evaluated. The Lysholm, Kujala and visual analog scale of pain questionnaires were applied before and 12 months after the surgery. Nuclear magnetic resonance images were evaluated 12 months after surgery according to MOCART (magnetic resonance observation of cartilage repair tissue) cartilage repair tissue score. Results: Of the seven patients evaluated, three presented defects classified as grade III and four as grade IV according to the International Cartilage Repair Society classification. Chondral defects were located in the medial femoral condyle (n = 2), patella (n = 2), and trochlea (n = 3). The mean age of the patients (six men and one woman) was 37.2 years (24-54 years). The mean chondral defect size was 2.11 cm2 (1.0-4.6 cm2). After 12 months, post-operative nuclear magnetic resonance showed resurfacing of the lesion site with scar tissue less thick than normal cartilage in all patients. The mean MOCART score was 66.42 points. A significant decrease in pain and an improvement in the Lysholm and Kujala scores were observed. Conclusion: The use of the collagen I/III porcine membrane was favorable for the treatment of chondral and osteochondral lesions of the knee when assessing the results using the VAS, Lysholm, and Kujala scores 1 year after surgery, as well as when assessing the magnetic resonance image of the lesion 6 months after surgery.

ABSTRACT Objective: To evaluate and collect epidemiological data on injuries of the anterior cruciate ligament of the knee in professional soccer players of the Brazilian Championship. Methods: Transversal study, with information extracted from the online data platform www.transfermrkt.com.br (Transfermarkt GmbH & Co. KG), with 5 seasons (2015 - 2019) of the Brazilian championship series A and B being analyzed and revised by 3 researchers. Results: 52 injuries of the anterior cruciate ligament were detected, with a mean age of 26,3 years, time off due to injury of 244.5 ±31.6 days, regardless of the division. Forwarders and defenders had the highest incidences of injury without statistically significant difference, but they had greater chances than goalkeepers, left/right backs and midfielders. Conclusion: The incidence of injury to the anterior cruciate ligament of the knee in professional football players in the first and second divisions of the Brazilian soccer championship between 2015 and 2019 is 0.414 per 1,000 hours of play, a value similar to described in the literature. The incidence of the injury varies according to the player’s field position, being the forwarders and defenders the most injured players. Level of Evidence II, Retrospective study.
