Abstract

Background: Secondary angle closure glaucoma (SACG) can be quite puzzling and can challenge even an experienced glaucoma surgeon. Unlike primary angle closure glaucoma, SACG can have various ocular and systemic associations. A careful review of the symptoms and past ocular and surgical history cannot be overemphasized. Some of the SACG can be refractory, requiring drainage devices. However, sometimes, all it takes is a prompt laser iridotomy. This can significantly reduce ocular morbidity and, in some situations, even blindness. Gonioscopy, often an underutilized technique, is critical in making the right diagnosis. With the advent of imaging techniques such as anterior segment optical coherence tomography and ultrasound biomicroscopy, one can easily pick up the etiology and treat early. Purpose: As many cases of SACG present acutely, it is critical that one makes a prompt diagnosis. We present here a video bouquet of illustrated examples of SACG and the steps to identify the cause by using different imaging techniques. Through this video, we aim to make the diagnosis of SACG a simpler, more streamlined, and logical process that will help in the accurate diagnosis and management. Synopsis: This video demonstrates various etiologies of secondary angle closure and methods to identify and treat the same. Highlights: Secondary angle closure can occur either with or without pupillary block. SACG with pupillary block involves mechanisms such as seclusio pupillae, aphakic/pseudophakic glaucoma, phacomorphic glaucoma, and silicon oil-induced glaucoma. In contrast, there are various other etiologies causing anterior pulling or posterior pushing mechanisms that contribute to non-pupillary block SACG. Irrespective of the fact that Machine learning has produced groundbreaking results, it demands an enormous amount of data in order to perform so. Even though data production has been in its all-time high, almost all the data is unlabelled, hence making them unsuitable for training the algorithms. This paper proposes a novel method of extracting the features using Dynamic Mode Decomposition (DMD). The experiment is performed using data samples from Imagenet. The learning is done using SVM-linear, SVM-RBF, Random Kitchen Sink approach (RKS). Abstract

This study presents an integrated analog front-end (AFE) tailored for photoplethysmography (PPG) sensing. The AFE module introduces a novel transimpedance amplifier (TIA) that incorporates capacitive feedback techniques alongside common drain feedback (CDF) TIA. The unique TIA topology achieves both high gain and high sensitivity while maintaining low power consumption. The resultant PPG sensor module demonstrates impressive specifications, including an input noise current of 4.81 pA/sqrt Hz, a transimpedance gain of 18.43 MΩ, and a power consumption of 68 µW. Furthermore, the sensory system integrates an LED driver featuring automatic light control (ALC), which dynamically adjusts the LED power based on the strength of the received signal. Abstract

Collagen crosslinking, mediated by lysyl oxidase, is an adaptive mechanism of the cardiac repair process initiated by cardiac fibroblasts postmyocardial injury. However, excessive crosslinking leads to cardiac wall stiffening, which impairs the contractile properties of the left ventricle and leads to heart failure. In this study, we investigated the role of periostin, a matricellular protein, in the regulation of lysyl oxidase in cardiac fibroblasts in response to angiotensin II and TGFβ1. Our results indicated that periostin silencing abolished the angiotensin II and TGFβ1‐mediated upregulation of lysyl oxidase. Furthermore, the attenuation of periostin expression resulted in a notable reduction in the activity of lysyl oxidase. Downstream of periostin, ERK1/2 MAPK signaling was found to be activated, which in turn transcriptionally upregulates the serum response factor to facilitate the enhanced expression of lysyl oxidase. The periostin‐lysyl oxidase association was also positively correlated in an in vivo rat model of myocardial infarction. The expression of periostin and lysyl oxidase was upregulated in the collagen‐rich fibrotic scar tissue of the left ventricle. Remarkably, echocardiography data showed a reduction in the left ventricular wall movement, ejection fraction, and fractional shortening, indicative of enhanced stiffening of the cardiac wall. These findings shed light on the mechanistic role of periostin in the collagen crosslinking initiated by activated cardiac fibroblasts. Our findings signify periostin as a possible therapeutic target to reduce excessive collagen crosslinking that contributes to the structural remodeling associated with heart failure. Significance statement: Excessive collagen crosslinking mediated by lysyl oxidase (LOX) alters the mechanical properties of the cardiac wall following myocardial injury. In our study, we identified a novel regulatory mechanism by which periostin mediates LOX via activation of the ERK1/2 MAPK pathway and transcriptional activation of serum response factor in angiotensin II and TGFβ1‐stimulated cardiac fibroblasts. In addition, we observed a positive correlation between periostin and LOX in the infarcted scar of an in vivo rat model of myocardial infarction. These findings provide valuable insights into the intricate molecular mechanisms contributing to the stiffening of the cardiac wall and periostin as a possible therapeutic target to reduce excessive collagen crosslinking that contributes to the structural remodeling associated with heart failure. Abstract

Purpose: The present research intended to find whether the technology readiness motivators, Optimism, and innovativeness, impact the user's intention to accept Online Behavioural Advertising (OBA), an area that requires additional research. Theoretical framework: Even though these personalized advertisements provide an enhanced user experience, they provide users with advertisements based on their online surfing behavior, and unfortunately, at the same time, it also creates privacy concerns. At the same time, Privacy Calculus Theory (PCT) is considered an appropriate theory when it comes to a rational benefits-costs assessment of the privacy-centred decision-making by internet users. Design/Methodology: Based on an extensive literature review, a conceptual model is being proposed, and a scenario-based questionnaire was developed and pursued on internet users' experience of the subject matter. Six hundred ten valid responses were received, and PLS-SEM was used to analyze the results. Findings: The results show that Optimism and innovativeness have a direct and indirect effect on the intention to accept Online Behavioural Advertising (OBA). Technology readiness Motivators have a detrimental impact on the intention to accept OBA. Research, Practical and Social implications: Based on our findings, we may be able to gain a deeper understanding of the theory behind targeted, data-driven digital advertising and the way in which it pertains to online shopping. Presented here is evidence to support the theory that the predisposition of users toward new technology could be the reason for users ranking perceived benefits higher than risk. Originality/Value: This study attributes original work in the sense that it is the first time that an individual’s predisposition towards technology has been taken into consideration while analysing the acceptance of OBA grounded on the Privacy Calculus framework.
