Abstract

The addition of montelukast to a second-generation H SB 1 sb -antihistamine, in people with CSU whose symptoms are not adequately responsive to H SB 1 sb -antihistamines alone. Prognosis Approximately 45% of people with CSU respond to H SB 1 sb -antihistamines at licensed doses,65 and reports estimate that up to two-thirds of the residual may respond to updosing.50 About two-thirds of those unresponsive to H SB 1 sb -antihistamines respond to omalizumab,66 and a similar, probably different but overlapping, proportion to ciclosporin, although there is less evidence for the latter.67 Of note, the response rates given include partial and complete responses, so full disease suppression may not be achieved in many people with CSU with these treatments alone. The GDG does not recommend routinely offering combinations of two different second-generation H SB 1 sb -antihistamines at the same time to people with CSU, although it was noted that some people may benefit.51 The safety of giving two H SB 1 sb -antihistamines at lower dosages has not been investigated and there is no published evidence on using such combination treatment. Summary: This study summarizes the use, since its inception, of the Cutaneous Lupus Disease Area and Severity Index (CLASI) as an outcome measure in clinical studies. We systematically searched the MEDLINE, PubMed, EMBASE and Cochrane databases for papers including the term 'cutaneous lupus disease area and severity index' and its abbreviations up to August 2017, identifying 205 abstracts. Following shortlisting, two independent physicians critically reviewed 71 papers for data extraction. We found that a limited number of high‐quality studies used the CLASI scoring as an outcome measure. We concluded that further validation is necessary to identify the effectiveness of the CLASI in the assessment of cutaneous lupus erythematosus subtypes. The use of standardized core patient‐ and physician‐reported outcome measures may reduce heterogeneity and allow comparisons between patients enrolled in clinical trials. The guideline aligns with the Medicines and Healthcare products Regulatory Agency recommendations on use of oral isotretinoin for severe acne, i.e. nodular/conglobate acne or acne at risk of permanent scarring resistant to adequate courses of standard therapy with systemic antibacterial and topical therapies; it also recommends isotretinoin for acne fulminans. However, future acne treatment developments, including topical antiandrogen and retinoid therapies, and alternative systemic treatments, may help. Acne remains the commonest chronic inflammatory dermatosis, affecting up to 96% of adolescents.1 Prevalence data suggest acne is presenting earlier and persisting longer.2 The negative impact of acne can be profound, and it ranks globally as one of the most burdensome skin conditions.3 High-quality, evidence-based guidelines that are practical, relevant, easily accessible and understood by all stakeholders are key to sharing the latest knowledge and implementing effective management. 40. Risk of Serious Infections in Patients with Psoriasis on Biologic Therapies: A Systematic Review and Meta-Analysis

Abstract

A comprehensive evaluation of the risk of serious infections in biologic therapies for psoriasis is lacking. We performed a systematic review and meta-analysis of randomized controlled trials (RCTs) and prospective cohort studies reporting serious infections in people taking any licensed biologic therapy for psoriasis compared with those taking placebo, nonbiologic therapy, or other biologic therapies. The quality of the studies was assessed using Grading of Recommendations Assessment, Development and Evaluation criteria. No significant heterogeneity was detected in data from 32 RCTs (n = 13,359 participants) and one cohort study (n = 4,993 participants). In adults, low- to very-low-quality RCT data showed no significant difference between any biologic therapy and placebo at weeks 12–16 (overall pooled Peto odds ratio = 0.71, 95% confidence interval = 0.36–1.41) and weeks 20–30 (odds ratio = 2.27, 95% confidence interval = 0.45–11.49). No significant differences were found in any of the other comparisons in underpowered RCT data. Prospective cohort study data of low quality suggests that only adalimumab (adjusted hazard ratio [adjHR] = 2.52, 95% confidence interval = 1.47–4.32) was associated with a significantly higher risk of serious infection compared with retinoid and/or phototherapy in adults. No association between biologic therapies and serious infections in patients with psoriasis who were eligible for RCTs was detected. Further observational studies are needed to inform the uncertainty around this risk in the real world.

Abstract

Summary: Background: Topical photodynamic therapy (PDT) is widely used to treat superficial nonmelanoma skin cancer and dysplasia, and is generally well tolerated. However, as with all treatments, adverse effects may occur and awareness may facilitate approaches to prevention and management. Objectives: To review the available evidence relating to the adverse effects of topical PDT, to help inform recommendations in updated clinical guidelines produced by the British Association of Dermatologists and British Photodermatology Group, and the efficacy of preventative and therapeutic approaches. Methods: This review summarizes the published evidence related to the adverse effects of topical PDT and attempts to interpret this evidence in the context of patient risk and management. Results: Pain and discomfort during PDT are acute adverse effects, which can be minimized through the use of modified and low‐irradiance PDT regimens and do not therefore usually limit successful treatment delivery. Other adverse effects include the risk of contact allergy to photosensitizer prodrugs, although this is rare but should be kept in mind, particularly for patients who have received multiple PDT treatments to larger areas. There are no other significant documented longer‐term risks and, to date, no evidence of cumulative toxicity or photocarcinogenic risk. Conclusions: Topical PDT is usually well tolerated, reinforcing the utility of this important therapeutic option in dermatology practice. The main acute adverse effect of pain can typically be minimized through preventative approaches of modified PDT regimens. Abstract

Summary: The 8th edition of TNM (tumour, node and metastasis) has numerous and important changes compared with the 7th edition. Public Health England and the Royal College of Pathologists, U.K., have adopted the 8th edition of TNM (TNM8) published by the Union for International Cancer Control for skin cancer staging. These changes will have an impact on the management and commissioning of melanoma and nonmelanoma skin cancer (NMSC). The T1–T3 categories for NMSC staging require the clinician to measure the maximum dimension (usually diameter) of every potential invasive cancer. For squamous, basal and adnexal carcinomas, but not Merkel cell carcinoma (MCC), the T1–T3 categories are defined by new 20‐mm and 40‐mm divisions based on the maximum dimension of the lesion. In addition, new risk factors upstage T1 or T2 to T3. For melanoma, mitotic index no longer influences separation of pathological stage (pT1). There is a new, additional stratification level at 0·8‐mm Breslow thickness. Subdivision pT1b, with a negative sentinel lymph node biopsy (SLNB) of pN0, is now stage IA compared with the previous IB. For MCC, SLNB is now included specifically in the pN staging system. The pT1 subdivision requires clinical information as to whether histologically involved nodes were clinically occult or detectable. For both melanoma and MCC the clinician must state whether the lymph nodes are occult or clinically detectable. Eyelid carcinoma continues to have a staging system different from that in general skin and the system is substantially revised in TNM8. What's already known about this topic? Knowledge of cancer stage is essential for optimal patient management.Cancer stage includes tumour characteristics, regional extension to lymph node(s) and the presence of any distant metastasis (termed TNM stage or stage group).TNM7, published in 2010, formed the basis of the professional datasets to handle and report skin cancer specimens, published online by the Royal College of Pathologists (www.rcpath.org).
