Objectives: The presentation and outcomes of acute decompensated heart failure (ADHF) during COVID times (June 2020 to Dec 2020) were compared with the historical control during the same period in 2019. Methods: Data of 4806 consecutive patients of acute HF admitted in 22 centres in the country were collected during this period. The admission patterns, aetiology, outcomes, prescription of guideline-directed medical therapy (GDMT) and interventions were analysed in this retrospective study. Results: Admissions for acute heart failure during the pandemic period in 2020 decreased by 20% compared to the corresponding six-month period in 2019, with numbers dropping from 2675 to 2131. However, no difference in the epidemiology was seen. The mean age of presentation in 2019 was 61.75 (±13.7) years, and 59.97 (±14.6) years in 2020. There was a significant decrease in the mean age of presentation (p = 0.001). Also. the proportion of male patients decreased significantly from 68.67% to 65.84% (p = 0.037). The in-hospital mortality for acute heart failure did not differ significantly between 2019 and 2020 (4.19% and 4.,97%) respectively (p = 0.19). The proportion of patients with HFrEF did not change in 2020 compared to 2019 (76.82% vs 75.74%, respectively). The average duration of hospital stay was 6.5 days. Conclusion: The outcomes of ADHF patients admitted during the Covid pandemic did not differ significantly. The length of hospital stay remained the same. The study highlighted the sub-optimal use of GDMT, though slightly improving over the last few years.

Due to poor healthcare capacity, low vaccination rate, increasing COVID-19 variants, socioeconomic disparities, and misinformation, it is inevitable that implications will prove to be damaging to both healthcare workers and civilians. Among these challenges, it is important to note the need for stronger epidemiological surveillance for both COVID-19 and dengue and the implementation of public health measures without endangering sources of livelihood. To sustain this, cooperation between WHO and Pakistan’s government must continue through smart lockdowns, dengue awareness campaigns, and double laboratory procedures.Keywords: dengue, COVID-19, Pakistan, healthcare infrastructure

Investigation of dielectric properties of materials has significant applications in the current scenario. A high sensitive microwave submersible sensor for measuring complex permittivity of liquids over a broad range is presented in this paper. An asymmetric coplanar strip (ACS) fed stepped monopole is employed as the sensing element, and the impedance contrast in the stepped monopole maximizes the sensor’s sensitivity. The sensor is immersed in the liquid sample, and its resonant frequency is exploited to determine the real part of permittivity, and the reflection phase or the radiation efficiency is utilized for the imaginary part of permittivity. Analysis of the sensor, loaded with liquid samples of various dielectric constants and loss tangents, is presented in this paper. The proposed sensor operating at 5.3 GHz under unloaded condition has a maximum numerical sensitivity of 13.7%. The sensor is fabricated and experimentally validated with liquids with dielectric constant ranging from 2–78. The proposed sensor is utilized to measure the glucose concentration in an aqueous glucose solution and the concentration of ethanol blending in petrol.

6. Introducing a Foundation Programme Aiming at Academic Cognition

- Abstract
The quality of education of an educational programme is a function of the value of the educational goals that the programme aims at, together with the effectiveness and efficiency of the means that it employs to achieve those goals, including pedagogical strategies and assessment tasks. Hence, in order to raise the quality of education, the current discourse of 'Outcome Based Education' needs to shift its focus from what the teacher does in the classroom to raising the quality of student learning. To do this, many universities both in India and abroad have initiated Foundation Programmes as part of both General Education and Specialized Education. This article proposes that such a foundation programme must aim at what we call Academic Cognition, in consonance with what NEP 2020 calls Higher-Order Cognition. We spell out the various strands of Academic Cognition, and explore ways in which foundation programmes in the curricula for Higher Education can provide the foundations for Academic Cognition.

Dose optimization of sirolimus may further improve outcomes in allogeneic hematopoietic cell transplant (HCT) patients receiving post-transplantation cyclophosphamide (PTCy) to prevent graft-versus-host disease (GVHD). Sirolimus exposure-response association studies in HCT patients (i.e., the association of trough concentration with clinical outcomes) have been conflicting. Sirolimus has important effects on T-cells, including conventional (Tcons) and regulatory T-cells (Tregs), both of which have been implicated in the mechanisms by which PTCy prevents GVHD, but there is an absence of validated biomarkers of sirolimus effects on these cell subsets. Considering the paucity of existing biomarkers and the complexities of the immune system, we conducted a literature review to inform a quantitative systems pharmacology (QSP) model of GVHD. The published literature presented multiple challenges. The sirolimus pharmacokinetic models insufficiently describe sirolimus distribution to relevant physiological compartments. Despite multiple publications describing sirolimus effects on Tcons and Tregs in preclinical and human ex vivo models, consistent parameters relating sirolimus concentrations to circulating Tcons and Tregs could not be found. Each aspect presents a challenge in building a QSP model of sirolimus and its temporal effects on T-cell subsets and GVHD prevention. To optimize GVHD prevention regimens, phase I studies and systematic studies of immunosuppressant concentration-effect association are needed for QSP modeling.

Due to their sub picosecond temporal resolution, coherent Raman spectroscopies have been proposed as a viable extension of Spontaneous Raman (SR) thermometry, to determine dynamics of mode specific vibrational energy content during out of equilibrium molecular processes. Here we show that the presence of multiple laser fields stimulating the vibrational coherences introduces additional quantum pathways, resulting in destructive interference. This ultimately reduces the thermal sensitivity of single spectral lines, nullifying it for harmonic vibrations and temperature independent polarizability. We demonstrate how harnessing anharmonic signatures such as vibrational hot bands enables coherent Raman thermometry.

We develop a model the dynamics of human mesenchymal stem cells (hMSCs) and chondrocytes evolving in a nonwoven polyethylene terephtalate (PET) scaffold impregnated with hyaluron and supplied with a differentiation medium. The scaffold and the cells are assumed to be contained in a bioreactor with fluid perfusion. The differentiation of hMSCs into chondrocytes favors the production of extracellular matrix (ECM) and is influenced by fluid stress. The model takes deformations of ECM and PET scaffold into account. The scaffold structure is explicitly included by statistical assessment of the fibre distribution from CT images. The effective macroscopic equations are obtained by appropriate upscaling from dynamics on lower (microscopic and mesoscopic) scales and feature in the motility terms an explicit cell diffusion tensor encoding the assessed anisotropic scaffold structure. Numerical simulations show its influence on the overall cell and tissue dynamics.

We propose a PDE-ODE model for tissue regeneration, obtained by parabolic upscaling from kinetic transport equations written for the mesoscopic densities of mesenchymal stem cells and chondrocytes which evolve in an artificial scaffold impregnated with hyaluron. Due to the simple chosen turning kernels, the effective equations obtained on the macroscopic level are of the usual reaction-diffusion-taxis type. We prove global existence of solutions to the coupled macroscopic system and perform a stability and bifurcation analysis, which shows that the observed patterns are driven by taxis. Numerical simulations illustrate the model behavior for various tactic sensitivities and initial conditions.

In Polaris, we introduced a cloud-native distributed query processor to perform analytics at scale. In this paper, we extend the underlying Polaris distributed computation framework, which can be thought of as a read-only transaction engine, to execute general transactions (including updates, deletes, inserts and bulk loads, in addition to queries) for Tier 1 warehousing workloads in a highly performant and predictable manner. We take advantage of the immutability of data files in log-structured data stores and build on SQL Server transaction management to deliver full transactional support with Snapshot Isolation semantics, including multi-table and multi-statement transactions. Increase in viscosity under increasing shear stress, known as shear thickening (ST), is one of the most striking properties of dense particulate suspensions. Under appropriate conditions, they exhibit discontinuous shear thickening (DST), where the viscosity increases dramatically and can also transform into a solid-like state due to shear induced jamming (SJ). The microscopic mechanism giving rise to such interesting phenomena is still a topic of intense research. A phenomenological model proposed by Wyart and Cates shows that the proliferation of stress-activated interparticle frictional contacts can give rise to such striking flow properties. Building on this model, a recent work proposes and verifies a universal scaling relation for ST systems where two different power-law regimes with a well-defined crossover point is obtained. Nonetheless, the difference in the nature of the flow in these two scaling regimes remains unexplored. Here, using rheology in conjugation with high-speed optical imaging, we study the flow and local deformations in various ST systems. We observe that with increasing applied stress, the smooth flow changes into a spatio-temporally varying flow across the scaling-crossover. India's contribution to cardiovascular research has been dismal with a share of only 1% of total number of papers published in the world during the period 1999–2008. Based on two recent studies published from Kerala, the Kerala ACS Registry and the CSI Kerala CRP Study and four other studies being undertaken in Kerala, we think that a Kerala model for cardiovascular research can be conceptualized. This model which consists of funding by professional organization of cardiologists with wide participation of cardiologists, physicians, health workers, nurses, and in some situations general public, logistics looked after by a central coordinator and study design by panel of experts or institutions of repute in the region and carried out at low cost can be considered for implementation in rest of India. Studies based on such a model may change practice pattern of cardiovascular diseases in India.

Vertical convection is the fluid motion that is induced by the heating and cooling of two opposed vertical boundaries of a rectangular cavity (see e.g. Wang et al. 2021). We consider the linear stability of the steady two-dimensional flow reached at Rayleigh numbers of O($10^8$). As a function of the Prandtl number, $Pr$, and the height-to-width aspect ratio of the domain, $A$, the base flow of each case is computed numerically and linear simulations are used to obtain the properties of the leading linear instability mode. Flow regimes depend on the presence of a circulation in the entire cavity, detachment of the thermal layer from the boundary or the corner regions, and on the oscillation frequency relative to the natural frequency of oscillation in the stably temperature-stratified interior, allowing for the presence of internal waves or not. Accordingly the regime is called slow or fast, respectively. Either the global circulation or internal waves in the interior may couple the top and bottom buoyancy currents, while their absence implies asymmetry in their perturbation amplitude. Six flow regimes are found in the range of $0.1 \leq Pr \leq 4$ and $0.5 \leq A \leq 2$. For $Pr \lessapprox 0.4 $ and $A>1$ the base flow is driven by a large circulation in the entire cavity. For $Pr \gtrapprox 0.7$ the thermal boundary layers are thin and the instability is driven by the motion along the wall and the detached boundary layer. Cyber attacks has always been of a great concern. Websites and services with poor security layers are the most vulnerable to such cyber attacks. The attackers can easily access sensitive data like credit card details and social security number from such vulnerable services. Currently to stop cyber attacks, various different methods are opted from using two-step verification methods like One-Time Password and push notification services to using high-end bio-metric devices like finger print reader and iris scanner are used as security layers. These current security measures carry a lot of cons and the worst is that user always need to carry the authentication device on them to access their data. To overcome this, we are proposing a technique of using keystroke dynamics (typing pattern) of a user to authenticate the genuine user. In the method, we are taking a data set of 51 users typing a password in 8 sessions done on alternate days to record mood fluctuations of the user. Developed and implemented anomaly-detection algorithm based on distance metrics and machine learning algorithms like Artificial Neural networks (ANN) and convolutional neural network (CNN) to classify the users. In ANN, we implemented multi-class classification using 1-D convolution as the data was correlated and multi-class classification with negative class which was used to classify anomaly based on all users put together. We were able to achieve an accuracy of 95.05% using ANN with Negative Class. From the results achieved, we can say that the model works perfectly and can be bought into the market as a security layer and a good alternative to two-step verification using external devices. This technique will enable users to have two-step security layer without worrying about carry an authentication device.

It has previously been demonstrated in both simulation and experiment that well aligned remote focusing microscopes exhibit residual spherical aberration outside the focal plane. In this work, compensation of the residual spherical aberration is provided by the correction collar on the primary objective, controlled by a high precision stepper motor. A Shack-Hartmann wave front sensor is used to demonstrate the magnitude of the spherical aberration generated by the correction collar matches that predicted by an optical model of the objective lens. The limited impact of spherical aberration compensation on the diffraction limited range of the remote focusing system is described through a consideration of both on-axis and off-axis comatic and astigmatic aberrations, which are an inherent feature of remote focusing microscopes., Comment: 14 pages, 8 figures

The theory of diffraction limit proposed by H.A Bethe limits the total power transfer through a subwavelength hole. Researchers all over the world have gone through different techniques for boosting the transmission through subwavelength holes resulting in the Extraordinary Transmission (EOT) behavior. We examine computationally and experimentally the concept of EOT nature in the microwave range for enhancing radiation performance of a stacked dipole antenna working in the S band. It is shown that the front to back ratio of the antenna is considerably enhanced without affecting the impedance matching performance of the design. The computational analysis based on Finite Difference Time Domain (FDTD) method reveals that the excitation of Fabry-Perot resonant modes on the slots is responsible for performance enhancement.
