Kemunculan strain parasit yang rintang terhadap hampir semua ubatan antimalaria telah mendorong para saintis mengkaji penggantian mekanisme tindakan alternatif yang lebih berkesan. Keberkesanan rawatan semasa antimalaria adalah terhad dari segi bio ketersediaan ubat yang rendah, ketoksikan ubat yang tinggi dan kadar keterlarutan dalam air yang rendah. Penghibridan adalah satu strategi menarik bagi mengembangkan konsep penemuan ubat antimalaria. Kerangka 4-aminokuinolina telah disasarkan dalam kebanyakan proses reka bentuk agen antiplasmodium kerana kos sintesisnya yang murah, selamat dan kurang toksik sejak 20 tahun yang lalu. Penemuan hibrid antiplasmodium menggunakan kerangka 4-aminokuinolina dan pelbagai moieti seperti artemisinin, piperidin, indolin, pirimidin telah menunjukkan aktiviti antiplasmodium yang baik. Walau bagaimanapun, sehingga kini penemuan hibrid ini masih tidak dapat dibangunkan dan memasuki ujian percubaan klinikal. Ulasan ini meringkaskan penemuan hibrid antiplasmodium yang telah diterbitkan dalam tempoh sebelas tahun ke belakang (2011-2021). Kelebihan dan kelemahan konsep penghibridan sebagai pengganti agen antiplasmodium sedia ada dibincangkan. Analisis kajian menunjukkan hibrid 4-aminokuinolina mempunyai aktiviti antiplasmodium yang setanding atau lebih baik secara in vitro berbanding rawatan profilaksis klorokuina. Hibrid kuinolina kelas IV adalah yang paling kerap dikaji dan diperoleh dalam kajian ini sepanjang tempoh sebelas tahun ke belakang. Kekurangan data praklinikal terperinci mengenai hibrid yang disintesis telah menghalang kajian lanjut dalam ujian klinikal.

Malaria masih lagi kekal menjadi salah satu penyakit endemik yang boleh membawa maut dan rawatan yang digunakan kini untuk merawat malaria adalah terapi gabungan artemisinin. Namun begitu, akses kepada rawatan ini adalah sangat terhad dan memberi kesan besar kepada negara miskin disebabkan kos terapi yang tinggi. Penilaian aktiviti antimalaria bagi kompleks organologam berasaskan ligan makroksiklik tetraaza masih kurang meluas dijalankan. Ligan makrosiklik tetraaza 5,5,7,12,12,14- heksametil-1,4,8,11- tetraazasiklotetradeka-7,14-dienium bromida, dikomplekskan dengan empat logam peralihan iaitu Cu(II), Pb(II) dan Ni(II) dan Cd(II). Aktiviti antimalarial secara in vitro ke atas strain parasit Plasmodium falciparum yang rintang terhadap klorokuina, K1 ditentukan dan dibandingkan dengan aktiviti antiplasmodial sebatian klorokuina dan artemisinin. Kompleks [Cd(L)(OAc)]Br- merencat pertumbuhan parasit dengan 50% kepekatan perencatan berkesan (EC50) dalam julat aktiviti antiplasmodium yang poten (EC50100 μM). Nilai indeks pemilihan yang tinggi turut ditunjukkan oleh kompleks Cd(II) iaitu 60.5 seterusnya menjadikan kompleks ini berpotensi untuk dibangunkan sebagai agen antimalaria.

Jangkitan malaria adalah penyakit berjangkit serius yang disebabkan oleh parasit plasmodium dan harus dirawat sebagai perubatan kecemasan. Sehingga kini, tiada vaksin yang sudah dikomersialkan untuk mencegah malaria. Enam sebatian porfirin logam nikel(II) dan zink(II) berasaskan porfirinmeso bebas bes 5,15-difenilporfirin (2), 5,15-diheksilporfirin (3) dan 5,10,15,20-tetrafenilporfirin (4) iaitu NiDDHP, NiDPP, NiTPP, ZnDHP, ZnDPP dan ZnTPP dihasilkan melalui penyejatan Lindsey sebelum dicirikan secara spektroskopi (resonans magnet nukleus, ultra lembayung boleh nampak, spektrometri jisim) dan fizikal (takat lebur). Aktiviti antiplasmodium dan kesitotoksikan secarain vitro terhadap strain rintang-klorokuina, P. falciparum K1 dinilai dan dibandingkan dengan aktiviti antiplasmodium dadah rujukan seperti klorokuina dan artemisinin. ZnDHP, ZnTPP dan NiDPP merencat pertumbuhan parasit dengan 50% kepekatan perencatan berkesan (EC50) dalam julat aktiviti antiplasmodium sederhana iaitu 21.4 sehingga 36.0 µM. Aktiviti kesitotoksikan terhadap sel mamalia Vero yang ditunjukkan oleh NiDPP, ZnDHP dan ZnTPP berada dalam julat tidak toksik iaitu 97 sehingga 587 µM. ZnDHP mempunyai nilai indeks pemilihan yang paling tinggi iaitu 27.2 µM, menunjukkan aktiviti antiplasmodium yang selektif terhadap perencatan plasmodium dan tidak toksik terhadap sel mamalia.

The role of Carica papaya L. leaf juice in immune dysregulation caused by dengue virus infection remains unclear. This study aimed to investigate the immunomodulatory activities of the freeze-dried C. papaya leaf juice (FCPLJ) on AG129 mice infected with a clinical DENV-2 (DMOF015) isolate. The infected AG129 mice were orally treated with 500 and 1000 mg/kg/day of FCPLJ, for three days. Platelet, leukocyte, lymphocyte and neutrophil counts were microscopically determined. The level of plasma proinflammatory cytokines was measured by multiplex immunoassay. The levels of intracellular cytokines and viral RNA were determined by RT-qPCR technique. The results showed that the FCPLJ treatment increased the total white blood cell and neutrophil counts in the infected mice. The FCPLJ treatment decreased the level of GM-CSF, GRO-alpha, IL-1 beta, IL-6, MCP-1 and MIP-1 beta in the plasma of the infected mice. The intracellular IL-6 and viral RNA levels in the liver of infected mice were decreased by the FCPLJ treatment. In conclusion, this study supports the potential immunomodulatory role of the FCPLJ in a non-lethal, symptomatic dengue mouse model. Further studies on the action mechanism of the C. papaya leaf juice and its possible use as adjunctive dengue immunotherapy are warranted.

Treatment of drug resistant protozoa, bacteria, and viruses requires new drugs with alternative chemotypes. Such compounds could be found from Southeast Asian medicinal plants. The present study examines the cytotoxic, antileishmanial, and antiplasmodial effects of 11 ethnopharmacologically important plant species in Malaysia. Chloroform extracts were tested for their toxicity against MRC-5 cells and Leishmania donovani by MTT, and chloroquine-resistant Plasmodium falciparum K1 strain by Histidine-Rich Protein II ELISA assays. None of the extract tested was cytotoxic to MRC-5 cells. Drug repurposing is commonly employed in the search for potential therapeutic agents. Andrographis paniculata, a medicinal plant commonly used for symptomatic relief of the common cold, and its phytoconstituent andrographolide, have been repeatedly identified as potential antivirals against SARS-CoV-2. In light of new evidence emerging since the onset of the COVID-19 pandemic, this rapid review was conducted to identify and evaluate the current SARS-CoV-2 antiviral evidence for A. paniculata, andrographolide, and andrographolide analogs. A systematic search and screen strategy of electronic databases and gray literature was undertaken to identify relevant primary articles. One target-based in vitro study reported the 3CLpro inhibitory activity of andrographolide as being no better than disulfiram. Another Vero cell-based study reported potential SARS-CoV-2 inhibitory activity for both andrographolide and A. paniculata extract. Eleven in silico studies predicted the binding of andrographolide and its analogs to several key antiviral targets of SARS-CoV-2 including the spike protein-ACE-2 receptor complex, spike protein, ACE-2 receptor, RdRp, 3CLpro, PLpro, and N-protein RNA-binding domain. In conclusion, in silico and in vitro studies collectively suggest multi-pathway targeting SARS-CoV-2 antiviral properties of andrographolide and its analogs, but in vivo data are needed to support these predictions.

It has been discovered that Plasmodium knowlesi (P. knowlesi) is transmitted from macaque to man. Thus, the aim of the present study was to determine P. knowlesi genetic diversity in both human (n = 147) and long-tailed macaque (n = 26) samples from high- and low-endemicity localities. Genotyping was performed using seven neutral microsatellite loci markers. The size of the alleles, multiplicity of infection (MOI), mean number of alleles (Na), expected heterozygosity (HE), linkage disequilibrium (LD), and genetic differentiation (FST) were determined. Malaysia has a national goal to eliminate malaria by 2020. Understanding the genetic diversity of malaria parasites in residual transmission foci can provide invaluable information which may inform the intervention strategies used to reach elimination targets. This study was conducted to determine the genetic diversity level of P. falciparum isolates in malaria residual foci areas of Sabah. Malaria active case detection was conducted in Kalabakan and Kota Marudu. All individuals in the study sites were screened for malaria infection by rapid diagnostic test. Blood from P. falciparum-infected individuals were collected on filter paper prior to DNA extraction. Genotyping was performed using merozoite surface protein-1 (MSP-1), merozoite surface protein-2 (MSP-2), glutamate rich protein (GLURP) and 10 neutral microsatellite loci markers. The size of alleles, multiplicity of infection (MOI), mean number of alleles (Na), expected heterozygosity (He), linkage disequilibrium (LD) and genetic differentiation (FST) were determined. In Kalabakan, the MSP-1 and MSP-2 alleles were predominantly K1 and FC27 family types, respectively. The GLURP genotype VI (751-800 bp) was predominant. The MOI for MSP-1 and MSP-2 were 1.65 and 1.20, respectively. The Na per microsatellite locus was 1.70. The He values for MSP-1, MSP-2, GLURP and neutral microsatellites were 0.17, 0.37, 0.70 and 0.33, respectively. Background The development of resistant to current antimalarial drugs is a major challenge in achieving malaria elimination status in many countries. Therefore there is a need for new antimalarial drugs. Medicinal plants have always been the major source for the search of new antimalarial drugs. The aim of this study was to screen selected Malaysian medicinal plants for their antiplasmodial properties. Methods Each part of the plants were processed, defatted by hexane and sequentially extracted with dichloromethane, methanol and water. The antiplasmodial activities of 54 plant extracts from 14 species were determined by Plasmodium falciparum Histidine Rich Protein II ELISA technique. In order to determine the selectivity index (SI), all plant extracts demonstrating a good antiplasmodial activity were tested for their cytotoxicity activity against normal Madin-Darby Bovine Kidney (MDBK) cell lines by 3-(4, 5-Dimethylthiazol-2-yl)-2, 5-diphenyltetrazolium bromide (MTT) assay. Investigations on the fundamental of malaria parasite biology, such as invasion, growth cycle, metabolism and cell signalling have uncovered a number of potential antimalarial drug targets, including choline kinase, a key enzyme involved in the synthesis of phosphatidylcholine, an important component in parasite membrane compartment. The effect on gene expression of Plasmodium falciparum K1 strain following 72 hours exposure to 2 microM (IC50 concentration) of the choline kinase inhibitor, hexadecyltrimethylammonium bromide (HDTAB) was evaluated by DNA microarray analysis. Genes important in P. falciparum intraerythrocytic life cycle, such as invasion, cytoadherance and growth were among those affected by at least 2-fold changes in their expression levels compared with non HDTAB-treated control.

Eurycoma longifolia, locally known as 'Tongkat Ali' is a popular local medicinal plant that possess a lot of medicinal properties as claimed traditionally, especially in the treatment of malaria. The claims have been proven scientifically on isolated compounds from the plant. The present study is to investigate the anti malaria properties of Eurycoma longifolia standardized extract (root) (TA164) alone and in combination with artemisinin in vivo. Combination treatment of the standardized extract (TA164) with artemisinin suppressed P. yoelii infection in the experimental mice. The 4 day suppressive test showed that TA164 suppressed the parasitemia of P. yoelii-infected mice as dose dependent manner (10, 30 and 60 mg/kg BW) by oral and subcutaneous treatment. By oral administration, combination of TA164 at 10, 30 and 60 mg/kg BW each with artemisinin respectively showed a significant increase in the parasitemia suppression to 63, 67 and 80 percent as compared to artemisinin single treatment (31%). Using subcutaneous administration, at 10 mg/kg BW of TA164 in combination with 1.7 mg/kg BW of artemisinin gave a suppression of 80% of infection. This study showed that combination treatment of TA164 with artemisinin gives a promising potential anti malaria candidate using both oral and subcutaneous route, the later being the most potent.

Seven Malaysian medicinal plants were screened for their antiplasmodial activities in vitro. These plants were selected based on their traditional claims for treatment or to relieve fever. The plant extracts were obtained from Forest Research Institute Malaysia (FRIM). The antiplasmodial activities were carried out using the pLDH assay to Plasmodium falciparum D10 strain (sensitive strain) while the cytotoxic activities were carried out towards Madin- Darby bovine kidney (MDBK) cells using MTT assay. The concentration of extracts used for both screening assays were from the highest concentration 64 microg/ml, two fold dilution to the lowest concentration 0.03 microg/ml. Goniothalamus macrophyllus (stem extract) showed more than 60% growth inhibition while Goniothalamus scortechinii root and stem extract showed a 90% and more than 80% growth inhibition at the last concentration tested, 0.03 microg/ml. The G. scortechini (leaves extract) showed an IC50 (50% growth inhibition) at 8.53 microg/ml, Ardisia crispa (leaves extract) demonstrated an IC50 at 5.90 +/- 0.14 microg/ml while Croton argyratus (leaves extract) showed a percentage inhibition of more than 60% at the tested concentration. Blumea balsamifera root and stem showed an IC50 at 26.25 +/- 2.47 microg/ml and 7.75 +/- 0.35 microg/ ml respectively. Agathis borneensis (leaves extract) demonstrated a 50% growth inhibition at 11.00 +/- 1.41 microg/ml. The study gives preliminary scientific evidence of these plant extracts in line with their traditional claims.

Malaria is a disease which is still endemic and has become a disastrous scourge because of the emergence of antimalarial drug resistant Plasmodium falciparum. A new approach in addressing this is in developing a combination drug. This study is to show the enhancement of antimalarial properties, when single compound, goniothalamin combine with standard drug, chloroquine. Based on 4 Day Test, percentage of parasite growth on treated infected mice were determined. Oral treatment with 1 mg/kg BW of chloroquine on experimental mice suppressed 70% and 76.7% of both Plasmodium yoelii and Plasmodium berghei, respectively. The infection of P. berghei in mice was inhibited less than 50% by goniothalamin individual treatment at all doses in this study. About 27.8% and 18.5% inhibition of infection were observed in P. yoelii infected mice treated with 30 mg/kg and 60 mg/kg of goniothalamin respectively and the suppression exceed more than 50% at higher doses (90 and 120 mg/kg). Combination of 1 mg/kg chloroquine with either 30 mg/kg or 60 mg/kg of goniothalamin decreased the parasitemia of P. yoelii infected mice more than 90% and prolong the survival up to 100% after treatment. Similar treatment to P. berghei infected mice only shows about 60% reduction of parasitemia. The study findings showed that antimalarial property of goniothalamin was enhanced by combination with chloroquine at lower dose of each drug.

In the present study we examined the effect of E. longifolia methanol extract (TA164) on the GSH levels of P. falciparum infected erythrocytes and uninfected erythrocytes. Our study on parasite growth shows the IC50 and IC75 values of TA164 to be 0.17 g/ml and 6 g/ml respectively while for BSO was 25.5 g/ml and 46.5 g/ml respectively. About 95% to 100% growth inhibition of P. falciparum infected erythrocyte was observed when treated with TA164 and BSO at 16 g/ml and 64 g/ml respectively. The study on GSH contents indicated that non-infected erythrocytes treated with 6 g/ml (IC75 values) of TA164 at 24 hours incubation showed less GSH content as compared to non-treated erythrocytes. A similar observation was seen on treated trophozoite infected erythrocyte (10% parasitemia) when treated with 6 g/ml at 3 hours incubation. Analysis of the GSH contents of parasite compartments treated with TA164 at the same concentration (6 g/ml) for 3 hours incubation indicated a reduction of GSH contents. At the same concentration, TA164 did not affect the GSH contents of enriched trophozoite infected erythrocytes (60-70% parasitemia). TA164 did affect the GSH content of non-infected erythrocyte at 24 hours (accept IC50 value) as well as the parasite compartments (trophozoite infected erythrocyte and parasite itself) but fails to affect the GSH content of enriched trophozoite infected erythrocyte.
